Objective To constmct the training system of specialized nurses in operation room.Methods The Delphi method was used to organize two rounds of consultations by 20 experts from July 1st to Nov.1st 2012.Results In the two rounds of expert consultations,experts' positive coefficients were 95％ and 100％,the authoritative coefficients of experts were 0.86 and 0.89.The evaluation system included 10 first level entries and 38 second-level entries,all entries' importance assignment was more than 4.0,all variable coefficients were less than 0.25.Conclusions The positivity and authority of experts in this study were in high degree,the concentration and harmonization of experts' opinions were relatively high,all entries' weight were relatively high.So the system has high scientific nature and reliability to provide a theoretical basis for specialized nurses in operation room.

Objective To construct an index system of the performance appraisal to clinic nurses.Methods According to the BSC,KPI and SMART principal,we constructed the primary evaluation indexes.And then two rounds of Delphi consultation were conducted.Results An index system of the performance appraisal to clinic nurses was composed of 5 level-1 indexes,including 22 indexes.Conclusions The index system has high scientific nature and reliability to provide a theoretical basis for the performance appraisal to clinic nurses.

Objective To examine the dissimulation in memory test by event-related potentials (ERP).Methods Recording the ERP of 20 students in the choosing tasks according to Binomial Forced-Choice Digit Memory Test (BFMDT) in three situations including reality,dissimulation and forced wrong choose. Results The reaction time was lengthened gradually by the situation of reality ( 681 ± 21 ) ms, dissimulation ( 741 ± 25 ) ms and forced wrong choose( 946 ± 31 ) ms (F= 115.73, P = 0.000). The main components in ERP waveforms, N2 ( 180 ～184 ms) and P2 ( 214 ～ 243 ms) had higher amplitudes in situations of dissimulation and forced wrong choose than that of reality. The amplitude of N2 showed significant difference in three groups(F=6. 896, P＜0.01 ). The amplitudes were increased gradually by the situation with reality( -0.376 ± 0.58 )μV, dissimulation( 0. 86 ± 0.71 )μV and forced wrong choose( 1.77 ±0.82)μV. Conclusion ERP may be an electrophysiological index of the intention of dissimulation in memory test.

Objective To observe the hematocyte change in patients peripheral blood with severe hepatitis and to investigate its causes and clinical significance.Methods Partial parameters of peripheral blood cell in 65 patients with severe hepatitis and 52 normal individuals were analysed using blood cell counter.Results WBC number in patients with severe hepatitis was significantly higher than that in normal individuals[(8.13±4.33)×109/L vs(6.02±1.07)×109/L,P＜0.01],and PLT,PCT,MPV,PDW,RBC and Hb in the patients with severe hepatitis Were also significantly lower than that in normal individuals(P＜0.01).Conclusions Severe hepatitis can cause the hematocyte changes.It is essential to examine the hematocyte parameters in diagnosis and therapy of severe hepatitis.

Objective To screen for clopidogrel metabolism related gene CYP2C19 in patients with coronary artery disease in Wuhan.Methods 316 patients,from Jan to Dec 2014,after cardiology percutaneous coronary interventional therapy (PCI) for the treatment of coronary artery disease were selected as research object.Clopidogrel metabolism related CYP2C19 geno-types (* 1,* 2,* 3)were detected by the gene chip,and for different types of metabolism of patients according to CYP2C19 gene type:strong metabolize type (*1/*1),intermediate metabolizer types (*1/*2,*1/*3),poor metaboli-zer types (*2/*2,*3/*3,*2/*3).Results According to the CYP2C19 gene polymorphism of metabolic function type, strong metabolic type carrying CYP2C19*1 (*1/*1)accounted was 43.4%,intermediate metabolizers carrying CYP2C19*2 or *3 (* 1/* 2 and * 1/* 3)and poor metabolizers (* 2/* 2,* 2/* 3 and * 3/* 3)accounted was 42.4% and 14.2%,respectively.Different gender had no statistical significance in CYP2C19 genotype differences.Conclusion Clopi-dogrel metabolism functional of CYP2C19 gene in patients with interventional coronary heart disease in Wuhan area had more deletion gene.

Objective To investigate the rivalry mechanism of ginkgo in rat brain edema after intracerebral hemorrhage.Methods Totally 80 healthy Wistar rats were divided into 4 groups randomly(20 rats in each group).Fifty microliters saline(group A and group B) or auto-hemoglobin(group C and group D) was infused into the right caudate nuclears using stereo tactic guidance.Groups A and C were lavaged 50 g/L carboxymethyl cellulose sodium solution of 3 mL,groups B and D were lavaged the same amount of ginkgo(200 mg/kg) solution dissolved by 50 g/L carboxymethyl cellulose sodium solution once a day for 3 weeks.All the rats were decapitated after raised for 3 weeks,and superoxide dismutase,malondialdehyde,and the number of hemeoxygenase-1(HO-1) positive cells in brains were assayed.Results The differences in SOD and MDA contents as well as HO-1 positive cell number between every two groups were significant(P

