OBJECTIVETo study the therapeutic efficacy of Lixue Xiaoyin Decoction (LXD) in treating psoriasis and its effect on plasma endothelin (ET).
METHODSTwo hundred and twenty patients were divided into two groups. LXD was taken orally by the 118 patients in the treated group, while Compound Qingdai Capsule (CQC) taken orally by the 102 patients in the control group. The therapeutic efficacy was evaluated after 8 weeks of treatment. The ET content was determined by radioimmunoassay before and after treatment.
RESULTSThe total effective rate was 73.72% in the treated group and 54.90% in the control group with significant difference (chi2 = 8.52, P < 0.01). The plasm ET level in patients was significantly higher than that in the healthy subjects. ET in both groups was all lowered after treatment, but the decrement was more obvious in the treated group, showing significant difference when compared with that in the control group (P < 0.01).
CONCLUSIONLXD shows good therapeutic efficacy in the treatment of psoriasis. It can improve the microcirculation, inhibit the division of epithelial cells, promote the epidermic cell differentiation, and shows regulative effect on plasma ET.

3.Progress on hidden blood loss after lumbar interbody fusion.

Lumbar interbody fusion is one of the most commonly used surgical treatment of lumbar disease at present, but the hidden blood loss after surgery is large, accounting for 1/3 or even more than 1/2 of total blood loss. If not monitored and treated for the hidden blood, it can result in anemia and prolong bed time, thereby increasing the chance of infection. This paper summarizes the mechanism, influencing factors, calculation and treatment of hidden blood loss after lumbar fusion, so that the surgeon can have a correct understanding and evaluation of the hidden blood loss for the patients undergoing lumbar intervertebral fusion, and so as to reduce the occurrence of complications. This is very important for helping patients to pass perioperative period smoothly.

4. Application of cyberknife in treatment of retroperitoneal lymph node metastatic tumors in 81 cases

Objective:
To evaluate the clinical efficacy and side effects of cyberknife therapy in the treatment of retroperitoneal lymph node metastatic tumor.
Methods:
Among the 81 patients presenting with postoperative retroperitoneal lymph node metastases, 33 cases suffered from abdominal pain or low back pain, 7 had unilateral hydronephrosis and ureteral dilatation, and 8 developed unilateral or bilateral lower limb swelling. Using stereotactic radiotherapy with a cyberknife, DT was delivered at 33-45 Gy/3-6 F. The improvement of symptoms, objective tumor response rate, and irradiation-induced side effects were observed.
Results:
At 4 weeks after treatment, pain and swelling of the lower extremities were completely mitigated, and hydronephrosis was fully healed in all patients. Enhanced CT or MRI was performed every 2 to 3 months. After 6-month follow-up, the complete response (CR) rate was calculated as 77%(62/81), 21%(17/81) for the partial response (PR) rate, 3%(2/81) for the stable disease (SD) rate and the effective rate (CR+ PR) was 98%.No case progressed. The main side effect was intestinal injury, including grade 1 in 16%(13/81), grade 2 in 9%(7/81). 5.Mechanism study of Qingre huashi decoction in the treatment of gastric cancer by up-regulating miRNA-155 and inhibiting Wnt/β-catenin signaling pathway

OBJECTIVE To study the mechanism of Qingre huashi decoction in the treatment of gastric cancer by intervening in miRNA-155 and inhibiting Wnt/β-catenin signaling pathway. METHODS Thirty nude mice were randomly divided into model group, control group (0.004 g/kg cisplatin+0.02 g/kg fluorouracil), overexpression group, Qingre huashi prescription low-dose, medium-dose and high-dose groups (2.71, 5.43, 10.86 g/kg), with 5 mice in each group. The overexpression group was inoculated with miRNA-155 AGS cell line, and the other groups were inoculated with AGS cells to induce tumor-bearing gastric cancer model. The control group was given relevant medicine intraperitoneally, and other groups were given relevant medicine or normal saline intragastrically, once a day, for 3 consecutive weeks. The weight of tumor tissue in nude mice was determined; the pathological morphology of tumor tissue was observed; the miRNA-155 expression, mRNA and protein expressions of Wnt7, β-catenin and T- cell factor-4(TCF-4) in tumor tissue were detected. RESULTS Compared with the model group, the tumor weights of nude mice in the control group, the overexpression group and Qingre huashi decoction high-dose group were significantly reduced (P<0.05); mRNA and protein expressions of Wnt7, β -catenin and TCF-4 were significantly decreased (P<0.05), while miRNA-155 expression was increased significantly (P<0.05). Tumor cells exhibited varying degrees of loose arrangement, shallow nuclear staining, and necrotic foci. CONCLUSIONS Qingre huashi decoction can inhibit the protein and mRNA expressions of Wnt7, β-catenin and TCF-4 in Wnt/β-catenin signaling pathway by up-regulating miRNA-155, thus inhibiting the tumor growth of tumor-bearing nude mice.
6.Current Situation and Reconsideration on the Study of Integrated Chinese and Western Medicine Andrology.

