Necrotizing enterocolitis of newborn (NEC) remains a significant cause of morbidity and mortality in neonatal intensive care units.Although many advances have been made in the understanding of this disease,the pathophysiology remains incompletely clear,and treatment is mainly limited to supportive care.In recent years,studies have focussd on the role of the inflammatory mediators,signaling and their impacts on the disease process.Investigators also made some advances in the fields of intestinal proteome and probiotics.Early breasffeeding and probiotics are promoted to prevent and/or amehorate neonatal NEC.In this artical,we reviewed the advances in the above fields for helping to the pathogenesis and treatment of NEC.

Neonatal necrotizing enterocolitis is a common gastrointestinal disease in neonate.Recently,with the establishment of the NICU and the all-round development of perinatal medicine and pediatrics,survival rate of premature especially very low birth weight neonates have obviously increased and the incidence of the disease is rising year by year.It is generally considered that multiple risk factors such as premature birth,low birth weight,infections,asphyxia,irrational use of drugs and so on,acting on the immature intestine,but the pathogenesis is still unknown.Therefore,it is significant to clearify the pathogenesis of the risk factors of NEC for studying the pathogenesis of the disease and providing prevention and control strategy.

Mitochondrial adenosine triphosphate (ATP) synthase is the key enzyme of mitochondrial oxidative phosphorylation reaction. The down-regulation of the mitochondrial ATP synthase is a hallmark of most human carcinomas, which is the embodiment of the bioenergetic signature of cancer in the performance of the decreased oxidative phosphorylation and increased aerobic glycolysis. Combining with the bioenergetic signature of cancer, studies showed that mitochondrial ATP synthase and multidrug resistance and adverse prognosis of tumor were closely related. Its mechanisms are related to post-transcriptional regulation of the ATP synthase, the hypermethylation of the ATP synthase gene and the inhibitor peptide of the mitochondrial ATP synthase, called ATP synthase inhibitory factor 1 (IF1). In this review, we stress the biological characteristics of mitochondrial ATP synthase and the relationship between ATP synthase and multidrug resistance and prognosis of Malignant tumor, in order to find a new way for tumor therapy.

Objective To determine the influencing factors on prognosis of neonatal necrotizing enterocolitis (NEC) in premature infants with a gestational age of 28-32 weeks.Methods Forty-six cases of NEC (Bell stage Ⅱ or Ⅲ) with a gestational age of 28-32 weeks admitted to Bayi Children's Hospital from January,2009 to January,2013 were analyzed retrospectively.Twenty-nine cases were assigned to the cured group and 17 cases were assigned to the poor prognosis group according to prognosis.General conditions,laboratory results,treatment and complications in the two groups were analyzed.The Chi-square test,Fisher's exact test and univariate analysis of variance were used for statistical analysis.Results There were no statistically significant differences regarding gender,average birth weight and mean age of onset between the two groups [average birth weight (1 410.52±281.59) g vs (1 266.47±280.32) g and mean age of onset:(20.79± 10.61) d vs (16.71 ±9.41) d for the cured group versus the poor prognosis group,respectively].There were no difference in changes in white blood cells and platelets between the two groups.There were six cases of positive blood culture in the poor prognosis group and none in the cured group.There were no differences in procedures such as blood transfusion and ibuprofen administration [41.4％ (12/29) vs 11/17 and 6.9％ (2/29) vs 5/17,both P＞0.05,in the cured group versus the poor prognosis group,respectively].There were significant differences in the use of Bifidobacterium between the two groups [69.0％ (20/29) vs 5/17,x2=6.758,P=0.009].Fourteen cases in the cured group and 10 cases in the poor prognosis group underwent surgery,and all 10 cases in the poor prognosis group died.Seven cases underwent repeated surgery,one infant in the cured group and six infants in the poor prognosis group and a significant difference was observed (Fisher's exact,P=0.007).A statistically significant difference in granulocyte colony-stimulating factor (G-CSF) treatment was observed between the two groups,where 15 cases in the cured group and three cases in the poor prognosis group were treated with G-CSF (x2=5.225,P＜0.05).Statistically significant differences in septicemia,patent ductus arteriosus,gastrointestinal perforation,multiple organ failure (MOF),disseminated intravascular coagulopathy (DIC) and septic shock were observed between the two groups [septicemia:44.8％ (13/29) vs 15/17,x2=8.478; patent ductus arteriosus:17.2％ (5/29) vs 9/17,x2=6.451; gastrointestinal perforation:3.4％ (1/29) vs 6/17; MOF:0.0％ (0/29) vs 5/17; DIC:0.0％ (0/29) vs 3/17; septic shock:3.4％ (1/29) vs 6/17,all P＜0.05 in the cured group versus the poor prognosis group,respectively).Conclusions Oral intestinal microbial preparations before the onset of NEC and G-CSF therapy after the onset of NEC may be protective factors in improving the prognosis of NEC,while patent ductus arteriosus,septicemia,gastrointestinal perforation,MOF,DIC and septic shock are risk factors.Emphasis should be placed on the administration of intestinal microbial agents,prevention of infection and treatment of complications.

