5.Preparation, dissolution and bioavailability of lycopene-Poloxamer 188 solid dispersion

Aim: To improve the solubility, dissolution and bioavailability of lycopene by preparing lycopene solid dispersion. Methods: Lycopene solid dispersion was prepared by the solvent method using Poloxamer 188 as car-rier. The physicochemical characteristics of the dispersion were determined by DSC, ultraviolet-visible spectra and the Dissolution Apparatus Ⅱ( Paddle). The oral bioavailability of lycopene was estimated in rat after oral dosing of lycopene solid dispersion or oil preparation. The plasma concentrations of lycopene in rats were determined by HPLC. The pharmacokinetic parameters were estimated by Kinetica software package. Results: In vitro dissolution of lycopene solid dispersion was greater than those of lycopene (raw material), and the physical mixture of lyco-pene and Poloxamer 188, partly due to the existing molecular state of lycopene in the dispersion. It was also found that the relative bioavailability of lycopene solid dispersion to lycopene oil preparation was (312. 2±96. 9) % . The optimal ratio of lycopene to carrier in the dispersion was about 1: 5. Conclusion: Lycopene-Poloxamer 188 solid dispersion could be prepared by the proposed simple, low-costly procedure resulting in improved bioavail-ability of lycopene, which is worthy of further development.
6.Study of postoperative incisal effusion following posterior lumbar spinal surgery

[Objective]To analyze the causes for postoperative incisal effuscin following posterior lumbar spinal surgery, to study a protocl for classification of effusion and afford according treatments.[Method]1186 cases of posterior lumbar spinal surgery were completed in the hospital from june 2001 to june 2005,877 operations with instrumentation, Il09 oprations with non-instrumentation, to compare the rate of incisal effusion between the two groups. The patients of incisal effusion were divided into three types:Type Ⅰ:the causes are ascertained, 28 cases: Type Ⅱ: infection, 11 cases; Type Ⅲ:the causes are uncertain, 14 cases o Patients of type Ⅰ were received puncturation, etiological treatment, nutrient therapy. Patients oftype II were received invisal drainaga,debridement, surgical irrigation and culture-directed antibiotic. Patients of type Ⅲ were received puncturation, and prophylactic antibiotics. 7.Diagnosis and surgical treatment of wrist soft tissue tumors

[Objective]To explore the diagnosis and surgical procedure of soft tissue tumors!in the wrist.[Method]A retrospective study was conducted to evaluate surgical excision of 132 cases of wrist soft tissue tumors in our hospital from jan 2003 to jun 2004.The preoperative diagnosis was compared with pathologic diagnosis and surgical complications was analyzed.[Result]Totally 132 cases included 110 ganglion and 22 non-ganglion were related to 11 disease.The correct rate of preoperative diagnosis was 86.4%(ganglion 95.5%,non-ganglion 40.9%).All cases were recieved surgical excision and were followed up 2 years except 5 cases,123 cases were cured,3 cases were recurrence and 1 died.Complications were seen in 2 dorsal cases and 6 volar cases.[Conclusion]Soft tissue tumors in the wrist are related to much broader spectrum of diseases.On cureing ganglion it is important to master surgical indications and crucial points of operation procedure for different sites.Because histological features of non-ganglion is difficult to determine,surgical treatment should be adviced for initial choise.
8.Using Sengstaken-Blakemore tube as assist treatment of escophageal and gastric variceal bleeding under emergency endoscopy

Objective To evaluate the effect of Sengstaken-Blakemore tube in assisting cyanoacrylate injection combined with sclerotherapy and variceal ligation under emergency endoscopy in treating escophageal and gastric variceal bleeding. Methods 60 patients were randomly selected, and were evenly divided into 2 groups single blindly. Endoscopic cyanoacrylate injection, sclerotherapy and variceal ligation were performed in control group, and additional Sengstaken-Blakemore tube were used in experimental group. Results All the 30 patients in study group got effective hemostasis (30 of 30, 100.00 %), whereas only 23 patients in control groups stopped bleeding (23 of 30, 76.67 %) (P < 0.05). escophageal and gastric variceal were cured 19 patients in the experimental group (19 of 30, 63.33 %) , and only 4 were cured in the control group (4 of 28, 14.28 %) (P < 0.05). Meanwhile, occurence of fever, chest pain, hospital days and hospitalization costs of experimental group was significantly lower than that in control group (P < 0.05). Conclusions Treating the escophageal and gastric variceal bleeding, using endoscopic cyanoacrylate injection, sclerotherapy; and variceal ligation under emergency endoscopy combined with Sengstaken-Blakemore tube could significantly increase the therapeutic efficiency as well as reduce complications and therapy cost.

10. Preparation of dl-tetrahydropalmatine ethosomes and their ex vivo transdermal permeation properties

Objective To prepare dl-tetrahydropalmatine (dl-THP) ethosomes (ETS) and elucidate their transdermal absorption properties. Methods Dl-tetrahydropalmatine ethosomes (dl-THP ETS) were prepared by ethanol injection combined with pH-gradient active drug-loading method. Their physicochemical properties including elasticity, vesicle size, morphology and entrapment efficiency were characterized. Franz diffusion cells were used to investigate the ex vivo skin permeation characteristics of the formulation with liposomes (LPS) and tinctures being used as reference preparations. Results According to a preferred formulation of dl-THP ETS [dl-THP 100 mg, vitamin E 1.3 mg, soybean lecithin 1 200 mg, cholesterol 120 mg, absolute ethanol 9 mL and citrate buffered saline (pH 3.0) 21 mL, 0.1 mol/L NaOH solution suitable quantity (to adjust the pH value to 5.5) ], the obtained dl-THP ETS had an elasticity index of (20.1 ± 1.1) mL, an average size of (85.8 ± 0.9) nm with a polydispersity index of (0.082 ± 0.003) and an entrapment efficiency of (81.7 ± 3.2)%. The cumulative permeated drug quantity per unit area (Qn) of dl-THP ETS in 24 h was (2 306.4 ± 592.3) μg/cm2 with no significant difference compared with the Qn of the LPS [(2 434.2 ± 564.4) μg/cm2] (P > 0.05) and about 4 times of that of the tincture [(633.1 ± 218.0) μg/cm2] (P < 0.05). And the averages of RSD of the Qn at each time point were (28.37 ± 10.9)% and (62.83 ± 44.1)% for the ETS and LPS, respectively, indicating that the Qn fluctuation among samples of the ETS was smaller than that of the LPS (P < 0.05). Average correlation coefficients of (0.968 ± 0.033) and (0.882 ± 0.078) (P < 0.05) were obtained for the ETS and LPS respectively when their 24 h permeation curves were fitted to linear relationship, indicating the permeation of the former was closer to zero-order kinetics than that of the latter. Conclusion The dl-THP ETS have a high elasticity, a suitable size, a high entrapment efficiency, and enhanced and stable percutaneous absorption in line with zero-order kinetics.
