1.Clinical Observation of Prednisone Acetate Combined with Leflunomide in the Treatment of IgA Ne-phropathy

OBJECTIVE:To observe therapeutic efficacy and safety of prednisone acetate combined with leflunomide in the treatment of IgA nephropathy. METHODS:80 patients with IgA nephropathy were randomly divided into observation group and control group,with 40 cases in each group. Control group was given prednisone acetate 1.0 mg/kg,qd,6 weeks later,decreasing gradually,decreasing to 0.5 mg/kg in 12th week;observation group was additionally given leflunomide 50 mg,qd,3 days later de-creasing to 20 mg. Both groups were given 3 months of treatment. Clinical efficacy,24 h urinary protein quantification,Scr and BUN levels were observed in 2 groups after treatment,and ADR were recorded during treatment. RESULTS:Total effective rate of observation group(95.0%)was significantly higher than that of control group(75.0%),with statistical significance(P<0.05). Af-ter treatment,24 h urinary protein quantification,Scr and BUN levels of observation group were significantly lower than those of control group,with statistical significance(P<0.05). The incidence of ADR in observation group(0)was significantly lower than in control group(7.5%),with statistical significance(P<0.05). CONCLUSIONS:Prednisone acetate combined with leflunomide has significant effect on IgA nephropathy,and will not increase the occurrence of ADR.
2.Optimization of Extracting Technology of Jiejiu Gankang Granules by Orthogonal Design

Objective To study the extraction process of Jiejiu Gankang granules by orthogonal test.Methods With the ethanol extraction rates of the medical materials like ginseng,radix salviae miltiorrhizae,the water extraction rates of the medical materials like hawthorn,and Tanshinone ⅡA as the parameters,the extract conditions of Jiejiu Gankang granules was optimized by orthogonal design.Results The optimal preparation process was as follows:the mixture of medical materials like ginseng and Schisandra chinensis was refluxed twice with total 10 times of 70% alcohol,2.0 hours for each time,and then the mixture of medical materials like hawthorn extracted twice with total 10 times of boiling water,2.0 hours for each time.Conclusion The optimal preparation process is reasonable and with high extraction rate of active components.

5.The Analysis of Postoperative Curative Effects of Facial Paralysis Caused by Middle Ear Cholesteatoma

Objective To study the effects of the duration and degree of the peripheral facial paralysis due to middle ear cholesteatoma on the recovery of postoperative nerve functions.Methods The clinical data of 30 cases (single ear) of patients with cholesteatoma complicated with facial paralysis were retrospectively analyzed.The mean age was 54.7±8.9 years old (range 21~77years old) including 17 males and 13 females.The duration of facial paralysis, degree and location of facial nerve injury,surgical timing of facial nerve decompression,recovery of facial paralysis after surgery were followed up.According to the course of paralysis, the patients were divided into ≤2 months group (14 cases) and >2 months group (16 cases).According to the degree of paralysis, the patients were divided into the incomplete facial paralysis group (III-IV grade,14 cases) and the complete facial paralysis group (V-VI grade,16 cases).The influence of the course and degree of paralysis on the postoperative recovery of neurological function were analyzed by the Fisher exact probability test.Results Three cases underwent open radical surgery with no facial nerve damage confirmed by intraoperative exploration, and their postoperative facial nerve functions were fully restored.Facial nerve canal damage was found in 27 cases, accompanied by facial nerve congestion, edema or granulation formation.Among them, the facial nerve damage location was the tympanic segment in 20 cases.The open radical operation and local facial nerve decompression were carried out, and the postoperative facial paralysis recovery rate was 46.67%(14/30).The facial nerve function recovery rate in less than 2 months group was higher than the >2 months group (P<0.05).The facial nerve function recovery rate of the incomplete paralysis group was higher than the complete facial paralysis group (P<0.05).Conclusion This study suggests that facial nerve damage most occurred in the tympanic segment of the facial nerve.Radical mastoidectomy and local facial nerve decompression are effective for the treatment of middle ear cholesteatoma complicated with facial paralysis.The shorter course and lighter degree of paralysis lead to the better postoperative recovery of neurological function.
6.The value of color doppler flow imaging in diagnosis of liver cirrhosis upper gastrointestinal bleeding about portal vein hemodynamic changes

