1.The influence of age on propofol pharmacodynamics

Objective To study the influence of age on the pharmacodynamics of propofol including the relationship between plasma concentrations of propofol and the time of loss of consciousness or return of consciousness.Methods Forty-two ASAⅠ-Ⅱ patients were assigned to one of three age groups: group A: age ranged from 18-34 yr(n=14); group B:35-60 yr(n=15); and group C: 61-90 yr(n=13). Once spinal anesthesia was completed, propofol infusion at the rate of 0.5 mg?kg -1?min -1(group A) or 0.4 mg?kg -1?min -1(group B and C )was started until burst suppression of EEG lasting 3s was observed. Venous blood samples were taken at 1, 2, 4, 8,10, 15, 30, 45,60,90,120 and 240 min after the start of propofol infusion for the determination of plasma propofol concentrations. The influence of age on relationship between propofol plasme concentration and loss of consciousness or return of consciousness was analyzed by non-linear regression. The relationships between age and time of loss of consciousness,duration of sleep,the total dose of propofol,the plasma concentration at the time of loss of consciousness or return of consiousness were determined by linear regression.Results As compared with those in group A and B,the time of loss of consciousness and the total dose of propofol required decreased markedly in group C. The observation showed increased sensitivity to propofol in elderly patients. The EC50 values for loss of consciousness were 2.86 (at age of 25 yr),2.26 (at 50yr), and 1.78 (at 75yr) ?g?ml -1,and the EC50 values for return of consciousness of 1.76 (at 25 yr),1.45(at 50yr),1.11(at 75yr)?g?ml -1 respectively.Conclusions Elderly patients are more sensitive to propofol than younger people in terms of hypnosis and EEG effects.
2.In vitro study of bone marrow-endothelial progenitor cells

Objective To explore the expansion and differentiation of bone marrow endothelial progenitor cells in vitro. Methods Rabbit bone marrow mononuclear cells were isolated by gradient density centrifugation.Attached cells were cultured and in endothelial cell growth supplement(ECGS) from bovine pituitary.Cell multiplication was observed,and their biological characteristics were detected. Results The number of attached cells increased 5~8 fold with each expanded.Cobblestone like structure was observed when the cells lined in single layer.Expanded cells expressed von Willebrand factor(vWF) and could ingest Dil acLDL. ConclusionEndothelial progenitor cells,which exist within adult bone marrow,have the potential to be expanded and differentiated when stimulated by ECGS in vitro.
3.Determination of propofol and lidocaine in human plasma by RP-HPLC

OBJECTIVE To establish an assay for the simultaneous determination of propofol and lidocaine in human plasma using reversed-phase high performance liquid chromatography(RP-HPLC).METHODS With thymol and bupivacaine as the internal standards for propofol and lidocaine respectively,the extraction was performed with cyclohexane.The organic layer was evaporated and the residue was redissolved by mobile phase.The concentrations of propofol and lidocaine were assayed on a hypersil BDS C18 column with a mobile phase consisting of acetonitrile-methanol-water(10∶60∶30)(including 0.14% n-butyamine 0.1% acetic acid)at a flow rate of 1 mL*min-1 and detected at 220 nm during 1~7 min and at 273 nm during 7~16 min.RESULTS The linearity between concentrations and peak area ratio was obtained from 0.1 μg*mL-1 to 25.6 μg*mL-1(r=0.998,r=0.9995).The detection limits were 0.1 μg*mL-1 for propofol and 0.05 μg*mL-1 for lidocaine.The within-day and between-day variation coefficients were less than 5%(n=5).The average recoveries were 93.33%,99.4% and 90%,95.67% respectively.CONCLUSION The method was found to be rapid,accurate and precise.It is suitable for clinical pharmacokinetic and pharmacodynamic study.
4.Effect of Fas ligand gene on reendothelialization in rat carotid artery with balloon withdrawal injury

AIM: To investigate the effect of Fas ligand on reendothelialization in the rat carotid artery with balloon withdrawl injury. METHODS: Balloon withdrawal injury was used to establish the denuded carotid artery rat model in twelve SD rats and then six SD rats were treated with ?-gal virus supernatant (?-gal group), and the others were treated with FasL virus supernatant (FasL group). After fourteen days, Evans blue dying was conducted via injection through rat tail vein at 30 min before the animals were killed, then fixation perfusion in situ was performed with 10% formalin and specimens were obtained. Histologic observation and morphometric analysis were made. OBJECTIVE:To study the accuracy of infusion of Midazoloam with plasma concentration as target. METHODS:The parameters of Midazoloam obtained from our researches were inputted into target-controlled infusion(TCI) system with C language. The clinical anesthesia of 12 patients undergoing selective operations was completed with plasma concentration as target-controlled infusion. Predicted value of plasma concentration of Midazoloam was compared with measured value. Parameters of Midazoloam sample were calculated such as performance error(PE),absolute performance error(absPE),median performance error(MDPE),median absolute performance error(MDAPE),constancy error(CE),absolute constancy error(absCE),median constancy error(MDCE) and median absolute constancy error(MDACE). RESULTS:PE,absPE,MDPE and MDAPE of plasma concentration were -2.57%,14.16%,-3.28% and 15.34%,respectively. CE,absCE,MDCE and MDACE were 0.06%,1.42%,0.03% and 1.21%,respectively. 6.Prediction of nuclear grade of renal clear cell carcinoma based on MRI texture analysis in combination with imaging features

