1.Retrospective Analysis of ADR Induced by Gynecological Chinese Patent Medicine in Our Hospital during 2008-2015

OBJECTIVE:To explore the characteristics and regularity of ADR induced by gynecological Chinese patent medi-cine,and to provide reference for rational use of gynecological Chinese patent medicine. METHODS:ADR reports were collected from gynecological ADR reporting information system of our hospital during 2008-2105;all ADR cases of Chinese patent medicine were collected and analyzed. RESULTS:No severe ADR was found among 41 cases of Chinese patent medicine-induced ADR,24 cases of new and common ADR(58.54%),17 cases of common ADR(41.46%);the incidence of ADR was in high level 1-3 d after medicating,being 97.6%. Main routes of administration were oral administration(19 cases,46.3%)and vaginal administra-tion(15 cases,36.6%)in ADR cases. Clinical manifestations were gastrointestinal reaction(29.97%),systemic reaction(19.15%) and mild injury of reproductive system(17.02%),etc. CONCLUSIONS:Chinese patent medicine is relatively safety in gynecologi-cal department. However,caution should be exercised in special cases and different medication.

4.Expression and significance of vasculogenic mimicry in endometrial carcinoma tissues

Objective: To investigate the expression of vasculogenic mimicry (VM) in endometrial carcinoma tissues and its rela-tionship with the clinicopathologic features and prognosis of the disease. Methods: A total of 267 paraffin-embedded endometrial carci-noma specimens of patients with complete follow-up data were collected from the Shijiazhuang Bethune International Peace Hospital between January 2005 and June 2014. CD31-PAS dual staining was performed to identify the VM structure. Tissue samples were then divided into VM-positive and VM-negative groups. CD133 expression was detected by immunohistochemical streptavidin peroxidase method. Results: Among the 267 endometrial carcinoma patients, 65 cases (24.3%) were VM positive. VM formation was closely corre-lated with the International Federation of Gynecology and Obstetrics Staging (χ2=9.987, P=0.002), histodifferentiation grade (χ2=11.795, P=0.001), myometrial invasion depth (χ2=5.499, P=0.019), vascular cancer embolus (χ2=22.599, P<0.001), and lymph node me-tastasis (χ2=7.848, P=0.005). Kaplan-Meier survival curve analysis showed that survival time was significantly shorter in the VM-posi-tive group (median survival time was 51 months) than in the VM-negative group (median survival time was 100 months) (χ2=70.973, P<0.001). Moreover, CD133 expression was significantly higher in the VM-positive group [75.4% (49/65)] than in the VM-negative group [58.9% (119/202)] (χ2=5.720, P=0.017). Conclusion: VM is closely correlated with the pathogenesis, development, and malignancy of endometrial carcinoma. Furthermore, VM is one of the important indexes influencing the prognosis of this disease. Therefore, CD133-positive cells may contribute to VM formation.
5.Clinical and neuroimaging features of reversible posterior leukoencephalopathy syndrome

Objective To investigate the clinical and neuroimaging characteristics of patients with reversible posterior leukoencephalopathy syndrome (RPLS). Methods The etiology, clinical manifestations, neuroimaging features, treatment, and prognosis were retrospectively analyzed in 8 patients (5 males and 3 females) with RPLS. Results The causes of RPLS included systemic lupus erythematosus (2 cases) eclampsia (one case), primary hypertension (one case), fungal encephalitis (one case), multiple myeloma (one case), renal transplantation (one case), immunosuppressant (three cases), chemotherapy (one case) and antifungal agent (one case). The clinical manifestations of these patients included headaches, seizures, visual abnormalities, and consciousness or mental disorders. Magnetic resonance imaging (MRI) of the head mainly showed symmetrical abnormalities in the posterior regions of the brain, as evidenced by low or equal signal on T_1WI, and high signal on T_2WI and FLAIR images. After treatment with antihypertensive agents, dehydration therapy, and heteropathy for 2~3 weeks, the neurological deficits of the patients were almost resolved and the initial lesions disappeared completely or almost completely in all patients at the follow-up MRI. Conclusions RPLS is a clinical entity characterized by reversible white matter damage in posterior brain. Prompt diagnosis and treatment may result in reversible resolution of its clinical symptoms and neuroradiological lesions.
6.Dopamine and 5-hydroxytryptamine signaling pathways in the pathogenesis of spinal cord injury-induced osteoporosis:new theory and novel therapeutic strategies

BACKGROUND:Current basic and clinical research have showed that increases in bone resorption and bone loss accur earlier after spinal cord injury (SCI) than disuse atrophy, revealing that other mechanisms are involved in the pathogenesis of the SCI-induced osteoporosis (SIO).
OBJECTIVE:To introduce the current lab and clinical research progress in SIO focusing on the functional changes of two major neurotransmitters in the spinal cord, including dopamine (DA), 5-hydroxytryptamine (5-HT) and their receptors, as well as their regulatory functions on bone metabolism, aiming at finding a new treatment strategy for SIO.
METHODS:A computer-based online search in PubMed and Embase databases was conducted for clinical and basic research related to SIO published from January 1967 to August 2016, using the keywords of“spinal cord injury;osteoporosis;dopamine;serotonin;5-hydroxytryptamine”in English. Irrelevant, poorly related and repetitive studies were excluded, and finally 41 eligible articles were included.
RESULTS AND CONCLUSION:DA and 5-HT are major neurotransmitters in the central nervous system, both involving in the regulation of bone remodeling. After SCI, loss of innervation and descending neurotransmitters especially DA, 5-HT and subsequent deregulation of their receptors are responsible for the onset of post-traumatic bone loss. The above research progress, in combination with the emerging clinical and lab investigations targeting 5-HT, DA and their receptors for improving neural functions after SCI, provides possible therapeutic pathways for SIO.
7.Monitoring the migration of bone marrow derived mesenchymal stem cells to intracranial glioma by sodium iodide sympoter

