3.30 Children with Hand Foot and Mouth Disease following Acute Flaccid Paralysis

ObjectiveTo investigate the clinical characteristics and epidemic analysis in children with hand foot and mouth disease(HFMD) following acute flaccid paralysis(AFP).Methods30 children with HFMD following AFP in Hunan Province Children's Hospital were surveyed retrospectively.ResultsAll the patients were below 5 years old, 83.34% were 1~3 years old, 90% from countryside. Besides AFP of limbs, 86.67% complicated with encephalitis. The major clinical characteristics as followed: fever(100.00%), skin rash(100.00%), startle and skip(80.00%), emesis(63.33%), neurogenic pulmonary edema(13.33%) and urinary retention(16.67%). The most palsy occurred in 3~5 days after onset, and the palsy in lower limbs was major, only a few had palsy in upper extremities or tetraplegia. 26 cases were infected with enterovirus type 71(EV 71), 1 case infected other EV, 3 cases were negatively. The EMG showed that the amplitude of active potential lowered in the femoral nervus, tibial nerve, peroneal nerve, axillay nerve and musculocutaneous nerve, or/and the motor conduction velocity slowed down, the damage of proximate nerve was the most common.ConclusionThe most children with HFMD following AFP were infected with EV 71. Encephalitis was the common complication. The symptom of EV 71 was more severious than distribute EV.
4.Locking plate combined with anti-osteoporosis drugs for osteoporotic proximal humerus fractures in elderly patients

Objective Proximal humerus fracture is the most common upper-arm osteoporotic fracture in elderly patients and the result of treatment directly affects the physiological function of the shoulder .This article discusses the clinical effect of the locking proximal humeral plate ( LPHP) combined with anti-osteoporosis drugs in the treatment of osteoporotic comminuted proximal humerus fractures in elderly patients . Methods This study included of 47 elderly osteoporotic patients with comminuted proximal humerus fractures treated from April 2009 to March 2012 and with complete follow-up data.According to the Neer classification , the patients were divided into groups A (LPHP treatment, n=23, including 13 cases of three-part fractures and 10 cases of four-part fractures) and B (LPHP+anti-osteoporosis drugs, n=24, including 13 cases of three-part fractures and 11 cases of four-part fractures). Results All the patients were followed up for 14 to 38 (mean 23.6) months.The postoperative healing time was significantly shorter in group B than in group A ([96.57 ±2.59]d vs [115.91 ±2.73]d, P<0.05), and the excellence rate was remarkably higher in B than in A (91.7%vs 78.3%,P<0.05). Conclusion With the advantages of short healing time and high excellence rate , LPHP+anti-os-teoporosis drugs is superior to simple LPHP in the treatment of osteoporotic comminuted proximal humerus fractures in elderly patients.

Objective To summarize the appearances of MS lesions on DWI, and to quantitatively study the ADC and AI values in different clinical MS subgroups Methods One hundred and eighty seven lesions were studied in 18 cases of MS at different clinical stages Routine MR and DWI were performed in all cases ADC and AI were calculated in the lesions,NAWM, and normal control group Results Hyperintensity (edema like on T 2WI) was demonstrated on DWI at acute primary progressive MS lesions Hyperintense ring sign was found at acute relapse MS lesions ADC increased in all subgroups of MS Significant difference of ADC was demonstrated among MS lesions, NAWM, and normal control groups Significant decrease of AI was demonstrated at later stages Hyperintensity was found in all MS lesions on T 2WI The hyperintensity lesions on DWI had enhanced or non enhanced appearances on CE T 1WI The ADC value of enhanced MS lesions was less than that of the non enhanced lesions ( t =4 19, P 0 05) Conclusion DWI could give quantitative information for different pathological changes in MS lesions of different clinical subgroups Quantitative measurement of diffusion has great potential value in the diagnosis, differential diagnosis, and the prediction of prognosis in MS
6.Effect of Early Rehabilitation at Bedside on Motor Function in Children with Acute Flaccid Paralysis

ObjectiveTo investigate the effect of early rehabilitation at bedside motor function in children with acute flaccid paralysis (AFP). Methods45 patients were divided into two groups. The treatment group (n=23) received rehabilitation when the vital signs were stable and nervous symptom did not advance. The control group (n=22) received rehabilitation after acute stage. ResultsThe muscular strength of treatment group recovered more significantly than the control group 2 weeks and 4 weeks after treatment (P<0.05).ConclusionEarly rehabilitation at bedside can facilitate the motor function recovery in AFP.
7.Human Chorionicgonadotropin Measurement Method Based on Magnetic Nanoparticle Probes

