Objective To investigate the application of fundamental cause in the preparation of surgical instruments. Methods The fundamental cause analysis method was used to analyze the situations of improper preparation of surgical instruments in the operation room to analyze the fundamental causes and work out countermeasures. The preparation of surgical instruments before and after the implementation was compared.Result The qualification rate of surgical instruments increased from 96.0% before the implementation to 98.6% after the implementation, with statistically significant difference (P<0.001).Conclusion The fundamental cause analysis can help the operation room to find out the problems in the surgical equipment preparation process so as to improve the working procedures and the qualification rate of prepared surgical instruments.

The mode of community home-based care with combination of medical care and elderly care is im-portant for promoting healthy aging. But now the services contents provided by community health service agencies were not clear, related system of policies and regulations were imperfect, human resources were insufficient and net-work of service resource was not perfect. All of these factors caused the service cannot met old people medical de-mands and the use for community health services is inadequate. So we should proposesupply-side reform on communi-ty health service to meet the medical demands for old people and even to stimulate new demand.

Objective The aim of this study is to summarize preverbal communication skills in children with cochlear implants, and to provide a clinical baseline of assessment in early stage after cochlear implantation. Methods 23 prelingually deaf children with homebred cochlear implants participated in this study. The age at cochlear im-plantation ranged from 12 to 66 months with a mean of 35 months. The evaluation was performed at switch-on ses-sion and 1,3,6,9 and 12 months after the first programming session.Tait video analysis was used to assess the preverbal communication skills including turn-taking,autonomy,eye contact and auditory awareness.CAP and SIR were used to evaluate the auditory and speech abilities.ResuIts The skills of turn-taking and auditory aware-ness increased significantly, especially in the first six months after switch-on session (P<0.05).The scores of autonomy and eye contact were relative low and there were no significant differences during the first year after surgery (P>0. 05).There was a correlation between the score of CAP/SIR and turn-taking or auditory awareness.ConcIusion Children with cochlear implants established vocal-auditory mode of early communicative behavior over time.Pre-verbal communication skills CAP and SIR have relativity and can comprehensively evaluate the auditory, speech and communication skills of children with cochlear implants.

OBJECTIVE:
The purpose of this study is to analyze the development of musicality in children after cochlear implantation, and provide a clinical database for the evaluation of their musicality.
METHOD:
Twenty-six children with cochlear implants (CI group) participated in this research. They received cochlear implants at the age of 11 to 68 months with a mean of 35.6 months. Seventy-six infants as a control group aged from 1 to 24 months with a mean of 6.1 months participated in this study, whose hearing were considered normal by passing the case history collection, high-risk registers for hearing loss and hearing screening using DPOAE. The music and young children with CIs: Musicality Rating Scale was used to evaluate their musicality. The evaluation was performed before cochlear implantation and 1, 3, 6, 9, 12, 24 months after cochlear implantation for children with cochlear implants. The evaluation was also performed at 1, 3, 6, 9, 12, 24 months for children with normal hearing.
RESULT:
The mean scores of musicality showed significant improvements with time of CI use for CI group (P<0.05). The mean scores of musicality also showed significant improvements with time for control group (P<0.05). There were no significant differences in mean scores between CI group and control group at 1, 3, 6, 9, 12 months of hearing age by rank sum test (P>0.05). Significant difference was noted between the two groups at 24 months (P<0.05).
CONCLUSION
The musicality of children with cochlear implants improved significantly with time after cochlear implantation. The most rapid growth was found in the first year after cochlear implantation.

【Objective】 To evaluate and analyze the impact of COVID-19 epidemic on inventory of red blood cells (RBCs)in local and municipal blood stations in China, and to provide reference for the management of public health emergencies. 【Methods】 Relevant data from 2018 to 2021 were collected, and the differences in the volume of qualified RBCs, the usage efficiency of inventory RBCs, the average daily distribution of RBCs,the blood distribution rate of RBCs prepared by 400 mL whole blood, the difference in the average storage days of RBCs at the time of distribution, the average daily inventory of RBCs and the time of the average daily inventory of RBCs to maintain the distribution in 24 local and municipal blood stations in China during the COVID-19 epidemic and non-epidemic periods were retrospectively analyzed. 【Results】 Compared with non-epidemic periods, the volume of qualified RBCs [(117 525.979 ±52 203.175)U] and the average daily distribution of RBCs [( 156. 468 ± 70. 186) U ] increased significantly, but the usage efficiency of inventory RBCs decreased(97.24%±0.51%) significantly (P<0.05).There was no significant difference in the blood distribution rate of RBCs prepared by 400 mL whole blood(73.88%±20.30%), the average storage days of RBCs distribution(13.040 ±3.486), the average daily stock quantity of RBCs[(2 280.542 ±1 446.538) U ] and the time of the average daily inventory of RBCs to maintain the distribution[(15.062 ±7.453) d] (P>0.5). 【Conclusion】 During the COVID-19 epidemic, the inventory management of RBCs operated well, the overall inventory remained relatively stable, the stock composition and storage period showed no significant change.