Objective To explore the effect of Ulinastatin on proinflammatory cytokines of patients undergoing operation of intracranial aneurysm embolization. Methods 40 patients undergoing operation of intracranial aneurysm embolization were randomly divided into 2 groups (n = 20): ulinastatin group (Group U), normal saline group (Group C). In group U, 6 000 IU/kg Ulinastatin was infused via vein before anesthesia induction, the group C received the injection of isovolume normal saline. Exsanguinated arterial blood at T1, T2, T3, T4 and T5 five time points were taken to measure TNF-α and IL-6 in serum. Results The contents of TNF-α and IL-6 in serum both increased at T3, T4 and T5 compared to that at T1 in both groups; both TNF-α and IL-6 significantly decreased in Group U compared to that in Group C at the same time point. Conclusion Ulinastatin may inhibit the release of inflammatory mediators and lighten cerebral ischemia-reperfusion injury in patients undergoing operation of intracranial aneurysm embolization.

Objective To isolate CESs by dynabeads coated with the specific antibody of CD146 from peripheral blood and its origin was identified.Methods One mL blood with or without admixed HUVECs was diluted(1∶2) in PBS-0.1% BSA.Anti-CD146-coated Dynabeads were added and incubated for 30mins at(4 ℃).Cells bound to anti-CD146-coupled beads were separated from blood in Dynal MPC and then washed and resuspended in(4 mL) buffer.After 4 additional washes with PBS-0.1% BSA using the magnet,the rosetted cells were flushed from the tube wall with (100 ?L) of PBS-BSA with acridine orange or Giemsa,and counted in a hemocytometer in a inverse phase contrast fluorescence microscope.Results The amount of CECs in healthy adult was 10.5(6～16.5)/mL(n=42).The recovery rate was 91%.Isolated CECs were confirmed by positive expression of vWF and CD31.Conclusion Isolation of CECs from blood by immunomagnetic beads coated with antibody of CD146 is characterized by its accuracy,time-saving,high recovery rate,less contaimination of blood and less damage to CECs.It can be used to quantify CECs and to analyze the functional performance of CECs from patients with vascular injury diseases.