Objective:To investigate the value of color doppler flow imaging (CDFI) in the diagnosis of liver cirrhosis upper gastrointestinal bleeding in portal vein hemodynamic changes. Methods: 96 cases of patients with liver cirrhosis were selected who were diagnosed in our hospital, according to the history whether patients had a gastrointestinal bleeding or not. They were divided into bleeding group(45 cases) and no bleeding group(51 cases). At the same time, we chose the hospital physical examination center of 42 cases of healthy volunteers as a control group, using color doppler flow imaging portal venous blood flow mechanics parameters, including diameter, average blood flow velocity and blood flow of portal vein(PV) and splenic vein(SV) and compare the data of the three groups.Results: Compared with control group, the patients with liver cirrhosis, the blood vessel diameter have increased whether bleeding or not. The average blood flow velocity is slower and PVF is larger, and the differences between them are statistically significant(t=3.579,t=3.670,t=4.750,t=3.951,t=6.116,t=5.371;P<0.05). Conclusion: The clinical application of color doppler flow imaging(CDFI) in patients with cirrhosis portal hemodynamic change is not only simple noninvasive, and there is important diagnostic value in the detecting parameters.
7.Application of asymmetrical Z-plasty in correction of epicanthus

Objective To investigate a new method in correction of epicanthus.Methods Asymmetrical Z-plasty was used in correcting epicanthus and lengthening palpebral fissure in 42 cases with 84 eyes.It could be combined with double-eyelid-blepharoplasty to create beautiful eyes.Results All 42 cases with 84 eyes had satisfactory outcome after half a year to 2 years follow-up.The epicanthus scar was conspicuous in the first 6 weeks and it subsided gradually.In the three cases.the hypertrophic scar occurred in the epicanthus,but it softened and became inconspieuous 6 mouths after operation.Conclusion Asymmetrical Z-plasty is a new and reliable method for correction of epicanthus with inconspicuous scar and little chance of recurrence that can be widely used.

10.THE CONNECTIONS OF THE VISUAL PORTION OF THE THALAMIC RETICULAR NUCLEUS IN GOLDEN HAMSTERS

The thalamic reticular nucleus (RT) is a sheet-like nucleus which surrounds the anterolateral aspect of the dorsal thalamus. In this study, we present data on the connections of the dorsocaudal portion of the RT in hamsters using anterograde, retrograde and transneuroual fiber tracing techniques. After injection of one eye with a mixture of ~3H-proline and ~3H-fucose (2 animals; 10 and 15 days survival time), transneuronal label was located in the dorsocaudal portion of the RT (dc RT) on both sides of the thalamus.After separate injection of horseradish peroxidase (HRP) into the thalamic lateral posterior nucleus (LP), dorsal and ventral nuclei of the lateral geniculate body (LGd, LGv) (8 animals, one day survival time), HRP labelled neurons wereobserved in the dorsal, middle and ventral area of the ipsilateral dc RT respectively.A small iontophoretic injection of a mixture of ~3H-proline and ~3H-leucine was made into different sites of area 17 in each of 3 hamsters (one day survival time),in each case an elongated patch of label was located in nearly the entire rostrocaudal extent of the ipsilateral dc RT. A caudal Injection in area 17 resulted in labelling the dorsal area while a progressively more rostral injection resulted in labelling a more ventral area of the dc RT.Thus, the dorsocaudal portion of the RT can be defined as the visual portion of the nucleus because it has connections with many of the known visual centres, namely, the visual cortex, LP, LGd, LGv Furthermore, because of the topographic connection with area 17 and the visual thalamic nuclei, the notion that the dc RT as a nucleus which is capable of processing specific and localised visual information. should be considered.
