1.The diagnostic value of transvaginal three-dimensional ultrasound for uterine cavity lesions

Objective To evaluate the value of transvaginal three -dimensional ultrasound(3D -TVS)in the diagnosis of uterine cavity lesions.Methods 100 patients with suspected uterine cavity lesions,firstly underwent transvaginal ultrasound examination,then underwent transvaginal three -dimensional ultrasonography,and the trans-vaginal ultrasound and transvaginal three -dimensional ultrasound examination was judged by hysteroscopy standard. The diagnostic methods and hysteroscopy examination results of the two groups were compared and analyzed,and the three -dimensional ultrasound imaging data were analyzed.Results The ultrasound findings of transvaginal two -dimensional showed that there were 91 cases in line with hysteroscopy(lesion 142),the accuracy was 64.54%,the differences were statistically significant(χ2 =4.872,4.913,8.198,13.254,10.137,all P <0.05).At the same time, the three -dimensional ultrasound examination results of vaginal showed consistent with hysteroscopy 132,the accuracy was 93.62%,the differences were statistically significant(χ2 =2.872,7.387,10.258,18.275,12.259,all P <0.05).Transvaginal three dimensional ultrasound examination of common uterine cavity lesions ultrasound image was clear,and the size,shape and location of the lesion were clearly visible.Conclusion Three dimensional ultra-sound in the diagnosis of uterine cavity lesions has high accuracy,its image data is more clear,and in the clinical diagnosis of uterine cavity lesions has high value of application,and it is worth of promoting.
2.An investigation and analysis for dietary nutrition in bedridden elderly patients

In total, 607 bedridden patients aged 60 years and over at Tianlin community hospital and four other hospitals or at home-care in Shanghai were investigated with questionnaire, physical examinations and related laboratory tests. Contents of investigation include general situation, medical conditions, 24-hour recall of diet, intake of nutritional supplement preparation, and so on. As compare to the reference for nutrient intake of the elderly set in the Guide to Diet for Chinese Residents (2007), total nutritional intake was obviously insufficient and three nutrients in diet were imbalanced for most bedridden elderly patients. Intake of most nutrients (including proteins, dietary fiber, calcium, iron, zinc, selenium,copper, magnesium, phosphor, retinol equivalent, vitamin B1 , vitamin B2, vitamin C) was obviously low in bedridden elderly patients, with a larger gap from the requirements of the Dietary Guide.
3.Effect of administrative intervention on smoking cessation of the medical personnel

Objective To investigate the effect of the administrative intervention on smoking cessation of medical personnel and demonstrate the feasibility and the validity of this approach.Methods Two comparable hospitals were selected,one hospital with 136 smoking medical staff members was used as the intervention group;the other hospital which had 127 smoking medical staff members was used as the control group.We applied administrative intervention and health education to the intervention group and health education only to the control group.The intervention time was 10 months,we used the questionnaire survey before and after intervening to evaluate the smoking rate,the intention to quit smoking,the willingmess to accept the help of quitting and the proportion of those who quitted smoking.Results The smoking rate of intervention group (37.4%) was lower than that of control group (77.2%),the difference was statistically significant (x2=40.99,P<0.01).The proportion of control group smokers in planning(51.1%) is statistically significant as compared with intervention group (46.18%) (x2=46.18,P<0.01).The proportion of people who were willing to accept the help from families and friends and smoking cessation counseling in intervention group was significanly higher than that in the control group.(x2=10.04,x2=7.73,x2=7.58;P< 0.01).But the proportion of accepting the medicine for quitting smoking was not significantly different (x2=0.16,P>0.05).The proportion of smokers who wanted to quit smoking on their own willingness in control group was significantly higher than that in intervention group (x2=36.27,P<0.01).After10 months,61 people (46.6%) in the intervention group succeeded in quitting smoking and 13 (10.6%) people in control group succeeded (x2=28.21,P<0.01).Conclusion Administrative intervention has feasibility and validity when hospitals take activities for smoking cessation.
4.Influence of specific siRNA silencing survivin gene on proliferation and invasion of human tongue cancer Tca8113 cells in vitro

