1.Studies on Chemical constituents in seeds of Euphorbia lathyris

Objective To study the chemical constituents in the seeds of Euphorbia lathyris.MethodsCompounds were isolated by methods of column chromatography(silica gel,including reversed phase),Sephadex,and recrystallization.On the basis of spectroscopic methods including IR,MS,NMR,and X-ray,structures of compounds were confirmed.Results Twenty-two multi-type compounds were isolated from ethanol extract in the seeds of E.lathyris.Their structures were identified as 5,15-O-diacetyl-3-O-phenyl-6(17)-epoxylathyrol(1),5,15-O-diacetyl-3,7-O-dibenzoyl-7-hydroxylathyrol(2),5,15-O-diacetyl-3-O-benzoyl-lathyrol(3),20-O-hexadecanoyl-ingenol(4),3-O-hexadecanoyl-ingenol(5),15,17-O-diacetyl-3-O-cinnamoyl-17-hydroxyjolkinol(6),5,15,17-O-triacetyl-3-O-benzoyl-17-hydroxyisolathyrol(7),5,15-O-diacetyl-3-O-nicotinoyl-lathyrol(8),5,15-O-diacetyl-3-O-benzoyl-7-O-nicotinoyl-7-hydroxy-lathyrol(9),ingenol(10),lathyrol(11),esculetin(12),?-sitosterol(13),benzene-1,2,3-triol(14),palmiticacid(15),2,3-dihydroxypropyl icosanoate(16), 2,3-dihydroxypropyl oleate(17),2,3,4-trihydroxybutyl hexadec-3-enoate(18),aurantianide acetate(19),benzoic acid(20),p-hydroxybenzoic acid(21),oleic acid(22).Conclusion Among these,compounds 10,11,1419 are obtained from this plant for the first time and compounds 1-3 are the main diterpenes.
3.Relationship of cytochrome P-450 epoxygenase metabolites of arachidonic acid with impaired endothelium-dependent vasorelaxation in the healthy elderly

Objective To determine the relationship of cytochrome P-450 epoxygenase metabolites of arachidonic acids with impaired endothelium-dependent vasorelaxation in the healthy elderly. Methods A case-control study was employed.The study enrolled 60 healthy young adults (group A) and 60 healthy senior citizens (group B) of Han population in Shanghai. Serum contents of 14,15-dihydroxyeicosatrienoic acid (14,15-DHET) (a stable metabolite of 14,15-EET), 6-Kote-PGF1a (a stable metabolite of prostaglandin) and TXB2 (a stable metabolite of thromboxane A2) were measured using ELISA kits. The endothelium-dependent vasorelaxation (EDV) and endothelium-independent vasorelaxation (NEDV) in brachial arteries were determined by color Doppler ultrasound. Results Compared with group A, group B had significant higher levels of hemoglobin A1c, triglyceride and cholesterol levels (P<0.01), and significant lower levels of 14,15-DHET and 6-Kote-PGF1a (P<0.001), leading to increased values of TXB2/6-Kote-PGF1a and TXB2/14,15-DHET. There was bigger basal interior diameter of brachial arterials with reduced EDV and NEDV response (P<0.01 and P<0.05, vs. group A respectively) in group B. Moreover, the age was negatively correlated with 14,15-DHET and TXB2/14,15-DHET. Conclusions Our results indicate that the impaired EDV and NEDV in aging are associated with reduced production of arachidonic acid metabolites through cytochrome P-450 epoxygenase pathway and cyooxygenase pathway of endothelium in the healthy elderly.
4.Feasibility Study on Test Method Establishment for Histamine-like Substances in Pirarubicin

Objective:To establish the test method for histamine-like substances in pirarubicin. Methods:According to the meth-od described in the appendix of Chinese Pharmacopoeia (2010 edition), the test for histamine-like substances was carried out. Re-sults:The test for histamine-like substances had no interference under the condition with pirarubicin concentration of 0. 01mg·ml-1 . The established test method for histamine-like substances in pirarubicin was applied in 10 samples, and the results were accordant with those of the test method for depressor substances. Conclusion:The test method for histamine-like substances in pirarubicin is feasible with the limit of 1 μg·mg-1 .

7.Analysis of clinicopathological features and risk factors for postoperative complications in the elderly gastric cancer patients.

