1.Application of acoustic elastography, conventional ultrasonography and mammography in the diagnosis of the breast cancer

ObjectiveTo explore the value of the diagnosis of breast cancer by acoustic elastography, conventional ultrasonography (including gray-scale ultrasonography, color Doppler flow imaging and Doppler spectrum) and mammography.MethodsOne hundred and four patients with breast disease conifrmed by surgical pathology were studied retrospectively in Zibo Maternal and Children Health Hospital. All the patients were examined by acoustic elastography, conventional ultrasonography and mammography before surgery.ResultsPathological results of 104 cases were: sixty-five malignanies and 39 benignities. The most common signs in the diagnosis of breast cancer by conventional ultrasonography were spicule sign (76.9%, 50/65), taller-than-wide sign (73.8%, 48/65), vascular resistance index>0.7 (70.8%, 46/65), calciifcation (53.8%, 35/65). And breast masses without these signs were regarded as benign lesions. The diagnostic criteria of breast cancer by acoustic elastography was 5-point method. We regarded the lesions got more than 3 points as malignancy. The most common signs in the diagnosis of breast cancer by mammography were dense shadow (73.8%, 48/65), spicule sign (70.8%, 46/65), calcification (69.2%, 45/65). And breast masses without these signs were regarded as benign lesions. The diagnostic accuracy of acoustic elastography, conventional ultrasonography, mammography, conventional ultrasound plus acoustic elastography, conventional ultrasound plus mammography and acoustic elastography plus mammography for breast tumor were 88.4% (92/104), 86.5% (90/104), 83.7% (87/104), 93.3% (97/104), 90.4% (94/104) and 89.4% (93/104), respectively. ConclusionsThe combination of any two imaging modalities can improve the diagnostic accuracy of breast cancer. And conventional ultrasound combined with acoustic elastography could be an effective method in the diagnosis of breast cancer.
2.Mycoplasma pneumoniae pneumonia with secondary thrombocytosis in infants and young children

Objectives To analyze the clinical characteristics and signiflcance of Mycoplasma pneumoniae pneumonia (MPP) combined with secondary thrombocytosis in infants and young children. Methods Clinical features, laboratory and imaging data of the infants and young children with MPP were collected, and compared between the two groups of children with and without secondary thrombocytosis. Results Sixty-seven (67) infants and young children with secondary thrombocytosis (28 males and 39 females, with onset at 2 years and 2 months after birth in average) and 269 infants and young children with normal platelet counts (138 males and 131 females, at 2 years and 3 months after birth in average) were included. Signiflcant longer durations of hospitalization(P =0.018) and fever(P =0.000), higher temperature peak(P =0.000), as well as higher morbidity of refractory MPP(P =0.001) and more complications(P =0.000)were observed in the group of MPP with secondary thrombocytosis. Moreover, the laboratory data of white blood cell count(P =0.000), C-reactive protein(P =0.000),procalcitonin (P =0.000), erythrocyte sedimentation rate(P =0.000)and higher morbidity of multiple pathogen infection(P =0.033) were observed between the two groups.Conclusions More severe clinical manifestations and higher complication morbidity could be observed in the group of MPP combined with secondary thrombocytosis, implicating the value of platelet count in clinical assessment of MPP in infants and young children at early stage.
3.Correlation Between Serum Uric Acid,Serum Lipids and Colorectal Polyps

