1.Comparison of the effect and compliance of Rifampicin by different administration in treating pulmonary tuberculosis in elderly patients

Objective To compare the effect and compliance of Rifampicin by vein and oral in treating pulmonary tuberculosis in elderly patients who were primary treated and had positive sputum. Methods A total of 100 first-time smear-positive pulmonary tuberculosis patients were randomly divided into two groups, the treatment group and the control group,with 50 cases in each group. During the observation enhancement period,patients of the treatment group were given HZE added rifampin inoculation fluid and HEZ plus rifampin capsules for the control group.The indicators were recorded, such as:the rate of digestive discomfort, liver digfunction ,symptom improvement, sputum smear negative conversion at the end of the second month and chest X-ray improvement. Results In therapy group and control group,the rate of digestive discomfort was 14% and 82% respectively,P <0. 05 ;the rate of liver disfunction at the first weekend was 16% and 12% respectively(P > 0. 05 ) ;The rate of liver disfunction at the second weekend was 36% and 30% respectively(P >0. 05) ;The rate of the symptom improvement was 96% and 70% respectively ( P < 0. 05 ) ;The rate of improvement remarkably in chest film was 80% and 32% respectively( P < 0. 05 ) ;The rate of sputum smear negative conversion at the end of the second month was 52% and 34% ,P >0. 05 ;the rate of.sputum smear negative conversion at the end of the sixth month was 96% and 82% (P < 0. 05 ) ;The rate of chemotherapy formula changed during the therapy was 14% and 82% ( P < 0. 05). Conclusion The Rifampicin was used by vein in elderly PTB that primary treated and got positive sputum could get better effect indexes,less side effect in digestive tract,similar liver disfunction and fine compliance to the standardized chemotherapy formula.
2.The effect of positive psychological intervention based on PERMA model on the fear of cancer recurrence in breast cancer patients

Objective:To explore the effect of positive psychological intervention based on PERMA model on the fear of cancer recurrence and psychological capital in breast cancer patients during chemotherapy.Methods:A total of 60 breast cancer patients who received chemotherapy from November 2019 to January 2020 were divided into intervention group and control group with 30 cases in each group. The control group received routine health education. On this basis, the intervention group was given 8 times of positive psychological intervention. The general situation questionnaire, Fear of Progression Questionnaire-Short Form (FoP-Q-SF) and Positive Psychological Capital Questionnaire (PPQ) were used to evaluate the results of the two groups before and after intervention.Results:Before, 4 weeks after and 8 weeks after the intervention, the score of fear of cancer recurrence in the intervention group were 39.67±4.24, 34.60±3.78 and 31.17±3.60. The difference of the score of fear of cancer recurrence at different time points was statistically significant ( F value was 257.757, P<0.05). The scores of psychological capital were 113.20±19.75, 125.57±18.79 and 134.43±17.59. The difference of psychological capital level at different time points was statistically significant ( F value was 154.225, P<0.05). Conclusions:Positive psychological intervention based on PERMA model can reduce the fear of cancer recurrence in breast cancer patients during chemotherapy and improve their psychological capital level.
3.Population pharmacokinetic/pharmacodynamic modeling of warfarin by nonlinear mixed effects model.

The study aimed to establish a population pharmacokinetic/pharmacodynamic (PPK/PD) model of warfarin. PCR-RFLP technique was used to genotype the CYP2C9 and VKORC1 polymorphisms of 73 patients. RP-HPLC-UV method was used to determine the 190 plasma concentrations of warfarin. Application of NONMEM, the clinical information and 263 international normalized ratio (INR) monitoring data were used to investigate the effect of genetic, physiological, pathological factors, other medication on clearance and anticoagulant response. The final model of warfarin PPK/PD was described as follows: CL = θCL · (WT/60)θWT · θCYP · eηCL (if CYP2C9*1/*1, θCYP = 1; if *1/*3, θCYP = 0.708); EC50 = θEC50 · θVKOR · eηEC50 (if VKORC1- 1639AA, θVKOR = 1; if GA, θVKOR = 2.01; V = θV; K(E0) = θK(E0); Emax = θEmax; E0 = θE0 · eηE0. Among them, the body weight (WT), CYP2C9 and VKORC1 genotype had conspicuous effect on warfarin PK/PD parameters. The goodness diagnosis, Bootstrap, NPDE verification showed that the final model was stable, effective and predictable. It may provide a reference for opitimizing the dose regimen of warfarin.

