2.Investigation of ?-lactamase genotypes of Chryseobacterium/Flavobacterium spp.

Objective To understand the distribution of the genotypes of ?-lactamases in Chryseobacterium/Flavobacterium spp.Methods Minimum inhibitory concentrations (MICs) of 43 Chryseobacterium meningosepticum strains,22 Chryseobacterium indologenes strains and 10 Chryseobacterium gleum strains against 15 antibiotics were determined by agar dilution method.3-D test and modified 3-D test were used to identify carbapenamase.2-mercaptopropionic acid inhibitory test was used to confirm metallo-?-lactamases (MBL).Genes of ?-lactamases were amplified with 6 pairs of primers special for Chryseobacterium/Flavobacterium spp.and the amplified genes were sequenced.Results MIC_(50) and MIC_(90) of quinolones were lower comparing to other antibiotics.MICs of C.gleum against 15 antibiotics were lower than other Chryseobacterium/Flavobacterium spp.Among 43 C.meningosepticum strains,26 strains (60.5%) produce MBL,but all strains (100%) produced extended-spectrum ?-lactamases (ESBLs);12 C.indologenes strains (68.8%) produced MBL;6 (60%) C.gleum strains had MBL.Genotypes of MBL in C.meningosepticum strains were Bla-B 1,2,3,5 and 11,and Bla-GOB 2,4,6 and 8,respectively.Only one genotype,namely CME-1,was identified for ESBL in C.meningosepticum.The genotype of MBL in 3 C.indolgenes strains was IND-1,and the 6 C.gleum strains contained CGB genotype.Meanwhile,there were 8 C.indolgenes strains and 3 C.gleum strains were confirmed to produce ?-lactamase,but their genotypes were unable to be detected using the current primers,implying that there were possible novel genotypes.Conclusions Investigation of genotypes distribution of ?-lactamase in Chryseobacterium/ Flavobacterium spp.can provide theoretical evidences and rational in the selection of antibiotics,control of noscomical infection and development of novel antibiotics.
3.Breast malignant tumor following polyacrylamide hydrogel injection: Two cases report

BACKGROUND: Breast augmentation with polyacrylamide hydrogel injection has been used nearly for a decade in some Chinese medical therapy units. More patients need to be removing injections or a second augmentation due to complications. More attention should be paid for possibility of complications concomitant with breast malignant tumor. OBJECTIVE: Two cases of breast invasive duct carcinoma diagnosed after removing injection were analyzed retrospectively in order to increase the importance of detection rate of breast malignant tumor. METHODS: Two cases in all 41 assembled patients which were removed polyacrylamide hydrogel injections from 82 breasts were diagnosed with invasive carcinoma. The characteristics of medical history, physical diagnosis, image diagnosis and pathological examination were analyzed retrospectively. RESULTS AND CONCLUSION: The detection rate of breast malignant tumor should arise more attention in patients requiring removal of polyacrylamide hydrogel injection. The following aspects should be emphasized such as the comprehensive analysis for the results of physical diagnosis and image diagnosis, tumor-free operation with more samples from suspicious nodules and frozen pathological examination. The principles of cancer surgery should be followed in pathological diagnosed cases in case of iatrogenic metastasis and spread of breast malignant tumor.
4.Evaluation and comparison of MRSA 1D chromogenic agar medium and other 3 methods for detecting and identifying methicillin-resistant Staphylococcus aureus

Objective To evaluate MRSA ID chromogenic agar medium for detecting and identifying methieillin-resistant Staphylococcus aureus(MRSA).Methods 118 Staphylococcus spp. clinical isolates(including 108 Staphylococcus and 10 coagulase-negative Staphylococcus(CNS)were identified by Bacteria Auto-Identification System.MRSA ID Chromogenic Agar,oxacillin screening agar, and cefoxitin disk diffusion method were compared for detecting methicillin-resistant Staphylococcus aureus (MRSA).Meanwhile,detection of mecA gene by polymerase chain reaction(PCR)was considered as the gold standard.Escheriehia coli ATCC25922,Staphylococcus aureus ATCC25923,Pseudomonas aeruginosa ATCC27853,Enterococcus faecalis ATCC29212 were used for interference test.Results meeA gene of 108 Staphylococcus was detected by PCR and 80 strains were positive.Compared with PCR,the agreement of screening MRSA by MRSA ID chromogenic agar,oxacillin screening agar and cefoxitin disk diffusion method were 100%,97.5%,and 100%,respectively.Only one methicillin-sensitive Staphylococcus(MSSA)and one CNS were shown in wrong color and became reseda micro-colonies on the MRSA ID chromogenic agar. Standard references strains did not grow on the MRSA ID chromogenie agar.Conclusion MRSA ID chromogenic agar can be used to rapidly and initiatively detect MRSA,which is useful to take quick infection control measures.