Objective To test the drug susceptibilities in Mycobacterium tuberculosis by microscopic observation drug susceptibility(MODS)and evaluate the method for the detection susceptibility in second-line drugs.Methods To set up the MODS method.the drug susceptibilities of 4 second-line drugs in Mycobacterium tuberculosis were tested in 24 well plates.The test conditions were discussed.The resistance to protionamide(PTH),amikacin(AMK),capreomycin(CPM)and levofloxacin(LVF)of 60 MTB isolated from 2007 to 2008 in Shanghai Pulmonary Hospital were detected and compared with that of L-J method.The isolates were tested by minimum inhibitory concentration(MIC)method when the drug susceptibilities were not consistent.Results Among the 60 Mycobacterium tuberculosis clinical isolates.the results of drug susceptibility were confirmed by MODS,absolute concentration method and the accordance rate of PTH,AMK,CPM and LVF were 96.7%(58/60),98.3%(59/60),91.7%(55/60)and 96.7%(58/60),respectively.If the result of absolute concentration method was as the standard.the sensitivity.specificity.positive and negative predictive value as well as accuracy were 100%,87.5%,87.5%,100%and 96.7%in PTH;100%,90.0%,90.0%,100%and 98.3% in AMK;76.9%,95.7%,83.3%,93.8%,91.7% in CPM;100%,96.0%,83.3%,100% and 96.7% in LVF respectively detected by MODS assay.Conclusion MODS is a rapid and simple method for susceptibility testing of second-line drug in Mycobacterium tuberculosis.

Objective To investigate the ultrasound diagnostic value for nipple discharge disease by comparison with fiberoptic ductoscopy. Methods Fiberoptic ductoscopy and ultrasonic inspection were performed on fifty-seven patients with spontaneous nipple discharge,and the results were analysed. Results In all patients, 55 cases (96. 49 % ) were abnormal in high-frequency ultrasound inspection:27 mammary duct expansion(47.36%),19 occupying lesion with mammary duct expansion or cyst (33.33%),6 cystic echo ( 10. 52% ) ,3 only solid echopattern (5.26%) ,and 2 nothing found (3.51% ). In ultrasonic inspection,color and pulse Doppler signal were found in 7 cases. On the other hand, 26 galactophoritis (45.61%), 12mammary duct ectasia (21.05 %), 17 intraductal papilloma (29.82%), 2 breast carcinoma (3.50%) were diagnosed by fiberoptic ductoscopy. In all 19 cases with occupying lesion,ultrasonography's sensitivity was 52. 63% ,specificity was 89. 47%, misdiagnosis rate was 10. 50%, rate of missed diagnosis was 47.36%.Conclusions To nipple discharge disease, high-frequency ultrasound has high sensitivity, easy to find abnormal indication,but is not accurate enough in the diagnosis of smaller occupying lesion.

Objective To investigate the changes in trafficking of GluRl-containing AMPA (GluR1-AMPA) receptor and GluR2-AMPA receptor from cytoplasm to cell membrane in the spinal cord dorsal horn in a rat model of incisional pain.Methods Thirty-two adult male SD rats aged 6-8 weeks weighing 280-300 g were randomly divided into 2 groups:control group (group C,n =8) and incisional pain group (group Ⅰ,n =24).An 1 cm long incision was made in the plautar surface of right hindpaw according to Brennan et al.in group Ⅰ.Cumulative pain score (CPS) and paw-withdrawal threshold to yon Frey stimuli (PWT) were measured at 3 h and day 1 and 3 afar incision ( T1,2,3 ).The animals were sacrificed after pain behavior assessment.Their lumbar segments of the spinal cord (L3-6) were removed.The expression of GluR1 and GluR2 in cell membrane and cytoplasm in spinal cord dorsal horn was determined by Western blot analysis.The co-expression of Stargazing with GluR1 and GluR2 in the spinal cord dorsal horn was examined by co-immuno-precipitation.Results The CPS was increased and PWT decreased; the GluR1 expression in cytoplasm was decreased while the expression of GluR1 in cell membrane and the co-expression of Stargazing with GluR1 were up-regulated in group Ⅰ as compared with group C.There was no significant change in the expression of GluR2 in cytoplasm and cell membrane and the co-expression of Stargazing with GluR2 in group Ⅰ as compared with group C.Conclusion GluR1-AMPA receptor transfers from cytoplasm to cell membrane but GluR2-AMPA receptor does not in rats with incisional pain.
