1.Therapeutic effect of trimetazidine on patients with ischemic cardiomyopathy heart failure and significance of plasma level of N-terminal pro-B type natriuretic peptide

Objective To observe the therapeutic effect of trimetazidine on patients with ischemic cardiomyopathy heart failure and the changes of plasma level of N-terminal pro-B type natriuretic peptide(NT-proBNP).Methods 62 patients with ischemic cardiomyopathy heart failure were randomly assigned to regular treatment group(3 1)and trimetazidine treatment group(31).All of them were given either regular medicine treatment or trimetazidine for 4 weeks.Concentration of NT-proBNP and echocardiography were detected before and after treatment and the correlation between NT-pmBNP and ultra-sound values was studied.Results There was no statistic difference in the NT-proBNP level between two groups before treatment,but after 4-week treatment the NT-proBNP level in both groups were deCreased(P<0.05),the decline in the trimetazidine group was more significant.The NT-proBNP level was significantly correlated to LVEF and LVEDD(γ=-0.472、0.45 1;P<0.05).Conclusions Trimetazidine has a positive effect on patients with ischemic cardiomyopathy heart failure,and decreases the plasma NT-proBNP level.NT-proBNP Can be used as a follow-up index.

Bioeleotrical activities of human cerebral cortex which control and regalate movements get to increase during voluntary movements, the metabolic level of the cells of motor areas change at the same time,and consequently regiond cerebrd blood flow (rCBF)changes. Different changes of rCBF indicate the range of motor areas with which the relavant movemats are involved.And studies of this take great significance in finding out the basic active regalarity and features of human brain. In this artide motheds of the determination of rCBF an introduced briefly, and changes and distribution of rCBF of rCBF of each cortical moter area during different types of volantary movements are discribed. 3.Effect of Exercise on Skeletal Muscle Metabolism

Influenced by the intensity, volume of exercise, the basic metabolic features of skeletal muscle are changed to such structural and functional adaptations as there are increases in the amount of contractile components, muscle glycogen and myoglobin, in the volume and quantity of mitochondria and in the density of capillary in skeletal muscle, which result in enhanced aerobic metabolism, contraction efficiency and prolonged time defore failure. It is also pointed out that the structural changes and decrease of function of sarcoplasmic reticula during acute exercise are important reasons for skeletal muscle fatigue , whereas, high- intensity training can improve sarcoplasmic reticulum function and its Ca2+ -intake rate, so that to improve the physiological efficacy of excitation- contraltion coupling. So it is put forward that anti- resistance training, as a means of exercise therapy, can efficiently increase muscle strength and prevent muscle atrophy. 4.CT Gemstone Spectral Imaging in Diagnosis of Thyroid Nodule Hemorrhage

PurposeTo explore the value of gemstone spectral imaging (GSI) in diagnosis of thyroid nodule hemorrhage.Materials and Methods Seventeen patients with surgery and pathology confirmed thyroid nodule hemorrhage underwent thyroid spectrum CT scan. The CT value, iodine concentration value, water concentration value and the effective atomic ordinal value as well as spectral curve slope of hemorrhagic nodules and surrounding normal thyroid tissue were measured respectively.Results Hemorrhagic thyroid nodules showed equal or high CT value, low concentration of iodine, high concentration of water, low spectral curve slope, and low effective atomic ordinal value on spectral CT imaging. There were statistically significant differences in concentration of iodine value, concentration of water value, and spectral curve slope between hemorrhagic nodules and surrounding normal thyroid tissue (Z=-5.438,-4.679 and-5.317,P<0.01), and there were statistically significant differences in CT value and effective atomic ordinal value (Z=-2.097 and-2.230,P<0.05).Conclusion Energy spectrum CT scan is important in detection and accurate diagnosis of thyroid nodule hemorrhage.
5.Clinical study on adaptive support ventilation in treatment of severe asthma

