In order to accelerate the comprehensive reform of clinical education,the Ministry of Education and the Ministry of Health decided to implement the program of excellent doctor education training plan in 2012.On the basis of the plan,taking a certain medical college as an example,on the problems such as lack of teachers,unreasonable age structure,the lack of advanced experimental technology and comprehensive talents and the lack of innovation and enterprising spirit and so on,the author puts forward some proposals from the development of experimental teaching,the teachers' personal and professional growth and the institutional and policy level,such as strengthening the foreign exchange,making learning training plan and personal development planning,establishing the evaluation system of the experiment teaching quality and opening up a variety of learning methods to improve the teachers' comprehensive ability.

3.The value of calprotectin and ischemia modified albumin in the diagnosis of adult acute appendicitis

Objective:To explore the early evaluation value of calprotectin (S100A8/A9) and ischemia modified albumin (IMA) for adult acute appendicitis (AA) and adult non-simple acute appendicitis.Methods:The data of 62 patients with histologically confirmed appendicitis and 57 healthy controls in the physical examination center of our hospital during the same period from May 2018 to October 2019 were collected. According to postoperative pathological data, patients with appendicitis were divided into the simple appendicitis group and the non-simple appendicitis group . The measurement data conforming to normal distribution were expressed as mean ± standard deviation (Mean±SD), and Student's t test was used for comparison between groups. S100A8/A9, IMA, C-reactive protein (CRP), procalcitonin (PCT), and blood routine parameters were compared after grouping.The area under the ROC curve (AUC) was used to evaluate the early efficacy of S100A8/A9 and IMA for acute appendicitis and non-simple acute appendicitis. Results:There were no significant differences in sex, age, platelet count (PLT), and red blood cell count (RBC) between the appendicitis group and healthy control (all P>0.05), while white blood cell count (WBC), CRP, PCT, neutrophils to lymphocytes ratio (NLR), S100A8/A9, and IMA levels in the appendicitis group were higher than those in healthy control (all P<0.05). The AUC of S100A8/A9 (≥366.9 μg/L), IMA (≥0.29), and S100A8/A9 combined with IMA in predicting acute appendicitis were 0.735, 0.891, and 0.913, respectively. There was no significant difference in WBC between the non-simple appendicitis group (21 cases) and the simple appendicitis group (41 cases) ( P>0.05).The levels of CRP, PCT, NLR, S100A8/A9 and IMA in the non-simple appendicitis group were significantly higher than those in the simple appendicitis group ( P<0.05). The AUC of S100A8/A9 (≥532.9 μg /L), IMA (≥0.41) and S100A8/A9 combined with IMA in predicting non-simple acute appendicitis were 0.866, 0.873 and 0.936, respectively. Conclusions:S100A8/A9 and IMA could be used as biomarkers for the diagnosis of AA, which have certain clinical value for the assessment of acute appendicitis and non-simple acute appendicitis.
4.Improving the teaching quality of cervical vertebra DR in radiology department with scenario simulation and rapid film reading

Objective:To explore the application of scenario simulation and rapid film reading training in the standardization of cervical vertebra DR examination in radiology department.Methods:Using the method of scene simulation in the examination room to take DR films of cervical vertebra, two senior technicians in charge evaluated and guided the deficiencies of junior technicians from the aspects of patient check, exposure conditions, body position placement, film printing, inspection safety and radiation protection. Every morning, DR images of cervical vertebra were randomly selected and read quickly, and the junior technicians should quickly browse the images within one minute and make quality control evaluation. Then the senior technician in charge pointed out the shortcomings of the evaluation and related improvement methods. After that, the technical specification manual of cervical vertebra DR was compiled based on the deficiencies and problems found by each technician, and 200 patients aged 18-55 years, without abnormal movement and no internal fixation before and after the establishment of standardization were randomly selected in group A (before standardization) and group B (after standardization). The image quality and repetition projection rate of group A and group B before and after the standardization were compared. SPSS 25.0 was used to analyze the data.Results:The image quality score of group B (8.83±0.33) was higher than that of group A (8.45±0.30), and the repetition projection rate of group B (2%) was lower than that of group A (9%), with statistical significance ( P<0.05). Conclusion:Through the training mode of rapid film reading and scenario simulation, the standardization of cervical vertebra DR examination technology suitable for the situation of the department has been established, which effectively solves the problem of abstraction in the technical teaching of radiology department. The standardization establishment plays an important role in standardized training, for which, the technicians can quickly grasp the work process of the department and the corresponding examination technology during participating in the standardization establishment and training process.
5.The expression and significance of programmed cell death 5 and Bcelllymphoma/lewkmia-2 in sinonasal squamous cell carcinoma.

OBJECTIVE:
To explore the expression and significance of programmed cell death 5 (PDCD5) and Bcelllymphoma/lewkmia-2(Bcl-2) in sinonasal squamous cell carcinoma (SNSCC).
METHOD:
Immunohistochemical method and Western Blot method was used to determine the expression of PDCD5 and Bcl-2 in specimen of SNSCC in thirty cases, sinonasal inverted papillomas (SNIP) in thirty-eight cases, and normal nasal mucosa in twenty cases.
RESULT:
(1) The expression of PDCD5 protein in SNSCC significantly decreased compared with SNIP and normal nasal mucosa. (2) The expression of Bcl-2 protein in SNSCC up-regulated obviously compared with SNIP and normal nasal mucosa. (3) Positive rate of PDCD5 protein and Bcl-2 protein in well, moderate and low differentiatied group is respectively 100.00%, 83.33%, 38.89% and 50.00%, 70.83% and 100.00%, the difference was statistically significant (P < 0.05). (4) In the follow-up cases, the survival rate of the patients with higher expression of PDCD5 protein was higher, but that with lower expression of Bcl-2 protein was higher.
CONCLUSION
The inactivation of PDCD5 protein and the activation of Bcl-2 protein may play an important role in the development of SNSCC, and there are a positive correlation between PDCD5 and Bcl-2 protein in SNSCC, which may be identified as a new therapeutic target.

