1.Observation of clinical effect of Xuebijing injection adjuvant therapy for severe lung infection

Objective To study the clinical effects and mechanism of Xuebijing injection combined with antibiotics in the treatment of severe lung infection in ICU.Methods 110 cases of ICU severe lung infection were randomly divided into control group(55 cases) and observation group(55 cases).The control group was administered with cefotaxime sodium and sodium benzene azole penicillin,while the observation group was co-administered with Xuebijing injection and cefotaxime sodium,sodium benzene azole penicillin.All treatment lasted for 7 days.Meanwhile,the serum levels of C-reactive protein (CRP),TNF-α,IL-6,COX-2 and SOD were measured before and after the therapy.Results After treatment,the levels of CRP and TNF-α,IL-6 were significantly reduced in the two groups[after treatment CRP,TNF-α,IL-6 levels of the control group:(46.50 ± 17.74) ng/L,(339.50 ± 112.61) ng/L,(141.20 ± 42.66) ng/L;and those in the observation group:(35.60 ± 16.89) ng/L,(268.20 ± 98.47) ng/L,(118.70 ± 39.81) ng/L;the control group:t =7.329,9.682,6.038;the observation group:t =11.012,14.335,14.335,all P < 0.01],and the reduced amplitude of the observation group was significantly lower than that of the control group (t =3.300,P < 0.01).The serum levels of COX-2 and SOD were significantly reduced [after treatment COX-2 and SOD levels of the control group:(189.50 ± 34.52) ng/L,(203.60 ± 67.26) U/mL;those of the observation group:(118.20 ± 25.36) ng/L,(162.30 ± 59.78) U/mL;COX-2:the control group:t =15.021,P < 0.01;the observation group:t =32.931,P < 0.01;SOD:the control group,t =4.183,P < 0.01;the observation group,t =7.682,P <0.01],and the reduced amplitude of the observation group was significantly lower than that of the control group(t =3.404,P <0.01).Conclusion Xuebijing injection combined with antibiotics in the treatment of severe lung infection in ICU has good effects,which is due to the inhibition of COX-2 and SOD to improve inflammation and oxidative stress damage in cells.
2.Epidemiological investigation on the prevalence of bladder hyperactivity in Huzhou,Zhejiang province

Objective To investigate the prevalence of bladder hyperactivity in Huzhou city,Zhejiang prov-ince.Methods 1 872 patients aged over 50 years old male residents were selected as research subjects in Huzhou. To investigate the prevalence of bladder hyperactivity in the elderly male population in the local area.The correlation between age,IPSS score,body mass index,diabetes and the bladder was investigated.Results In 1 872 cases,the data were complete and in accordance with the requirements of this study were 1 863 cases.1 863 cases,505 patients with bladder hyperactivity,bladder disease prevalence was 27.1%.Bladder hyperactivity patients with IPSS score (17.9 ±8.1 )points,prostate volume (36.6 ±18.5 )mL,QOL score (3.6 ±1.1 )points,residual urine volume (26.9 ±3.5 )mL in the bladder,were higher than those with non bladder hyperactivity,IPSS score (3.2 ± 16.8)points,prostate volume (28.1 ±1.3)mL,QOL score (2.3 ±1.8)points,the differences were statistically sig-nificant (t =7.277,5.910,19.814,2.406,P <0.01 -0.05).Qmax(12.6 ±6.3)mL/s in the patients with bladder hyperactivity was significantly lower than Qmax(17.1 ±7.4)mL/s of non bladder hyperactivity,and the difference was statistically significant (t =3.577,P <0.01).With the increasing age of male population,the prevalence of blad-der diseases was increased.With the increase of IPSS score,the incidence rate of bladder excessive activity increased. Diabetes,higher body mass index,the incidence of bladder excessive activity was higher.Conclusion Men with blad-der hyperactivity disorder has higher prevalence in Huzhou city,Zhejiang province.Age,lower urinary tract symptoms, diabetes,obesity are the risk factors in the incidence of bladder disease.
3.The effect of intrathecal low doses of meperidine on shivering during cesarean delivery

