1.The Value of CT Diagnosis in Broadening of Right Upper Mediastinum:An Analysis of 56 Cases

Objective To explore the value of CT diagnosis in broadening of right upper mediastinum.Methods X-ray plain film and CT manifestations of broadening of right upper mediastinum in 56 cases proved by surgery and pathology or CT enhancement or heart ultrasound were analyzed. All cases were undergone radiography and CT scan. The contrast-enhanced CT scans were carried out in 51 cases.Results The study showed:un-invasive thymoma in 6 cases,invasive thymoma resulted in superior vena cava syndrome in 2 cases,intrathoracic thyroid tumors in 2 cases,ascending aorta or innominate artery dilatation in 14 cases,thoracic aortic aneurysm in 3 cases,pulmonary artery stenosis resulted in aortopulmonary dilatation in 1 cases,right sided aorta in 2 cases,lymphoma in 4 cases,the right mediastinal lung carcinoma in 16 cases,larvate focus of lung carcinoma in 2 cases,low esophageal carcinoma resulted in upper esophagectasia in 2 cases,foreign body of esophageal secondary upper mediastinal abscess in 2 cases. The cases proved by plain and contrast-enhanced CT scan,operation,pathology and heart ultrasound were 33,10,12 and 1 cases respectively. Conclusion CT scan is a effective method in the diagnosis of broadening of right upper mediastinum.
2.Image quality evaluation of 64-slice spiral CT angiography in diagnosis of coronary artery disease

Objective To assessed the image quality and diagnostic value of the coronary artery diseases with 64-slice spiral computed tomography (64-SCT) angiography. Methods A retrospective image analysis of 39 patients who were diagnosed with coronary heart disease by both the 64-SCT and the invasive coronary angiography (CAG). Results In 507 coronary artery segments (13 coronary segments for each patient),1-,2-,3-class image quality by 64-SCT were in 185 (36.5%),236 (46.5%),86 (16.9%) segments respectively,and the heart rate range was respectively 62?5,66?4 and 75?10 per minute. Totally 421/507 (83.1%) segments that were of 1-,2-class image quality,met the requirements of clinical diagnosis,whereas 86/507 (16.9%) segments could not be sufficiently evaluated because of severe calcifications (51/86,59.3%) and/or motion artifacts (35/86,40.7%). Taking CAG as the gold standard diagnostic technique for coronary heart disease,of the lesions≥50% stenosis by 64-SCT (without any exclusion criteria),the overall sensitivity,specificity,accuracy,positive predictive value,and negative predictive value were 89.8%,94.9%,93.3%,88.5%,95.5%,respectively. Conclusion Our results indicate high diagnostic accuracy of coronary artery disease by 64-SCT compared to CAG. 64-SCT is a simple,convenient,safe and reliable,non-invasive effective method for diagnosis of coronary heart disease.
3.Preliminary Study on Error Control of Medical Devices Test Reports Based on the Analytic Hierarchy Process.

In this paper, the common errors in medical devices test reports are classified and analyzed. And then the main 11 influence factors for these inspection report errors are summarized. The hierarchy model was also developed and verified by presentation data using MATLAB. The feasibility of comprehensive weights quantitative comparison has been analyzed by using the analytic hierarchy process. In the end, this paper porspects the further research direction.

4.Changes of T-lymphocyte function subsets and IL-4 and IFN-γ after irradiation injury

