1.Effects of Tiaojing Zhuyun Capsules on the Number of Blastocyst Implantation Sites,Estrogen/Progesto-gen and Their Receptors in Mice with Implantation Dysfunction

OBJECTIVE:To study the effects of Tiaojing zhuyun capsules on the number of blastocyst implantation,estrogen, progestogen and their receptors(ER,PR)in mice with implantation dysfunction. METHODS:48 pregnant mice were randomly di-vided into normal control group (normal saline),pathological model group (normal saline) and Tiaojing zhuyun capsules group (24 g/kg),with 16 mice in each group. They were given relevant medicine intragastrically,once a day,for consecutive 3 d. On the 4th day,those mice were given mifepristone 0.1 ml subcutaneously to induce implantation dysfunction model except for normal control group. On the 5th day,the mice were sacrificed. The number of blastocyst implantation sites was observed. The serum lev-els of estradiol (E2) and progesterone (P) were determined by radioimmunoassay,and the expression of ER and PR protein were determined by immunohistochemistry. RESULTS:On the 5th day of pregnancy,the number of embryo implantation sites,serum levels of E2 and P and the protein expression of ER and PR in pathological model group were significantly lower than in normal con-trol group(P<0.05);the number of embryo implantation sites,levels of E2 and P,the protein expression of ER and PR in Tiao-jing zhuyun capsules group were significantly higher than in pathological model group (P<0.05). CONCLUSIONS:Tiaojing zhuyun capsules can effectively improve the serum levels of E2 and P,the protein expression of ER and PR,and increase the num-ber of blastocyst implantation sites in mice.
2.Risk-Treatment Paradox in Acute Coronary Syndrome

Objective This study aimed to assess the adherence to guideline-recommended therapies according to risk stratification in the management of acute coronary syndrome(ACS). Methods We analyzed 1,001 consecutive patients admitted with ACS. Patients were stratified using the GRACE risk score into low- and high-predicted risk of mortality at 6 months. We evaluated the use of hospital angiography,revascularization,and guideline-recommended medications between high and low-risk patients. Results High-risk compared to low-risk patients were less likely to underwent coronary angiography and/or revascularization during the hospitalization. The use of hospital-initiated pharmacotherapies was also lower in high-risk patients(P<0.05). Advanced age, increased creatinine level and higher GRACE score were independent predictors for failure to administer evidence-based therapies. Conclusion Patients with ACS at high risk of mortality were paradoxically less likely to undergo revascularization or receive medications according to guidelines. Better adherence to evidence-based therapies in high-risk patients may improve clinical outcome and quality of health care.
3.Experimental methodology of simultaneous determination of carbamazepine, phenytoin, and phenobarbital in serum by high-performance liquid chromatography

Objective To establish a high-performance liquid chromatography (HPLC) method with diode array detection to simultaneously determine carbamazepine,phenytoin,and phenobarbital in serum.Methods Extraction solvent (800μl ethylene acetate) and sample (0.2 ml) was mixed,extracted for 2 min,and centrifuged (3500 r/min,4 minutes).A volume (600 μl) of extract liquor was volatilized to dryness in water bath with the volatilization temperature 75 ℃,then was redissolved with 1.0 ml mobile phase.Analysis conditions was column temperature 30°,mobile phase (methanol∶ water =40∶60),and detection wavelength of 254 nm.Three metabolites were effectively separated.Results Under the optimized condition,calibration curves of three metabolites were linear in the ranges of (1.52 ~ 120 mg/L) and the correlation coefficients were not less than 0.999.The detection limits (S/N =3) were in the range of 0.4 ~ 1.5 mg/L.The spiked recoveries were in the range of 91.3% ~ 111% with relative standard deviations (RSD) less than 5%.Conclusions The optimal pretreatment condition for the sample was established.The chromatographic separation and the detection condition were optimized.The method was sensitive and accurate,and could meet the need of monitoring serum drug concentration.
4.The impact of head cooling on regional homogeneity during passive hyperthermia

