1.Determination of alkaloids in Dendrobium nobile Lindl. by automatic potentiometric titration

AIM: Determination of total alkaloids in Dendrobium nobile Lindl.METHODS: Dried materials were subjected to exhaustive extraction with ethanol: water (9:1,v/v) under reflux.Following filtration and condensation,the residue was dissolved in 3% hydrochloric acid and filtered.The solution was refined using a strongly acidic cation exchange resin column through sequential elution.After elution of deionized H2O,the eluent with 50% of alcoholic plus 5% of ammonia was collected until alkaloids were completely desorbed and evaporated under reduced pressure.The test sample was obtained.The test sample was dissolved in glacial acetic acid and titrated with perchloric acid titrand(0.1 mol/L) with automatic titrator.The herb should contain alkaloids,calculated as dendrobium(per 1ml of perchloric acid titrand is equivalent to 26.3 mg dendrobium).RESULTS: The results of three batches of material were 0.85% ,0.63% ,1.03% respectively.CONCLUSION: The method is convenient,fast,effective and reliable.Automatic potentiometric titration can be used for determination of alkaloids in Dendrobium nobile Lindl.
3. Clinical observation of gastric emptying rate in patients with diabetes

Objective: To compare the gastric emptying rate in course-matched patients with type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM), and to explore the impact of gastric emptying rate on premeal insulin therapy design in patients with T1DM. Methods: The gastric half-emptying time (T50) and clinical data of 16 patients with T1DM and 32 course-matched patients with T2DM treated at the Department of Endocrinology and Metabolism, Renji Hospital, Shanghai Jiao Tong University School of Medicine from September 2013 to June 2014 were analyzed, retrospectively. The T50 and clinical data of the course-matched patients with T1DM and T2DM were compared. The impact of T50 on premeal insulin therapy design in patients with T1DM was analyzed. Results: The T50 of patients with T1DM was significantly longer than that of the course-matched patients with T2DM (P=0.013). T1DM patients with delayed gastric emptying rate tended to shorten the time between insulin injection and meal than those with normal gastric emptying rate. Conclusion: The gastric emptying rates of patients with T1DM are slower than those of the course-matched patients with T2DM. Measurement of gastric emptying rate not only detects abnormal gastric emptying rate in patients with diabetes but also guides the choice of premeal insulin therapy, optimizes glycemic control and avoids large glucose fluc-tuations in patients with TIDM.
4. Screening of differential proteins from follicular fluid of patients with polycystic ovarian syndrome employing protein chip array

Objective: To identify the differential proteins from follicular fluid of patients with polycystic ovarian syndrome (PCOS) employing protein chip array, and provide a theoretical basis for further study of PCOS. Methods: AAH-BLG-507 array was used to test the follicular fluid of 6 PCOS patients with normal body mass index (BMI) and 6 non-PCOS controls with age and BMI-matched. The differential proteins were identified and bioinformatics analysis was carried out by functional annotation analysis, KEGG pathway analysis and protein-protein interaction network analysis. Results: Compared with non-PCOS controls, 25 up-regulated proteins and 49 down-regulated proteins were identified from follicular fluid of patients with PCOS. The classification of differential proteins based on molecular function revealed that they were mainly involved in cytokine activity and chemokine activity. KEGG pathway analysis was performed and pathways associated with cytokine-cytokine receptor interaction and chemokine signaling pathway were significantly enriched. The core proteins filtered by protein-protein interaction network analysis were mainly chemokines and their receptors. Conclusion: Protein chip array can be used to establish the differential expressed protein profile from follicular fluid of patients with PCOS. The cytokinecytokine receptor interaction and chemokine signaling pathway may be involved in the pathogenesis of PCOS.
5.Development of Multifunctional and Multipoint Fixed Support Drainage Device for Digestive Tract.

OBJECTIVE:
To discuss the development of a multifunctional and multipoint fixed support drainage device for the digestive tract, as well as the effect of its application on animal experimental models and patients.
METHODS:
The digestive tract multifunctional and multipoint fixed support drainage device is designed according to the requirements of the various gastrointestinal surgery and interventional procedures. It has metal flaps and airbags to achieve multi point fixation. The cuffs and shears are used to achieve endoscopic removal. And through different tube diameters and lengths, surgeons can achieve different surgical purposes.
RESULTS:
A multifunctional and multipoint fixed support drainage device for the digestive tract was successfully designed and developed. The application experiment of the winged pancreatico-intestinal supporting drainage tube on animal models and patients, showed lower drainage fluid amylase level, faster amylase recovery speed, and better perioperative safety.
CONCLUSIONS
The support drainage device has the characteristics of simple operation, firm fixation, and good controllability of removal. It is an ideal choice among support drainage tubes in gastrointestinal surgery and interventional operations.

