1.The curative effect of fiberoptic bronchoscope on mechanical ventilation with merger lung infection

ObjectiveTo explore the clinical effect of fiberoptic bronchoscope on mechanical ventilation patients with lung infection. Methods42 patients with serious lung infection acted mechanical ventilation were carried by fiberoptic bronchscope to check airstract,clear the waset,waste cultivating,and lung washing. ResultsThe waste cultivating was Positive. The conditions of 39 patients were improved effectively. The PaO2 ( t =-188. 539, P <0. 001 ), PaCO2 ( t =74. 481, P < 0. 001 ), SaO2 ( t =-9. 025, P < 0. 001 ) were statistically different after fiberoptic bronchoscope treatment. ConclusionFiberoptic bronchoscope could effectively identify the etiology and provide specif.ic antiinflamation treatment ;lung washing was beneficial for the airabstract waste extraction to improve the condition which was a relatively safe and effective method.
2.Three-dimensional conformal radiation therapy for postoperative local recurrence or metastasis in colorectal carcinoma

Objective To evaluate the effects of three-dimensional conformal radiation therapy (3DCRT) on the local control and survival of patients with local recurrence or metastasis in colorectal cancer. Methods Twenty-three patients with local recurrence or metastasis in colorectal cancer were irradiated by 3DCRT. The regimen was: total dose 40-45?Gy, 5-7?Gy/f, 1f/2d. Results Of the 23 patients, 8 achieved CR and 9 PR with a total response rate of 74%(17/23). The overall 1-, 2-and 3-year survival rates were 78%,52% and 30%. Conclusion 3DCRT is able to prolong the survival and improve the life quality of patients with local recurrence or metastasis in colorectal cancer.
3.Application and effect analysis of the specialized management of telephone follow-up in patients ;with tumor intermission of chemotherapy

Objective To explore the application and effects of the specialized management of telephone follow-up in patients with tumor intermission of chemotherapy. Methods A total of 400 patients with tumor intermission of chemotherapy were randomly divided into the observation group and the control group with 200 cases in each group. Combined with the current general telephone follow-up and through the status of the investigation, content and process of specialized management of telephone follow-up in patients with tumor intermission of chemotherapy were designed and applied to the observation group. The control group did not implement the intervention except for the ordinary telephone follow-up. The two groups of patients were evaluated by self-designed questionnaire, the effect of follow-up and the satisfaction degree of the patients were evaluated. Results In the observation group (5 cases were lost to follow-up), the mental state steady, reasonable diet, moderate activity, understand medical medicine, law and adopt measures to prevent infection accounted for 73.85% (144/195), 74.36% (145/195), 67.69% (132/195), 80.51% (157/195) and 72.31% (141/195), 77.95% (152/195);while in the control group (8 cases were lost to follow-up), the proportion was 53.13% (102/192), 51.04% (98/192), 43.75%(84/192), 55.73%(107/192), 52.08%(100/192), 53.13%(102/192),χ2 values ranged from 16.84 to 27.41, P < 0.01. The satisfaction degree with follow-up time, health guidance and problem solutions during follow-up process of the observation group accounted for 67.69% (132/195), 69.23% (135/195), 86.15% (168/195), the proportion of the control group was 45.83% (88/192), 46.88% (90/192), 47.92%(92/192),χ2 values were 20.49, 49.01, 77.65 (P<0.01). Conclusions The specialized management of telephone follow-up will help to promote the treatment of patients with tumor intermission of chemotherapy of home care norms, improve the effect of follow-up and satisfaction.

The fibrovascular membrane of proliferative diabetic retinopathy(PDR)cause severe damage to the structure and function of eye.The pathogenesis and development of fibrovascular membrane are associated with muhipie cellular factors,such as transforming growth factor(TGF),connective tissue growth factor(CTGF),basic fibroblast growth factor(bFGF),vascular endothelial growth factor(VEGF),platelet-derived growth factor(PDGF),tumor necrosis factor(TNF),extracellular matrix(ECM),matrix metalloproteinases(MMPs),endothelin and chemotactic factor,etc..So,the molecular mechanism of fibrovascular membrane of PDR is extremely complex.The relationship of pathogenesis on the fibrosis of PDR to cytokines,extracellular matrix,MMPs and other factors were summarized.

