Objective To design an arthroscopic universal aiming device for cruciate ligament repairing and evaluate its clinical efficacy. Methods The aiming device was designed on the principle of “a line crossing any point on a spherical surface and perpendicular to it must cross the sphere center”, which enables the surgeon to locate the ligamentous attachment of cruciate ligament precisely for reconstruction. Twelve cases of PCL injuries have been treated with this method. Results The average follow up period was 12.1 months (5-19 months). According to Lysholm scoring system, the average score was improved from 59.2 points to 92.7 points, increased by 33.5 points. Lachman test turned negative in all patients, the average range of motion was 120?. Conclusion The instrument was reasonably designed for easy handling and precise orientation. It is particularly preferable to locate the tibial attachment of cruciate ligament for endoscopic reconstruction on the posteromedial approach.

Zhejiang province takes an innovative approach in its rural health service strategy featuring routine health examination, which proved an initial success.Intending to assess the effect and dilemma of such a system, and seek sustainable development in future,we made investigations in Pinhu and Xianju in Zhejiang province.Overall, rural health service strategy based on rural health routine examination,to some extent, improved patient-physician relationship, changed and strengthened their rural health concepts, and upgraded non-communicable disease control. Furthermore, the practice has economically and socially influenced rural health service institutions and rural physicians in terms of their concepts and methodology. There are also setbacks and problems in need of govemment support, programme improvement and resource optimization for further sustainability.

ObjectiveTo compare the effect of the neurosurgical clipping and endovascular for treating intracranial aneurysm.MethodsRetrospectively analyzed the clinical data of 110 cases with intracranial aneurysms. The patients were divided into intervention group(55 patients with endovascular treatment) and surgery group(55 patients with neurosurgical clipping treatment) according to different methods, followed forup 1 ~4 months, and immediately evaluated the efficacy. The Chinese stroke scale(CSS) and activities of daily living(ADL) score were used for prognostic evaluation.ResultsThe immediate effect:the marked efficacy in 40 cases, efficacy in 10 cases and inefficacy in 5 cases, and the total effective rate in the intervention group was 90.9%,while the surgery group were 32 cases,10 cases,13 cases, and the total effective rate was 76.4%.There was significantl differences between the two groups ( x2 =3.96,P < 0.05 ) ; the CSS and ADL scores significantly lower than pre-treatment ( t =9.12,8.05,P <0.05).ConclusionEndovascular treatment for intracranial aneurysms could improve the clinical efficacy and improve patients self-care ability, and significantly improve the prognosis.

Objective We investigate lipid peroxidation of compressed myeloid tissue at early stage after decompression of chronic compressive spinal cord injury.Method SOD and MDA of compressed myeloid tissue are measured respectively before compression, before decompression and 3h after decompression.Result Increased MDA while decreased SOD of compressed myeloid tissue at 3h after decompression than before decompression. Conclusion The increased lipid peroxidation of compressed myeloid tissue at early stage after decompression of chronic compressive spinal cord injury. It is possible that it was resulted from ischemical reperfusion injury.

Objective By observing the effects of"three methods and three points"tuina technique on the expression of interleukin-17F(IL-17F),interleukin-17 receptor C(IL-17RC),activator 1 of nuclear transcription factor-κB(Act1),tumour necrosis factor receptor-associated factor 6(TRAF6)and M1 microglial cell expression in the spinal dorsal horn of rats with mild chronic compressive injury(minor CCI)model,we explored the immediate analgesic mechanism of tuina on peripheral neuropathic pain(pNP).Methods Thirty-six SD rats were divided into the sham group, the model group and the tuina group according to the random number method, twelve rats in each group, and the minor CCI model was replicated by ligating the right sciatic nerve. The rats in the tuina group were subjected to pointing, plucking and kneading at the BL37,BL57 and GB34 points on the affected side using a tuina simulator, while the sham group and the model group were only grasped and restrained, and were intervened for one time. The mechanical pain test and cold plate test were used to evaluate the response of rats to mechanical stimulation and cold stimulation after immediate intervention. The protein expression of IL-17F and TRAF6 in the spinal dorsal horn of rats in each group was detected by Western blotting. The mRNA expression of IL-17F,IL-17RC,Act1 and TRAF6 in the spinal dorsal horn of rats in each group was detected by real-time PCR.The average fluorescence intensity of M1 microglia in the spinal dorsal horn of rats in each group was detected by immunofluorescence. Results Behavioral results showed that before intervention, compared with the sham group, paw mechanical withdraw threshold(PMWT)decreased and cold sensitivity threshold(CST)increased in the model group and the tuina group;after tuina intervention,PMWT in the tuina group was increased, and CST was decreased compared with the model group;after intervention,PMWT in the tuina group was increased, while CST was decreased(P<0.05).RT-PCR results showed that compared with the sham group,mRNA expression levels of IL-17F,IL-17RC,TRAF6 and Act1 in the spinal dorsal horn of the model group were increased;compared with model group, the mRNA expression levels of above indexes in the tuina group were decreased(P<0.05).Western boltting results showed that compared with the sham group, the expression levels of IL-17F and TRAF6 protein in the spinal dorsal horn of the model group were increased;compared with the model group, the expression levels of IL-17F and TRAF6 protein in the tuina group decreased(P<O.05).Immunofluorescence results showed that the mean fluorescence intensity of CD40 in the spinal dorsal horn of model group was enhanced compared with the sham group;compared with the model group, the mean fluorescence intensity of CD40 in the tuina group was decreased(P<0.05).Conclusion The"three methods and three points"tuina technique can produce immediate analgesia by inhibiting the expression of IL-17F,IL-17RC,Act1,TRAF6 and the activation of M1 microglia in the dorsal horn of the spinal cord after one intervention.
