1.Clinical Observation on 50 Cases of Schizophrenia Treated by Jieyu Huatan Anshen Formula Combined with risperidone

Objective To compare the effect and safety of Jieyu Huatan Anshen Formule(Formula for relieving stagnation,dissolving phlegm and calming the mind) combined with risperidone and risperidone used singly in the treatment of schizophrenia.Methods Totally the 100 schizophrenia patients were randomized into a control group(n=50),treated with risperidone,and a study group(n=50),treated by Jieyu Huatan Anshen Formula combined with the half dosage of risperidone.The observation lasted 8 weeks.The therapeutic effect and safety were evaluated by the positive and negative syndrome scale(PANSS),clinical global impression(CGI),and treatment emergent symptom scale(TESS).Results After treatment,the PANSS total scores and the factor scores of both groups were significantly decreased(P0.05).The in-group comparison of CGI effect showed a significant difference(P0.05).The incidence of side effect of the study group and the control control group was 18% and 52% respectively,there exsisted a significant difference(P

Hashimoto thyroiditis(HT) is the most common autoimmune thyroid disease,which can present as euthyroidism,hypothyroidism or hyperthyroidism.Currently,there are still disputes on the treatment of such diseases.This paper summarizes the researches on the HT treatment at home and abroad in recent years,introduces the latest developments of iodine restriction,thyroid hormone replacement therapy,systemic immune regulation therapy,local immune regulation therapy,statins and surgical treatment.It elaborates the international consensus on recommended iodine intake and the importance of the individualized principle of thyroid hormone replacement therapy.
3.Treatment experience of 36 cases of simple renal cysts

ObjectiveTo summarize the treatment of simple renal cysts experience,the advantages and disadvantages of the two surgical options. MethodsRetrospective analysis of 15 cases of renal cysts after general anesthesia in patients with laparoscopic cyst decompression in the clinical data and 21 cases of renal cysts underwent open surgery in patients with clinical data,comparative analysis of advantages and disadvantages of both types of surgery were conducted. ResultsIn this study,14 patients with laparoscopic surgery were successful,1 patient converted to open surgery,laparoscopic surgery in patients with blood loss,postoperative complications and time of staying hospital after operation postoperative hospital stay were significantly less than group of open surgery. ConclusionThe laparoscopic renal cyst decortication was a safe and effective treatment for simple renal cysts,compared to traditional open surgery in patients with trauma,less bleeding postoperative pain,rapid recovery,shorter hospital staying and so on.
4.Clinical and endoscopic diagnosis in the differentiation of Crohn's disease from intestinal tuberculosis

Objective To compare the clinical features and endoscopic findings of Crohn's disease(CD) and intestinal tuberculosis(ITB) in order to differentiate CD from ITB. Methods The clinical and endoscopic data from 168 patients with CD and 156 patients with ITB between June 2003 and February 2009 were retrospectively analyzed. Results The salient features of CD were male patients in predominance (male : female was 108 :60) and high incidence of colectomy (CD 33.3% vs ITB 10.9%, P<0.01). Diarrhea (66.1%), hematochezia (32.1%), perianal disease (16.1%), intestinal obstruction (28.0%) were more frequent in CD patients than in ITB patients (47.0%, 7.7%, 3.4%, 9.4% respectively, all P values<0.05). The salient features of ITB were night sweating, pulmonary tuberculosis, ascites, hyperglobulin, increased erythrocyte sedimentation rate and the positive serum antibody to mycobacterium. The endoscopic examination showed that the fissure-shape ulcer, grid-shape ulcer, cobblestone sign and intestinal stricture were more frequent in CD patients than in ITB patients (all P values <0.05). Whereas the circular ulcer and involved ileocecal valve with fixed bouche shape were more common in ITB patients (P<0.05). Conclusions The clinical characteristics are different in CD and ITB patients. The endoscopic findings including fissure-shape ulcer, grid-shape ulcer, circular ulcer, cobblestone sign and the status of involved ileocecal valve are important in the differentiation of ITB from CD.
5.Standardized early medical care of Lushan earthquake victims in army hospitals

