1.Effect of monocular depth perception on accommodative response

Background Different depthes or distances are prompted with many visual cues on the flat screen.Which kind of depth cues plays a major role in the accommodation of human eyes,in addition,whether this effect exists in the different distance are still below understood.Objective This study was to research the effect of the monocular space depth perception on accommodative response at different distances,and to analyze the effect of linear perspective and size-constancy perceptual depth cues on the level and amplitude of accommodative fluctation.Methods Four groups of visual targets were designed by FLASH software and placed at the front of eyes with the distance of 50 cm or 33.3 cm,representing static change from far to near and dynamic change from far to near,from near to far respectively.Twenty-six healthy volunteers aged 22-26 years were included in this study,with the spherical power of (-4.06± 1.99) D and cylindrical power of (-0.38±0.41) D under the informed consent.The accommodative response and accommodative fluctuations during monocularly watching the targets were recorded at 5 times/second by Grand Seiko WAM 5500 automatic infrared refractor.This study was approved by Ethic Committee of Wenzhou Medical University,and written informed consent was obtained from each subject before entering the study group.Results At the distance of 50 cm,the accommodative response caused by the depth perception of near was (1.52±0.46) D,the one of distance was (1.37±0.46) D,with a significant difference of 0.15 D under the combined effect of linear perspective and size-constancy (P =0.016).The accommodative response caused by the depth perception of big car was (1.43 ± 0.35) D,and the one of small car was (1.43 ± 0.36) D,without statistically significant difference under the effect of size-constancy alone (P =0.467).At the distance of 33.3 cm,the accommodative response caused by the depth perception of near was (2.40±0.53)D,and the one of distance was (2.35 ±0.51) D,without significant difference under the combined effect of linear perspective and size-constancy (P=0.379).The accommodative response caused by the depth perception of big car was (2.38±0.48)D,the one of small car was (2.39±0.52)D,without statistically significant difference under the effect of size-constancy alone (P =0.820).In addition,the total curve of accommodative microfluctuation showed apparent fluctuation periodically caused by the distance perception involving linear perspective only.Conclusions Monocular space depth perception has some impacts on the accommodative response at certain distance.Linear perspective,which is one of the depth cues of space perception,plays a major role in the accommodative response.Compared with the size-constancy,linear perspective can provide depth perception at higher level.The size-constancy has a little or contrast impact to the accommodation,because it depends on the assistance from other depth cues like linear perspective.
2.Monitoring on dust particle and airborne microbe counts in the operating rooms in some municipal hospitals of Xi’an

Objective To understand environmental quality status of clean operating rooms in municipal hospitals of Xi’an,and provide reference for ensuring the clean quality of operating rooms.Methods On-site specimen collec-tion and detection were conducted to detect and analyze the cleanlinss of air in clean operating rooms in 15 hospital of Xi’an.Results A total of 51 operating rooms were monitored,secondary and tertiary hospitals had 24 and 27 operating rooms respectively;26 were grade-I and 25 were grade-Ⅲ operating rooms.The qualified rates of dust particle≥0.5μm/particle size in grade-I and grade-Ⅲ clean operating rooms were 76.92% and 80.00% respective-ly,≥5μm/ particle size were 73.08% and 88.00% respectively.The qualified rates of dust particle counts in grade-I and grade-Ⅲ clean operating rooms were 61 .54% and 80.00% respectively,difference was not significant(χ2=2.092,P >0.05 ).The qualified rate of dust particle counts in clean operating rooms in tertiary hospitals was significantly higher than secondary hospitals(85.19% vs 58.33%,χ2 =4.600,P <0.05 ).The qualified rates of airborne microbes in operating zone of secondary and tertiary hospitals were 87.50% and 92.59% respectively(P >0.05),in surrounding zone were 95.83% and 92.59% respectively (both P >0.05).Conclusion Dust particle and airborne microbes in some clean operating rooms in municipal hospitals of Xi’an are beyond the standard,supervi-sion and management should be strengthened.
3.Sterilization performance of pressure steam sterilizers in medical institutions of Xi'an city

Objective To understand the current situation of pressure steam sterilization in medical institutions in Xi'an city,and provide reference for the monitoring and quality control of pressure steam sterilization.Methods Central sterile supply departments(CSSDs) in medical institutions in Xi'an in 2012-2014 were investigated.Physical,chemical,and biological monitoring methods were used to evaluate the pressure steam sterilizers.Results A total of 135 medical institutions in Xi' an were monitored,including 40 tertiary medical institutions and 95 secondary and below medical institutions.A total of 540 specimens were collected,454 were qualified,overall qualified rate was 84.07 %.Qualified rate of monitoring result of pressure steam sterilization of tertiary medical institutions was higher than secondary and below medical institutions(93.13 % vs 80.26 %,x2 =13.91,P<0.01).Qualified rates of chemical PCD and biological test pack of tertiary as well as secondary and below medical institutions were 100% and >90% respectively.The qualified rate of recoded temperature in secondary and below medical institutions was 88.42 %,but actually measured temperature was 38.95 %,which was significantly lower than 72.50 % of tertiary medical institutions(x2 =12.68,P<0.01).The rates of the measured temperature> 134℃ of pressure steam sterilizer in each year were all low,but measured temperature<132℃ was about 50.00 %,difference between actually measured temperature and recorded temperature was-4℃-1 ℃,difference between-2.5 ℃-0.5℃ accounted for 80.00%,the main unqualified temperature was 130℃-131 ℃,accounting for 82.61 %.Conclusion Efficacy of pressure steam sterilization is different in different levels of medical institutions in Xi'an,physical monitoring measures are not standardized,the supervision of physical parameters in key sectors should be strengthened to ensure the sterilization quality of medical institutions and reduce the occurrence of iatrogenic infection.
4.Repeatability and reproducibility of manual measurements to peripapillary choroidal thickness in healthy children using enhanced depth imaging OCT

