1.Hepatectomy for hepatolithiasis: an analysis of 98 eases

Objective To investigate the effect and long-term prognosis of patients who underwent hepatectomy for hepatolithiasis. Methods We retrospectively analyzed the medical records of 98 patients with hepatolithiasis who were treated by hepatectomy in Peking Union Medical College Hospital.Results Male/femah:1/1.7;median age:55 years old.58 cases(59.2%)had been treated before;among them.50 by surgery.In 88 cages(89.8%)hepatolithiasis involved the left lobe only,in 2(2.0%) only the right lobe involved.and in 8(8.2%)both left and right lobe were involved.51(52.0%)had extrahepatic biliary stones,30(30.6%)had biliary duct strictures,28(28.6%)had a history of biliary ascariasis.and 5(5.1%)had a concurrent biliary tract malignancy.All received partial hepatectomy according to the stone location;for the 8 bilaterally involved patients,left hepatectomy and right lithotomy were performed.Postoperative complications occurredin 14 cases(14.3%),and there were2perioperative deaths(2.0%).Seventy-eight patients(79.6%)have beenfollowedupfor over1 yearwith no tumors;the results were excellent or good in 91.0%;the stone residue and recurrence rate were both 2.0%.Conclusions Hepatectomy not only eliminates calcuci,but also removes diseased biliary tracts,with advantages of low residue stone and recurrence rate.
2.Revision and reliability and validity testing of the Arteriovenous Fistula Assessment Scale

Objective:To translate and revise the Arteriovenous Fistula Assessment Scale (AVF-AS), test the reliability and validity of the Chinese version of AVF-AS.Methods:The modified Brislin translation model was used to translate, back translate and cross culture adjust AVF-AS, forming the Chinese version of AVF-AS. Using the convenient sampling method, 220 hemodialysis patients from the First Affiliated Hospital of Zhengzhou University were selected for investigation from July to September 2022. Two weeks later, 30 patients were randomly selected for retesting. The valid data were used for project analysis and reliability and validity evaluation.Results:The Chinese version of AVF-AS consisted of 3 factors and 18 items. Consistency level between evaluators was 0.94, the item level content validity index was 0.83-1.00, average scale level content validity index was 0.94, and the calibration validity was 0.68.Three common factors(autogenous arteriovenous fistula blood flow, stenosis and ischemia, puncture location) were extracted from exploratory factors, and the cumulative variance contribution rate was 75.255%. The total scale's Cronbach α was 0.946, the half reliability of each dimension was 0.826 - 0.898, and the test-retest reliability was 0.907.Conclusions:The Chinese version of AVF-AS has good reliability and validity, and can be used as an effective tool to evaluate the autogenous arteriovenous fistula functional status of hemodialysis patients in China.
3.Efficacy and safety of baricitinib in the treatment of moderate-to-severe atopic dermatitis: a clinical observation

Objective:To evaluate the efficacy and safety of baricitinib in the treatment of moderate-to-severe atopic dermatitis (AD) .Methods:From June 2020 to June 2021, patients with moderate-to-severe AD who were insensitive or intolerant to topical agents were enrolled from Department of Dermatology, Peking University First Hospital. Before treatment, the patients were evaluated by 4 scales, including the Investigator′s Global Assessment (IGA), Eczema Area and Severity Index (EASI), Itch Numeric Rating Scale (NRS), and Dermatology Life Quality Index (DLQI) ; meanwhile, photos of skin lesions were taken, routine blood test was performed, blood biochemical indices and total IgE levels were measured. After exclusion of contraindications, the patients were treated with oral baricitinib at a dose of 2 mg/d for 16 weeks. Regular follow-up was conducted at weeks 1, 2, 4, 8, 12, 16 and 20 after the start of treatment, clinical evaluation was carried out with the above 4 scales, and adverse events were recorded during the treatment.Results:A total of 24 patients were enrolled in the study, and all completed 16-week oral treatment and 20-week follow-up. All the 4 scale scores showed a continuous downward trend within 20 weeks after the start of treatment. At week 20, the patients′ IGA, EASI, NRS, and DLQI scores significantly decreased from 4.13 ± 0.61, 37.59 ± 14.86, 6.83 ± 2.26 and 18.67 ± 8.64 points respectively at baseline to 1.12 ± 0.49, 4.53 ± 3.78, 0.72 ± 0.58 and 1.39 ± 0.85 points respectively ( t = 22.70, 10.55, 10.69, 8.40, respectively, all P < 0.001). During the follow-up period, no serious adverse reactions were observed; 3 patients experienced gastric discomfort at the start of oral treatment, but the symptoms disappeared after the treatment continued; 3 developed acute allergic manifestations (1 case of allergic conjunctivitis, 2 cases of acute urticaria), which resolved rapidly after the use of antihistamines without recurrence. Conclusion:Baricitinib can provide a safer and more effective treatment option for patients with moderate-to-severe AD, especially those who are insensitive or intolerant to topical agents and need systemic treatments.
4.Effect of Xiaoyao Wan on CYP2E1 and FasL / TNF-α signal pathway in metabolic associated steatohepatitis