Objective To investigate the effect of different intrathecal doses of meperidine on shivering during cesarean delivery under spinal-epidural anesthesia.Methods Sixty parturient women,ASA grade Ⅰ-Ⅱ grade,scheduled for cesarean delivery were enrolled in four groups by random digits table with 15 cases each group.All patients underwent combined spinal-epidural anesthesia.Spinal anesthesia consisted of isobaric bupivacaine 0.5% (10 mg) in the standard group (group A),isobaric bupivacaine 0.5% (10 mg) plus 5 mg meperidine in group B,isobaric bupivacaine 0.5% (10 mg) plus 10 mg meperidine in group C,isobaric bupivacaine 0.5%(10 mg) plus 15 mg meperidine in group D.The signs and symptoms were recorded 10,20,30,60 min after anesthesia respectively.Results All patients were successfully operated,not unexpected.Four groups at each time point of vital signs,nausea and vomiting,and the number of cases of neonatal 1,5 min Apgar scores was no significant difference (P > 0.05).Group A of shivering and the third grade shivering were significantly more than the number of group B,C,D(11 cases vs.2,1,1 cases;4 cases vs.0,0,0 cases,P< 0.01).Conclusion Epidural anesthesia,intrathecal meperidine 5-15 mg can effectively prevent shivering in cesarean section.
4.Clinical analysis of 125I radioactive seeds implantation treatment in 26 cases of advanced pancreatic cancer

Objective To investigate the effect of iodine-125 seed implantation in the treatment of unresectable pancreatic carcinoma.Methods A total of 26 pancreatic carcinoma patients were retrospectively analyzed.Patients undergoing palliative operation in combination with iodine-125 seed implantation.jundice recovery,objective tumor response,pain relieved,clinical benefit response,median survival time and complication were investigated.Results Half of cases with jundice has reduced and totally normal in 3 weeks.Abdominal pain was relieved in 94.7% patients,average recovery duration was (5.0 ± 1.5) d.18 cases had gastroenterol function disorder (69.2%),gastroparesis in six cases (23%) and average recover time was (16 ± 5.1) d.24 of 26 cases were follow up study,median survival time was (12 ± 5.1) months,objective tumor response was smaller than preoperation in 9,no change in 10 and 5 cases became larger than before.Conclusions Palliative operation in combination with iodine-125 seed implantation is safe and effective in the treatment of unresectable pancreatic carcinoma.

AIM: The effects of Yigu capsule on tartrate resistant acid phosphatase (TRAP) secretion and apoptosis in rat osteoclasts were investigated in order to further explore its mechanism of preventing and treating osteoporosis. METHODS: (1) Twenty-month-old Sprague-daweley rats were randomly divided into two groups(Yigu capsule group and saline group), and the drug-containing serum and control serum were prepared. (2) The newborn Sprague-daweley rat osteoclasts were cultured with different concentrations of Yigu capsule drug-containing serum. AIM: The effects of YIGU capsule on proliferation and IGF-I mRNA protein expressions in osteoblasts were studied. Objective To investigate the preliminary experience of mechanical thrombectomy with a tri-axial system of the Solitaire AB stent through a Neuro delivery catheter to treat intracranial large artery occlusion. Methods A tri-axial system was used to deliver the Solitaire AB stent through a Neuro delivery catheter to provide intracranial aspiration in close proximity to the stent. This technique was used in 1 case of acute middle cerebral artery occlusion and 1 case of acute basilar artery occlusion. Results Successful revascularization was achieved in these 2 cases. Thrombolysis in cerebral infarction (TICI)score was 3. The clot length of acute middle cerebral artery occlusion was 3 cm and the modified Rankin Scale (mRS)score of this case was 3 at 90 days follow-up. Another patient with acute bilateral vertebral occlusion was revealed successful recanalization by angiography. Conclusion The results suggest that this technique of a tri-axial system used of the Solitaire stent through a Neuro delivery catheter can effectively retrieve clots from the occlusive artery and minimize the chance of antegrade blood flow dislodging the thrombus.
9.Construction and inhibitory effect of microRNA expression vector targeting survivin on proliferation of human colorectal carcinoma HT-29 cells

