1.Nursing of percutaneous and transhepatic portal venous autologous bone marrow stem cell transplantation

Objective To summarize the nursing experience of treatment of percutaneous and transhepatic portal venous autologous bone marrow stem cell transplantation for chronic hepatic failure.Methods 19 patients who were definitely diagnosed as chronic liver failure received pertinent nursing in different perioperative period of transhepatic portal venous autologous bone marrow stem cell transplantation.Results All of the 19 patients went through perioperative period safely without any adverse reactions or complications.Conclusions In the process of treatment of autologous bone marrow stem cell transplantation for chronic hepatic failure,sufficient preoperative preparation,good communication and close cooperation among doctors,nurses and patients during operation,careful nursing and rehabilitation instruction after operation,are important assurances for autologous bone marrow stem cell transplantation to run smoothly.
2.An experimental study of quantitative diagnosis of fatty liver by imaging technology

Objective To compare the values of biochemistry, ultrasonography ( US) , computed tomography(CT) and proton magnetic resonance spectroscopy ('H MRS) in the quantitative diagnosis of fatty liver. Methods Forty-five healthy New Zealand rabbits were enrolled in the study. Hepatic steatosis models were established by giving high fat, high sugar diet with drinking water containing five percent ethanol. Eighteen variable indexes were measured by biochemical examination, US,CT and ' H MRS. ROC analysis and Z test were used to compare the diagnostic efficacy of different clinical examinations. Results Among eighteen variable indexes,serum TC,ultrasound attenuation coefficient,liver CT value and ' H MRS fat peak area had the highest degree of relationship by biochemical examination, US, CT, ' H MRS in diagnosis of fatty liver, correlation coefficients were 0.886,0.483, -0. 764, 0. 558, and areas under curve were 0. 981, 0. 581, 0. 810, 0. 713, respectively. There were significant differences in areas under curve between every two groups except 'H MRS fat peak area and ultrasound attenuation coefficient, liver CT value. Conclusions The diagnostic values of imaging modalities in the hepatic steatosis grade, their order is CT >'H MRS> US; Serum TC maybe have important diagnostic value in evaluating hepatic steatosis grade,this is very worth further studying.
3.Application of nursing research group model in a training program for clinical nurses′scientific research ability

Objective:To explore the practical effect of nursing research group model in a training program for clinical nurses′ scientific research ability.Methods:From August 2020 to July 2021, 30 clinical nurses from the Fifth Affiliated Hospital of Sun Yat-sen University were trained by case teaching mode through the construction of a complete nursing scientific research training course system with the nursing research group as the carrier. The scores of nursing scientific research ability of clinical nurses before and after the training were compared, and the evaluation of teaching effect and the output of scientific research results were analyzed.Results:After the training, the total scores of clinical nurses′ scientific research ability, problem discovery ability, literature reading ability, scientific research design ability, data processing ability, scientific research practice ability and thesis writing ability were 78.33 ± 7.48, 8.83 ± 1.42, 14.47 ± 1.22, 13.27 ± 1.36, 11.07 ± 1.60, 14.73 ± 1.28, 15.97 ± 1.35 respectively. The scores were significantly higher than those before the training (59.00 ± 7.44, 6.53 ± 1.22, 11.80 ± 1.37, 9.53 ± 1.31, 8.10 ± 1.37, 11.30 ± 1.34, 11.73 ± 1.46), and the differences were statistically significant ( t values were -52.80 - -21.14, all P<0.05). The proportion of clinical nurses who believed that the training with nursing research group as the carrier could improve learning attitude, enhance clinical comprehensive ability and promote professional development was ≥90%. After the training, 8 papers were published, 3 projects were approved and 2 patents were approved, which were significantly higher than those before the training (2, 1 and 0), and the total scientific research and innovation achievements increased by 333.33% (10/3) compared with those before the training. Conclusions:Scientific research training with nursing research group as the carrier can improve the scientific research ability of clinical nurses and improve their innovative thinking.
4.Assessment of ablation zone of liver tumor by 3DUS-US fusion imaging based on electromagnetic positioning:an experimental study

