1.Analysis of the curative effect and survival of postoperative adjuvant chemotherapy for stage Ⅱ/Ⅲ colon cancer

Objective To study the efficacy of stageⅡ/Ⅲcolon cancer postoperative adjuvant chemotherapy and survival time.Methods 108 stage Ⅱ/Ⅲ colon cancer patients with postoperative adjuvant chemotherapy were selected as the study group,and in the same period 52 stage Ⅱ/Ⅲ colon cancer patients with only simple surgery treatment were selected as the control group.The adjuvant curative effect of the two groups were analyzed and the sur-vival time was followed up.Results The patients of the two groups were followed up for 3 years.The median disease-free survival time of the study group was 18 months,disease-free survival rate was 69.44%,26 cases in recurrence and transfer,accounted for 24.07%,and 7 cases(6.48%) died;The median disease-free survival time of the control group was 12 months and the disease-free survival rate was 46.15%,18 cases in recurrence and transfer, accounted for 34.62%,and 10 cases(19.23%) died.The median disease-free survival,recurrence and transfer rate of the control group were statistically significant better than those of the control group(χ2 =8.07,4.74,all P<0.05).Postoperative adjuvant chemotherapy in patients with adverse reaction was mainly for nausea and vomiting,loss of appetite,pigmentation and hair loss up to see, but were comparatively light, and very little serious most patients could be tolerated.Conclusion The postoperative adjuvant chemotherapy for patients with stageⅡ/Ⅲcolon cancer can reduce postoperative recurrence,metastasis,improve patients'survival time,prolong patient life.
2.Immunotoxins composed of monoclonal antihuman T Iymphocyte antibody and single chain ribosome—inactivating proteins:antitumor effects in vitro and in Vivo

Two new single chain ribosome—inactivating proteins,polygonin2 from the roots ofPolygonatum odoratum and momordin2,from the seeds of Momordica charantia,inhinited pro-teib synthesis potentially in rabbit reticulocyte lysate,but were relatively low toxic to Molt—4 cells.polygonin2 and momordin2 were conjugated to H65 monoclonal antibody that recognizedhuman T lymphocyte CD5 surface antigen using a heterobifunctional crosslinking reagent 2—iminothiolane.The resulting immunotoxins,referred to as H65—PGN2 and H65—MOR2,showed potent specific cytotoxicity to target cell Molt—4 and human peripheral blood T lympho-cyte,but had no effect on human hematopoietic cells.Injection of Molt—4 cells s.c.into Balb/cnu/nu mice developed into a solid tumor.Administration of the H65—PGN2 or H65—MOR2immunotoxins i.p.10 ug per mice at 48—h intervals suppressed tumor growth specifically.Theresulting showed that clinical studied were warranted to use these conjugates for the therapy ofautoimmune diseases as well as the ex vivo purging of T lymphocyte in ABMT.
3.Discussion about mode,mechanism and effect in the popularization and demonstration of country Feasible Medical Technology

Objective:Offer practical experience and theoretical basis for the popularization of Country Feasible Medical Technology(CFMT)in our nation,and explore the best mold and mechanism for popularization of CFMT at the actual situation of our nation,especially to the western rural areas.Methods:We launch the popularization and demonstration of CFMT in two countries of Chongqing City.Results:In 2005 and 2006,we adopted different modes of popularization about CFMT,and obtained different effects;besides,the CFMT reduces the difficult and expensive condition of seeing a doctor to some extent,and wins wide welcome and praise from health workers and masses.Conclusions:For the popularization of CFMT in western rural areas,the best mold is 'government-control & market-guidance' Mold.And the most suitable mechanism is a synergistic effect of many mechanisms,in which Consultation & Combined Action mechanism,Social Mobilization mechanism,Proper Financial Compensation mechanism,and Restriction & Inspiritment mechanism is the four main mechanisms.Besides,to summarize experiences the participation of college and scientific research institute is needed in the whole process of research project.
4.Distribution Characteristic and Drug Reisitance Analysis of 207 Children Infected Klebsiella Pneumoniae

Objective Analysis of K.pneumoniae infection in the Pediatrics and the Neonatology clinical characteristics of the resistance,that can guide and use medicine clinically rationally.Methods The sensitiveness test and ESBL detection to the 20 kinds of antibiotics were examined by Automatic Instrument bacterial identification and susceptibility testing ;ESBL positive strains with the standards disk diffusion method phenotypic confirmatory test.Results The 207 strains of K.pneumoniae from fecal specimens were 77.3% and from respiratory specimens were 18.8%.Imipenem and meropenem sensitivity 100%,followed by compound piperacillin,ciprofloxacin,compound amoxicillin and cefoxitin,that sensitive rates were 98%,97%,96% and 96%,;ceftazidime,cefepime,cefotaxime lower sensitivity were 75%,75%,78%.50 ESBL-producing strains,accounting for 24.2%.Conclusions K.pneumoniae have different levels of the drug resistance expcept Imipenem and meropenem,and the multi-drug resistant strains ratio and the positive rate are high.The sensitiveness of penicillin compound containing enzyme inhibitor is second only to the two described above,and they are economy and effective.Considering by medicine economics,the kinds of medicine is preferred on according to the drug sensitivity.
5.Analysis of 206 ethic reviews in a hospital within 3 years

