1.Molecular Epidemiology of Staphylococcus aureus Originated from Respiratory Tract of Patients after Transplantation

OBJECTIVE To know the gene type of Staphylococcus aureus which caused the respiratory tract infection of patients after transplantation in our hospital,and to explore the epidemiology,preventing measure and nursing strategy of the lung infection by S.aureus among them.METHODS Using random amplified polymorphic DNA(RAPD) to analyze the gene type fingerprint of the 20 S.aureus strains originated from the respiratory tract of patients after transplantation,and using cefoxitin paper strip method to detect the mecA gene of the S.aureus.RESULTS Twenty S.aureus strains were divided into 8 gene types,and among them,the detectable rate of gene types Ⅰ,Ⅱ,and Ⅲ was 40.0%,15.0%,and 15.0%,respectively.The positive rate of mecA gene reached to 80.0%.CONCLUSIONS The S.aureus originated from respiratory tracts of the patients after transplantation is mainly of gene type Ⅰ.And the positive rate of mecA gene is very high.So the management of hands washing,sterilization and isolation of medical and nursing staffs should be strengthened,and effective method should be took to prevent the propagation of mecA positive S.aureus among the patients after transplantation.
2.Clinical Study of Aerosol Transmission in Two Oxygen Humidifiers

OBJECTIVE In order to find the way to reduce nosocomial pneumonia through preventing delivery of microbio-aerosol.we investigate whether oxygen humidification process generate and deliver aerosol to the end of oxygen tube,and the difference between two types of humidifier.METHODS Two types of humidifier were used.One was unique bionic humidifier through surface of humidification material,the other was conventional bubble humidifier.The amount of aerosol was counted by laser particle counter at the end of oxygen cannula.Different solute of ferric chloride and soluble starch in two humidification materials were used to testify the generation and transmission of aerosol during oxygen therapy.RESULTS Bubble humidifier produced large amount of aerosol of 0.3,0.5,1.0,3.0,5.0 ?m at 31089.9、28488.6、2873.9、197、1.1particle/cubic meter respectively.Most of the aerosol were between 0.3 and 3 microns in diameter.While bionic humidifier produced small amount of aerosol of 0.3,0.5,1 ?m at 95.3,39.8,2.0 particle/cubic meter respectively,there were no aerosol particle larger than 3?m.The difference of 0.3,0.5,1.0,3.0,5.0?m aerosol between bubble and bionic humidifier was significant(P

Objective To observe the effect of Sijunzi Decoction(SD) and Huangqi Sijunzi Decoction(HSD) on mitochondrial succinate dehydrogenase,cytochrome oxidase activity of liver in rats with spleen asthenia,thus to reveal the pathogenesis mechanism of spleen asthenia from energy metabolism,and to clarify the therapeutic mechanism of spleen-invigorating and Qi-tonifying herbs.Methods Forty SD rats were randomly divided into 4 groups:normal control group,spleen-asthenia model group,SD(10.5 g/kg) group and HSD(18 g/kg) group,10 rats in each group.Except that the normal control group,the rats in the other groups were fed with half-full diet once every other day and with gastic gavage of Xiaochengqi Decoction 60 g/kg every day to establish animal model of spleen asthenia.After 15 days,the model rats were administered the corresponding drugs according to the experimental design for 4 continuous weeks.The changes of mitochondrial succinate dehydrogenase,cytochrome oxidase activity of liver were measured,and the general status of the rats were observed.Results Compared with the normal control group,the level of mitochondrial succinate dehydrogenase and cytochrome oxidase activity in liver of spleen asthenia group were decreased obviously(P
4.Effect of Astragalosides on Gastrin Receptor in Gastric Parietal Cells of Rats with Spleen Deficiency

[Objective] To observe the regulatory effect of astragalosides (AST) on the binding capacity of gastrin receptor in gastric parietal cells (GPC) of rats with spleen deficiency. [Methods] SD rats were given gastric gavage of Radix et Rhizoma Rhei (Dahuang) decoction 10 g/kg to induce spleen deficiency. Gastric parietal cells were isolated and purified by Lewin gastric sac method and Percoll density gradient separation method. 2 mL suspension of GPC from rat models was incubated in vitro with AST 20 ?L at the concentrations of 10, 20 and 30 g/L respectively. Suspension of GPC from rat models incubated with phosphate buffered saline (PBS) served as the model group and suspension of GPC from the normal rats incubated with PBS as the normal group. The binding capacity of gastrin receptor in GPC was detected by radioligand receptor assay. Objective To establish a quality standard for Kangtai Capsule. Methods Kangtai Capsule was identified by TLC; the effective components in Kangtai Capsule were determined by HPLC. Results The relevant spots in Radix Paeoniae Alba, Radix Saposhnikoviae and Radix Aucklandlae were identified by TLC. the content of Paeoniflorin showed a good linearity in the range of 0.204~ 2.040 ? g, the average recovery rate was 97.9 % and RSD was 0.88 % . Conclusion This method is simple, feasible and repeatable, and can be used for quality control of Kangtai Capsule.
6.Determination of Selenium Contents for Some Chemical Forms in Rice Rich-Selenium

【Objective】 Selenium (Se) contents of some chemical forms as total Se,inorganic Se,organic Se and selenomethionine (SeMet) in rice e nriched-Se were determined, in order to provide scientific basis for applying r ice enriched-Se to cancer prevention. 【Methods】 Fluorometric determination wi th 2,3-diaminonaphthalene (2,3-DAN) for total Se, inorganic Se and organic Se, and with CNBr-2,3-DAN for SeMet. 【Results】 Total Se contents of rice enrich ed-Se reaping in summer and autumn were (2.71±0.97) μg/g, (9.87±1.18 ) μg/g, respectively, both higher than that of ordinary rice (P<0.001); the organic Se occupying over 99% of total Se were 2.68, 9.77 μg/g, respectively . Main component of the organic Se was SeMet, and its Se content (Se-SeMet) was (1.45±0.67) μg/g for rice enriched-Se reaping in summer, (4.96±0.98) μ g/g in autumn,and their contents/total Se contents were 53.5, 50.5 %, respecti vely. 【Conclusions】 Most part of Se in rice enriched-Se was natural organic Se and main component of the organic Se was SeMet.
7.The anxiety level of the patients who will have ultrasound-guided puncture biopsy: a survey study.