Objective To summarize the experiences with early treatment of the wounded associated with 4 · 20 Lushan earthquake in army hospitals.Methods Emergency response programs were started and tent hospitals were set up.According to the basic principle of wartime wound treatment regulation,emergency triage,emergency medical care,transferring,specialized treatment and evacuation were performed for the wounded in this earthquake.Results After the earthquake,the emergency rescue programs were started in the hospital.Twenty minutes later,emergence medical team with full equipment were set out for the epicenter and the tent hospitals were set up.A total of 181 earthquake cases were admitted within two weeks,of which 68 (37.6%) were transferred from the scene of earthquake and 113 (62.4%) were transferred after the primary treatment (including cases evacuating from other hospitals).Forty-nine surgeries had been done,including 30 emergence ones.Thirty-four cases (including 14 severe wounded) were evacuated to superior hospitals as their vital signs turned stable.There was no death or surgical wound infection.Conclusions Peacetime complete emergence programs and materials reserves,unified and well-organized command as well as normative grading treatment system in army hospitals assure the standardized medical care with high efficiency and high quality for earthquake victims.However,more researches are needed over diagnosis methods and standards for wound severity,equipment improvement and standards for casualty evacuation.
6.Identification of Lipid Metabolism Genes in Cerebral Infarction and Intervention Effect of Huoxue Rongluo Prescription

Objective To identify lipid metabolism genes in cerebral infarction;To explore the intervention effect of Huoxue Rongluo Prescription.Methods Multi-chip combined differential analysis(GSE61616,GSE30655)was used to identify lipid metabolism genes in cerebral infarction in combination with Reactome database,and the expression differences of lipid metabolism genes in cerebral infarction were identified and verified in GSE97537 chip;Pearson correlation analysis was used to analyze the correlation of 51 cerebral infarction samples in GSE61616,GSE30655,GSE97537,GSE137595,GSE22255,GSE163614,and GSE78731 datasets;PPI,GO and KEGG analysis of lipid metabolism genes in cerebral infarction were performed through STRING database and R clusterProfiler package.SD rats were made to the model of cerebral infarction,and was administered with Huoxue Rongluo Prescription extract 11.7 g/kg by intragastric administration for 7 days.The symptoms of neurological deficit,the changes of Nissl bodies and the mRNA expressions of PLA2G4A,SPHK1,and PTGES key genes in lipid metabolism in cerebral infarction were observed.Results TSPO,CYP1B1,PLIN2,CH25H,PLA2G4A,ANGPTL4,PTGS1,SPHK1,and PTGES were identified as lipid metabolism genes in cerebral infarction,and were significantly highly expressed and positively correlated in cerebral infarction.Among them,PTGS1,PLA2G4A,and SPHK1 interacted with each other,which were the key genes of lipid metabolism in cerebral infarction;the lipid metabolism gene in cerebral infarction mainly exerted molecular functions such as oxidoreductase activity,iron ion binding,heme binding,etc.,mediating arachidonic acid metabolism,phospholipase D signaling pathway,VEGF signaling pathway,involved in regulation of lipid metabolism process,fatty acid metabolism process,fatty acid derivative metabolism process.The symptoms of neurological deficit in the model rats with cerebral infarction were severe(P<0.001),and Huoxue Rongluo Prescription could effectively improve the neurological deficit of model rats(P<0.001).The Nissl staining indicated that the neuronal structure was abnormal and the number was significantly reduced after cerebral infarction(P<0.001).Huoxue Rongluo Prescription could increase the number of neurons(P<0.001)and repair the neuronal structure.RT-qPCR showed that the key genes of lipid metabolism in cerebral infarction were significantly higher in cerebral infarction(P<0.001),corroborated with the bioinformatics results,and Huoxue Rongluo Prescription could reduce the expression of key lipid metabolism genes of PTGS1,PLA2G4A,and SPHK1(P<0.001,P<0.01,P<0.05).Conclusion Huoxue Rongluo Prescription can down-regulate the expressions of PTGS1,PLA2G4A,SPHK1,exert molecular functions such as oxidoreductase activity,iron ion binding,heme binding,and mediate arachidonic acid metabolism,phospholipase D signaling pathway,and VEGF signaling pathway.It participates in the process of lipid metabolism regulation,fatty acid metabolism,and fatty acid derivative metabolism,increases the number of Nissl bodies,improves the symptoms of neurological deficits,and exerts neuroprotective effects.