Background Current researches of choroidal structure changes primarily focus on macular choroidal thickness in adolescents and children,but there are few studies on peripapillary choroidal thickness in children with enhanced depth imaging (EDI)-OCT.In addition,the reliability of manual measurements to peripapillary choroidal thickness is an important factor for the estimation of EDI-OCT.Objective This study was to determine the repeatability and reproducibility of manual measurements of peripapillary choroidal thickness in children by EDI-OCT.Methods A reliability evaluation of diagnosis test was performed.EDI-OCT technology was used to image the peripapillary choroidal thickness in 49 children aged 7-14 years in Eye Hospital of Wenzhou Medical University from February to March 2015 under the informed consent of their custodians.An annular scanning was carried out surrounding optical disc by the same ophthalmologist with Spectralis OCT,and the peripapillary choroidal thickness was manually measured at global,temporal,superotemporal,superonasal,nasal,inferonasal and inferotemporal zones.The intraclass and interclass repeatability and reducibility of measuring values from intraobserver,inter-observer and intra-session were evaluated with intraclass correlation coefficients (ICC),and BlandAltman agreement analysis was used to assess the consistency of repeated measurements.Results The average age in the subjects was (9.9 ± 1.3) years.No significant difference was found in the mean difference of reduplicative measured peripapillary choroidal thickness at each zone (all at P>0.05).The ICCs of intra-observer,inter-observer and intra-session were 0.971-0.993,0.827-0.952 and 0.974-0.991,respectively,and the 95% limit of agreement of global peripapillary choroidal thickness were-12.4-9.7 μm,-15.2-11.6 μm and-16.3-19.1 μm,respectively.Conclusions The repeatability,reproducibility and consistency are favorable in manually measured values of peripapillary choroidal thickness from EDI-OCT image.EDI-OCT appears a good application in the assessment of peripapillary choroidal thickness in children.
5.Monitoring of hand hygiene status of health care workers in clinical laboratories of medical institutions in Xi'an City

Objective To investigate the current status of hand hygiene(HH) among health care workers(HCWs) in clinical laboratories in medical institutions in Xi'an City.Methods HH status of HCWs in clinical laboratories in medical institutions in Xi'an was performed random on-the-spot sampling and monitoring.Results A total of 240 HH specimens of HCWs in clinical laboratories in 80 medical institutions in Xi'an City were collected, 127 detected results were qualified, the total qualified rate was 52.92%.The qualified rates of medical institutions were as follows: municipal hospitals 62.67%,workers' hospitals 55.95%,private hospitals 40.74%;comprehensive medical institutions 67.68%,specialized medical institutions 42.55%;tertiary medical institutions 79.63%(n=43),secondary and below medical institutions 45.16%(n=84),there were significant differences in HH qualified rate among HCWs in different types of medical institutions(all P<0.01).Of different HH detection items, detection rates of Escherichia coli and Staphylococcus aureus were 0.83% and 8.33% respectively.There were significant differences in HH compliance rates among HCWs of all age groups(χ2=9.103,P<0.05), HCWs aged≥50 years had the highest qualified rate of HH(71.43%), followed by those aged<30 years (67.82%),HCWs in 40~ year age group had the lowest HH qualified rate (39.66%).Conclusion The qualified rate of HH of HCWs in clinical laboratory of medical institutions in Xi'an City is low, it is necessary to enhance the procaution awareness of HCWs in clinical laboratories, strengthen quality control of HH, strictly implement standard hand-washing procedures to reduce occurrence of HAI.
6.Effect of "321" health education model on uncertainty of disease in patients with diabetic retinopathy