Objective:To investigate the effect of siRNA interfering survivin gene on proliferation of human tongue cancer Tca8113 cells,and clarify the effect of survivin Tca8113 cells biomechanism.Methods: The Tca8113 cells at logarithmic growth phase were selected and divided into interference group,negative control group,and blank control group.The relative levels of survivin mRNA and survivin protein expression of 3 groups were detected by RT-PCR and Western blot assay,the inhibitory rate of proliferation of Tca8113 cells was checked by MTT method,the apoptotic rate was assessed by flow cytometry(FCM),the the migration ability of Tca8113 cells were detected by Wound healing assay.Results: Compared with control group,the survivin mRNA and protein expression was markedly down-regulated in Tca8113 cells following RNA interference treatment(P<0.05),the cell proliferation were down-regulated in interference group(P<0.05),the cell apoptotic rate were up-regulated in interference group(P<0.05),the migration ability was significantly decreased in interference group(P<0.05).Conclusion: The expression of siRNA-survivin can significantly inhibit the invasion in tongue cancer Tca8113 cells,indicating it might be a potential biological therapeutic target for tongue cancer.

Objective: To study the optimal combined ratio of baicalin, icariin and Astragalus saponin I from a Chinese herbal compound Biminne. Methods: Firstly, a mouse model of allergic rhinitis was established by intraperitoneal injection of ovalbumin (OVA) and aluminum hydroxide gel suspension, and the effective dose range of baicalin, icariin and Astragalus saponin I was detected by 2,3-bis(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carboxanilide inner salt method. Secondly, 10 groups of combinations of baicalin, icariin and Astragalus saponin I assembled by U*(10)(10(8)) form were employed to determine the optimal combination by means of analyzing of the inhibitory effect on the splenocyte proliferation. Finally, the effects of each effective ingredient and the optimal combination were compared by observing the splenocyte proliferation, the contents of interleukin-4 (IL-4), IL-5, interferon-gamma (IFN-gamma) in supernatant of the splenocyte cultures and the ratio of IL-4 to IFN-gamma in order to verify the result. Results: Baicalin or icariin at concentrations ranging from 2 to 10 mumol/L, and Astragalus saponin I from 1 to 10 mumol/L effectively suppressed the splenocyte proliferation. When the proportion of baicalin, icariin and Astragalus saponin I was 1:2.14:2.65, the inhibitory effect was most remarkable. Further research confirmed the rationality of the optimal combination. Conclusion: An optimal combination of the major effective ingredients from Chinese herbal compound Biminne most effectively suppresses the proliferation of splenocytes from sensitized mice and regulates the cytokine secreting.
7.Study on the dose prediction of deep learning-based VMAT after surgery for endometrial carcinoma

Objective:To explore the predictive value of deep learning based on three dimensional deep residual network(3D Res-Unet)model for the dose accuracy of postoperative volume modulated arc therapy(VMAT)plan of endometrial carcinoma.Methods:A retrospective collection of 154 VMAT radiotherapy plans for endometrial carcinoma from The First People's Hospital of Neijiang was conducted.The data set was divided into one training set with 108 cases,one validation set with 15 cases and one test set with 31 cases as the ratio of 7:1:2 through randomly sampling.The approved dose of clinical application was used as"gold standard"to compare the difference between predictive radiotherapy dose of 3D Res-UNet and clinically radiotherapy dose.Results:There were statistical differences in the conformity index(CI)of target area and average dose(Dmean)between deep learning and clinical gold standard(t=-3.115,-0.124,P<0.05),and the difference of bladder V40 of organ at risk(OAR)between them was significant(t=0.510,P<0.05),and the difference of rectum V50 between them was significant(t=-2.121,P<0.05).The predictive dose of the left femoral head V30 was significantly lower than that of clinical dose(t=0.415,P<0.05).The predictive dose of the right femoral head V30 was significantly lower than that of clinical dose(t=-3.102,P<0.05).The predictive dose of pelvic Dmean was significantly higher than that of clinical dose(t=1.224,P<0.05).The predictive dose of small intestine V40 was significantly higher than that of clinical dose(t=0.461,P<0.05).There were no statistically significant difference in other indicators(P>0.05).The difference plot of dose showed that there was few difference between predictive results and clinical results,and the dose volume histogram of prediction basically coincided with that of clinical application.Conclusion:The 3DRes-UNet model can effectively predict the three-dimensionally spatial dose of VMAT plan after surgery for endometrial carcinoma,which can guide clinical radiotherapy work.
8.Efficacy and safety of anti-B cell maturation antigen chimeric antigen receptor T-cell for retreatment of relapsed/refractory multiple myeloma