Acquired immunodeficiency syndrome (AIDS) is a serious infectious disease caused by human immunodeficiency virus (HIV). Increasing cases of HIV infection have been detected in adolescents. Intravenous drug abuse and sexual behavior, which are affected by various individual and environmental factors, are the major ways of HIV infection. The current situation and influential factors of HIV infection in adolescents are introduced in this paper.
8. Comparison of morphology and growth kinetics of adult normal and herniated nucleus pulposus cells cultured in vitro

Objective: To investigate the morphology and growth kinetics of the cells derived from the adult normal and herniated nucleus pulposus tissues. Methods: The cells derived from the adult normal and herniated nucleus pulposus were cultured in vitro. Passage 1,3,5 cells were chosen for light microscope and electron microscope observation. The cell growth rate, MTT value, maximal growth density, cellular mitotic index, cloning efficiency, and other parameters were observed in normal and herniated nucleus pulposus and the results were compared. Results: Compared with adult normal nucleus pulposus cells, the cells isolated from herniated nucleus pulposus were irregularly arranged and lost polarization, with rough endoplasmic reticulum slightly dilated. Under the same cellular density and culture conditions, the growth rate of passage 1 cells from adult normal and herinated nucleus pulposus had no significant difference, but the differences of passage 3 cells(P<0.01) and passage 5 cells (P<0.05) were statistically significant. The maximal growth density, MTT value, cellular mitotic index, and cloning efficiency of passage 1 cells of adult normal and herinated nucleus pulposus were respectively 7.28±0.23 and 7.41±0.17, 0.186±0.015 and 0.173±0.008, (25.36±2.14)‰ and (23.13±2.55)‰, and (52.4±2.8)% and (47.7±2.6)% (all P>0.05); those of passage 3 cells were respectively 6.96±0.19 and 5.48±0.22, 0.154±0.018 and 0.110±0.009, (22.52±2.16)‰ and (16.68±2.35)‰, and (48.7±3.3)% and (40.7±2.5)%; and of passage 5 cells were respectively 2.42±0.13 and 0.98±0.14, 0.085±0.006 and 0.050±0.007, (15.61±1.96)‰ and (9.34±2.12)‰, and (40.2±2.3)% and (28.34±2.9)%. There were significant differences between the parameters of passage 3 and 5 cells(P<0.05). Conclusion: The adult normal and herinated nucleus pulposus cells are different in cellular morphology when cultured in vitro. The continuous proliferation ability of the herniated nucleus pulposus cells is obviously decreased than that of the adult normal control group, and aging is more easily seen in the herniated ones.
9.Effect of ketamine on plasma adhesion molecules and myocardial injury in patients undergoing cardiac valve replacement

Objective It has been shown that ketamine can effectively suppress expression of cell adhesion molecules in vitro. The purpose of this study was to investigate the effect of ketamine on plasma adhesion molecules in vivo during cardiac valve replacement.Methods Twenty ASA Ⅲ-Ⅳ patients (7 males, 13 females) aged 23-65 yrs, weighing 31-77 kg undergoing cardiac valve replacement were randomly divided into two groups of 10 patients each : ketamine group received ketamine 2 mg?kg-1 i.v. before induction of anesthesia and at the start of CPB and control group received normal saline instead of ketamine. Blood samples were taken from radial artery for determination of plasma P-selectin, L-selectin, ICAM-1, IL-6 and cTnI concentrations before induction of anesthesia (T0 ,baseline) , 20 min after initiation of CPB (T1) , 30 min after aortic declamping (T2) 4 h and 24 h after CPB (T3, T4) . Results There was no significant difference between the two groups in age, weight, operation time, CPB time and aortic cross-clamping time. 10.Urinary injuries in laparoscopic-assisted vaginal hysterectomy: A report of 5 cases

Objective To investigate urinary tract injuries in laparoscopic-assisted vaginal hysterectomy(LAVH).Methods Clinical data of 5 cases of urinary tract injuries from 415 cases of LAVH from December 1995 to January 2006 in this hospital were retrospectively studied. Results Intraoperative bladder injury occurred in 2 cases and an immediate repair was given.Postoperative ureteral injury occurred in 3 cases: 2 cases of ureteral injury were found 4 days after operation because of abdominal pain,and 1 case was found on the 30th day after operation because of massive vaginal discharge.Conclusions Urinary tract injury is one of common complications in laparoscopic-assisted vaginal hysterectomy.High-risk factors include tumor size more than 5 cm,tumor protruding to the broad ligament,and tumor adjacent to the isthmus.Surgical repair is the major treatment.
