1.Research in the pudendal nerve block anesthesia timing during second stage of labor

Objective To discuss the influence of pudendal nerve block anesthesia on pain degree and labor course when presentation at different positions during the second stage of labor,in order to find the better anesthesia timing and method to reduce pain,shorten labor,increase maternal comfort to a maximum extent and provide the best service for the maternal.Methods 200 cases of primiparas with fullterm and single-birth were selected and numbered randomly,patients with odd numbers were set to the observation group,patients with even numbers were set to the control group,100 cases in each group.In the observation group,when the presentation reached S+1,1% lidocaine was used to perform pudendal nerve block anesthesia.In the control group,the pudendal nerve block anesthesia was performed before perineal incision.Results The pain intensity,time of the second labor stage,perineum damage degree between the two groups were statistically different.The observation group with Ⅱ-grade level,Ⅲ-grade level pain were 32 cases less than that of the control group.The perineum incision later crack number in the observation group was 24 cases less than that of the control group.The number of delivery within 1 hour in the second labor process in the observation group was 21 cases more than that in the control group.Conclusions Implementation of bilateral pudendal nerve block anesthesia in the second stage of labor when the presentation reaches S+1 can alleviate childbirth pain,shorten the second stage of labor,reduce the degree of perineal trauma.

3.Reasonable Application of Multi-enzyme Abluent in Process of Flexible Endoscope Cleaning and Disinfecting

OBJECTIVE To discuss the reasonable application of multi-enzyme abluent in the process of endoscopic cleaning and disinfecting.METHODS The multi-enzyme abluent was reasonably used to clean the endoscope in the pre-cleaning and disinfection process at thebedside,to enable it to achieve the best cleaning effect.RESULTS The reasonable application of multi-enzyme abluent could enhance the effect of endoscope cleaning to ensure the success of endoscopic disinfection.CONCLUSIONS Multi-enzyme abluent can protect the successful endoscopic disinfection and prevent the iatrogenic infection.
4.Detection and separation of side population cells in haematological tumor cell lines

BACKGROUND:Side population(SP)cells,with varied contents,are widely distributed in adult tissues,embryos,and certain tumor cells.Haematological tumor is one of the main pathological conditions,which endangers human life.Thus,it is important to recognize SP cells in haematological tumor cell lines.OBJECTIVE:To identify whether hematologic tumor cell lines contain SP cells,and to explore a stable detection and separation methods.METHODS:Cells with the characteristics of stem cells being capable of fluorescent dye Hoechst33342 were isolated by flow cytometry;whether there were SP cells in logarithmic growth period of NB4,Raji,K562/ADM and K562 or not were detected.After sorting K562 subpopulations,the purity of sorted cells was detected.RESULTS AND CONCLUSION:After Hoechst33342 staining,flow cytometry results showed that the SP cells appeared in the haematological tumor NB4,Raji,K562/ADM and K562 cell lines,which accounted for 0.8%,2.7%,1.3% and 2.7%,respectively.These cells could be blocked by Verapamil.The purity was greater after a second detection of SP and Non SP cells in K562 cells.
5.The association of TESTIN and Caspase-3 protein expressions with clinicopathological features and prognosis of esophageal squamous cell carcinomas

Objective To investigate the expressions of TESTIN gene and Caspase-3 protein and thier relations with the clinicopathological features and prognosis of esophageal squamous cell carcinomas(ESCC). Methods The expressions of TESTIN and Caspase-3 in 50 ESCC tissues and paracancerous tissues (>5 cm apart form the ESCC tissue) were detected by immunohistochemistry.The expression of TESTIN in 65 matched-pairs of ESCC tissues was detected by Western blotting and RT-PCR. The association of TESTIN and Caspase-3 with clinicopathological features and prognosis of ESCC were analyzed. Results The immunohistochemistry examination showed that the positive rates of TESTIN protein [30. 0% (15/50)] and Caspase-3 protein [24.0% ( 12/50)] were significantly lower in ESCC tissues than those in paracancerous tissues [(84. 0% (42/50) and 94. 0% (47/50),respectively, P<0.01)]. The mRNA level of TESTIN was down-regulated in ESCC tissues (P<0.05). Whereas the protein level of TESTIN was down-regulated in 45 (69.2%) of 65 ESCC tissues in comparison with paracancerous tissues. TESTIN expression was positively correlated with the differentiation of ESCC, but not associated with gender, age, tumor size, TNM stage and lymph node metastasis. There was a positive correlation between TESTIN and Caspase-3 in protein expressions (P<0. 05). Patients with negative expression of TESTIN had lower survival rate compared to those with positive expression (P<0. 05). Conclusions The positive relation between low-expression of TESTIN and Caspase-3 implicates that both are involved in the development of esophageal squamous cell carcinogenesis, and TESTIN might be a novel ESCC marker with prognostic significance.
6.Endoscopic full-thickness resection for gastric stromal tumor