Objective To reveal the average pass rate of a general hospital's ethic review and common problems.Methods The records generated by the ethics committee of a hospital in 3 consecutive years were reviewed and analysed with ridit of CMH and Bootstrap statistical methods.Results The pass rate for drug clinical trial is 36.09%,medical instruments 18.42%,and subject evaluation 14.29%.Among the problems,those concerning informed consent take up 63.59%,of protocol 14.56%,of CRF 7.77% and of the others 22.820%.The domestic applicants had more problems than those abroad.Besides,the pass rate of drug trials decreased with year from 45.83% in 2006 to 19.44% in 2008,which was statistically significant (P=0.029).In terms of drug clinical trial,the pass rate of revised projects in foreign countries was significantly higher than the domestic ones (P=0.028).ConclusionEthics Committee in a hospital plays a very important role in clinical researches.And informed consent and research protocol are very important in the ethic review.
6.An analysis of the model with sustainable development for promotion of appropriate technologies in rural health

Objective To explore the promotion model with the sustainable development of appropriate technologies in rural health. Methods After a large-scale government-led, administrative intervention mode (Ⅰ mode) , the mode (Ⅱ mode) dominated by the third sector and participated in by the whole society was used in some counties, and Ⅲ model, an enterprise-led and market economy controlled mode was adopted in Tongliang County. Results The primary care doctor and patient satisfaction in the third sector participation mode, and the personal experiences of doctors in enterprise-led mode were evaluated as excellence. The differences of experience of both doctors and patients between the third sector-led and government-led were statistically significant (P<0.01) ; he third sector-led mode was better than government-led mode. Conclusion It needs to establish a long-term pattern with the government supervision and policy support, the third sector and business management, full community participation, and regulation of market economy rules for the sustainable promotion model of appropriate technologies in rural health.
7.Effects of DingKunDan on the endometriosis model rats

We built rats model of endometriosis applying autologus endometrial transplants to compare the graft vol-ume of the modeling rats before and after dosing,serum tumor necrosis factor alpha(TNF-α)levels in rats,and the expressions of vascular endothelial cell growth factor(VEGF)in ectopic endometrium.To compare with the control group,the graft volume of DingKunDan group was reduced and the serum TNF-αlevel declined ,and also the ex-pression of VEGF in ectopic endometrium was reduced.Their differences were statistically significant (P <0.05). DingKunDan can inhibit the growth of ectopic endometrium in endometrosis model rats.The mechanism may be re-lated to the decrease of the serum TNF-αlevel and the VEGF expression.

Objective:To investigate the expression of minichromosome maintenance proteins 4(MCM4)and cell division cycle 6(CDC6)in uterine cervical carcinomas and its relationship with human papilloma virus(HPV)16/18 infection.Methods:The expression of MCM4 and CDC6 was examined in 50 squamous cell carcinoma specimens,20 cervical intraepithelial neoplasia(CIN)Ⅱ-Ⅲ specimens,20 CINⅠ specimens,and 20 normal cervical tissues by immunohistochemical method.The infections of HPV type 16,18 DNA were determined by PCR.Results:(1)The expression of MCM4 and CDC6 in uterine cervical carcinoma tissues was significantly higher than that in the CIN specimens and normal cervical tissues(Both P0.05).(2)The positive rates of(HPV)16/18 were significant different between cervical carcinomas,CIN and normal tissues(P0.05).(3)MCM4 expression were positively correlated with the expression of CDC6 in uterine cervical carcimonas(r=0.390,P

Objective To combine grey relation analysis and partial least squares regression model to establish the forecasting model of per-patient hospitalization expenses .Methods Gray relational analysis was used to filter out the main factors affecting per-patient hospitalization expenses ,and then collinearity was examined between these factors .Partial least squares regression was used to establish prediction model of per-patient hospitalization expenses ,and the prediction accuracy was proved .Results After filtered by gray relational analysis ,the order of the importance of factors affecting per-patient hospitalization expenses was the west-ern medicine fee ,traditional Chinese medicine fees ,diagnosis and treat fees ,other fees ,inspection fees ,bed fees and operation fees . The established partial least squares regression model had a higher accuracy on fitting and prediction ,with low average relative er-ror ,respectively ,-0 .000 2% and 0 .349 3% .Conclusion The gray relational analysis and partial least squares regression are suit-able for the influencing factors and prediction analysis of hospitalization costs .It provides a reference for data with the small sample size and high collinearity between the variables .

Objective To investigate the electrophysiological characteristics of the atrium in patients with atrial fibrillation (AF) during electrophysiological study (EPS) Methods Seventy two patients with presence of AF during EPS were selected as Group AF , and the other 78 patients without AF as Group control All the patients had no AF history and structural heart diseases P A, P CSd, S1 A1, S2 A2, Max CD, CD zone and ERP HRA were measured as the electrophysiological parameters Results P A, P CSd, S1 A1, S2 A2, Max CD and CD zone were significantly longer in Group AF than in Group control , ERP HRA was shorter in Group AF Conclusion The decreased atrial conduction and the shorter atrial effective refractory period may be the electrophysiological basis for the occurrence of AF
