2.Evaluation of the changes in microcirculation of the pulmonary cancers before and after interventional therapy by DSA

Objective According to the changes of microcirculation in the pulmonary cancers, the treatment effect was evaluated after the vascular interventional therapy.Methods Angiography of 138 primary pulmonary carcinomas, and the feeding arteries were performed. Areas of mass blush were measured for 81 cases before and after therapy. The tumour blush was considered to be the imaging appearance of the microcirculation of the lung carcinoma. The angiographic images were collected by digital image system (DSA and movie). Results (1) The rate of the tumour blush appearance was 88.8% in this group. 3.Lung transplantation from donor of cardiac death in China (report of 3 cases)

Objective To summarize the clinical experience of harvesting and using the lungs of cardiac death donor.Methods The lungs from donation after cardiac death (DCD) were harvested and used for lung transplantation.The donors suffered from severe craniocerebral trauma or brain neoplasms and were identified after cardiac death post declaration of brain death.Written consent about DCD was obtained from the consanguinities.The donor lungs were harvested after clinical evaluation of donors with considerable function and after the determination of DCD.The preoperative lymphocytotoxic cross match test was negative,ABO blood type was compatible,and the donors were all suitable for the transplant procedure.Results Two bilateral lung transplantations and one single lung transplantation were performed,with the warm ischemic time being 23,27,and 32 min,respectively.The operative course was uneventful The ICU stay was 31,18,and 26 days respectively,with dramatic improvement of pulmonary function postoperatively.Acute rejection occurred in two cases,which was treated with bolus of corticoids.There were no infection in our 3 patients,and the life quality was satisfactory during the follow-up period.Conclusion The lung from DCD may be one of the available resources used for lung transplantation on the basis of efficient management of the potential donors and clear evaluation of the donors.

5.Advances in the study of new BCR-ABL kinase inhibitors

The oncogenic product of BCR-ABL is an abnormal tyrosine kinase that causes chronic myeloid leukemia (CML). With further research into the pathogenesis of CML, the discovery of compounds that selectively inhibit abnormal BCR-ABL tyrosine kinases is a research focus worthy of attention. The first three generations of BCR-ABL inhibitors are orthosteric inhibitors, which competitively block the binding of ABL protein tyrosine kinase to ATP and prevent it from activating downstream signals. The fourth-generation BCR-ABL inhibitors allosterically inhibit ABL protein tyrosine kinase by binding to the myristoyl pocket, providing greater selectivity and maintaining activity against drug-resistant mutations proteins. Novel drug design strategies such as proteolytic targeting chimera (PROTAC), covalent inhibitors and dual targeting inhibitors also provide new directions for the development of BCR-ABL kinase inhibitors. This paper reviews recent research advances on BCR-ABL kinase inhibitors and discusses drug design strategies for various novel BCR-ABL inhibitors.
6.Identification of aminoalcohol-diterpenoid alkaloids in Aconiti Lateralis Radix Praeparata and study of their cardiac effects.

In order to affirm the cardioactive components in Fuzi, we identified a group of aminoalcohol- diterpenoid alkaloids in Fuzi using ultra high-performance liquid chromatography coupled with electrospray ionization mass spectrometer (UPLC-ESI-MS) method. Among a total of forty-one isolated ingredients, thirteen major aminoalcohol-diterpenoid alkaloids were identified by comparing their retention times and MS spectra with those of the reference substances. Moreover, Fuzi samples from different places of origin and with different processing methods were examined and their components displayed a pattern of high similarity, though the relative abundance varies probably due to their different processing methods. Furthermore, the cardiac effect of each identified alkaloid was individually evaluated using the isolated bullfrog heart perfusion experiment. Among the thirteen aminoalcohol diterpenoid alkaloids tested, six of them significantly enhanced the amplitude rates. Taken together, we affirm that the cardioactive components in Fuzi are aminoalcohol-diterpenoid alkaloids, shedding light on future studies of the mechanisms and development of these cardioactive compounds.

8.Immunohistochemical study using T-cell lymphoma antibody 1 and CD44 in diagnosis of Burkitt's lymphoma.