Background:Colorectal polyps(CRP)has nonspecific clinical manifestation,and is closely related to the pathogenesis of colorectal cancer. Studies have shown that abnormalities of serum uric acid(UA),serum lipids are closely related to pathogenesis of CRP,but is still in controversial. Aims:To investigate the correlation between serum UA,TG, TC,LDL-C,HDL-C and incidence of CRP. Methods:A total of 142 CRP patients from Jan. 2016 to Oct. 2016 at Qingdao Municipal Hospital were enrolled,and 116 patients without CRP were served as controls. The serum levels of UA, TG,TC,LDL-C,HDL-C were determined,and correlation between UA and serum lipids was analyzed. Results:Compared with controls,serum levels of TG,UA were significantly increased in CRP patients(P<0.05),HDL-C level was significantly decreased(P<0.05),however,no significant differences in TC,LDL-C levels were found between the two groups. Serum UA level was positively correlated to TG,TC,LDL-C levels in patients with CRP(r=0.32,r=0.21, r=0.15;P<0.05),and negatively correlated to HDL-C level(r= -0.07,P<0.05). Conclusions:Increased serum TG and UA levels and decreased serum HDL-C level may be correlated to the pathogenesis of CRP. The interaction between serum UA,TG,TC,LDL-C and HDL-C may increase the risk of CRP.
4.Clinical study on treatment of poststroke depression by acupuncture combined with conventional therapy of western medicine

Objective To evaluate the curative effect of acupuncture combined with conventional therapy for the treatment of post-stroke depression (PSD). Methods Sixty patients were divided into two groups according to the computer. Acupuncture group was treated with acupuncture combined with conventional therapy of western medicine. Non-acupuncture group was treated with sham acupuncture combined with conventional therapy. Both groups were treated for 4 weeks. The patients was evaluated using the Hamilton depression rating for scale (HAMD). Adverse reactions during treatment were assessed by the Treatment emergent Symptom Scale (TESS). Results The HAMD score at the first week (27.1 ± 6.1 vs. 30.8 ±5.0,t=-5.532),the second week(26.6 ±6.3 vs.15.8 ±7.3,t=2.744),the 4th week(15.8 ±7.3 vs.20.7 ± 5.2,t=3.570),and the 8th week(9.9 ±6.2 vs.15.6 ±5.7,t=6.154)in the acupuncture group were lower than those in the non-acupuncture group (P<0.01). During the treatment, 2 cases of insommia occurred in acupuncture group, 8 cases of insommia occurred in nonacupuncture group, 2 cases of nausea and vomiting occurred in acupuncture group, 9 cases of nausea and vomiting occurred in nonacupuncture group(χ2=10.550, P=0.001).Conclusions Acupuncture combined with conventional therapy can improve the depressive state of PSD patients, reduce the adverse reactions and improve the quality of life of patients.
5.The correlation between the polymorphism of IL-10 promoter and the susceptibility of syphilis serum fixation

Objective:To explore the association of -592A/C and -1082A/G single nucleotide polymorphism in interleukin (IL)-10 gene with susceptibility to serofast in patients with syphilis.Methods:The SNPs of -592A/C and -1082A/g in the promoter region of IL-10 were detected by multiple single base extension (SNaP-shot) assay in 123 patients with syphilis(syphilis group), 118 patients with seronegative syphilis (seronegative syphilis group) and 120 healthy controls (healthy control group). The clinical characteristics, genotypes and allele frequencies of different subjects were compared.Results:There was no significant difference in age and gender between syphilis group, seronegative syphilis group and healthy control group ( P>0.05). There was no significant difference in the number of sexual partners, initial rapid plasma reagin test for syphilis (RPR) titer, stage, and Jihai reaction between the syphilis group and seronegative syphilis group ( P>0.05). There was no significant difference in the genotype and allele frequency of -592A/C and -1082A/G in the promoter region of IL-10 between the syphilis group, seronegative syphilis group and the control group ( P>0.05). Conclusions:There seems to be no evidence for association between -592A/C and -1082A/G single nucleotide polymorphism in IL-10 gene and susceptibility to serofast in patients with syphilis.
6.Microbiological surveillance result of endoscopes after INTERCEPT Foam Spray: a quasi-experimental pilot study in Singapore