Objective To explore the impact and protective mechanisms of head cooling on neural activity during passive hyperthermia.Methods Sixteen subjects were randomly exposed for 1 h to three different conditions: normal (25℃), hot (50℃) and head cooling (chamber:50℃,cold packs:5℃),after environment exposure, rs-fMRI were performed.Regional homogeneity(ReHo) datum at three different conditions were analyzed by REST2.0 to obtain brain areas with statistical difference.Brain voxel with statistical difference were selected as ROIs to ReHo values and were analyzed by One-Way ANOVA with SPSS18.0.Neural activity of brain areas with statistical difference were compared in any two groups by Post hoc.Results The brain regions showing differences among three groups included right orbital frontal cortex,left middle frontal gyrus,bilateral amygdala,left middle temporal gyrus,left hippocampus,bilateral parietal inferior, left precentral gyrus.Compared with normal group, ReHo increased in right orbital frontal cortex, and decreased in left precentral gyrus,left middle frontal gyrus,left parietal inferior,but no changed in bilateral amygdala,left middle temporal gyrus,left hippocampus,right parietal inferior in head cooling group.Compared with hot group,head cooling group showed increased ReHo in left middle temporal gyrus,left hippocampus,right parietal inferior,and decreased ReHo in bilateral amygdala,left parietal inferior,unchanged ReHo in right orbital frontal cortex, left precentral gyrus, left middle frontal gyrus.Conclusion The specified alterations of ReHo may reflect that the head cooling could partially eliminate the impact of passive hyperthermia, and is closely linked with emotional function.
5.Study on correlation between OCT for detecting characteristics of coronary artery plaque and matrix metalloproteinase

Objective To apply the optical coherence tomography(OCT) to detect the characteristics of coronary artery plaque and to investigate its correlation with levels of serum matrix metalloproteinase 7(MMP 7),MMP9 and MMP12.Methods The patients undergoing coronary arterial angiography for diagnosing coronary arterial lesions in the cardiology department of our hospital from October 2014 to March 2016 were collected and included into the research subjects.The subjects were divided into the stable plaque group and unstable plaque group based on the results of OCT scanning.The neovascularization characteristics such as the fibrous cap thickness of plaque,angle of lipid pool,macrophage infiltration and plaque cracks were detected by using OCT.ELISA was used to measure serum MMP7,MMP9 and MMP12 levels.Results (1) The fibrous cap thickness in the stable plaque group was more than that in the unstable plaque group(P<0.01);the lipid pool angle,microphage infiltration,intima erosion and plaque cracks in the unstable plaque group were more than those in the stable plaque group(P<0.05).(2) The MMP7 and MMP9 levels in the unstable plaque group were higher than those in the stable plaque group and control group(P<0.05).(3) The fibrous cap thickness had significantly negative correlation with serum MMP9 level(r=-0.336,P=0.034);the MMP7 and MMP9 levels in the microphage infiltration group were higher than those in the non-microphage infiltration group(P<0.05);the MMP9 level in the intima erosion group was higher than that in the non-intima erosion group(P<0.01).Conclusion OCT can detect and find unstable plaque and the serum levels of MMP7 and MMP9 are significantly elevated in the patients with unstable plaque,which can be used as an important basis for predicting unstable plaque and guiding the treatment decisions.
6.Diagnosis and treatment of nasal sinus osteoma.

OBJECTIVE:
To discuss the clinical characteristic and surgical treatment of nasal sinus osteoma.
METHOD:
Fifty-one cases with nasal sinus osteoma were diagnosed by nasal sinus X ray or CT scan. All of these osteoma patients were cured by surgical resection. Supraorbital Lynch approach were applied in forty-two cases, lateral rhinotomy Moure approach in 4 cases, Caldwell-Luc approach in 3 cases and Dandy approach that combined with cranium and facial route in 2 cases.
RESULT:
Operative incisions in all cases healed in 1 period, no complications such as brain spinal fistula occurred. All cases were followed up for 6 months to 5 years, there were no recurrence of osteomas and occurrence of mucus cysts.
CONCLUSION
Osteomas in nasal sinus grow slowly. X ray or CT scan is helpful to confirm the diagnosis and choose the surgical approach reasonably.