Objective To study the effect of adaptive support ventilation (ASV) in treatment of severe asthma.Methods Forty-nine cases of severe asthma were divided into ASV group (25 cases) and control group (24 cases,tradition mechanical ventilation).The arterial blood gas,respiratory dynamics,mechanical ventilation time,hospital stay and thorax barotrauma was compared between two groups.Results The arterial blood gas and respiratory dynamics was improved after mechanical ventilation compared with that before mechanical ventilation in two groups,and there was significant difference (P < 0.05 or < 0.01).The airway peak voltage,lung dynamic compliance and platform pressure after mechanical ventilation of 2,12 and 24 h in ASV group was better than that in control group[2 h:(33 ± 12) cm H2O(1 cm H2O =0.098kPa) vs.(37 ± 11) cm H2O,(16 ± 9) ml/cm H2O vs.(17 ± 10) ml/cm H2O,(27 ± 6) cm H2O vs.(30 ±12) cm H2O; 12 h:(23 ± 12) cm H2O vs.(25 ± 11) cm H2O,(28 ± 6) ml/cm H2O vs.(23 ± 10) ml/cm H2O,(20 ±6) cm H2O vs.(25 ±4) cm H2O; 24 h:(18 ± 12) cm H2O vs.(20 ± 11) cm H2O,(32 ±9)ml/cm H2O vs.(28 ± 10) ml/cm H2O,(12 ±7) cm H2O vs.(16 ±7) cm H2O],and there was significant difference(P< 0.05 or < 0.01).The mechanical ventilation time and hospital stay in ASV group was shorter than that in control group [(46 ± 8) h vs.(56 ± 6) h,(7 ± 2) d vs.(10 ± 3) d],and there was significant difference (P< 0.01).The thorax barotrauma was not observed in ASV group; 3 cases showed subcutaneous emphysema and 2 cases showed pneumothorax in control group.Conclusions ASV mode could decrease airway peak voltage and platform pressure,improve arterial blood gas and lung dynamic compliance,shorten mechanical ventilation time and hospital stay.It is safe and effective for patients with severe asthma.
6.Changes of coagulation function and hemodynamic indexes in acute exacerbation of chronic obstructive pulmonary disease and effects of depside salt from salvia miltiorrhiza

Objective To investigate the changes of coagulation function and blood rheology in acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and effects of depside salt from salvia miltiorrhiza.Methods The patients with AECOPD were randomly divided into depside salt from salvia miltiorrhiza anticoagulation group (45 patients) and conventional treatment group (45 patients).At the same time 50 normal were choosed as control group (50 patients).On the 14th day after treatment,4 indexes of coagulation,D-dimer and of blood rheology in depside salt from salvia miltiorrhiza anticoagulation group and conventional treatment group were determine and compared with those of normal control group.Results The levels of Fbg (4.6 ± 0.7) g/L and D-dimer (1.58 ± 1.13) mg/L were significantly different between depside salt from salvia miltiorrhiza anticoagulation group and normal controls.After depside salt from salvia miltiorrhiza anticoagulation,The levels of FbgFbg (3.3 ± 1.2) g/L and D-dimer (0.48 ± 0.36)mg/L were decreased significantly (P < 0.01).The 1 evels of Fbg (3.3 ± 1.2) g/L and D-dimer (0.48 ± 0.36)mg/L were decreased significantly in depside salt from salvia miltiorrhiza anticoagulation group compared with those in Conventional treatment group (P < 0.01).But PT,APTT and TT level were not significantly different in three groups (P <0.05).After treatment,hemodynamic indexes:whole blood high shearing viscosity (4.76 ± 1.35)mPa.S,low shearing viscosity (8.69 ± 2.36) mPa.S plasma viscosity (2.32 ± 0.26) mPa.S and hematocrit (40.3 ± 2.38)% in depside salt from salvia miltiorrhiza anticoagulation group indicated significant differences compared to those in Conventional treatment group[(5.50.3 ± 1.34) mPa.S,(12.30 ± 2.30) mPa.S,(2.32 ± 0.26) mPa.S,(47.89 ± 3.13)%)] (P < 0.01).Conclusion Patients with in AECOPD have hypercoagulable state.depside salt from salvia miltiorrhiza could improved hypercoagulable state in acute exacerbation of chronic obstructive pulmonary disease and decrease blood viscosity.
7.Influence of Metformin on Metabolic Syndrome in Patients With Coronary Artery Disease