This study aimed to assess the impact of INTERCEPT Foam Spray (IFS) application on delayed endoscope reprocessing through microbiological surveillance culture (MSC).
Methods:
A quasi-experimental, matched-comparison pilot study was conducted using gastrointestinal endoscopy. IFS was applied to the endoscopes after precleaning and before reprocessing the next day. An equal number of endoscopes, matched by endoscope type, were subjected to routine reprocessing. The MSC were subjected to high-level disinfection to detect any contamination. Data were analyzed using the chi-square test or Fisher exact test (categorical data) and Student t-test (continuous data).
Results:
In total, 150 MSCs were collected from 42 endoscopes. Positive MSCs were observed in 4.0% (4/75) of the sprayed group and 1.3% (1/75) of the control group (95% confidence interval, 30.34–0.31; p>0.05), all of which were contributed by colonoscopes. Colonoscope were more prone to positive MSC (mean difference in percentage, p<0.05). Mean spraying hours were not associated with detected growth (11.7% vs. 13.6%; 95% confidence interval, 1.43 to –5.27; p>0.05), with environmental and skin flora being the primary contaminants.
Conclusions
IFS may be applied when delayed endoscope processing is necessary, but with caution when applied to colonoscopes. However, further research is warranted to verify the result.

9.Psychological resilience characteristics of visually impaired adolescents: a qualitative study

ObjectiveTo analyze the psychological resilience characteristics of visually impaired adolescents. MethodsFrom February to May, 2023, 13 visually impaired adolescents (average age of 16.5 years) from Nanjing Normal University of Special Education, Yangzhou Special Education School and Nanjing School for the Blind in Jiangsu, China were selected for semi-structured interviews with phenomenological method, and the interview data were sorted by theme analysis method. ResultsFour themes and twelve sub-themes were obtained; namely multiple sources of difficulties, including travel difficulties, learning difficulties, social difficulties and difficulties in daily life; need for external support, including insufficient family support, school support needs to be improved, need for peer support and insufficient social support; negative and positive adaptation coexist, including negative adaptation and positive adaptation; and negative coping and positive coping coexist, including positive coping and negative coping. ConclusionThe psychological resilience of visually impaired adolescents is characterized by multiple sources of difficulties, insufficient external support, coexistence of positive and negative adaptation, and coexistence of positive and negative coping. A joint family-school-society support system should be formed to reduce the source of their difficulties, and help them establish positive adaptation and positive coping methods, to enhance their psychological resilience.
10.Soft tissue changes in midfacial healthy and affected sides of unilateral cleft lip and palate patients after orthognathic surgery

Objective:To investigate the alterations in soft tissue morphology and thickness in the mid-face region of patients with cleft lip and palate (UCLP) secondary to maxillofacial deformity following Le Fort I osteotomy.Methods:A total of 22 patients (16 males and 6 females aged from 17 to 28 years with an average of 20 years) diagnosed with cleft lip and palate secondary to maxillofacial deformity were collected from the Wuhan University Hospital of Stomatology from July 2012 to August 2020. All patients underwent Le Fort I osteotomy. CBCT scans were obtained at T0 (3 days before surgery), T1 (7 days after surgery), and T2 (1 year after surgery). The Dolphin11.95 software and 3D Slicer software were utilized to measure and analyze the soft tissue near the mid-face osteotomy line. Differences in soft tissue thickness before and after surgery were compared.Results:Before and after the operation, the soft tissue thickness at P3, P5, P6, and P8 on the affected side was thicker than that on the healthy side, and the difference was statistically significant, with a P-value of <0.05. At P5, P6, P7, P8, and P9 below the osteotomy line at T2-T0, the degree of postoperative thinning on the affected side was more apparent than that on the healthy side, and there was statistical significance at P6 ( P<0.05). The postoperative soft tissue asymmetry in the Ck region was improved compared with the preoperative one. The preoperative average protruding of the affected side was 0.63 compared with the healthy side, and the postoperative value was 0.17. The preoperative and postoperative Mann-Whitney U tests showed significantly statistical difference. Conclusions:After Le Fort I osteotomy, the facial asymmetry of patients with unilateral cleft lip and palate secondary to maxillofacial deformity is improved. However, there is still a difference in the soft tissue thickness between the healthy side and the affected side, and the change in soft tissue thickness on the affected side is more significant than that on the healthy side.