Objective To investigate the feasibility and accuracy of three-dimensional ultrasound-ultrasound(3DUS-US)fusion imaging based on electromagnetic positioning in the assessment of ablation zone of liver tumor through experimental study in phantom models.Methods Sixteen phantom models which could be ablated and used to assess the ablation zone were established.Then,they were ablated and divided into complete ablation group(n =8)and incomplete ablation group(n =8)randomly.Their ablation effects were assessed by 3DUS-US fusion imaging.If the tumor model was assessed as incomplete ablation,the maximum thickness of the residual tumor was measured.The assessments of gross specimen were used as golden standard to validate the feasibility and accuracy of assessment of 3DUS-US fusion imaging.The success rate of registration,duration time of fusion imaging,accuracy rate of assessment and measurement error in incomplete ablation group were recorded.Results All of the 16 phantom models were produced successfully.The success rate of registration was 100% and the duration time of 3DUS-US fusion imaging was(3.9±1.0)min.The accuracy rate of assessment was 100%.There was no significant difference between the assessment of residual tumor thickness in incomplete ablation group by 3DUS-US fusion imaging and gross specimen(t=-0.814,P =0.442),and the measurement error was(1.1±0.6) mm.Conclusions 3DUS-US fusion imaging based on electromagnetic positioning is a feasible,accurate and convenient method in the assessment of ablation zone of liver tumor.It would be a new immediate precise assessment method during ablation procedure for liver tumor.
5.A study on the current status and influencing factors of emotional intelligence of standardized training nurses

Objective:To investigate the current status of emotional intelligence of standardized training nurses and explore the influencing factors.Methods:A total of 312 standardized training nurses from the Fifth Affiliated Hospital of Sun Yat-sen University and the First Affiliated Hospital of Nanchang University were investigated from May to June 2020 with general information questionnaire and Nurses′ Emotional Intelligence Scale, and analyze the influencing factors of nurses′ emotional intelligence during the standar dized training period.Results:The total score of standardized training nurses′ emotional intelligence was (59.01 ± 8.24), the scoring rate was 73.76%, and the total average score of items was (3.69 ± 0.52). The average scores of the four dimensions from low to high were: self emotion assessment (3.86 ± 0.66), self emotion management (3.67 ± 0.71), self emotion application (3.64 ± 0.67) and emotion assessment of others (3.58 ± 0.65). Multiple linear regression analysis showed that political outlook, whether it was an only child, and training time and willingness to engage in nursing work for a long time were the main influencing factors of standardized training nurses′ emotional intelligence, accounting for 32.6% of the variance.Conclusions:The emotional intelligence of standardized training nurses is at the medium level, which needs to be further improved. Hospital managers need to carry out positive intervention combined with relevant influencing factors to cultivate more excellent nursing talents for clinical practice.
6.Relationship between emotional labor and emotional intelligence of nurses in standardized training period

Objective:To investigate the current status of emotional labor of nurses in standardized training period, and to analyze the relationship between emotional labor and emotional intelligence.Methods:From May to June 2020, 312 nurses in the standardized training period were selected as the research objects, which were investigated with general information questionnaire, Emotional Labor Scale and Emotional Intelligence Scale.Results:The total score of standardized training nurses' emotional labor was (52.27±10.18) points, and the total average score of items was (3.73±0.73) points. The average scores of the three dimensions from high to low: deep acting (4.62±0.86) points, expression of genuine emotion (3.62±0.99) points and surface acting (3.42±0.95) points. There was a correlation between emotional intelligence and emotional labor. Multiple linear regression analysis showed that the emotional evaluation of others ( β value was -0.189, P<0.05) ( β value was -0.255, P<0.01). Self emotional assessment ( β value was 0.297, P<0.01) ( β value was 0.177, P<0.05). Self emotional assessment ( β value was 0.269, P<0.05), which had a positive predictive effect on emotional expression requirements. Conclusions:The emotional labor of nurses in standardized training period is at the medium level, and the deep acting score is the highest between three dimensions. Hospital managers could promote nurses to use appropriate emotional labor strategies by strengthening the cultivation of emotional intelligence during standardized training period.
7.Olanzapine causes non-alcoholic fatty liver disease via inhibiting the secretion of apolipoprotein A5.