Objective To construct microRNA (miRNA) expression vector targeting surviving,and to investigate its effect on transfected human colorectal carcinoma (HT-29) cell apoptosis and proliferation.Methods miRNA targeting survivin was synthesized and transfected HT-29 cells by lipofectin.HT-29 cells were cultured in the 6 orifices.The cultured cells were divided into control,liposome,negative control and positive control groups.Transient transfected cells were collected and the proliferation index and apoptosis rate of HT-29 cells were detected by flow cytometry.The expressions of survivin mRNA and protein were detected by RT-PCR and Western blot.Results The proliferation index and apoptosis rate of the positive control group were significantly higher compared with normal group,transfection group and mock-vehicle group (17.98% ± 2.35% vs 38.04% ±2.11% vs 36.73% ±2.51% vs 36.57% ±3.05%; t =20.05,P<0.01; t =18.75,P<0.01; t=18.59,P<0.01; 19.54% ±1.74% vs 3.13% ±0.29% vs 3.70% ±0.44% vs 3.61% ± 0.50% ; t =16.40,P < 0.01 ; t =15.84,P < 0.01 ; t =15.92,P < 0.01).Survivin mRNA and protein expression levels were specifically suppressed in transfected HT-29 cells (t =0.68,P <0.01 ; t =0.58,P < 0.01; t=0.61,P<0.01;t=0.64,P<0.01; t=0.62,P<0.01;t=0.67,P<0.01).Conclusion Survivin targeted silence can effectively decrease the expression of survivin mRNA and protein,induce colorectal carcinoma HT-29 cell apoptosis and inhibit cell proliferation.
10.Clinical application of cardiopulmonary resuscitation with abdominal lifting and compression in emergency treatment

Objective To investigate the clinical value of cardiopulmonary resuscitation (CPR) with abdominal lifting and compression on patients with breathing and cardiac arrest induced by severe chest trauma.Methods Sixty-six breathing and cardiac arrest patients induced by severe chest trauma admitted to the General Hospital of Jingyuan Coal Industry Group Company from October 2011 to October 2016 were enrolled,and they were divided into abdominal lifting and compression group (n =32) and unarmed abdominal compression group (n =34) by random number table.The patients in both two groups were given the airway open,respiration support,defibrillation treatment,venous access establishment,vasoactive drugs application and other conventional treatments.On the basis of the routine treatment,the patients in abdominal lifting and compression group were given application of abdominal lifting and compression device with 100 times/min frequency and continuously alternating press down to lift the abdomen,the amplitude of pressing and pulling were 3-5 cm below or above the original level of the abdomen.Those in unarmed abdominal compression group were given abdominal CPR pressing method by hand,the frequency of pressing and depth of subsidence was the same as abdominal lifting and compression group.Heart rate (HR) and arterial blood gas at 30 minutes after CPR as well as the success rate of resuscitation were compared between the two groups.The changes in HR,mean arterial pressure (MAP) and pulse oxygen saturation (SpO2) before and 30 minutes and 60 minutes after CPR were dynamically observed in patients with restoration of spontaneous circulation (ROSC) after abdominal lifting and compression CPR treatment.Results Compared with the unarmed abdominal compression group,HR (bmp:136.13±6.14 vs.148.45±5.16) and arterial partial pressure of carbon dioxide [PaCO2 (mmHg,1 mmHg =0.133 kPa):48.51 ±2.60 vs.62.51 ±2.50] at 30 minutes after CPR in abdominal lifting and compression group were significantly lowered,and arterial partial pressure of oxygen (PaO2) was significantly increased (mmHg:88.07±3.92 vs.74.12±2.12,all P < 0.05).Four patients with ROSC were found in abdominal lifting and compression group,and 2 in unarmed abdominal compression group.The success rate of resuscitation in abdominal lifting and compression group was significantly higher than that of unarmed abdominal compression group (12.50% vs.5.82%,P < 0.05).In 4 patients with ROSC after abdominal lifting and compression CPR showed a downward trend in HR and an upward trend in MAP and SpO2 with CPR time prolongation.Conclusions The effect of abdominal lifting and compression CPR is better than that of unarmed abdominal compression CPR,which is of great value for the life saving of patients with breathing and cardiac arrest induced bv severe chest trauma.