Objective :
To study the mechanism of the treatment of metabolism associated steatohepatitis ( MASH) in rats with Xiaoyao Wan.
Methods :
Twenty-four SD male rats were randomly divided into control group ( CON group,n = 8) and model group ( n = 8 ) .The model group was given high-fat diet ,carbon tetrachloride ( CCl4 ) subcutaneous injection,hunger and satiety disorder and tail clamping for four weeks to establish the MASH model, and the rats were randomly divided into the MOD group and the Xiaoyao Wan group (XYW group) ,with 8 rats in each group.The rats of the XYW group were given Xiaoyao Wan,the other two groups were given normal saline. Four weeks after administration ,the serum biochemical indexes and oxidative stress indexes of rats in different groups were detected.The pathological sections of rat liver tissue were observed by hematoxylin-eosin ( HE) stai- ning and oil red O staining.The expression of cytochrome P450 2E1 ( CYP2E1) ,factor-related apoptosis ligand (FasL) ,tumor necrosis factor-α ( TNF-α) and tumor growth factor-β1 ( TGF-β1 ) in rat liver were detected by Western blot method.The relative contents of CYP2E1,FasL,TNF-α and TGF-β1 mRNA in liver tissue were de- tected by RT-qPCR.
Results :
The general condition of rats in the XYW group was significantly improved compared with the MOD group ; the level of hepatic index was significant decrease compared with the MOD group (P<0. 01) and the level of body mass index was significant increase compared with the MOD group (P <0. 01) ; the serum levels of triglyceride ,total cholesterol ,low -density lipoprotein cholesterol ,aspartate aminotransferase ,alanine aminotransferase,malondialdehyde,monocyte chemotactic protein-1,TNF-α , and interleukin (IL) -18 were signif- icantly reduced compared with the MOD group (all P<0. 05) ,and the levels of high-density lipoprotein choles- terol,superoxide dismutase,and IL-10 were significantly elevated compared with the MOD group (all P<0. 05) ; the content of lipid droplets in hepatocytes of rats was significantly reduced compared with the MOD group under light microscope ; the protein levels of CYP2E1,FasL,TNF-α and TGF-β1 in liver tissues were significantly re- duced compared with the MOD group ( all P <0. 05 ) ,and the relative contents of CYP2E1 ,FasL,TNF-α and TGF-β1 mRNA were significantly reduced compared with the MOD group ( all P <0. 05) .
Conclusion
Xiaoyao Wan can regulate the expression of CYP2E1,FasL,TNF-α and TGF-β1 in the liver tissue of MASH rats by reduc- ing the accumulation of fat in the liver,so as to achieve the purpose of treating MASH.
5.The key role of CYC2 during meiosis in Tetrahymena thermophila.

Meiotic recombination is carried out through a specialized pathway for the formation and repair of DNA double-strand breaks (DSBs) made by the Spo11 protein. The present study shed light on the functional role of cyclin, CYC2, in Tetrahymena thermophila which has transcriptionally high expression level during meiosis process. Knocking out the CYC2 gene results in arrest of meiotic conjugation process at 2.5-3.5 h after conjugation initiation, before the meiosis division starts, and in company with the absence of DSBs. To investigate the underlying mechanism of this phenomenon, a complete transcriptome profile was performed between wild-type strain and CYC2 knock-out strain. Functional analysis of RNA-Seq results identifies related differentially expressed genes (DEGs) including SPO11 and these DEGs are enriched in DNA repair/mismatch repair (MMR) terms in homologous recombination (HR), which indicates that CYC2 could play a crucial role in meiosis by regulating SPO11 and participating in HR.