6.Effect of Cordceps Sinensis on the expression of ICAM-1 and VCAM-1 in the kidney of spontaneously hypertensive rats

Objective To observe the effect of Cordceps Sinensis (CS) on the expression of intercellular adhesion molecule-1 (ICAM-1) and vascular cell adhesion molecule-1 (VCAM-1) in the kidney of spontaneously hypertensive rats(SHR), and to investigate the mechanism of CS. Methods Male SHRs (23 week old) were randomly divided into 4 groups: a group without any treatment (Group S), a group treated with Cordceps sinensis at 4 F), and a group received daily intragastric administration of CS at 4 male WKY rats were used as normals controls. At the end of 8 weeks, all rats were sacrificed. Serum creatinine(Scr), 24 h urinary protein count, and the expression of ICAM-1 and VCAM-1 were examined by immunohistochemical technique and RT-PCR. Results Compared with the WKY rats, blood pressure, 24 h urinary protein count, Scr,and the expression of ICAM-1 andVCAM-1 in the kidney of SHR significantly increased (P<0.05).Compared with Group S, blood pressure decreased after treatment by fosinopril (P<0.05). Compared with Group S, the levels of Scr, 24 h urinary protein count, and glomerular lesion were significantly reduced in the CS and/or fosinopril treatment group. The expression of ICAM-1 and VCAM-1 was significantly decreased in these groups (P<0.05).Conclusion CS may play a role in the protection and anti-fibrosis in the process of renal injury in SHR through reducing the expression of ICAM-1 and VCAM-1.
7.Uropathogenic Escherichia coli with usp gene induces the rapid early stage apoptosis of HeLa cells

Objective To understand the distribution of pathogenic genes in uropathogenic Esche-richia coli (UPEC) from urinary specimen and to analyze the pathogenicity of UPEC and their mechanism of apoptosis to HeLa cells. Methods We have analyzed 6 pathogenic genes among the 28 strains of the clini-cally isolated E. coli from urinary tract infected patients. The 6 pathogenic genes were surveyed by using the PCR amplification of the target genes. The adhesion experiments and tryphan-blue staining was used to screen the phenotype of the pathogenic strains, while Annexin V/PI method was applied to study the strains to cause apoptosis of the HeLa cells, which was further confirmed with electronic microscopy. Results We have detected 6 strains that carried the usp gene. Phenotype screening identified two high virulent isolates (strain 6N and 27N) from the 6 strains. Strain 27N and 6N contained very similar virulence gene profile ex-cept that strain 27N did not contain usp gene. Both strains can destroy HeLa cell within 3 hours causing cell death. Results of apoptosis detected by flow cytometry revealed that strain 6N induced 20.75% of HeLa cells to an early stage apoptosis within 1.5 hours. On the other hand, strain 27N induced only 1.55% of HeLa cells to apeptosis. Conclusion High virulent UPEC strain carrying usp gene can induce HeLa cell rapid early apoptosis.
8.Laparoscopic anatomical resection of left lateral liver

Objective To evaluate the feasibility of and indications for laparoscopic left lateral hepatectomy. Methods The candidates for laparoscopic left lateral lobectomy were 7 cases with liver lesions including 2 cases of primary liver cancer,5 cases of liver hemangioma. Results These patients underwent laparoscopic hepatectomy uneventfully. The operation time averaged at 210 min. The blood loss in operation averaged at 700 ml. The postoperative hospital stay averaged at 5.7 days. No complications occurred. Conclusion Based on these preliminary results, we conclude that laparoscopic anatomical left lateral lobectomy can be carried out safely and effectively by a surgeon with the sufficient expertise of laparoscopy and hepatobiliary surgery.
