Chinese Journal of Postgraduates of Medicine 2010;33(16):23-25
Objective To investigate the relationship between the serum levels of CEA,cAl25,CA19-9 and clinical pathological characteristics in gastric cancel patients.Method The serum levels of CEA,CA125 and CA19-9 in 107 gastric cancer patients before the surgery were detected by electrochemiluminescence immunoassay. Results The preoperative seurm levels of CEA,CA125 and CA19-9 were not correlated with sex,age and the location of the tumor. The serum level of CEA,CA125 and CA19-9 in patients with lymph node metastasis were higher than those in patients without lymph node metastasis. The serum levels of CEA,CA125 and CA19-9 were positively correlated with the depth of tumor invasion and the size of the tumor. The serum levels of CA125 were correlated with the degrees of differentiation.Thehigher the CA125 levels, the lower the degrees of differentiation. But the levels of CEA and CA19-9 were not correlated with differentiation. There were positive.correlations among the serum levels of CEA,CA125 and CA19-9.The correlation coefficient of CA125 and CA19-9 was 0.623(P<0.01),CA125 and CEA was 0.572(P<0.01),CA19-9 and CEA was 0.412(P<0.01).Conclusions The serum levels of CEA,CA125 and CA19-9 in gastric cancer patients are significantly related with the size of the tumor, the depta of tumor invasion and lymph node metastasis. The preoperative serum levels of CEA,CA125 and CA19-9 can provide some reference for formulating the treatment of the gastric cancer patients.
2.Models of end-stage liver disease in evaluating the prognosis of cirrhosis patients after endoscopic injection sclerotherapy
Chinese Journal of Postgraduates of Medicine 2010;33(7):36-39
Objective To evaluate CTP,MELD,MELD-AS,MELD-Na,i-MELD and MESO in prognosis of cirrhosis patients who underwent endoscopic injection sclerotherapy (EIS). Methods Seventy-eight cirrhosis patients with integral clinical and follow-up data were enrolled into the study. The area under the receiver operating characteristic (ROC) curve (AUC) was used to compare these six models in predicting mortality risk. The optimal threshold of them to determine the prognostic death risk was calculated by ROC curves. Results Eight patients died within half one year, 11 patients died within the first year. There was no significant difference in gender and age between the survival and death groups. The AUC of the six models higher than 0.800 were. that, 6-month: MELD-AS (0.837), MESO (0.830), MELD (0.830) ; 12-month: MELD (0.822),MELD-AS (0.821),MESO (0.814). Conclusions MELD-AS,MESO,MELD and so on are all accurate predictive systems for 6-month and 12-month of patients who underwent EIS. The value of them are very important in the selection of patients for EIS, and have a certain clinical significance.

To observe the analgesic effects of morphine and bupivacaine for the patient controlled epidural analgesia (PCEA) after prostatectomy. Methods: 21 patients after receiving prostatectomy were treated with PCEA including morphine and bupivacaine, and 24 patients were treated with routine analgesia as control. The effects of analgesia with visual analogue scale (VAS) were evaluated indirectly, and episodes of bl adder spasm were recoreded. Results: The effects of PCEA on postoperative analgesia and controlling bladder spasm were much better than that of routine analgesia. Conclusion: Effects of morphine and bupivacaine used in PCEA for post-prostatectomy analgesia are confirmed adm inistration is slimple. Because of their dosage is smalller and side effects are fewer, this treating protocol is beneficial for the recovery of the patients re ceiving prostatectomy.

This study was aimed to prepare solid dispersions of Acanthopanax leaves total flavonoids in order to im-prove its bioavailability. PEG4000, PEG6000, F68, PVPK30 were used as carrier materials in the preparation of four different types of solid dispersion to screen the best type of carrier material and evaluate the amount of carrier mate-rial and its influence on the drug dissolution. Rutin was used as reference substance. NaNO2-Al(NO3)3-NaOH was used as the color system, with a UV spectrophotometer measured absorbance at 500 nm. The dissolution characteris-tics of different proportions of solid dispersions were examined in vitro. The results showed that compared with raw material, the in vitro drug release rate with PVPK30 as carrier material in the obtained solid dispersion of the pro-portion of the raw material was significantly improved, and the cumulative release rate was also increased significant-ly. It was concluded that the solid dispersion prepared by solvent method significantly improved in vitro drug release in water.

