Objective To study the molecular mechanism of acute myocardial infarction(AMI),searching for protective genes and injury-related ones,aimed to fight myocardial infarction disease.Methods Rat model of AMI was established with left anterior descending (LAD) coronary artery ligation.Gene chip was used to verify the mechanism of AMI at molecular level.Results The author screened out 190 differential expression genes between AMI rat myocardium and sham operated rat myocardium in the gene expression profile study;In the Model/Sham operation gene expression profile,there were 89 genes depressed expression and 101 genes elevated expression.Most of them involve in the 12 known categories,and relate to the metoblolism,signal and trandport pathways.Conclusion The major possible mechanism of AMI is to regulate the local expression of related gene through a multi-pathway.

Objective To investigate the distribution characteristics of bone marrow(BM)cell morphological results and their role in the etiological diagnosis of blood diseases,and to understand the characteristics of hematological diseases spectrum in this ar-ea.Methods The BM puncture specimens in 372 cases of hematological diseases in our hospital from March 2012 to April 2014 were performed the Wright-Giemsa staining and cytochemical staining.The cell morphology and cytochemical staining were ob-served by microscope,which was combined with the related clinical data for obtaining the morphological report.Results Among 372 cases,368 cases were diagnosed with hematological diseases,which were dominated by 3 kinds of main type,leukemia(95/372), hyperplasia anemia(36/372)and iron-deficiency anemia (26/372).In 82 cases of anemia,hyperplasia anemia(36/82),iron-deficiency anemia (26/82)and megaloblastic anemia (11/82)were predominant.Conclusion The BM morphological examination is one of im-portant measures for the etiological diagnosis in hematological diseases.The analysis on the diseases spectrum is conducive to guide the diagnosis and treatment in clinic.
5.Effect of Epidural Analgesia with Morphine and Bupivacaine in Patients after Receiving Prostatectomy

To observe the analgesic effects of morphine and bupivacaine for the patient controlled epidural analgesia (PCEA) after prostatectomy. Methods: 21 patients after receiving prostatectomy were treated with PCEA including morphine and bupivacaine, and 24 patients were treated with routine analgesia as control. The effects of analgesia with visual analogue scale (VAS) were evaluated indirectly, and episodes of bl adder spasm were recoreded. Results: The effects of PCEA on postoperative analgesia and controlling bladder spasm were much better than that of routine analgesia. Conclusion: Effects of morphine and bupivacaine used in PCEA for post-prostatectomy analgesia are confirmed adm inistration is slimple. Because of their dosage is smalller and side effects are fewer, this treating protocol is beneficial for the recovery of the patients re ceiving prostatectomy.
6.Effects of Shuanglong Prescription on Heart Hemodynamics and Myocardial Oxygen Consumption in Dogs

Objective To investigate the pharmacological action of Shuanglong Prescription on cardiac function in dogs and to study its mechanism. Methods The changes of the parameters such as coronary blood flow, myocardial oxygen consumption and heart rates were observed in normal anesthetic dogs. Results Compared to those in the normal group, the coronary blood flow increased and the heart rates decreased in Shuanglong Prescription group. Conclusion Shuanglong Prescription can regulate and improve the cardiac function of dogs.
7.Pathological assessment of gastric mucosa using OLGA system

Objective To evaluate the clinical value of OLGA (Operative Link on Gastritis Assessment) system in pathological assessment of gastric mucosa from Chinese patients.Methods Endoscopic and pathological data of patients,who underwent the endoscopic examination of upper gastrointestinal tract from 2007 to 2012,were analyzed.Using the Sydney protocol,we graded atrophy and staged the Helicobacter pylori infection status in gastric biopsies from 108 patients with dyspepsia.We applied the OLGA system to assess the relationship between OLGA staging and grading and dysplasia/cancer,and the relationship between OLGA grading and Helicobacter pylori infection.Results Two cases of early gastric adenocarcinoma were detected at stage Ⅱ and Ⅳ,respectively.The detection rates of precancerous lesions (including intestinal metaplasia and dysplasia) in cases clustered in stages Ⅲ-Ⅳ were significantly higher than those in stages 0-Ⅱ (intestinal metaplasia:100.0% VS 65.3%,P <0.001 ;dysplasia:77.8% VS 13.9%,P <0.001,respectively),and there were significant differences between OLGA staging and the degree of dysplasia (r =0.671,P <0.001).In the subjects at stage 0-Ⅱ,we observed only 10 cases (13.9%) of mild dysplasia.The difference in rate of Helicobacter pylori infection was significant at different OLGA grade (P < 0.001),and the correlation between OLGA grading and infection degree of Helicobacter pylori was significant (r =0.586,P < 0.001).Conclusion Precancerous lesions and gastric adenocarcinoma tend to be discovered in patients clustered at high-risk (Ⅲ-Ⅳ) OLGA stages.OLGA staging of dysplasia can effectively differentiate the risks of gastric cancer.The OLGA grading system is consistently associated with the Helicobacter pylori infection.