Objective To investigate the clinical values of multiple ultrasound soft markers in screening for fetal chromosomal abnormality during first-trimester.Methods Two thousand seven hundred and eighty-nine nulliparas in Department of Obstetrics,the First Affiliated Hospital of Jinan University during early pregnancy (11-13+6 gestational weeks) were selected for this study.Fetalnuchal translucency (NT),facial angle (FA),ductus venosus (DV),fetal heart rate (FHR),tricuspid reverse (TR),nasal bone (NB) and fetal structures were scanned and measured.Risk calculation software (Astraia) was used to calculate the chromosomal abnormal risk (cut-off line:＞1/300) based on ultrasound records.The chorionic villi or amniotic fluid of high risk patients was collected with informed consent for karyotype analysis (prenatal diagnosis).All patients were followed up until six months after delivery.Chi-square test or Fisher exact test was used to compare the difference.Results One hundred and seven cases were high-risk of trisomy 21 among which 96 cases accepted invasive prenatal diagnosis.Sixteen chromosomal anomaly and six trisomy 21 cases were diagnosed out the 96 fetuses.Among 2789 cases,four were high-risk of trisomy 21 according to ultrasound screening.Six cases were diagnosed as trisomy 21.The false positive rate of ultrasound screening was 3.6％(101/2783).There were 196 cases whose NT ≥2.5 mm,in which 66 cases were high risk of chromosomal abnormality,and 16 fetal chromosomal abnormalities were diagnosed after chorionic villus sampling.The invasive procedure rate was 2.3％ (66/2789).Totally,186 pregnant women were older than 35 years,among which 32 cases were high risk.There was no significantly difference on the of rate fetal chromosomal abnormality between the groups of age≥ 35 pregnant women and the general population (P=0.055).But 29.9％ (32/107) high risk cases were detected in the group of age≥35.Five of thirteen fetal malformations cases were associated with abnormal karyotype.Conclusions Multiple ultrasound soft markers screening during early pregnancy could increase the diagnosis rate of chromosomal abnormality and decrease the false positive rate,false negative rate and invasive procedure rate.Early ultrasound screening might be effective in not only identifying chromosomal abnormality,but also diagnosing severe structure deformity of fetus.

Objective To investigate the level and influencing factors of caregivers′burden among primary caregivers of elderly stroke patients. Methods Totally 298 caregivers of elderly stroke patients were investigated with Zarit Burden Interview (ZBI), Hospital Anxiety and Depression Scale (HADS) and Simplified Coping Style Questionnaire(SCSQ). The patients were investigated with Modified Barthel Index (MBI). Results The caregivers′burden was in the mild level with a total score of 35.21±11.45. The influencing factors were the number of days of care, daily caring hours, hospital anxiety and depression score, the coping style and the ability of patients to perform ADL. Conclusions The primary caregivers of stroke patients have mild burden. The nurses should be synchronized to assess the health of patients and primary caregivers at the same time, understand the caregivers′burden, and provide specific care advice, in order to reduce the burden of their carer, so as to improve the quality of patients′life.

Objective To investigate the effects of Shuangligan and Glycine on Thl/Th2 balancing on severe acute pancreatitis ( SAP) in rats. Methods Thirty-two Wistar rats weighing (260 ± 20) g were randomly divided into sham operation (SO) group, SAP group, SAP + Slg (with the treatment by Shuangligan) group and SAP + Gly (with the treatment by Glycine ) group. Each group included 8 rats, which accepted different treatment according to the experimental design. Changes of plasma level of endotoxin ( ET) and serum amylase (AMY) and the effects of Shuangligan and Glycine on Thl/Th2 ratio at the 24th hour after operation were observed respectively. Results The plasma endotoxin (ET) level ( (0. 67 ±0. 11) EU/ml),proinflammatory cytokine (INF-γ:(8.43 ± 0.86) ng/L, IL-12: (8.26 ± 1.97) ng/L) and Thl/Th2 ratio (0.36 ± 0.07) in SAP group were significantly higher than those in SO group( ET: (0. 44 ±0.07) EU/ml, INF-γ: (3. 80 ±0. 55) ng/L, IL-12: (3. 34 ± 1. 34)ng/L,Thl/Th2 ratio (0. 24 ±0. 05) ) (P <0. 05). Compared with SAP group, SAP + Slg and SAP + Gly group had remarkably decreased plasma ET level ( (0. 57 ± 0. 08,0. 52 ± 0. 04) EU/ml) (P < 0. 05) and the Thl/Th2 ratio reached equilibrium ( SAP + Slg group; (0. 29 ± 0. 04 ), SAP + Gly group: (0. 25 ± 0. 06 )) . Conclusions In the earlier stage of SAP, the rising plasma ET level may cause the overreaction of the cell mediated immune response, which leads to the aggravated damages in tissue cells. Our data indicates that Shuangligan and Glycine can restrain the formation of intestinal endotoxemia and alleviate or prevent the tissue injuries.