ObjectiveTo evaluate the therapeutic effect of endoscopic full-thickness resection (EFTR) for gastric stromal tumors.MethodsA total of 33 patients with gastric stromal tumor orgination from deep muscularis propria layer received EFTR from January 2010 to July 2011.The effectiveness and safety of EFTR were compared with those of other 34 patients with gastric stromal tumor origination from muscularis propria layer who underwent endoscopic submucosal dissection (ESD).ResultsExcept in 2 patients with lesions larger than 3.0 × 3.0 cm,EFTR was successful in others 31 patients,who recovered well and had no recurrence during the follow-up within 12 months.There were no significant differences in resection rate,incidence of complications,body temperature,white blood cell counts or recovery time between 2 procedures (P > 0.05 ).However,the number of clips used in EFTR ( 7.0 ± 3.5 vs.4.9 ± 3.1,P =0.013 ) and postoperative fasting days (3.4 ± 1.5 vs.2.0 ± 1.0,P =0.001 ) were significantly higher than those of ESD procedures.ConclusionEFTR is effective and safe for gastric stromal tumors with no higher risk than ESD,but it is more complex technically.EFTR can be used as an expanding method of ESD in endoscopic treatment of gastric stromal tumors.
7.Community intervention in management of ankylosing spondylitis

Sixty five patients with of ankylosing spondylitis were randomly divided into intervention group (n =32) and control group (n =33).Patients in control group received conventional treatment only ;in addition to conventional treatment,patients in intervention group also received community intervention,including health education,psychological counseling and joint function exercise.The treatment lasted for 6 months,29 patients in intervention group and 28 in control group completed the trial.The results showed that the spinal motion (flexion,extension,lateral bending),chest expantion,finger to floor distance,occiput to wall gap in intervention group were better improved than those in control group after treatment (P < 0.05).The scores of self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were lower in intervention group than those in control group after treatment (P < 0.05).The results indicate that community intervention can effectively improve spinal mobility,reduce spinal deformity; relieve the anxiety and depression and improve the quality of life for patients with ankylosing spondylitis.
8.Determination of dexamethasone in human plasma by solid phase extraction with ultra performance liquid chromatography-tandem mass spectrometer

Objective To develop the analytical method to determine the content of dexamethasone in human plasma by solid phase extraction with ultra performance liquid chromatography-tandem mass spectrometer. Methods The human plasma was extracted with a solid phase extraction(SPE) and determined by UPLC-MS/MS. LC-MS/MS was performed in ESI source with MRM mode for quantification. Results The lowest detectable limit was 0.05ng/mL, the linear range was 1~100ng/mL. The absolute recovery was more than 78.1%. The intra- and inter day precision was within 15% at three concentrations. Conclusion Since the procedure proved to be simple, quick and effective, it could be used for the determination of dexamethasone in human plasma.
9.Curative Effect of Micro Plasma on Facial Acne Atrophic Scar

Objective To observe the clinical efficacy of micro plasma in the treatment of acne atrophic scar on face.Methods 82 cases with facial acne atrophic scar were randomly divided into the observation group and the control group.Each group had 41 cases.The observation group received micro plasma treatment while the control group were treated with ultrapulsed fractional CO2.The treatment was carried out 6 weeks interval, in total 3 times.After each treatment, the curative effect and the side effects were compared between the two groups.Results The total effective rates had no statistical difference between two groups, and the more treatment times, the better effect.The observation group had less adverse effects, which the duration of main side effects was shorter in the observation group than in the control group, with statistically significant difference.Conclusion The micro plasma technology on treating facial acne atrophic scar was an effective skill with less adverse reactions and worth of wide use.
10.Efficacy and safety of glucocorticoids for prevention of esophageal stricture after endoscopic submucosal dissection

Objective:To evaluate the efficacy and safety of oral prednisone acetate and oral prednisone acetate combined with local injection of triamcinolone acetonide for the prevention of esophageal stricture after endoscopic submucosal dissection (ESD) for early esophageal cancer and precancerous lesions.Methods:Data of 52 patients with early esophageal cancer or precancerous lesions hospitalized from December 2014 to February 2019 in Zhongda Hospital of Southeast University were analyzed retrospectively. They were divided into the control group (group A, n=20), oral hormone group (group B, n=17) and oral medication combined with local injection group (group C, n=15). The rates of stenosis and refractory stenosis, endoscopic dilatation times after ESD, time interval of first dilation after ESD and adverse events related to the operation and administration of glucocorticoids were compared among the three groups. Results:The differences in stenosis rates [85.0% (17/20) VS 47.1% (8/17) VS 46.7% (7/15), P<0.01], rates of refractory stenosis [75.0% (15/20) VS 23.5% (4/17) VS 20.0% (3/15), P<0.01], and endoscopic dilatation times [3.50 (2.25, 6.00) VS 0.00 (0.00, 2.50) VS 0.00 (0.00, 2.00), P<0.01] were statistically significant among the three groups. In pairwise comparison, the above indicators in group B and group C were significantly lower or less than those in group A ( P<0.05), but there were no statistical differences between group B and group C ( P>0.05). The time intervals of the first dilation after ESD were significantly different among the three groups (27.7±9.4 d VS 110.1±46.0 d VS 147.4±9.4 d, P<0.01). In pairwise comparison, the first dilation intervals in group B and group C were longer than that in group A ( P<0.01), and this indicator in group C was longer than that in group B ( P<0.01). Two patients in the control group developed perforation after ESD but they were treated actively and recovered finally. No severe ESD, steroid, or perforation related adverse events occurred in any other patient of the three groups. Conclusion:Both oral prednisone and oral prednisone combined with local injection of triamcinolone acetonide after ESD can effectively and safely prevent esophageal stricture after ESD. Oral administration combined with local injection of corticosteroids can prolong the time interval of the first dilation after ESD, which is beneficial to the psychological recovery and the improvement of patients’ life quality after the operation.
