Journal of Clinical Pediatrics 2017;35(1):24-27
Objective To explore the features of nerve electrophysiology in Miller-Fisher syndrome (MFS) in children. Methods Thirty-four children with MFS were selected. The examinations of routine motor sensory nerve conduction,H reflexes of soleus muscle and F wave of median and tibial nerevs were performed. At the same time,33 healthy children of the same age were selected as control group and 30 children with typical Guillain-Barre syndrome (GBS) selected as GBS group, which were compared with children with MFS.Results In children with MFS,routine motor nerve conduction velocity and amplitude, and sensory nerve conduction velocity were in normal range.Twelve cases (35.29%) showed a decrease in sensory nerve amplitude potential (SNAP),5 cases (14.71%) showed sural sparing phenomena (the decline degrees of SNAP in median or ulnar nerve greater than in sural nerve),22 cases (64.71%) showed abnormality of H reflex, and 12 cases (35.29%) showed the decline of SNAE Compared with control group, there was no difference in sensory nerve conduction velocity and SNAP in sural nerve in children with MFS (P<0.05);While there were significant decreases in SNAP in both median nerve and ulnar nerve (P<0.05).Compared with GBS group, children with MFS had significantly higher sensory conduction amplitude and faster velocity (P<0.01).Conclusion A decrease of terminal sensory nerve conduction amplitude could be found in children with MFS,but, the SNAP had limited impact on it.H reflex abnormality is the most common electrophysiologic abnormalities in MFS.

Objective To investigate the probability and timing of childhood Miller-Fisher syndrome (MFS) progressing to Bickerstaff brainstem encephalitis (BBE), classical Guillain-Barre syndrome (GBS), and pharyngeal-cervical-brachial (PCB-GBS). Methods The clinical data of 128 children with confirmed MFS diagnosis were retrospectively analyzed. Results Among 128 children, 60 cases were simple MFS (ocular muscle paralysis, ataxia, reflexes diminished or disappeared, without limbs weakness and lethargy; laboratory tests suggest cerebrospinal fluid protein-cell separation and/or serum anti-GQ1b antibody positive), 28 cases developed MFS/PCB-GBS (met MFS diagnosis criteria, accompanied by weakness of pharynx, neck and upper limb, weakened or disappeared of upper limb reflex, without weakness of lower limb), 22 cases developed MFS/GBS (met MFS diagnosis criteria, accompanied by weakness of limb), 18 cases developed MFS/BBE (met MFS diagnosis criteria, accompanied by lethargy, pyramidal tract positive). There were no differences in the age at onset, the interval from onset to the start of the treatment, Hughes functional grading, and the percentage of cases having a history of preceding infections, the rate of positive serum anti-GQ1b antibody, the ratio of albumin cytological dissociation in cerebrospinal fluid among 4 groups (P>0.05). The interval from MFS onset to progression to MFS/PCB-GBS, MFS/GBS, or MFS/BBE was within 10 days. Conclusions In children with MFS, 50% developed PCB-GBS, GBS, or BBE, which occurred within 10 days after onset. Clinicians should pay attention to the time window and adjust the medicine rationally.

BACKGROUND:A series of inflammatory signal pathways are activated accompanied by increasing expressions of inflammatory factors after periodontal tissues being stimulated by exogenous substances like bacterium. Inflammation is closely related to periodontal tissue repair and regeneration.
OBJECTIVE:To il ustrate the correlation of immune response of periodontitis and periodontitis-related inflammatory cytokines to p38 mitogen activated protein kinase (p38MAPK) pathway, and to provide a new idea and method for the treatment of periodontitis in the future.
METHODS:The related literatures of periodontitis and p38MAPK pathway published from January 1990 to January 2016 were retrieved from the databases of CBM, CNKI, SWIC and PubMed. The research and progress of periodontal tissue repair and regeneration after periodontitis were analyzed.
RESULTS AND CONCLUSION:Total y 36 literatures were enrol ed final y. Inflammation-regulated and inflammation-associated signaling pathways as wel as subsequent expression of inflammatory mediators can partly control the excessive disease-induced inflammation and immune responses of the host, among which p38MAPK signaling pathway may be involved in the occurrence and development of periodontitis. More studies, however, are needed to validate these findings. The regulation of p38MAPK signaling pathway is stil of great significance in the treatment of periodontal disease, and we hope to provide a new insight and basis for clinical diagnosis and treatment of periodontitis.

To study the expression of herpes simplex virus type 2 latency-associated transcript (LAT) open reading frame 1 (ORF1) and its anti-apoptosis function induced by actinomycin D in Vero cells. The recombinant plasmid pEGFP-ORF1 was constructed and transfected into Vero cells, and the expression of ORF1 was identified by RT-PCR. The changes of Vero cells morphology induced by actinomycin D were observed by fluorescence microscopy, Hochest33258 fluorescence staining. Cells viability was evaluated by MTT assay and cells apoptosis rate was detected by flow cytometry. Double digestion and sequencing confirmed the pEGFP-ORF1 was constructed successfully, RT-PCR showed that the target gene was highly expressed in Vero cells. Hochest33258 staining reaveals that Vero cells transfected with pEGFP-ORF1 and induced apoptosis by actinomycin D had no changes in morphology. MTT assay showed that the viabilities of Vero cells transfected with recombinant plasmid pEGFP-ORF1 and induced apoptosis by actinomycin D has no statistically significant difference compared with the untreated normal control group (P > 0.05), but remarkable higher than Vero cells transfected with empty plasmid pEGFP-C2 and induced apoptosis by actinomycin D, the difference was statistically significant (P < 0.05). Flow cytometry assay shows that the cells apoptosis rate had no significant difference between pEGFP-ORF1 group and the normal group, but the cells apoptosis rate ofpEGFP-ORF1 was lower than the pEGFP-C2 group. HSV-2 LAT ORF1 gene can be expressed in Vero cells and can protect Vero cells from apoptosis induced by actinomycin D.

Objective To explore the effect of continued self-efficacy health education after discharge on patients with benign prostatic hyperplasia. Methods Toally 100 patients with benign prostatic hyperplasia during January to December 2015 were equally assigned to control group and observation group by random digit table:the former was treated with discharge instructions and the latter with discharge instruction plus self-efficacy health education. The two groups were compared before intervention and 6 weeks after intervention in terms of health behavior and self-management ability. Results Before intervention, there were no significant differences between the groups in health behavior and total score of self-management ability as well as the scores on its dimensions (P>0.05). After intervention, however, the total scores on health behavior and self-efficacy ability as well as the scores on the dimensions in the observation group were significantly higher than those of the control group (all P<0.01). Conclusion Post-discharge continued self-efficacy health education can be effective in improving their health behavior and self-management ability, which can promote their recovery.
