1.Source, Structure, Biological Activity and Application of Natural Arabinogalactan: A Review

Natural arabinogalactan, an important polysaccharide, has a wide range of sources, a complex structure, various biological activities, and great application potential. Natural arabinogalactan is mainly rich in plants and microorganisms, and its structure varies due to different sources, including types Ⅰ, type Ⅱ, type Ⅱ-related types, and new configurations. Natural arabinogalactan has shown a variety of biological activities, such as anti-tumor, anti-oxidation, anti-coagulation, anti-aging, blood glucose-lowering, intestinal health-maintaining, anti-inflammatory, and immunomodulatory activities. In addition, natural arabinogalactan shows good biocompatibility and low toxicity, serving as a potential material in the biomedical field. Natural arabinogalactan has been designed as a carrier in the drug delivery system to effectively improve drug stability and targeting. Natural arabinogalactan is often added to skin care products to help delay skin aging and enhance skin barrier function because of their moisturizing and antioxidant properties. Additionally, natural arabinogalactan acts as a thickener, stabilizer, and emulsifier to improve the texture and taste while enhancing the nutritional value of food products. The review of latest research reports is helpful to further understand the relationship between the structure, biological activity, and functional application of natural arabinogalactan and provides an important reference for future research and development.

In order to promote the development of grassroots hospitals through targeted assistance,ZhuJiang Hospital of Southern Medical University took the lead in responding in Guangdong Province and established a tight-ly-knit specialty alliance belonging to Zhujiang.It introduces the construction content,principles and measures of Zhujiang Tightly-Knit Specialty Alliance,and proposes a dual-promotion management model for the tightly-knit Spe-cialty Alliance.A comprehensive comparison of the changes in specialized technical capabilities and medical quality of Guangning County People's Hospital before and after aid shows that the close specialist alliance has achieved initial results in effectively promoting the sinking of resources and improving the medical technology capabilities and quality of member units.It is of great practical significance for further promoting the development of specialist alliances and filling the shortcomings of primary medical institutions.

6.METTL3 regulates glucose transporter expression in placenta exposed to hyperglycemia through the mTOR signaling pathway

Background::Alterations in the placental expression of glucose transporters (GLUTs), the crucial maternal-fetal nutrient transporters, have been found in women with hyperglycemia in pregnancy (HIP). However, there is still uncertainty about the underlying effect of the high-glucose environment on placental GLUTs expression in HIP.Methods::We quantitatively evaluated the activity of mammalian target of rapamycin (mTOR) and expression of GLUTs (GLUT1, GLUT3, and GLUT4) in the placenta of women with normal pregnancies (CTRL, n = 12) and pregnant women complicated with poorly controlled type 2 diabetes mellitus (T2DM, n = 12) by immunohistochemistry. In addition, BeWo cells were treated with different glucose concentrations to verify the regulation of hyperglycemia. Then, changes in the expression of GLUTs following the activation or suppression of the mTOR pathway were also assessed using MHY1485/rapamycin (RAPA) treatment or small interfering RNA (siRNA)-mediated silencing approaches. Moreover, we further explored the alteration and potential upstream regulatory role of methyltransferase-like 3 (METTL3) when exposed to hyperglycemia. Results::mTOR, phosphorylated mTOR (p-mTOR), and GLUT1 protein levels were upregulated in the placenta of women with T2DM compared with those CTRL. In BeWo cells, mTOR activity increased with increasing glucose concentration, and the expression of GLUT1, GLUT3, and GLUT4 as well as GLUT1 cell membrane translocation were upregulated by hyperglycemia to varying degrees. Both the drug-mediated and genetic depletion of mTOR signaling in BeWo cells suppressed GLUTs expression, whereas MHY1485-induced mTOR activation upregulated GLUTs expression. Additionally, high glucose levels upregulated METTL3 expression and nuclear translocation, and decreasing METTL3 levels suppressed GLUTs expression and mTOR activity and vice versa. Furthermore, in METTL3 knockdown BeWo cells, the inhibitory effect on GLUTs expression was eliminated by activating the mTOR signaling pathway using MHY1485. Conclusion::High-glucose environment-induced upregulation of METTL3 in trophoblasts regulates the expression of GLUTs through mTOR signaling, contributing to disordered nutrient transport in women with HIP.
7.Group cognitive behavioral therapy for insomnia in the treatment of comorbid insomnia and obstructive sleep apnea: a case report

This article reported a patient who initially presented with insomnia complaints and was subsequently diagnosed with severe obstructive sleep apnea (OSA) on polysomnography (PSG). The patient tried continuous positive airway pressure (CPAP)but gave up because wear the ventilator made it more difficult to fall asleep. Then the patient only received group cognitive behavioral therapy for insomnia (CBT-I), which not only alleviated insomnia severity but also promoted severe OSA into mild status. Such case suggested that, firstly, due to the high comorbidity of insomnia and OSA, evaluation of OSA should be considered a part worth enough attention of the clinical diagnosis and treatment of insomnia patients. Secondly, by relieving insomnia, CBT-I can alleviate both nocturnal apnea and daytime somnolence in patients with comorbid insomnia and sleep apnoea (COMISA), so the application of CBT-I should be emphasized in the treatment of such patients.