6. Role of bronchoalveolar lavage fluid analysis in the diagnosis of hypersensitivity pneumonitis

Hypersensitivity pneumonitis is a group of granulomatous lung disease caused by repeated exposure and sensitization to a variety of organic antigens. Establishing a diagnosis of HP is challenging as no gold standard for diagnosis of HP is currently available. The diagnosis is based on integration of history, clinical manifestations, pulmonary function, high-resolution chest CT(HRCT), bronchoalveolar lavage fluid(BALF) analysis and histopathological features, which is a multidisciplinary diagnosis. The BALF cell profile in HP is characterized by a significant increase in the percentage of lymphocytes. BALF cellular analysis can provide additional supportive evidence of the diagnosis of HP, which narrows the differential diagnosis of HP and helps some patients avoid surgical lung biopsy.
7. Variations and subtypes of gastrocolic trunk and its significance in colon cancer surgery

With the increasing standardization of right colon cancer radical resection, more emphasis is put on keeping the mesentery intact along with vascular root ligation and extensive lymph nodes dissection which inevitably demands higher level of surveillance on the possible complications especially around gastrocolic trunk with large variation of tributary and complex vascular course, to achieve a successful surgical and oncological outcome. This article discusses the complex anatomy of the gastrocolic trunk and its implication in the choice of surgical approaches.
9.Expression of epithelial Na+ channel in human nasal polyp.

OBJECTIVETo evaluate the expression of epithelial sodium channel in nasal polyps and normal nasal mucosa and to characterize the ENaC-mediated Na(+) absorption and the mechanism of the liquid transport in human upper airway epithelia.
METHODSThe tissue from 12 patients with nasal polyps (NP) and the normal ethmoid cornu mucosa (ECM) from 5 patients were obtained through endoscopic surgery. The expression of ENaC was detected by Immunofluorescence and the concentration of ENaC alpha, beta, gamma-mRNA were detected by RT real-time PCR.
RESULTSThe ENaC in NP group (35.79 +/- 5.47) was higher than that in ECM group (22.17 +/- 5.43, t = 4.687, P < 0.01). The expression of ENaC-alpha, beta, gamma mRNA in NP group (respectively 2.06 +/- 0.42, 1.97 +/- 0.32, 1.96 +/- 0.54) was higher than that in ECM group (respectively 1.01 +/- 0.10, 0.98 +/- 0.08, 0.97 +/- 0.06; t = 5.482, 6.659, 4.036, all P < 0.01). The mRNA expression of three subunits of ENaC was as follows: alpha > beta > gamma in both groups. The mRNA expression of alpha, beta, gamma subunit of ENaC in NP group was higher than that in ECM group(P < 0.01). The expression of three subunits of ENaC was correlated respectively with that of mRNA in NP group.
CONCLUSIONSThe up-regulation of ENaC in human NP was associated with the expression of ENaC mRNA, that made hydrops and might be one of the most important cause of the develop of nasal polyp.

10.Bladder large cell lymphoma: a case report and literature review

【Objective】 To improve the understanding and diagnosis and treatment level of ALK negative anaplastic large cell lymphoma (ALK-ALCL) by sharing the diagnosis and treatment process of a patient with ALK-ALCL treated in Hangzhou Bay Hospital of Ningbo. 【Methods】 The clinical data and diagnosis and treatment process of the patient were retrospectively analyzed, and relevant literature was reviewed. 【Results】 The patient was a young male, with recurrent gross hematuria and right low back pain as the initial symptoms.Imaging examination indicated bladder tumor.After resection, the tumor was reduced and confirmed to be ALK-ALCL.After chemotherapy and autologous hematopoietic stem cell transplantation, the patient’s condition continued to improve.During the follow-up, no recurrence was observed. 【Conclusion】 Primary ALK-ALCL in the bladder is very rare and prone to misdiagnosis and missed diagnosis in clinical practice.The successful diagnosis and treatment experience of this patient can provide clinical reference.