4.Influencing factors and health management of female patients with urinary incontinence

Objective To investigate the prevalence,related factors and individualized health management strategy of female urinary incontinence.Methods Questionnaire survey has been adopted on 4105 female subjects in the east coastal,middle and southwest areas of Shandong Province from Jan.2011 to Dec.2011.Logistic regression model was used to analyze the influencing factors of urinary incontinence.The degree of risk was conducted according to the UI affected to the quality of life.Results The available questionnaires were 4105.Logistic analysis showed that age,delivery frequency,first delivery age,newborn birth weight,perineum injury history,chronic cough and constipation were risk factors of urinary incontinence.However,the protective factors of urinary incontinence included abdominal delivery and whitecollar worker.Mild,moderate and strong impacts of urinary incontinence on the quality of life were found 77.68% (1079/1389),21.38% (297/1389) and 0.94% (13/1389) of the participants,respectively.Unfortunately,doctor's office visiting was as low as 1.08% (15/1389).Along with the increase of age,parity,age of first time delivery,injury of perineum,chronic cough,constipation,may add the risk of incidence of urinary incontinence while caesarean section and brainwork may decrease the risk of incidence of urinary incontinence.Conclusions The overall of incidence of urinary incontinence in Shandong province is in a high level.Urinary incontinence may significantly reduce the quality of life of the patients.More investigation on individualized health management strategy for female patients with urinary incontinence should be needed in the future.
5.Comparison of four pre-column treatment methods for human plasma coagulation factorⅧpre-purification

Objective To compare four pretreatment methods of human plasma coagulation factor Ⅷ( FⅧ) before column purification.Methods Cryoprecipitate was dissolved in Tris , before the supernatant was treated with glycine precipitation, PEG precipitation, acid precipitation and aluminum hydroxide gel adsorption , respectively.Activated partial thromboplastin (APTT) was used to measure the activity of the supernatant clotting FⅧ after treatment.The total activity recovery and specific activity of the final samples were used to weigh the efficacy of those methods .The purity of the intend-ed protein was estimated by non-reducing SDS-PAGE electrophoresis .Results Total activity recovery of glycine precipitati-on was the highest (94.00%±7.60%), followed by that of acidic precipitation (89.47%±2.60%) and PEG precipita-tion (80.92%±9.67%) methods.The lowest was aluminum hydroxide gel adsorption (78.65%±7.52%).Glycine precipitation and PEG precipitation could more effectively remove contaminating protein than acid precipitation and aluminum hydroxide gel adsorption .Treated by four different methods , the specific activity of FⅧ of glycine precipitation sample was the highest (0.6856 ±0.1258 IU/mg), followed by PEG precipitation (0.5773 ±0.0787 IU/mg) and acidic precipitation (0.3885 ±0.0301 IU/mg).The specific activity of aluminum hydroxide gel adsorption was the lowest (0.2879 ±0.0472 IU/mg).Conclusion PEG precipitation is more effective for the actual production process than the other three methods .
6.The discussing of nurses collocating and training in clinical health education

The setup of clinical education nurses had the advantages as follow: First, the clinical health education with high levels and high qualities can effectively strengthen the patients attending consciousness;second,scientific trainings of health education nurses were useful to pre control the qualities of the health education and improve the level; At last, respecting and caring for patients were the key points to make the patients satisfy the health education.
7.STUDIES ON THE EFFECT OF VITAMIN B12 AND FOLIC ACID ON RADIATION DAMAGE 1. URINARY EXCRETION OF FORMIMINOGLUTAMIC ACID

Weanlimg male rats fed on synthetic diet containing 1% succinylsulfa-thiazole and devoid of vitamin B12 and folic acid for 3-5 weeks, developed syndromes of the vitamins deficiency, i.e. poor food consumption, low growth rate, decrease in white cell count and marked increase in formimi-noglutamic acid excretion.Nevertheless, the others had an intake of vitamin B12 (0.02 ?g/day/-rat) and folic acid (0.5?g/day/rat) for a 36-day period, also showed signs of vitamins deficiency. But when given a daily dose of 0.1 ?g of vitamin B12 and 2.5 ug of folic acid, the rats had a good appetite and grew well, and excreted more formiminoglutamic acid than normal rats, when they were given a loading dose (0.05 g/rat) of histidine. Under the similar feed- ing condition, rat ingesting a daily dose of 0.3 ug of vitamin B12 and 7.5ug of folic acid, showed no obvious syndomes of the vitamins deficiency.After the rats were fed on the same diet and given with vitamin B12 (0.02 ug/day/rat) and folic acid (0.5ug/day/rat) for a 36-day period, the animals developed increased radiosensitivity. After total-body irradiation (700 rad), the food consumption, growth rate and survival rate in this group were lower than those in other groups ingesting individually 0.1 ug of vitamin B12 and 2.5 ug of folic acid or 0.3ug of vitamin B12 and 7.5 ug of folic acid daily.After irradiation, in the group of 0.02 ug of vitamin B12 and 0.5 ug of folic acid or 0.1 ug of vitamin B12 and 2.5 ug of folic acid in daily in-gestion there was a considerable increase in formiminoglutamic acid with a loading test of histidine(0.05 g/rat) than before. The results showed that the radiation intensified vitamins deficiency in rats.
8.The study of mannose-binding lectin gene polymorphism and serum concentration in patients with systemic lupus erythematosus