The magnetic nanoparticle probe was prepared by specifically connecting the streptavidin-conjugated magnetic nanoparticles and the antibody of analyte via the strong streptavidin-biotin interaction. Based on the magnetic nanoparticle probes, the concentration of human chorionicgonadotropin (HCG) was detected and a new CL method for of hormone was further established. The performances of the magnetic nanoparticle probes were characterized by UV-Vis spectrometry, transmission electron microscopy and dynamic light scattering. The experimental conditions that affected the chemiluminescence were optimized. The optimal concentrations of luminal and H_2O_2 were 2×10~(-4) mol/L and 8×10~(-4) mol/L, respectively, and optimal pH was 13. Under the optimized experiment conditions, a linear response of chemiluminescence intensity to HCG concentration was obtained with a correlation coefficients of 0.9924. The linear range was from 0.5 to 250 μg/L and wider than the conventional ELISA method (5-200 μg/L). The relative standard deviation was 3.8%. Correlation analysis showed that there was significant correlation between the method of magnetic nanoparticle probes and ELISA in 34 clinical samples. The proposed method with characters of sensitive, effective, fast response and wide detection range provided good application prospect in analysis of other ultra-micro protein.
8.Detection of Human Chorionic Gonadotropin by Highly Sensitive Magnetic EnzymE-linked Chemiluminescent Immunoassay

A highly sensitive magnetic enzymE-linked chemiluminescent immunoassay method was developed for the detection of human chorionic gonadotropin(HCG). The monoclonal antibody was covalently coupled on the surface of carboxylated magnetic beads to generate magnetic-biotargeting; Alkaline phosphatase(ALP) was utilized as a labeled reagent of another monoclonal antibody, whereas 3-(2-spimadamantane) 4-methoxy-4-(3-phosphoryloxy)phenyl-1,2-dioxetane(AMPPD) was utilized as the chemiluminescent substrate. Based on this concept, a highly sensitive chemiluminescent immunoassay method was established to test HCG. Then, several modifications were made to optimize the method, and the detection sensitivity and procedure were improved accordingly. The detection of the assay could be fulfilled within 60 min and the test result of HCG concentration was linear over the range of 0.15 150 IU/L with good relativity(r=0.960). The relative standard deviation(RSD) were below 5% and the sensitivity of this method was 0.15 IU/L. The proposed method with wide linear range, simple operation and fast detection showed good prospect in practical application on-site.
9.Comparative analysis of the recurrent mutations between Uigur and Han ethnic deaf group in Xinjiang region of China

Objective:To investigate the recurrent mutations between Uigur and Han ethnic deaf group in Xinjiang region and determine the relationship between ethnicity and mutations.Method:DNA were extracted from peripheral blood of 125 deaf patients from Urumqi and Korla special educational schools in Xinjiang.Audiologic examinations showed that all patients had severe to profound bilateral sensorineural hearing hoss. The coding region of GJB2 gene, SLC26A4 and mitochondrial DNA target fragments were amplified by polymerase chain reaction(PCR).Mutations in GJB2 gene, SLC26A4IVS7-2 A>G, mtDNA 1494C>T and mtDNA1555 A>G were identified by sequencing analysis.Result:Allelic Frequency of the GJB2 35delG and SLC26A4IVS7-2 A>G mutations in Han deaf students were 7.4%and 10.1%,respectively, whereas not found in Uigur deaf groups.The difference was statistically significant. We did not find significant differences in GJB2 235 delC, 299-300delAT, mtDNA A1555G and C1494T allelic frequency between Uigur and Han students.Conclusion:Prevalence of the recurrent mutations between Uigur and Han ethnic deaf group in Xinjiang has a great diversity.

To investigate the relationship between plasma levels of TXB2, 6-KPGF1?, cAMP and cGMP and the hemodynamics of hypoxemia, 30 patients with chronic cor pulmonale (CCP) were studied. The influence of hypoxemia and isosorbide dinitrate therapy was also observed. The results showed: 1) Plasma TXB2 level was significantly higher and plasma 6- KPGF1? level was significantly lower in CCP patients than in healthy controls. There was a negative correlation between 6 KPGFl? and-Ppa levels and a positive correlation between TXB2, TXB2/ 6 KPGF1? ratio and Ppa levels. 2) High levels of plasma TXB2 and TXB2 / 6-KPGF1? were found in hypoxemia cases when the PaO2 level was less than 6.67 kPa (50 mmHg). 3) Reduced Ppa after isosorbide dinitrate infusion elevated the plasma levels of 6-KPGF1?, cAMP, and the cAMP/cGMP ratio, and reduced those of TXB2 and the TXB2/6-K.PGF1? ratio.
