1.Effects of low-dose ketamine on minimum alveolar concentration of sevoflurane in patients undegoing gynecological abdominal surgery

Objective To investigate the effects of the low-dose ketamine on the minimum alveolar concentration (MAC) of sevoflurane in patients undergoing gynecological abdominal surgery.Methods Fifty-one ASA Ⅰ or Ⅱ patients aged 36-49 yr with body mass index ≤ 30 kg/m2 scheduled for gynecological abdominal surgery were randomly divided into control group (group C, n = 29) and ketamine group (group K, n = 22) . The paitents were unpremedicated. In group K, a loading dose of ketamine 0.2 mg/kg was injected intravenously followed by infusion at a rate of 14 μg·kg-1 ·min-1 , while equal volume of normal saline was given in group C. Anesthesia was induced with inhalation of sevoflurane (end-tidal concentration 4%-5%, maintaining for 5 min) in both groups. Endotracheal intubation was performed. The patients breathed sevoflurane until the predetermined target end-tidal sevoflurane concentration was reached for at least 15 min before skin incision. Occurrence of body movement was determined by any visible contraction of the muscle bundle of trunk, limbs, head and neck during skin incision and/or within 60 s after skin incision. The initial end-tidal concentration of sevoflurane was set at 1.8 % .If body movement occurred, the end-tidal concentration of sevoflurane was increased by 0.2% , while if not, decreased by 0.2% . MACs of sevoflurane were calculated. Results The MAC of sevoflurane was 1.51% (95% confidence interval 1.45%-1.58%) in group K, and 2.49% (95% confidence interval 2.40%-2.57%) in group C, and there was significant difference between the two groups ( P < 0.05) . Conclusion Intravenous infusion of the low-dose ketamine can enhance the anesthetic effect of sevoflurane in patients undergoing gynecological abdominal surgery.
2.Endothelial dysfunction in young pre-hypertension and its influencing factors

Objective To investigate the endothelial dysfunction in pre-hypertension and its influencing factors.Methods A total of 373 youth were divided as the subjects into hypertension group (HBP group),prehypertension group (PHT group) and normal blood pressure group (NBP group).Endothelial function was assessed based on carotid intima-media thickness (IMT),brachial artery flow-mediated dilation (FMD) and brachial-ankle pulse wave velocity (baPWV).Results IMT and baPWV in PHT group were higher than those in NBP group (P<0.05),but did not reach the significant difference when compared with HBP group (P>0.05).Compared with HBP,the levels of FMD in PHT group significantly increased (P< 0.05);however,no difference was observed in comparison with NBP group (P>0.05).In the early stage of hypertension,diastolic BP (β=-0.120,P<0.05) and body mass index (β=-0.115,P<0.05) were negatively correlated with FMD;diastolic BP (β=0.146,P<0.05),2-hour glucose (β=0.147,P<0.05),high-density lipoprotein cholestrol (β=0.150,P<0.05),and waist-hip ratio (β=0.126,P<0.05) showed a positive correlation with IMT.baPWV was correlated with systolicBP (β=0.358,P<0.01),waist circumference (β=0.254,P<0.05),fasting glucose (β=0.155,P<0.05),postprandial 2 h blood glucose (β =0.152,P <0.05),uric acid (β =0.206,P < 0.05),and C-reactive protein (β=0.099,P<0.05).Corclusion Our study shows that endothelial dysfunction may exist in the prehypertensive young,and several cardiovascular risks contribute to its development in the early stage of hypertension.
3.Quantile regression analysis of health-related quality of life of rural residents in Shaanxi and its associated factors

Objective This study aimed to apply quantile regression to study Hanzhong rural residents health survey data,explore the local distribution characteristics of health-related quality of life (HRQOL) and influencing factors and present the value of quantile regression applying in analysis of HRQOL.Methods In this cross-sectional population-based study,we evaluated the HRQOL of 2 737 subjects through filling Short-Form Health Survey (SF-36).Quantile regression model was used to compare MCS and PCS scores and evaluate the associated factors.Results With different quantiles MCS and PCS score,the associated factors and influence degree were different.In general,the influences of marital status,educational level,physical activity,history of disease and HRQOL in the part of the percentile scores were significant.Conclusion Analysis of the distribution of HRQOL of rural residents in Hanzhong and influencing factors would benefit the improvement of HRQOL of local residents.
4.Relationship between hypoadiponectinemia and hypertension in young individuals with normal renal function: Hanzhong adolescent hypertension study

【Objective】 To explore the relationship between plasma adiponectin levels and hypertension based on the Hanzhong adolescent hypertension cohort. 【Methods】 A total of 361 young individuals with normal renal function, aged 32 to 41 years old, were taken as the research subjects. We collected or measured data including general characteristics, blood pressure (BP), height, weight, pulse rate, and biochemistry indexes such as fasting glucose, blood lipid and plasma hs-CRP. The concentration of plasma adiponectin was determined by ELISA method. Binary logistic regression was performed to analyze the relationship between plasma adiponectin and hypertension. 【Results】 The plasma adiponectin level in patients with hypertension was significantly lower than that in normotensive patients [3.56 (2.57-5.02)) μg/mL vs. 4.82 (3.19-6.89) μg/mL, P=0.012]. Partial correlation analysis showed a weak correlation of plasma adiponectin level with systolic BP and diastolic BP (r=-0.155, P=0.003 and r=-0.144, P=0.006). Binary logistic regression analysis showed that after adjusting for confounding factors such as age, BMI, and high-sensitivity C-reactive protein, the risk of hypertension was 2.46 times higher in patients with plasma adiponectin in the lowest gender-specific tertile than those in the highest tertile (OR=2.46, 95% CI: 1.22-4.99). 【Conclusion】 Hypoaponectinemia is an independent risk factor for hypertension in young individuals with normal renal function.