9.Microscopic measurement of intercellular space of squamous epithelium in lower part esophagus of patients with gastro-esophageal reflux disease

Objective To evaluate the clinical significance of intercellular space diameters (ISD)of squamous epithelium by light microscopy (LM) in lower esophagus of erosive reflux esophagitis (ERD),non-erosive reflux disease ( NERD), Barrett esophagus (BE) and healthy controls. Methods A total of 21 ERD and 21 NERD patients with reflux symptoms and confirmed with 24-hour esophageal pH monitoring, 13 BE patients diagnosed by gastroscopy and biopsy, and 20 other healthy controls were enrolled in the study.Samples of ERD, NERD and control group were collected at 2 cm above dentate line, and made HE slides in the conventional way. Images for measurement of ICS were acquired with oil lens ( × 1000). ICS of squamous epithelium was quantitatively measured by computer-assisted morphometry. Ten cells were taken for each sample, 10 consecutive ISD for each cell, i.e. 100 ISD for each subject. Mean ISD was calculated.Results Mean ISDs by LM in control, BE, ERD, and NERD groups were 0. 59, 0. 99, 1.29 and 1.06 μm, respectively. The mean ISDs in BE, ERD, and NERD group were much greater than that in control (P<0. 05). The mean, maximal and minimal ISDs of group ERD were greater than those of NERD and BE (P = 0. 000). However, the ISDs of NERD and BE are of no significant difference ( P > 0. 05 ). The cut-off value of mean ISD for diagnosis of gastro-esophageal reflux disease (GERD) was 0. 85 μm. Diagnostic sensitivity and specificity for ERD, NERD and BE were 89. 1% and 100. 0%, with reference to clinical symptoms, endoscopy and ISDs above the cut-off value. Conclusion Larger ISDs in lower esophagus by using LM will be found in all subgroups of GERD, including ERD, NERD and BE. Increased ISDs may be one of the markers for diagnosis of ERD, NERD and BE.
10.Clinical results of limb salvage operation with major vascular reconstruction for sarcomas of extremities

Objective To determine the feasibility of limb salvage with major vascular reconstruction for sarcomas of extremities, focusing on the early complications, oncological and functional outcomes.Methods Between August 2004 and June 2009, 13 patients (mean age 38 years, range 14-63 years) underwent wide resection of upper and lower extremity sarcomas to include the involved arterial and venous segments. 4 patients had bone sarcomas and 9 soft tissue sarcomas, 6 patients only had underwent artery replacement and the other 7 had underwent both artery and vein. 8 resected vessels were reconstructed with vascular graft and 5 with ipsilateral or contralateral saphenous vein. Results No patient suffered from reperfusion injury and infection. The patient with osteosarcoma in proximal humerus had suffered acute arterial graft thrombosis at first day postoperatively; however, the limb was salvaged after successful thrombectomy. 5 of 8 patients with synthetic graft developed minor delayed wound healing and hematoma, but the other 5 patients with autologous saphenous vein replacement did not. The mean follow-up was 19.4 months (7-45months). One patient died of pulmonary metastasis; the patient with osteosarcoma in proximal humerus had developed local recurrence, but refused amputation and was alive with metastasis. No arterial occlusion was observed at final follow-up or at the time of death, but the patency of artery and vein was 100% and 28.6%respectively. One patient who has pulmonary metastases before operation died of metastases 7.5 months after operation. Four patients developed pulmonary metastases, therefore, Kaplan-Meier survival analysis showed that 2-year overall and metastasis-free survival rate was 90.9% and 63.6% respectively. Functional status were judged as good or excellent (mean MSTS score 72%) in 12 of 13 patients. Conclusion The study indicate that malignant involvement of major vessels is not a contraindication for limb-salvage. Vascular reconstruction is a feasible option in limb salvage surgery. Wide resection with vascular reconstruction provide acceptable oncological and functional outcome of limb salvage.