OBJECTIVETo study the values of immunohistochemistry using T-cell lymphoma antibody (TCL) 1 and CD44 in the diagnosis of Burkitt's lymphoma.
METHODSImmunohistochemical study for TCL1, CD44, CD10, bcl-2, bcl-6, c-myc and Ki-67 was performed on paraffin-embedded sections of lymphoma cases, including 25 cases of Burkitt's lymphoma and 25 cases of diffuse large B-cell lymphoma.
RESULTSBurkitt's lymphoma commonly expressed TCL1 (96%, 24 cases), CD10 (88%, 22 cases), bcl-6 and c-myc (92%, 23 cases). Only 1 case (4%) expressed CD44 and bcl-2. The Ki-67 proliferation index ranged from 95% to 100%. On the other hand, diffuse large B-cell lymphoma expressed CD44 (84%, 21 cases), CD10 (32%, 8 cases), bcl-6 (72%, 18 cases) and bcl-2 (72%, 18 cases). Four cases (16%) were weakly positive for TCL1. The staining for c-myc was all negative. The Ki-67 proliferation index ranged from 40% to 90%.
CONCLUSIONImmunohistochemical staining for TCL1 and CD44 is a useful ancillary tool in the pathologic diagnosis of Burkitt's lymphoma which is also helpful for the differential diagnosis from diffuse large B-cell lymphoma.

9.Prevention of bronchopulmonary complications by ambroxol after thoracotomy in patients with chronic obstructive pulmonary disease

Objective To evaluate the effects of ambroxol on prevention of bronchopulmonary complications after thoracotomy in patients with chronic obstructive pulmonary disease(COPD).MethodsTotal 161 elderly patients aged ≥70 years with COPD undergoing thoracotomy were randomly allocated to two groups:ambroxol group (300 mg/d,6 d) and placebo group as control.Pulmonary complications were evaluated by clinical symptoms,radiographic changes,and blood gas analysis. Results The incidence rates of atelectasis were 8.6% in ambroxol group and 28.8% in placebo group,respectively.The PaO2 values after surgery in ambroxol group decreased more than in placebo group (P<0.05) compared with the preoperative values.Side effects were not found in all patients.Conclusions Ambroxol should be considered as an alternative pharmacologic approach for the prevention of post-thoracotomy pulmonary complications in the elderly patients with COPD.
10.Correlation of serum eosinophil cationic protein with the severity of allergic rhinitis in childhood.

OBJECTIVETo investigate the relationships between the severity of childhood allergic rhinitis (AR) and the peripheral blood eosinophil count, serum eosinophil cationic protein (ECP), total IgE (tIgE), and allergen-specific IgE (sIgE) levels.
METHODSA total of 138 children with AR aged 3 to 17 (9.96 ± 3.78, x() ± s) years old were enrolled in the study. All children had persistent AR sensitized to house dust mites with a clinical history of 3 months to 12 (4.21 ± 2.72) years. The disease severity was evaluated using 10 cm-visual analogue scale (VAS), and the serum levels of ECP, tIgE and sIgE were determined using an ImmunoCAP system. Statistical analysis was conducted with SPSS11.0 software.
RESULTSAmong 138 children with AR, the VAS scores for global severity of rhinitis and nasal obstruction symptom were 5.32 ± 2.16 and 4.78 ± 2.45, respectively. Blood eosinophil count was 0.39 [0.24; 0.63] (M[P(25); P(75)]) ×10(9)/ml. Serum levels of ECP and total IgE were 10.60 [3.26; 30.80] µg/L and (2.50 ± 0.53) log kU/L, respectively. Serum levels of allergen-sIgE against Dermatophagoides pteronyssinus and Dermatophagoides farinae were 58.20[24.75; > 100] kUA/L and 54.95 [24.55; > 100] kUA/L, respectively. The VAS scores of nasal obstruction symptom, but not global severity of rhinitis, were positively related to the duration of AR (r = 0.215, P = 0.011) and the levels of serum ECP (r = 0.196, P = 0.022) in bivariate correlation analysis. There was also a significant correlation between the serum ECP level and the blood eosinophil count (r = 0.295, P = 0.000). No relationships of blood eosinophil count, and serum tIgE and sIgE levels with global severity of rhinitis as well as nasal obstruction symptom were found (all P > 0.05).
CONCLUSIONSThese results suggested that the severity of nasal obstruction was positively correlated with the duration of rhinitis and the levels of serum ECP in childhood persistent AR due to house dust mites, indicating the disease severity might be related to chronic inflammatory process.