Objective To observe the effects of T-lymphocyte function subsets,IL-4 and IFN-γ cell factor in different dose and stage after irradiation.Methoda The C57BL/6j mice were divided into shammed irradiation group and model groups.The radiation hurt model was induced by 60Co gamma rays(0.7,1.4,2.8 and 5.6 Gy).The changes of T-lymphocyte subsets CD3,CD4,CD8,IL-4 and IFN-γ in spleen cells were analyzed by flow cytometry in acute injury stage and recovery stage after irradiation.Results The lymphocyte subsets CD3 +,CD4+ and CD8 + decreased after irradiation,which were related to the irradiation dose.At 1 day after irradiation,the decreasing level of IFN-γ was higher than that of IL-4.When irradiation dose was over 2.8 Gy,IL-4 / IFN-γ showed a markedly increased compared with control group.At 25 days after irradiation,CD3 +,CD4+,CD8 +,CD4 +/CD8 +,IL-4 and IFN-γ recovered obviously,but they did not recover to the normal level of shammed irradiation group.Conclusions The depression of mouse immune function induced by γ-irradiation might be caused by changes of CD3,CD4,CD8 and CD4/CD8 ratio,especially the imbalance of IL4 and IFN-γ.

6.Practice and effects of pregnancy weight control program based on structure - process - outcome model

Objective To evaluate the effects of pregnancy weight control program based on structure- process- outcome model to improve women′s health status and pregnancy outcomes. Methods On the basis of Donabedian′s structure- process- outcome quality of care model, a pregnancy weight control program designed and carried out for the middle and late pregnancy. The structure, process and outcome of the pregnancy weight control program were evaluated to ensure the quality of the pregnancy weight control program. In addition, the women′s pregnancy outcomes were investigated after intervention. Results the prenatal BMI in the intervention group [ (23.06±1.11) kg/m2 ] were lower than those in the control group [(25.52±1.89) kg/m2 ], and the incidence of cesarean section (including cesarean section without indication), neonatal macrosomia, labor block, neonatal asphyxia, postpartum hemorrhage, pregnancy induced diabetes and hypertension in the intervention group were significantly lower than those in the control group, P<0.01 or 0.05. Conclusion Donabedian′s structure- process- outcome quality of care program is an effective approach to improve women′s pregnancy outcomes.
7.The preparation of 99Tcm-J591 and its SPECT imaging of nude mice bearing human prostate cancer

Objective To study the binding performance of 99Tcm labeled anti-human prostatic specific membrane antigen (PSMA) monoclonal antibody J591 (99Tcm-J591) and prostate cancer cells in vitro,the biodistribution and SPECT imaging of 99Tcm-J591 in nude mice bearing human prostate cancer in vivo.Methods The monoclonal antibody J591 was labeled with 99Tcm by improved Schwarz method.Labeled antibody was purified by Sephadex G-50.The labeling efficiency and radiochemical purity were measured by paper chromatography and trichloroacetic acid method.The binding performance of J591 and prostate cancer cells was measured by flow cytometry in vitro.The nude mice bearing PSMA-positive C4-2 prostate carcinoma xenografts served as experiment group,mice bearing PSMA-negative PC3 tumors served as control group.6.2-8.5 MBq of 99Tcm-J591 (25 μg) was intravenously injected into mice.Gamma imaging was performed 2,6,12 and 24 h after injection,T/NT was calculated by ROI technique.After scanned 12 h post injection,4 mice of the experiment group and 5 mice of the control group were sacrificed and the tracer in vivo biodistribution was measured by gamma-counting,and the % ID/g was calculated.Two-sample t test was carried out to validate significant difference of %ID/g between two groups.Results The labeling efficiency and radiochemieal purity of 99Tcm-J591 were (78.9±6.2)% and (92.3±5.1)%,respectively,and the specific activity of 99Tcm-J591 was 68.7 MBq/mg.The antibody J591 and 99Tcm-J591 could strongly combine with PSMA-positive C4-2 cells in vitro,and didn't combine with PSMA-negative PC3 cells in vitro.SPECT imaging results showed that radioactive concentration was obvious in tumor 6 h post injection,the concentration scope became large and the tumor image was clear 12 h post injection.T/NT was 1.9±1.1 at 2 h,4.3±1.8 at 6 h,5.6±2.7 at 12 h,1.4±0.6 at 24 h,respectively.In the control group,no radioactivity concentration was found in tumor,and T/NT was less than 2.The biodistribution results showed that %ID/g of tumor tissue was 20.1±5.2 in the experiment group and 5.8±2.6 in the control group,and there was significant difference (t=5.37,P<0.001).No significant tracer uptake occurred in other tissues and organs between the two groups (all t< 1.98,all P>0.05).Conclusion The immunoactivity,characteristics of biodistribution and tumor targeting property of monoclonal antibody J591 show promising future and potential values in diagnosis and therapy of prostate cancer.
8.Nursing care in osteopetrosis treated by optic nerve decompression under image guidance system combined with endoscopic approach