Objective To investigate the relationship between polymorphism of codon54of the man-nose-binding lectin(MBL)gene and serum concentration in patients with systemic lupus erythematosus(SLE).Methods The MBL gene54(GGC/GAC)polymorphism was analyzed by polymerase chain reaction and restrict fragment length polymorphism(PCR-RFLP)in44patients with SLE and45healthy controls.Serum levels of MBL were also determined by ELISA.Results The frequencies of the wild homozygons(GGC/GGC)and het-erozygotse(GGC/GAC)of MBL codon54did not differ significantly between patients(GGC/GGC71%,GGC/GAC27%,GAC/GAC2%)and healthy controls(GGC/GGC78%,GGC/GAC22%,GAC/GAC2%).The serum MBL level of the three genotypes was(1.8?0.6)mg/L(n=31),(1.5?0.6)mg/L(n=12),1.17mg/L(n=1)respective-ly in SLE group;(3.0?1.0)mg/L(n=35),(2.2?1.0)mg/L(n=10)in control group.The(GGC/GGC and GGC/GAC)genotypes in SLE group were significantly lower than the control group.Conclusion The condon54mutation of the MBL gene may be not associated with low serum level of MBL in patients with SLE.
9.Serum nesfatin-1 levels in gestational diabetes mellitus in relation to insulin resistance and pancreatic β-cell function

The association of serum nesfatin-1 levels with insulin resistance and pancreatic β-cell function in gestational diabetes mellitus was investigated.Oral glucose tolerance test(OGTT) was performed in ninety pregnant women from 24,to 28 gestational weeks.They were divided into three groups according to OGTT:45 nomal controls,27 gestational diabetes mellitus with fasting plasma glucose (FPG) of 5.1 mmol/L to 7.0 mmol/L (GDM1),18 gestational diabetes mellitus with FPG more than 7.0 mmol/L (GDM2).Serum nesfatin-1 levels were significantly higher in patients with GDM1 and GDM2 than in controls (P<0.01),and in GDM2 group it was also higher than GDM1 group(P<0.05).Fasting serum nesfatin-1 was positively correlated with FPG,30 min plasma glucose,1 h plasma glucose,2 h plasma glucose,homeostasis model assessment for insulin resistance,and PGAUC,but negatively correlated with homeostasis model assessment for β-cell function.Furthermore,multiple stepwise regression analysis showed that FPG was the independent influencing factor of serum nesfatin-1 level.Nesfatin-1 was positively correlated with insulin resistance,while negatively correlated with pancreatic β-cell function.Nesfatin-1 may play a role in the pathogenesis of gestational diabetes mellitus.
10.Pharmacodynamics Study of Different Lactone Ratios of Hydroxycamptothecin on Mice Models of H22 Hepatoma

Objective To investigate the anti-tumor effects of hydroxycamptothecin ( HCPT ) with different lactone ratios on the mice models of H22 hepatoma. Methods Mice models of H22 hepatoma were established. Tumor inhibiting rates of HCPT with different lactone ratios ( 0%, 25%, 50%, 75%, 100%) and the growth status of model mice before and after chemotherapy were observed. Serum biochemical indices were determined to investigate the effects of HCPT with different lactone ratios on hepatic and renal function of the mice. Results Positive control drug and HCPT with different lactone ratios all inhibited the tumor in mice with H22 hepatoma, the inhibition rate was 65. 30%, 12. 57%, 49. 23%, 75. 47%, 90. 06% and 93. 22%, respectively. Compared with the model control group, the living conditions of the mice in HCPT groups were improved. With increasing of lactone ratios, the hepatic injury was alleviated markedly, but the renal injury was aggravated. Conclusion There is a positive correlation between lactone ratios and its anti-tumor effect, and HCPT with 75% lactone can achieve preferable anti-tumor effect with less toxicity as compared with that with 100% lactone ratio.