Objective To investigate the influence of metformin on metabolic syndrome in patients with coronary artery disease.Methods 52 patients with coronary artery disease and metabolic syndrome were randomly assigned into 2 groups.the control group with 20 cases and the study group with 32 cases.All the patients were treated with standard coronary artery disease therapy and meanwhile the study group was administered with metformin in addition to the basic therapy above.The follow-up time is one year.FPG,2h postprandial glucose,FINS,TG,TC,HDL-C,insulin resistance index and other standards were compared before and after the therapy,and between the 2 groups when the observation was finished.The rate of cardiovascular events was also compared between the 2 groups.Results Compared with the control group,the study group had lower levels of FPG,2h postprandial glucose,FINS,TG and insulin resistance index when the therapy was completed(P<0.05),and the rate of cardiovascular events was also lower(P<0.05).Conclusion Metformin could lower the levels of glucose,FINS,BMI,TG and insulin resistance index in patients with coronary artery disease and metabolic syndrome.Thus,it could reduce and undermine cardlovascular risk factors,protect the cardiovascular system,reduce the rate of cardiovascular events,which fully benefits the patients.
8.The study of metabolic characteristics in Type 2 Diabetes Mellitus with Different BMI

Objective To study the metabolic characteristics in type 2 diabetes mellitus with different BMI.Methods 233 patients with type 2 diabetes mellitus were divided into 2 groups. The first group was the BMI <25kg/m2 ,the second group with BMI ≥25kg/m2. The blood sugar、 blood-fat 、blood uric acid、type-B ultrasonic of liver were analyzed. Results The waistline、hip circumference、limosis c-peptide 、total cholesterol 、triglyceride and morbidity of fatty liver and hypertension of the second group were higher than the first group. Conclusion Over weigh patients of type 2 diabetes had IR、hyperlipemia、hypertension 、fatty liver. The rate and the severity of the artery diseases in type 2 diabetes mellitus increased wit the increase of the BMI. So weigh reduction could decrease the development of the artery diseases in type 2 diabetes.
9.Investigation on prevalence rate of Parkinson's disease in population aged 55 years old and above in Kashi, Xinjiang between 2008 and 2009

Objective To investigate the prevalence of Parkinson's disease(PD)in a community cohort of elderly in southern Xin Jiang. Methods A total of 6229 residents of Uygurs and Hans ethnicity in Kashi district were investigated from 2008 to 2009 by using a stratified, staged and cluster sampling methods. Results In this study, 57 PD patients were found. The prevalence rate of PD was 0. 7%(21/2871)in Hans and 1.1%(36/3274)in the Uygurs. The prevalence rate of PD increased with age, especially in people aged 85-89 years old. The prevalence rate of PD was 6. 0%(9/150)in illiterate people,followed by 1.0%(30/2968),0. 6%(14/2180)and 0. 5%(4/842)in those having received primary, junior high or senior high school education. Conclusions The prevalence rate of PD increased with age and decreased with education level. There was a correlation between the prevalence of PD and age.
10.Protective effect of carvedilol on abnormality of L-type calcium current induced by oxygen free radical in cardiomyocytes.

The protective effect of carvedilol on abnormality of L-type calcium current induced by oxygen free radical in single guinea pig ventricular myocytes was studied. Whole-cell patch clamp technique was used to study the effect of H2O2 (0.5 mmol/L) on L-type calcium current in single guinea pig ventricular myocytes and the action of pretreatment with carvedilol (0.5 micromol/L). 0.5 micromol/L carvedilol had no significant effect on ICa,L and its channel dynamics. In the presence of 0.5 mmol/L H2O2, peak current of ICa,L was reduced significantly (P<0.001), the I-V curve of ICa,L was shifted upward, steady-state activation curve and steady-state deactivation curve of ICa,L were shifted left and recovery time of ICa,L was delayed significantly (P<0.001). 0.5 micromol/L carvedilol significantly alleviated the inhibitory effect of H2O2 on ICa,L as compared with that in H2O2 group (P<0.01). In addition, carvedilol reversed the changes of dynamics of ICa,L induced by H2O2. It was concluded that carvedilol could alleviate the abnormality of L-type calcium current induced by oxygen free radical in cardiomyocytes. It shows partly the possible mechanism of the special availability of carvedilol in chronic heart failure.