OBJECTIVETo investigate the clinicopathological features and postoperative short-term complications in the elderly gastric cancer patients.
METHODSClinical data of 270 elderly patients with gastric cancer who underwent gastrectomy in Department of Surgery, Ruijin Hospital, Shanghai Jiaotong University School of Medicine between July 2012 and June 2014 were analyzed retrospectively. Among 270 patients, 220 were 70 to 79 years old( old group) and 50 were ≥80 years old(oldest group). The clinicopathological features were compared between the two groups. Perioperative factors were analyzed to determine if they are associated with postoperative complications. Multivariate logistic regression model was performed.
RESULTSBefore operation, most elderly patients (n=161, 59.6%) had 2 or more than 2 comorbidities, including hypertension (n=154, 57.0%), anaemia (n=126, 46.7%), diabetes (n=53, 19.6%), arhythmia (n=52, 19.3%), cardiovascular disease(n=33, 12.2%), and chronic pulmonary disease(n=28, 10.4%). Elderly gastric cancers were more likely to locate at the lower third of the stomach (n=116, 43.0%). The pathological type was mainly the poorly differentiated carcinoma (n=152, 56.3%), and stage III was more common in TNM staging(n=138, 51.1%). As compared to the old group, the oldest group had more preoperative comorbid diseases(P=0.048), more previous surgery(P=0.029), more preoperative transfusion (P=0.019), more combined cholecystectomy (P=0.007) and feeding jejunostomy (P=0.037), but less tumor invasion of nerves(P=0.045). No significant differences in other clinicopathological parameters were found between the two groups (all P>0.05). A total of 121 (44.8%) patients presented postoperative complications, including severe complication in 30 cases(11.1%) and death in 4 cases(1.5%). Forty-seven patients(17.4%) presented operation-associated complications, including infection in 28 cases(10.4%) and leakage in 21 cases(7.8%). One hundred and seven(39.6%) patients presented non-operation-associated complications, including pneumonia in 48 cases(17.8%), hypertension in 23 cases(8.5%), and arhythmia in 17 cases(6.3%). Postoperative morbidities of Clavien-Dindo class II complication and non-operation-associated complication were higher in the oldest group compared with old group(P<0.05), while other postoperative complications were compared between the two groups, only urinary tract infection was significantly different(P<0.05). Univariate analysis showed that postoperative complications were significantly associated with age(χ(2)=7.308, P=0.007), number of comorbid diseases (χ(2)=10.872, P=0.001), cardiovascular disease (χ(2)=9.412, P=0.002), hypertension (χ(2)=4.934, P=0.026) and preoperative transfusion (χ(2)=3.911, P=0.048). Multivariate analysis showed that only the number of comorbid diseases was an independent risk factor for postoperative complications(OR=2.810, 95% CI: 1.710 to 4.616, P=0.000).
CONCLUSIONNon-operation-associated postoperative complications are more likely to occur in the elderly patients due to more comorbid diseases. Perioperative intensive care should be carried out for the elderly gastric cancer patients with comorbid diseases in order to improve surgical safety and efficacy.

8.The formula of ω-3 polyunsaturated fatty acids improving cognitive impairment in patients with depres-sion:a clinical randomized double-blind controlled trial

Objective To investigate the effects of different ratios of ω-3 polyunsaturated fatty acids(ω-3 PUFA)on depression and cognitive impairment in patients with major depression.Methods A randomized,double-blinded controlled trial was used to randomly assign patients with depression to a cognitive improvement group,a depression improvement group,and a placebo group.The cognitive improvement group took 1388 mg of docosahexaenoic acid(DHA)and 692 mg of eicosapentaenoic acid(EPA)every day and the depression improvement group took 1248 mg of EPA and 832 mg of DHA every day.The placebo group took the same dose of soybean oil for 12 weeks,during which psychiatric medication was maintained.The 24-item Hamilton depression scale(HAMD-24)was used to evaluate depressive symptoms,and the standardized MATRICS consensus cognitive battery(MCCB)was used to evaluate cognitive function after 6 weeks and 12 weeks,respectively.Results The study recruited a total of 46 patients with depression including 22 in the cognitive improvement group,12 in the depression improvement group,and 12 in the placebo group.After 6 weeks of treatment,the HAMD-24 scores were significantly lower in the depression improvement group(19.00±10.70)and cognitive improvement group(16.58±9.39)than in the placebo group(31.10±10.03)(P<0.01).After 12 weeks of treatment,HAMD-24 scores were significantly lower in the depression improvement group(13.58±8.43)than in the placebo group(28.10±15.04)(P=0.02).No significant interaction effect was found on the cognitive assessment scores in any dimension after 6 weeks and 12 weeks of treatment(P>0.05).The incidence rate of adverse events in the depression improvement group was 16.7%(2/12),and no adverse events were reported in the other two groups.There was no significant difference in the incidence of adverse events among the three groups(P=0.13).Conclusion Treatment with ω-3PUFA for 6 weeks can improve the depressive symptoms of patients with depression.The formula with a higher ratio of EPA exhibits higher effectiveness while the two groups of ω-3PUFA formulas with different ratios do not improve cognitive function.