AIM: To explore the nursing cooperation highlights of eight osteopetrosis patients underwent optic nerve decompression via transsphenoidal microsurgical approach instead of routine pathway, and to improve the quality of nursing cooperation. ·METHODS: We enrolled 8 cases ( left eye in 3 cases, right eye in 5 cases ) of osteopetrosis patients referred from the Eye Hospital of Wenzhou Medical University during February 2012 to November 2016. Patients received ophthalmic examinations including visual acuity and diagnostic imaging tests in pre-operation and post -operation. All eyes were performed surgical optic nerve decompression through endoscopic approach in assist of image guidance system. We retrospectively analyzed the clinical data and surgical cooperation procedure of these cases and summarized nursing cooperation experience. ·RESULTS:The operations of 8 patients were completed successfully without massive hemorrhage. Mean visual acuity improved from pre-operation (2. 5±2. 1) to post-operation (3. 4±1. 9). Cerebrospinal fluid leakage occurred in 1 patient and was instantly repaired during the operation. We performed the nursing strategy as postural drainage, condition monitoring and conscious assessment intra-and post-operation. ·CONCLUSION: It is the critical for this kind of surgery that both circulating nurse's high-skilled cooperation to the connection and operation of the navigation system, to treat with complication during the surgery, and scrub nurse's sufficient preparation of surgical instruments and consumables, proficient equipment delivery, meticulous management, use and maintenance of equipment.
9.Research advance on BAP1 and its function in tumor suppression

BAP1 is a member of the ubiquitin C-terminal hydrolases (UCH) subfamily of deubiquitylases with basic function of removing monoubiquitin or ubiquitin chains from the specific substrate proteins. As well, it is a key factor in regulating gene expression, cell cycle, cell differentiation,cell apoptosis and DNA damage response, dependent or independent of its deubiquitination function. Evidences haverevealed that mutation or downregulation of BAP1 can prominently increase the occurrence of cancers, including uveal melanoma, mesothelioma, renal cell carcinoma, breast cancer and lung cancer. Currently, the tumor spectrum and the pathogenic mechanism on BAP1 have not been illustrated clearly, and need to be further researched,which might bring a new opportunity in treatment of cancers.
10.Patchy echogenicity of the liver in patients with chronic hepatitis B does not indicate poorer elasticity

PURPOSE: This study was conducted to investigate whether the presence of patchy echogenicity in the liver of patients with chronic hepatitis B (CHB) is predictive of liver stiffness. METHODS: A total of 200 CHB patients with and without patchy echogenicity of the liver were assigned to two groups, with 100 patients in each group, and 32 of them underwent liver biopsy. Additionally, 80 healthy subjects, 100 inactive HBV carriers, and 100 patients with decompensated hepatic cirrhosis were assigned to the control groups. Laboratory tests and clinical data were collected, and shear wave velocity (SWV) of the liver was measured for all 480 subjects. RESULTS: The median SWV in patients with a normal liver, inactive hepatitis B virus carriers, CHB patients with and without patchy echogenicity, and decompensated hepatic cirrhosis were 1.07 m/sec, 1.08 m/sec, 1.16 m/sec, 1.16 m/sec, and 2.02 m/sec, respectively; there was no significant difference in SWV values between CHB patients with patchy echogenicity and those without patchy echogenicity. Furthermore, among CHB patients with and without patchy echogenicity, no significant difference in SWV was found according to fibrosis stage. CONCLUSION: The presence of patchy echogenicity of the liver does not indicate a higher degree of liver stiffness.