In order to fully understand the anxiety level of the patients who will have ultrasound-guided puncture biopsy, and the survey results will guide the future clinical care to reduce the anxiety score of the patients, and to im-prove the satisfaction of them, which reflects the quality of care, we used Zung Self-rating Anxiety Scale (SAS) to investigate outpatients who will have ultrasound-guided puncture biopsy in a upper first-class hospital by convenience sampling way. The results showed that the SAS standard score of the patients was higher than the Chinese normal population's, and there is significant difference between the patients and the normal population's score (t = 47.12, P < 0.001). So for reducing the patients' anxiety level we should give health education and care intervention to the patients before the operation. This reflects the Patient-centered Culture and Excellent-quality Care in China.

8.Effect of the integration pattern of hospital-community on the grade-based management for hypertension in elders.

OBJECTIVE:
To investigate the effect of the integration pattern of hospital-community on the grade-based management for hypertension in elders.
METHODS:
We randomly chosen 218 (male, n=121; female, n=97) primary senile hypertension patients from a Community Health Service Center of District in Changsha City, from June, 2013 to December, 2013. Based on the risky factors, the subjects were divided into three groups and every group received grade-based management on blood pressure with a integration pattern of hospital-community for six months. According to the HILL-BONE high blood pressure compliance scale and the self- designed blood pressure monitoring form, we assessed the effect of compliance and blood pressure control on senile hypertension patient.
RESULTS:
Hypertension treatment rate for the elders ranged from 22.9% to 88.1% (P<0.01). The levels of blood pressure of the subjects were significantly decreased compared with baseline. The level of diastolic blood pressure in the low, average, high and very high-risk group was decreased by 17, 20 and 23 mmHg, respectively (P<0.01). The level of systolic blood in the low, average, high and very high-risk group was decreased by 6, 5 and 7 mmHg, respectively (P<0.01). The compliance rate of HILL-BONE hypertension rose from 54.5% to 87.4% (P<0.01).
CONCLUSION
The integration pattern of hospital-community with the grade-based management for hypertension significantly improved the senile hypertension control rate and compliance of drug treatment. The rational for drug usage rate rose obviously. The integration pattern of hospital-community with the grade-based management for elders deserves to spread.

9.Depression in patients with facial acne vulgaris and the influential factors.

OBJECTIVE:
To understand the influential factors for depression in patients with facial acne vulgaris and to provide scientific evidence for a comprehensive and systematic treatment for acne vulgaris.
METHODS:
A total of 287 outpatients with facial acne vulgaris, who visited the dermatology of the Third Xiangya Hospital, were surveyed by Beck Depression Inventory (BDI). The data was collected by Epidata software (version 3.1) and processed by SPSS software package (version 18.0). The influential factors for the depression of outpatients with facial acne vulgaris were analyzed by multinomial logistic regression.
RESULTS:
A total of 181 patients with facial acne vulgaris showed various degrees of depression (BDI score≥5) and the rate was 63.1%. The symptoms for depression included sad and pessimistic attitude as well as the decreased attention to others (social withdrawal). The influential factors for mild, moderate or severe depression were gender, the degree and the course of acne. Female patients were more likely to suffer mild, moderate or severe depression (OR=3.62, 2.63, respectively); the risk of depression in acne patients was increased with the increase in degree of the severity (OR=2.31, 4.51, respectively); the patients with the acne course more than a year were more likely to show mild depression than those with a course less than a year (OR=4.30, 7.44, respectively). The patients with acne course more than 3 years were more likely to show moderate or severe depression compared to those with a course less than a year (OR=3.60).
CONCLUSION
Most of facial acne patients show a different degree of depression. The acne course is longer in female patients. The more severe the acne vulgaris is, the more suffering of the depression is. Psychological care should be considered to improve the treatment and quality of life.

10.Analysis of the mechanism of curcumol in regulating iron death and autophagy based on systemic pharmacology

Objective:
To explore the molecular biological mechanism of curcumol in regulating ferroptosis and autophagy.
Methods:
The Pharm Mapper database was used to screen out curcumol targets, various known related databases were used to establish a database of ferroptosis and autophagy related targets, and the String database was used to construct a protein-protein interaction network. The key targets were enriched and analyzed using DAVID database.
Results:
152 curcumol targets, 259 ferroptosis targets, and 796 autophagy targets were obtained; curcumol mainly regulated the ferroptosis process through PTGS2, ALB, MAPK1, MAPK8, and MAPK14 targets, and curcumol mainly through HSP90 AA1, MAPK1, MAPK8, ALB, NOS3 targets regulated the autophagy process, and curcumol mainly regulated the ferroptosis and autophagy process through ALB, MAPK1, MAPK8 targets.
Conclusion
Curcumol can exert pharmacological effects by regulating ferroptosis and autophagy.
