1.Urinary frequency: going beyond the tract

Obsessive-compulsive disorder (OCD) is a common anxiety disorder which can be chronic and
sustained. An OCD suferer experiences intrusive and repetitive thoughts, impulses, and behaviors,
which ultimately cause extreme discomfort. We report a case of a patient that primarily presented
with lower urinary tract symptoms who was subsequently treated with antibiotics. Nonetheless, the
symptoms persisted.
In subsequent consultations, the patient clarifed the compulsive nature of his symptoms and was
treated as a case of OCD. Terefore, it is crucial for physicians to correctly identify the nature of the
symptoms to manage the disorder properly and to avoid unnecessary consultation and treatment.
To the best of our knowledge, this is the frst report of other presentations of OCD.
2.Metabolic and Immunologic Alterations of Ginger Rhizome among Streptozotocin-Nicotinamide Induced Diabetic Rats

This study was conducted to determine immunological and metabolic
effects of different concentrations of ginger rhizome (Zingiber officinale Roscoe)
in streptozotocin (STZ)-nicotinamide (NA) induced diabetic rats. Methods: Forty-eight fasted male Sprague-Dawley rats were induced diabetes using a single intraperitoneal injection of NA(110 mg/kg b.w.) and STZ (65 mg/kg b.w, 15 min after NA). Diabetic rats orally received either different concentrations (250, 500 and 750 mg/kg body weight) of ginger rhizome suspension or glibenclamide (10 mg/kg body weight) for 6 weeks. Two control diabetic and normal groups were gavaged with only distilled water as a vehicle. Results: The results indicated that the lower concentrations of ginger modulated body weight, fasting blood glucose, level of triglyceride and tumor necrosis factor-a (TNF-a) (p<0.05). In contrast, ginger
could not enhance atherogenic indices due to a decline in the level of HDL-c. Immunological features of ginger were evident by a significant lymphocyte proliferation
in all treated groups at stimulation by 5 mg/ml PHA (p<0.001). Level of CD45ra+ (B cell marker) increased significantly in the lowest dose of ginger (p<0.05, 58%), and 250 mg/kg body weight of ginger was found to be safe for not altering the level of CD4+CD25+ marker (p>0.05). Conclusion: Ginger indicated better impact on metabolic and immunologic parameters in lower doses of supplementation
compared with high doses of treatment.

4.Imaging findings of an isolated deep cerebral venous thrombosis in the absence of superficial sinus thrombosis

Thrombosis of the deep cerebral venous system in the absence of superficial sinus thrombosis is a very
rare disease. The clinical and radiological findings can be diagnostically challenging due to the subtle
appearances on computed tomography (CT) scan. Magnetic resonance imaging (MRI) examination is
a preferred imaging modality to complement the CT findings for an accurate diagnosis of venous sinus
thrombosis. We present a case of this unusual condition which present as unilateral thalamic lesion
on CT scan and the role of contrast enhanced MRI with fast spoiled gradient echo (FSPGR) sequence
and 3D reconstruction which led to the diagnosis of thrombosis in the deep cerebral venous system.

5.Cutaneous Rosai-Dorfman disease.

Rosai-Dorfman disease (RDD) is a rare benign proliferative disorder of histiocytes in the lymph nodes with or without extranodal involvement. RDD limited to the skin without nodal involvement, known as cutaneous Rosai-Dorfman disease, is very rare. We describe a 34-year-old female with RDD of the skin over the chest. A large nodule with satellite lesions was excised for histopathological examination. Microscopically, there were many large histiocytes (Rosai-Dorfman cells) exhibiting emperipolesis, among many plasma cells, lymphocytes and neutrophils throughout the dermis and the subcutaneous tissue. The histiocytes were immunohistochemically positive for S-100 protein but negative for CD 1a. Physical examination showed no lymphadenopathy or any extra-cutaneous lesions. Serological tests indicated a past infection with Epstein-Barr virus and cytomegalovirus. The diagnosis of cutaneous RDD may be difficult in the absence of associated lymphadenopathy or any specific features of the skin lesion. Hence, not only is histopathological examination required for definitive diagnosis but a high index of suspicion by the clinicians and pathologists is essential to help diagnose this very rare disease.

6.Ptosis due to cavernous sinus syndrome as a rare presentation of advanced breast metastasis in a patient with delayed diagnosis

Complications of breast cancer metastasis may be the first manifestation of the disease. We report a rare case of left eye ptosis secondary to cavernous sinus syndrome in a 34-year-old lady due to refusal of proper medical treatment for metastatic breast cancer. The delay in seeking medical treatment was attributed to her use of Traditional Chinese Medicine (TCM).

8.An Observational Study of Blood Glucose Levels during Admission and 24 Hours Post-Operation in a Sample of Patients with Traumatic Injury in a Hospital in Kuala Lumpur

Background: Traumatic brain injury (TBI) has been associated with an acute stress response mediated by the sympathoadrenomedullary axis, which can be assessed by measuring blood glucose level.
Methods: This prospective observational study was conducted for a year in 2007 among 294 patients who had been treated for TBI in Hospital Kuala Lumpur. Patients fulfilling the set criteria were recruited into the study and data, including blood glucose level and Glasgow Outcome Score at 3-month follow-up, were collected.
Results: 294 patients were included in the study: 50 females (17.0%) and 244 males (83.0%). The majority of cases were young adult patients (mean age of 34.2 years, SD 13.0). The mean blood glucose level during admission and post-surgery were 6.26 mmol/L (SD 1.30, n = 294) and 6.66 mmol/L (SD 1.44, n = 261), respectively. Specifically, the mean admission glucose level associated with mild TBI was 5.04 mmol/L (SD 0.71); moderate TBI, 5.78 mmol/L (SD 1.02); and severe TBI, 7.04 mmol/L (SD 1.18). The mean admission glucose level associated with a poor outcome in patients with isolated TBI was 6.98 mmol/L (SD 1.21). Patients with admission glucose of 5.56 mmol/L (SD 1.21) were more likely to have a favourable outcome.
Conclusion: Mild, moderate, and severe TBI were associated with an increase in blood glucose levels during admission, and the mean increase in glucose levels is based on the severity of the isolated TBI. Surgical intervention did not cause further significant changes in blood glucose levels. Patients with isolated TBI and minimal increases in blood glucose levels were more likely to have a favourable outcome.
9.Correlation Of Safety Perceptions And Safety Behavior In University Teaching Laboratory

Chemicals usage has been reported as the cause of accidents in a laboratory. Those accidents have caused many injuries, even fatality in many sectors, one of which happened in the University Teaching Laboratory. Accidents usually happen because of unsafe behavior, while unsafe behavior is caused by bad safety perceptions. This paper investigates the correlation of perceptions and behavior in University Teaching Laboratory. The result measured by regression analysis to find out the relationship between two variables showed that there is a strong relationship between perceptions and behavior. Therefore, safe behavior can be created through creating good perception.