7.Dosimetric effect of multi-leaf collimator transmission on intensity-modulated radiation therapy

Objective To investigate the dosimetric effect of multi-leaf collimator (MLC)transmission on intensity-modulated radiation therapy (IMRT).Methods MLC transmission through theleaves and rounded ends were measured with ion chamber for Varian Trilogy linear accelerator with the X-ray of 6 MV and 10 MV.The intraleaf and interleaf transmission were also measured with the electronic portal imaging device of aS1000.10 tumor patients treated with IMRT were used to evaluate the MLC parameters modeling in the Eclipse treatment planning system.Results The average transmissions of Millennium MLC were 1.6% and 1.8% for 6 MV and 10 MV X-rays.The transmission increased with the field size and depth of measurement.The transmissions at off axis position were a little lower than those on the central axis.The intraleaf transmissions measured with aS1000 were 0.8%-1.2% and 1.1%-1.6%,the interleaf transmission were 1.3%-1.9% and 1.6%-2.5% for 6 MV and 10 MV X-rays respectively.Modeling with the measured MLC parameters,the Eclipse treatment planning system could calculate the dose distribution accurately.The γ pass rate at 3% 3 mm was above 95%,except for two patients with nasopharyngeal cancer with the pass rate of 93.6% and 94.5%.Conclusions The transmissions through the MLC leaves and the leaf ends contribute to the dose throughout the target significantly for IMRT.MLC transmission varied with the field size,depth of measurement and off axis position.
8.Efficacy and dosimetry of computed tomography image-guided 125 I radioactive seed implantation for locally recurrent rectal cancer

Objective To evaluate the efficacy of computed tomography ( CT ) image?guided 125 I radioactive seed implantation for locally recurrent rectal cancer ( LRRC ) , and to analyze the relationship between the dosimetry and prognosis. Methods A retrospective analysis was performed on the clinical data of 36 patients with LRRC who received CT image?guided 125 I seed implantation in our hospital from 2003 to 2011. Dosimetric verification was performed using CT scan immediately after 125 I seed implantation. The D90 , D100 , V100 , and V150 values were evaluated. In all the patients, the median activity of seeds was 0?7 mCi (0?4?0?8 mCi) and the median number of implanted seeds was 74(33?137). The local control (LC) and overall survival ( OS ) rates were calculated using the Kaplan?Meier method. The log?rank test and Cox regression model were used for the univariate and multivariate analyses, respectively. Results The median OS time was 16?2 months ( 95% CI= 13?5?18?9 months ) . The median LC time was 10?0 months (95% CI=6?2?13?8 months). The D90 and V100 values were (118.6±25?1) Gy and (90.0±0?3)%, respectively. The univariate analysis suggested that D90 was correlated with the LC time ( P=0?048) and V100 was correlated with the OS time ( P=0?035) . The multivariate analysis showed that a V100 value higher than 90% was a prognostic factor of OS (P=0?044). Conclusions In the treatment of LRRC using CT image?guided 125 I radioactive seed implantation, a D90 value larger than 140 Gy and a V100 value higher than 90% in the postoperative verification plan help improve the LC and OS rates. The D90 and V100 values in the postoperative verification plan may predict treatment outcomes in patients.
9.Surgical treatment of differentiated thyroid carcinoma with laryngeal or tracheal invasion.

OBJECTIVE:
To analyze the clinical characters, surgical treatments and prognosis of differentiated thyroid carcinoma (DTC) with laryngeal or tracheal invasion.
METHOD:
Forty-six cases including 33 papillary and 13 follicular were retrospectively reviewed. Thirty-four cases with tracheal invasion received conservative resection (17), window resection (11) and sleeve resection (6). One case with subglottic invasion was performed subglottic partial laryngectomy. Eleven cases with trachea-laryngeal invasion received total laryngectomy(4), 3 shave technique and 4 preservative laryngectomy.
RESULT
Complications included infections and hypocalcaemia, 15 patients got permanent fistula. 5-year survival rate in cases of tracheal shave resection was 88.2%, while 63.6% in those of window resection and 83.3% in those of sleeve resection. Survival rate within 5 years of patients received preservative laryngectomy was 62. 5%. Conclusion: With meticulous preoperative examination and positive surgical treatment, both survival rate and quality of life could be improved in patients of DTC with laryngeal or tracheal invasion.