Objective To explore the influence of "321" health education model on the quality of life, physiological and psychological treatment and uncertainty of disease in patients with diabetic retinopathy. Methods From January to December 2016, 104 patients with fundus surgery in the hospital for the treatment of diabetic retinopathy were selected as the research object. They were randomly divided into observation group and control group by the random number table method. Patients in the control group received routine health education and the patients in the observation group received the "321" model of health education. Patients in the two groups were tested with Mishel's uncertainty in illness scale (MUIS) before and after receiving education. The uncertainty of disease between two groups before and after education was compared, and two groups were assessed by using SF-36 scale and diabetes specific quality of life (DSQL). Results Before the education, the scores of disease uncertainty in the control group and the observation group were (104.9±6.7) and (105.1±6.5) points, and the difference was not statistically significant (t=-0.154, P=0.878); After receiving education, the scores of disease uncertainty in the control group and the observation group were (92.6±6.2) and (82.2±6.1) points, and the difference was statistically significant (t=8.622,P< 0.001); The scores of psychological/mental health and treatment were significantly different between the two groups after receiving education (P<0.05). Conclusions The "321" health education model for diabetic retinopathy can significantly reduce the uncertainty of disease, and has positive significance for the psychological/mental health and treatment of the patients.
7.Qualitative research on post-vitrectomy prone position experience of rotating nursing students

Objective To explore rotating nursing students′ feelings and experience of patients′postoperative prone position during their practice in department of ophthalmic surgery, so as to make them more considerate about the patients′ pain and difficulty out of transference, more patient to the patients, and thus increase patients′ comfort and satisfaction. Methods Hermeneutic phenomenological method in qualitative research was adopted. Every rotating nursing student, during the rotation, spent 40 min experiencing the prone position, and 12 were interviewed fully by researchers about their actual feelings and thoughts. Results With generic analysis, after repeated introspection, comparison and induction, four major themes were extracted:comfort level of the prone position was lower than expected; existing prone position equipments needed to be improved; nursing students were lack of patience to the prone-position patients; clinical teaching methods of nursing needed to be improved. Conclusions By experiential teaching, nursing students are fully aware of the discomfort of prone position and shortcomings of equipments, which help them to be more empathetic and more patient to the patients.
8.Association of serum β2-microglobulin levels with post-stroke cognitive impairment

Objective To investigate whether an association existed between elevated serum β2-microglobulin levels and post stroke cognitive impairment(PSCI).Methods A total of 140 patients with ischemic stroke in Wenzhou People's Hospital during December 2022 to December 2023 were prospectively were enrolled including 73 patients in the PSCI group and 67 patients in the non-post stroke cognitive impairment(PSNCI)group.Serum β2-microglobulin and other biochemical indicators were measured within 24 hours after admission.PSCI was assessed using the mini-mental state examination(MMSE)score with a threshold of≥27 points serving as the diagnostic criterion.Results The age,National Institutes of Health Stroke Scale(NIHSS)score at admission,proportion of low education level,proportion of multiple or large area infarction and serum β2-microglobulin level in the PSCI group were higher than those in the PSNCI group.Multivariate Logistic regression analysis showed that age,NIHSS score at admission,multiple or large area infarction and serum β2-microglobulin level were risk factors for PSCI,and education level was a protective factor for PSCI.Conclusion The increase of β2-microglobulin level is a risk factor for cognitive impairment after ischemic stroke.The detection of β2-microglobulin level in stroke patients may realize the early detection of PSCI.
9.Clinical observation and nursing care of high dose hormone therapy in patients with uveitis

Objective To explore the nursing program of large dose hormone therapy for the treatment of uveitis patients , and to provide the reference for clinical nursing program .Methods A total of 196 patients with uveitis were randomly divided into observation group and control group with 98 cases 136 eyes each group by the random number table method .All patients were treated with high-dose hormone therapy .The control group received routine nursing care , while the observation group was given comprehensive nursing intervention measures.The clinical efficacy and recurrence rate of two groups were compared .Results After treatment, the total effective rate of observation group was 90.44%, and it was 89.71%in the control group (χ2 =0.041,P>0.05).Over 1 year of follow-up, the recurrence rate in the observation group was 5.15%, while it was 30.88%in the control group, and the recurrence rate of the observation group was significantly lower than that of the control group (χ2 =30.381,P<0.05).Conclusions During the process of high dose hormone therapy of uveitis patients , rigorous observation and comprehensive nursing interventions can help clinical treatment effect, reduce the recurrence rate of uveitis , and is worth of further application .
10.Correlation between the HPV type and clinical course of juvenile onset recurrent respiratory papillomatosis

OBJECTIVE To study on the influence of human papilloma virus(HPV) type on the clinical course of juvenile onset recurrent respiratory papillomatosis(JORRP).METHODS Fresh tumor specimens of 38 patients were collected and the HPV type of the specimens was detected by flow fluorescent hybridization method.According to the results,children were divided into HPV6 and HPVll positive groups.The clinical data of the two groups were compared.RESULTS Among 38 children with recurrent respiratory papillomatosis,21(55.2%) were infected with HPV6,14(36.8%) were infected with HPVll,and 3(7.9%) were negative for HPV 6 or HPV 11.The proportion of aggressive cases in HPV6 and HPV11 groups were similar.The age of onset,preoperative clinical symptom score,number of anatomic locations,anatomic Derkay/Coltrera score and Dikkers score were significant different between the two groups (P=0.002,0.040,0.023,0.001,0.005,respectively).CONCLUSION JORRP patients with HPV11 infection had the features of smaller onset age,more severe clinical symptoms and broader invasion compared with HPV6 infection.