Objective To construct a recombinant lentiviral expression vector containing NIS and EGFP gene,and to explore the feasibility of NIS gene for monitoring the bone marrow derived mesenchymal stem cells (BMSCs) migration to the intracranial glioma.Methods The NIS and EGFP gene fragments were subcloned into lentiviral vector pLVX-puro,then packaged and amplified in HEK293T cells to obtain recombinant lentivirus pLVX-CMV-NIS-EGFP.pLVX-CMV-0-EGFP was constructed as control.BMSCs were isolated,cultured,and transfected by lentivirus.The antibiotic-resistant transfected BMSCs (BMSCs-NIS-EGFP and BMSCs-EGFP) were selected.The expression of NIS gene was examined by Western blot.Functional NIS activity was confirmed by the uptake of 125I and the inhibition effect of NaClO4.The nude mice intracranial glioma models were established.MicroSPECT was performed at 24 h post BMSCs-NIS-EGFP injection via the tail vein.Results pLVX-CMV-NIS-EGFP and pLVX-CMV-0-EGFP were successfully constructed and packaged.BMSCs were successfully isolated and cultured.Stable cell lines BMSCs-NIS-EGFP and BMSCs-EGFP were constructed after lentivirus transfection and puromycin selection.The expression of NIS gene was detected by Western blot in BMSCs-NIS-EGFP,but not in BMSCs-EGFP.BMSCs-NIS-EGFP showed significantly more uptake of 125I (nearly 10 times than the uptake in BMSCs-EGFP) and the uptake could be significantly inhibited by NaClO4.The nude mice intracranial glioma models were successfully established and the BMSCs-NIS-EGFP in glioma foci could be visualized by microSPECT imaging at 24 h post injection.Conclusions A recombinant lentivirus containing NIS gene could be successfully constructed for monitoring BMSCs migration towards intracranial glioma.It might provide evidence on the research of BMSCs and NIS gene mediated therapy for glioma.
8.Effect of dexmedetomidine combined with ropivacaine for ilioinguinal and iliohypogastric nerve block un-der sevoflurane anesthesia

Objective To investigate the clinical efficacy of dexmedetomidine combined with ropivacaine for ilioinguinal and iliohypogastric nerve block in elderly patients undergoing inguinal her-nia surgery.Methods Sixty elderly patients undergoing inguinal hernia surgery were randomly divided into dexmedetomidine group (group D)and control group (group C),30 patients in each group.All patients received ilioinguinal and iliohypogastric nerve block by the same anesthesiologist. 0.375% ropivacaine and dexmedetomidine 1 μg/kg 20 ml were used in ilioinguinal and iliohypogastric nerve block in group D,while 0.375% ropivacaine 20 ml were used in ilioinguinal and iliohypogastric nerve block in group C.Sensory block onset time and blockade duration were recorded.Side effects were recorded in both groups.Results Sensory block onset time in group D was significantly shorter than that in group C (10.6±4.3 min vs 14.4±5.1 min,P <0.05).Sensory block durations in group D was significantly longer than that in group C (832.7±136.6 min vs 669.8±140.1 min,P <0.05). There were two bradycardia cases in group D.No adverse reactions or anaesthesia complications oc-curred in all patients following postoperative follow-up.Conclusion Dexmedetomidine combined with ropivacaine can be used in ilioinguinal and iliohypogastric nerve block safely,and dexmedetomidine shortens the onset time and prolonged the duration of sensory block.
9.Pathogenicity of Staphylococcus epidermidis Causing Urinary Tract Infection

OBJECTIVE To understand the pathogenicity of Staphylococcus epidermidis causing urinary tract infection. METHODS Thirty five isolates were collected in the patients with urinary tract infection during 2003-2004 to test the minimal inhibitory concentration(MIC) to erythromycin,ampicillin,cefoxitin,ceftriaxone,teicoplanin,ciprofloxacin,tetracycline,trimethoprim-sulfamethoxazole and vancomycin.The genes of ermA,ermB,ermC,msrA,mecA,and icaA were amplified by PCR.The biofilm producing of the isolates was determined by the quantitative method.The pulsed-field gel electrophoresis(PFGE) was used to discriminate the homology among the isolates. RESULTS Among the isolates,MRSE accounted for 71.4%.The antibiotic susceptibility to erythromycin,ampicillin,ceftriaxone,ciprofloxacin,tetracycline and trimethoprim-sulfamethoxazole were 22.8%,6.34%,34.9%,26.9%,44.4% and 62.2%,respectively,whereas to teicoplanin and vancomycin were 99.2% and 100%.All the isolates resistant to cefoxitin carried mecA.ermC Was the predominant determinant among the erythromycin resistant isolates.Among the 14 icaA positive isolates,10 isolates were biofilm producing.All the isolates were classified into 18 types by PFGE. CONCLUSIONS The high and multiple antibiotic resistance of S.epidermidis isolates causing urinary tract infection were shown.The detection of icaA gene in clinical S.epidermidis isolates was a signal of their pathogenicity.
10.Psychological Analysis and Nursing Intervention in Urodynamic Test

Objective To investigate the influence of psychological intervention on the urodynamic test. Method The 959 patients in the urodynamic test were divided into two groups:the control group and the the test group,then we analyzed and compared the results. Results In the incidence of the abnormal abdominal pressure, the unnatural detrusor contraction and the Unnatural urethral sphincter contraction, there were statistically significant differences between two groups ( <0.05) . Conclusion It effective and necessary for urodynamic test to do psychological intervention.