Objective:To explore the efficacy and safety of anti-B cell maturation antigen (BCMA) chimeric antigen receptor T-cell (CAR-T) for the retreatment of relapsed and refractory multiple myeloma (RRMM).Methods:The clinical data of 10 RRMM patients who received anti-BCMA CAR-T therapy for the second time (CART2) in Henan Province Hospital of Traditional Chinese Medicine due to failure or recurrence after their first anti-BCMA CAR-T (CART1) therapy from January 2017 to June 2021 were retrospectively analyzed. The treatment, efficacy and adverse events of patients receiving CART2 therapy were summarized; and the objective response rate (ORR), median duration of response (DOR) and incidence of adverse reactions were compared between CART1 and CART2.Results:Among 10 patients, 8 were males and 2 were females, with a median age of 57 years (41-70 years). Patients' 3-month ORR after CART1 therapy was 90%, and the median DOR was 16.0 months (3.0-27.0 months). CART2 used human-derived anti-BCMA CAR-T to treat 6 cases and mouse-derived anti-BCMA CAR-T to treat 4 cases. The 3-month ORR of patients receiving CART2 therapy was 40%, and the median DOR was 8.5 months (3.0-11.0 months). Among 9 patients who received mouse-derived anti-BCMA CAR-T in CART1 therapy, 4 of them received the same product again and none of them showed curative effect. Among 6 patients retreated with human-derived anti-BCMA CAR-T, 4 patients (66.7%) of them achieved partial remission (PR) or better. During CART1 therapy, 10 patients developed grade 1-2 cytokine release syndrome (CRS), and 7 patients developed different degrees of decrease in leukocyte, neutrophil absolute count (ANC) and platelet. Among patients who achieved effective outcomes after receiving CART2 therapy, 4 patients of them developed grade 1-2 CRS, and different degrees of decrease in white blood cell, ANC and thrombocytopenia. Immune effector cell-related neurotoxicity syndrome was not observed.Conclusions:Anti-BCMA CAR-T is effective and safe to retreat RRMM. The ORR and DOR of patients receiving CART2 therapy are lower than those of patients receiving CART1 therapy. CRS and cytopenia are common adverse reactions.
9.Clinical Comprehensive Evaluation of Cefazolin and Cefuroxime in Pediatric Clean Surgical Prophylaxis

Abstract
OBJECTIVE To carry out clinical comprehensive evaluation of prophylactic drugs for clean surgery(type I incision) in children, and to promote the rational use of antibiotics during perioperative period. METHODS According to the Guidelines for the Management of Comprehensive Clinical Evaluation of Drugs(trial edition of 2021), the clinical comprehensive evaluation system of prophylactic drugs for clean surgery(type I incision) was established by using the methods of literature research, brainstorming, expert interview and Delphi, and the evaluation evidence was collected to carry out the comprehensive evaluation of multi-dimensional and multi-level evidence. RESULTS The clinical comprehensive evaluation system was constructed, including 5 first-class indicators(safety, effectiveness, economy, suitability, and accessibility), 11 second-class indicators and 30 third-class indicators. The clinical comprehensive evaluation score of cefazolin was 86.09, and that of cefuroxime was 81.86. The former was better than the latter in safety, effectiveness, economy and suitability, while the latter was better than the former in accessibility. CONCLUSION The clinical comprehensive evaluation of prophylactic drugs for clean surgery(type I incision) in children constructed in this study is scientific, and the results provides a basis for the selection of antibiotics for clean surgery(type I incision) in hospital