OBJECTIVES:
Long-term treatment of olanzapine, the most widely-prescribed second-generation antipsychotic, remarkably increases the risk of non-alcoholic fatty liver disease (NAFLD), whereas the mechanism for olanzapine-induced NAFLD remains unknown. Excessive hepatic fat accumulation is the basis for the pathogenesis of NAFLD, which results from the disturbance of TG metabolism in the liver. Apolipoprotein A5 (ApoA5) is a key regulator for TG metabolism in vivo that promotes TG accumulation in hepatocytes, thereby resulting in the development of NAFLD. However, there are no data indicating the role of apoA5 in olanzapine-induced NAFLD. Therefore, this study aims to investigate the role of apoA5 in olanzapine-induced NAFLD.
METHODS:
This study was carried out via animal studies, cell experiment, and ApoA5 gene knockdown experiment. Six-week-old male C57BL/6J mice were randomized into a control group, a low-dose group, and a high-dose group, which were treated by 10% DMSO, 3 mg/(kg·d) olanzapine, and 6 mg/(kg·d) olanzapine, respectively for 8 weeks. The lipid levels in plasma, liver function indexes, and expression levels of ApoA5 were detected. HepG2 cells were treated with 0.1% DMSO (control group), 25 μmol/L olanzapine (low-dose group), 50 μmol/L olanzapine (medium-dose group), and 100 μmol/L olanzapine (high-dose group) for 24 h. HepG2 cells pretreated with 100 μmol/L olanzapine were transfected with siRNA and scrambled siRNA (negative control), respectively. We observed the changes in lipid droplets within liver tissues and cells using oil red O staining and fat deposition in liver tissues using HE staining. The mRNA and protein levels of ApoA5 were determined by real-time PCR and Western blotting, respectively.
RESULTS:
After intervention with 3 and 6 mg/(kg·d) olanzapine for 8 weeks, there was no significant difference in body weight among the 3 groups (P>0.05). Olanzapine dose-dependently increased the plasma TG, ALT and AST levels, and reduced plasma ApoA5 levels (all P<0.05), whereas there was no significant difference in plasma cholesterol (HDL-C, LDL-C, and TC) levels among the 3 groups (all P>0.05). Olanzapine dose-dependently up-regulated ApoA5 protein levels in liver tissues (all P<0.05), but there was no significant change in ApoA5 mRNA expression among groups (P>0.05). In the control group, the structure of liver tissues was intact, the morphology of liver cells was regular, and only a few scattered lipid droplets were found in the cells. In the olanzapine-treated group, there was a large amount of lipid deposition in hepatocytes, and cells were balloon-like and filled with lipid droplet vacuoles. The nucleus located at the edge of cell, and the number of lipid droplets was increased significantly, especially in the high-dose group. Likewise, when HepG2 cells were treated with olanzapine for 24 h, the number and size of lipid droplets were significantly elevated in a dose-dependent manner. Moreover, olanzapine dose-dependently up-regulated ApoA5 protein levels in HepG2 cells (all P<0.05), but there was no significant difference in ApoA5 mRNA expression among groups (P>0.05). Compared with the HepG2 cells transfected with scrambled siRNA, the number and size of lipid droplets in HepG2 cells transfected with ApoA5 siRNA were significantly reduced.
CONCLUSIONS
The short-term intervention of olanzapine does not significantly increase body weight of mice, but it can directly induce hypertriglyceridemia and NAFLD in mice. Olanzapine inhibits hepatic apoA5 secretion but does not affect hepatic apoA5 synthesis, resulting in the pathogenesis of NAFLD. Inhibition of apoA5 secretion plays a key role in the development of olanzapine-related NAFLD, which may serve as an intervention target for this disease.