The development of Chinese medicine and Western medicine andrology is based on different social background and academic systems, either Chinese medicine or Western medicine andrology has their limitations, therefore, integration of Chinese and Western medicine (ICWM) andrology is in a great need. After more than 30 years of development, andrology has made great achievements in the construction of specialized academic association, holding academic conferences and publication of academic monographs, and the research progress on this field is mainly in the combination of disease and syndrome, microdifferentiation of symptoms and signs and basic research development. However, the comprehensive theoretic system of ICWM andrology has not yet established, and the related studies are still on the primary stage. In the future studies, great efforts still need to be made to expand the methods for the investigation of ICWM, and make innovations in the field of andrology.

7.Efficacy and Safety of Hydrotalcite Combined with Omeprazole versus Omeprazole for Gastric Ulcer :A Meta-analysis

OBJECTIVE: To systematically evaluate the efficacy and safety of hydrotalcite combined with omeprazole for gastric ulcer, and to provide evidence-based reference for clinical treatment. METHODS: Retrieved from PubMed, Embase, Medline, the Cochrane library, CNKI, VIP and Wanfang database, randomized controlled trials (RCTs) about hydrotalcite combined with omeprazole (trial group) versus omeprazole alone (control group) for gastric ulcer during the database establishment to Aug. 2018. After data extraction of included literatures met inclusion criteria, and quality evaluation with Cochrane evaluator manual 5.0.1, Meta-analysis was performed for response rate, the incidence of ADR, recurrence rate of gastric ulcer bleeding, needed time of clinical symptom improvement and hospitalization stays by using Rev Man 5.3 statistical software. RESULTS: A total of 16 RCTs, involving 1 802 patients were included. The results of Meta-analysis showed that response rate [RR=1.24, 95%CI(1.19,1.29), P<0.001] of trial group was significantly higher than that of control group; recurrence rate of gastric ulcer [RR=0.27,95%CI(0.17,0.45),P<0.001], clinical symptom improvement time [MD=-2.04,95%CI(-2.25, -1.83),P<0.001] and hospitalization time [MD=-4.25,95%CI(-4.55,-3.95),P<0.001] of trial group were significantly lower or shorter than those of control group, with statistical significance. There was no statistical significance in the incidence of ADR [RR=0.68,95%CI(0.46,1.02),P=0.06] between 2 groups. CONCLUSIONS: Compared with omeprazole alone, hydrotalcite combined with omeprazole for gastric ulcer can obviously increase the clinical response rate, decrease the recurrence rate of gastric ulcer and shorten the needed time of clinical symptom improvement and hospitalization time, but do not increase the incidence of ADR.
8.Meta-analysis of Therapeutic Efficacy and Safety of Mirtazapine Combined with Selective Calcium Ch annel Blocker in the Treatment of Irritable Bowel Syndrome

OBJECTIVE: To systematically review therapeutic efficacy and safety of mirtazapine combined with selective calcium channel blocker (SCCB) in the treatment of irritable bowel syndrome (IBS), and provide evidence-based reference for clinical medication. METHODS: Retrieved from the Cochrane Library, PubMed, Embase, Medline, CNKI, VIP and Wanfang database, randomized controlled trials (RCTs) about mirtazapine combined with SCCB (trial group) versus SCCB (control group) for IBS were collected. After literature screening and data extraction, quality evaluation was performed by using Cochrane system evaluator manual 5.1.0 recommend bias risk evaluation tool. Meta-analysis was performed by using Stata 14.0 software. RESULTS: A total of 14 RCTs involving 1 005 patients were included. The results of Meta-analysis showed that the total response rate [RR=1.34,95%CI(1.25,1.44),P<0.001],neuropeptide-Y level after treatment [SMD=0.77,95%CI(0.49,1.05),P<0.001], response rate of abdominal pain therapy [RR=1.32,95%CI(1.06,1.66),P=0.014] and response rate of treatment for abnormal stool characteristics [RR=1.75,95%CI(1.36,2.27), P<0.001] were significantly higher than control group; the scores of depression scale after treatment [SMD=-1.87, 95%CI (-2.35, -1.39), P<0.001], anxiety scale after treatment [SMD=-2.25, 95%CI (-3.35, -1.15), P<0.001], abdominal pain symptom score after treatment [SMD=-7.41, 95%CI (-8.30,-6.51), P<0.001], diarrhea symptom score after treatment [SMD=-6.39, 95%CI (-7.96,-4.81), P<0.001] were significantly lower than those of the control group. There were no statistical significance in response rate of abdominal distension therapy [RR=1.07,95%CI(0.90,1.28),P=0.421] and response rate of abnormal defecation therapy [RR=1.05,95%CI(0.88,1.26),P=0.588], the incidence of abdominal pain [RR=0.45,95%CI(0.11,1.97), P=0.291] and exhaustion [RR=5.00,95%CI(0.60,41.79),P=0.137] between 2 groups. CONCLUSIONS: Mirtazapine combined with SCCB can significantly improve therapeutic efficacy of IBS patients, promote clinical symptoms, but do not increase the occurrence of ADR as abdominal pain and exhaustion.