6.Design, synthesis and biological evaluation of novel 3-(2-oxo-2-substituted acetamido)benzamides as PARP-1 inhibitors.

Poly(ADP-ribose)polymerase-1 (PARP-1) plays a significant role in the DNA repair process by catalyzing the transfer of ADP-ribose from NAD+ to its receptors. It is a promising anticancer drug target and many PARP-1 inhibitors have been developed and used in the clinical trial. In this work, a series of 3-(2-oxo-2-substituted acetamido)benzamides have been synthesized and their inhibitory activities against PARP-1 were evaluated. Of all the tested compounds, six compounds displayed inhibitory activities with IC50 values ranging from 0.23 to 5.78 µmol.L-1 . The binding pose of compound 5a was predicted using molecular docking to facilitate further structural modification.
7.Liver X receptors attenuate high glucose-induced apoptosis in H9C2 cells through NF-κB signaling pathway

Aim To investigate whether liver X recep- tors attenuate high glucose-induced apoptosis in H9C2 cells through inhibiting nuclear factor-NF-κB.Methods The lentiviral vector of LXRs was constructed and H9C2 cells cultured in high glucose were infected.The H9C2 cells were divided into 6 groups:control group (5.5 mmol·L -1 glucose),mannitol group(5.5 mmol ·L -1 glucose +27.5 mmol·L -1 mannitol),high glu-cose group(33 mmol·L -1 glucose),green fluorescent protein(GFP)group LXRαgroup,and LXRβgroup. The inhibition rate of H9C2 cells,the mRNA of Bax, Bcl-2,the protein content of NF-κB,Bax,Bcl-2, cleaved caspase-3,and the cell apoptosis were com-pared among these groups.Results LXRs overexpres-sion significantly attenuated high glucose-induced in-crease in Bax NF-κB,cleaved caspase-3 and cell ap-optosis(P <0.05),and increased high glucose-induced decrease in Bcl-2.Conclusion Liver X receptors at-tenuate high glucose-induced apoptosis in H9C2 cells through NF-κB signaling pathway.
8.Analysis on significance of HBV envelope large protein and its correlation with virus replication in patients with chronic hepatitis B

Objective To analyze the relationship among serum hepatitis B virus (HBV ) envelope large protein (HBV‐LP ) , HBV‐DNA and HBV marker(HBV‐M ) for investigating the clinical significance of HBV‐LP to reflect the HBV in vivo replication in the patients with HBV infection .Methods Total 540 cases of chronic HBV infection treated in the Longgang District Hospital of Traditional Chinese Medicine from April 2013 to September 2015 were selected .The real‐time fluorescence quantitative PCR meth‐od was used to detect serum HBV‐DNA ,HBV‐LP and HBV‐M were detected by the enzyme linked immunosorbent assay (ELISA) . The correlation among HBV‐LP ,HBV‐M and HBV‐DNA were analyzed .Results The positive rate of HBV‐LP in HBeAg‐positive patients was 96 .39% ,and which of HBV‐DNA was 93 .33% ,there was no statistically significant difference between them (P>0 .05);The serum HBV‐LP level was positively related with the logarithmic value of HBV‐DNA copies ;the positive rate of HBV‐LP in HBeAg‐negative patients was 63 .33% ,and which of HBV‐DNA was 51 .11% ,the difference between them was statistically significant(P<0 .05) .Conclusion HBV‐LP can effectively reflect the HBV in vivo replication in the patients with chronic hepatitis B and its sensitivity is higher than that of HBeAg ,HBV‐LP can even more reflect the HBV in vivo replication status in patients with HBeAg‐negative chronic hepatitis B .
9.Screening and identification of cardiac-specific peptides

Objective To screen cardiac-specific short-acting peptides on live myocardial slices using phage display technology,so as to improve the targeted delivery efficiency of drugs in myocardium and provide effective candidates for the targeted therapy of Duchenne muscular dystrophy (DMD) and other cardiomyopathies.Methods Myocardial tissue slices were prepared and cultured in vitro.The protein activities of the tissues were examined by immunohistochemistry.The in vitro cultured myocardial tissue slices were co-incubated with phage library (1×1012 pfu),and the phages that bound to the myocardium were recovered and amplified.The cardiac-specific targeting phages were identified by five rounds of in vitro phage biopanning.The candidate phage-related insertion sequence was sequenced,and the in vivo tissue distribution of the highly enriched phages was verified.Results A platform for in vitro culturing of live myocardial slices was established.Myocardial slices with good biological activity were obtained.After 48 hours of culturing,the normal expression and localization of Dystrophin protein were detected.Using phage library,candidate phages were screened after five rounds of phage biopanning.The results of the sequencing analyses and in vivo tissue distribution verification indicated that the selected candidate phages showed significant enrichment in myocardium and skeletal muscle,and showed low levels in liver and kidney tissues.Conclusions The candidate phages showed higher binding efficiency in both myocardium and skeletal muscle,indicating that the candidate peptides had myocardial targeting property,and that can provide a new method for myocardial targeting therapy of DMD.