(DBI) in rats by observation on the changes of Homer 1a expression and apoptotic nerve cells . Methods Spraque-Dawlley(SD) rats were randomly ( random number ) divided into control group and severe DBI group .DBI rat model was established according to the de-scription of Marmarou′s diffused brain injury .No injury was done on control group .The changes of neuron pathology were observed by light microscopy and electron microscope .The expression of Homer1a was observed by immunohistochemistry and western blot .The quan-tity of apoptotic cells was measured by terminal deoxynucleotidyl transfernase medicated nick end labeling ( TUNEL) method. Results The death rate of rats in severe DBI group was 49.3%.Compared with the control group , the ultrastructures in hippocampal neurons in-cluding organelle , axonal and capillary were damaged seriously after injury , the survival rate of nerve cells decreased significantly at 1 h after injury ([99.4 ±0.6]%vs [94.4 ±5.6]%, P<0.05), and peaked at 72 h ([99.2 ±0.8]%vs [54.7 ±33.8]%, P<0 .05) in DBI group. The expression of Homer1a protein increased significantly at 1 h after injury(0 .136 ±0.024 )and peaked at6 h(0.178 ± 0.028) and maintained to 24 h (0.176 ±0.027), while decreased at 48 h (0.145 ±0.02)and 72 h (0.117 ±0.012) in DBI group;the expression of Homer 1a was obviously higher at each time point in DBI group than that in control group (P <0.05).The apoptoticindex of TUNEL positive cells increased at 6 h and demonstrated significant difference at 72h in comparison to control group ([41.78 ±3 .96]%vs [1.92 ±0.22]%, P<0.05).The correlation analysis indicated that Homer1a expression from 1~24 h and 24 h~72 h was related to the survival rate of nerve cells ( r=-0.726, P<0.05; r=0.842, P<0.05) and the quantity of TUNE positive cells(r=0.738, P<0.0;5 r=-0.898, P<0.05). Conclusion The dynamic expression of Homer1a in hippocampus after severe DBI can reflect nerve cell loss.

AIM: To investigate the protein patterns of bone marrow mesenchymal stem cells(MSCs) induced by 5-azacytidine and Shuanglong Formula(Radix et Rhizoma Ginseng,Radix et Rhizoma Salviae Miltiorrhizae) and molecular mechanism of induced differentiation was discussed at the level of proteome. METHODS: MSCs extracted from Chinese miniswine were induced by 5-azacytidine and Shuanglong Formula.Proteins were separated by two-dimensional gel electrophoresis, and were detected by silver staining method. The image was analyzed by PDQuest software, and the intensities of spots in the gels were used to quantify the expressions of proteins.Proteins of interest were chosen for in-gel digestion and MALDI-TOF-MS analysis, and then further identified by peptide mass fingerprinting(PMF). RESULTS: 16 altered proteins were identified by PMF and their regulation might be caused by the addition of medicate sera containing the active components of Shuanglong Formula. The effects of medicated sera containing Shuanglong Formula on the induced-differentiation could be discussed from the regulated expression of functional proteins; CONCLUSION: Shuanglong Formula may promote the 5-azacytidine-induced differentiation of bone marrow MSCs in vitro, and our study indicate that proteomics can be used for the analysis of molecular mechanism for different cell process.

Objective:To investigate the expression levels of microRNA-124 (miR-124) and microRNA-494(miR-494) in the serum of elderly patients with Parkinson disease (PD) and its clinical significance. Methods:Ninety PD patients (PD group) who were hospitalized in Zhengzhou Central Hospital Affiliated to Zhengzhou University from March 2018 to April 2020 were selected. At the same time, 100 non-PD elderly people examined in the physical examination center of the same hospital who matched with age and gender of PD patients were selected as the control group. After 12 hours of fasting, 4 ml of venous blood was taken from all subjects. All PD patients were graded by unified Parkinson disease rating scale(UPDRS) from the aspects of mental state, behavior and emotion, quality of life and motor examination, and graded by the Hoehn-Yahr rating scale for Parkinson disease. The expression levels of miR-124 and miR-494 in serum were detected by real-time fluorescent quantitative PCR (qRT-PCR), and the diagnostic values of miR-124 and miR-494 in PD patients were evaluated by ROC curve.Results:Hoehn-Yahr grade of PD patients with UPDRS≤60 points was significantly lower than that of patients with UPDRS >60 points((2.47±0.43) vs (3.42±0.47))( t=9.055, P<0.001), and there was no significant difference in serum miR-124 and miR-494 expression levels((0.72±0.14) vs (0.70±0.12), (1.17±0.19) vs (1.18±0.22)) ( t=0.633, 0.230, P=0.529, 0.819). Compared with that in control group, the expression of miR-124 in PD group was down-regulated ((0.71±0.20) vs (1.05±0.24)), and the expression of miR-494 was up-regulated((1.18±0.26) vs (0.96±0.22)) ( t=10.542, 6.315, P<0.001). The results of ROC showed that the area under curve (AUC) of serum miR-124 and miR-494 in the diagnosis of PD were 0.847 and 0.760 respectively, the cutoff values were 0.901 and 1.126, respectively, the sensitivities were 86.67% and 61.11% respectively, and the specificities were 75.03% and 79.00% respectively. The AUC of the combined diagnosis of PD was 0.898, and the sensitivity and specificity were 85.56% and 85.00% respectively. Conclusions:The expression of miR-124 is low in PD patients, while the expression of miR-494 is high, which suggests that the changes of the two miRNA levels may be related with the occurrence and development of PD.Both of them have a certain diagnostic value for PD, and the value of combined diagnosis is higher.