Objective:To explore the application value of MRI texture analysis in combination with imaging features to predict the WHO/International Society of Urological Pathology (ISUP) nuclear grading in pre-operative patients with clear cell renal carcinoma (ccRCC).Methods:MRI images of 78 patients diagnosed as ccRCC by surgical pathology from July 2016 to July 2020 in First Affiliated Hospital of Fujian Medical University were retrospectively analyzed. According to the WHO/ISUP grading system, the patients were divided into low grade group (49 cases, grade Ⅰ in 2 cases and grade Ⅱ in 47 cases) and high grade group (29 cases, grade Ⅲ in 25 cases and grade Ⅳ in 4 cases), and then were assigned to training set ( n= 63) and validation set ( n=15) in a ratio of 7∶3 using random indicator method. MRI radiological features were evaluated and MRI imaging texture features were extracted. The largest-diameter slice of lesion on cross-sectional images was selected and ROIs were drawn on T 2WI and corticomedullary phase (CMP) images, respectively. Quantitative texture analysis software MaZda was used to extract texture features, including gray-scale histogram, co-occurrence matrix, run-length matrix, gradient, autoregressive model and wavelet transform. The extracted texture features were preliminarily selected by the combination of Fisher, probability of classification errorand average correlation coefficient, and interaction information, and then the reduced texture parameters or imaging features were tested by the independent sample t test, Mann-Whitney U test or χ 2 test. Parameters with statistically significant differences were used to construct a multi-factors binary logistic regression model and the ROC curve was used to analyze its effectiveness in predicting high grade ccRCC. Results:In training set, there were significant differences intumor length, shape and margin, enhancement degree of CMP, vein thrombosis and 47 texture features between the low and high grade ccRCC groups. In the training set, 7 multi-factors binary logistic regression model were constructed, including radiological features model (M1), T 2WI texture features model (M2), CMP image texture features model (M3) and combination radiological features of T 2WI texture features model (M4), combination radiological features of CMP images texture features model (M5), combination T 2WI texture features of CMP images texture features model (M6) and combination of all features model (M7). The area under ROC curve of M7 in predicting nuclear grading of ccRCC was the largest, which were 0.901 (95% CI 0.828-0.974) and 0.820 (95% CI 0.564-0.974) in the training set and validation set, respectively. Conclusion:MRI texture analysis combined with imaging features is hopeful to be an effective preoperative noninvasive method in predicting WHO/ISUP grading of ccRCC.
7.Sedative and hypnotic interaction between propofol and remifentanil by target-controlled infusion during induction of anesthesia

Objective To investigate the sedative and hypnotic interaction between remifentanil and propofol by target-controlled infusion (TCI) during induction of anesthesia.Methods Third-two ASA Ⅰ or Ⅱpatients,aged 22-63 yr,body mass index 18-25 kg/m2,scheduled for elective surgery under general anesthesia,were randomly divided into 4 groups(n=8 each).Group Ⅰ only received TCI pmpofol.GroupⅡ,Ⅲ,and Ⅳreceived a target concentration of 2,4 or 6 ng/ml remifentanil respectively.While the blood-effect site concentrations of remifentanil were equilibrated,patients received TCI of propefol,with an initial target concentration of 0.5μg/ml.After the blood-effect site concentrations of propofol were equilibrated then with 0.5μg/ml increments until the loss consciousness was achieved.The eyelash reflex and state of consciousness were assessed and radial arterial blood sample 6 ml was taken every 3 min to determine the remifentanil and propofol concentrations in blood.Propofol and remifentanil concentrations in blood were measured by reversed-phase high-performance liquid chromatography and high-performance liquid chromatography with ultraviolet detection respectively.The sedative and hypnotic interaction between propofol and remifentanil was determined with a pharmacodynamie interaction model by regression analysis and determined using the isobolographic method.Results Propofol concentrations in blood were lower in group Ⅱ,Ⅲ and Ⅳ than group Ⅰ(P<0.05).The propofol concentratopms in blood were significantly decreased in trun with the increase in the remifentanil concentrations in blood in group Ⅱ-Ⅳ(P<0.05).At loss of eyelash reflex and loss of consciousness of patients,the pharmacodynamic interaction model by curve fitting was superior to linear regression (P<0.05).At loss of eyelash reflex of patients,the curve fitting result showed EC50,prop=2.77μg/ml and EC50,rem=26.67 ng/ml,and the isobolographic method equation is ECprop/2.77+ECrem/26.67=0.69.At loss of consciousness of patients,the curve fitting result showed EC50,prop==3.76μg/ml and EC50,rem=31.56ng/ml,and the isobolographic method equation is Ecprop/3.76+Ecrem/31.56=0.65.Conclusion Remifentanil (Cp 2-6 ng/ml) and propofol by TCI shows a synergistic type of pharmacodynamic interaction on the sedative and hypnotic during induction of anesthesia.
8.Effect of autologous bone marrow mononuclear cell local transplantation on peripheral neovascularization

AIM: To investigate the effect of autologous bone marrow mononuclear cells (BM-MNCs) local transplantation (BM-LT) on a rabbit model of hindlimb ischemia. METHODS: After unilateral femoral artery and its branches were surgically ligated and excised in rabbits (n=14) 1 hour or so, autologous BM-MNCs were injected into the ischemic skeletal muscles in 7 rabbits, and phosphate buffered saline for another 7 ones as control. Hemodynamics parameters were measured at day 0, day 7 and day 28, respectively. On day 28, the animals were sacrificed in groups for pathological analysis. 9.Simultaneous determination of six flavonoids in Apocynum venetum by HPLC.

OBJECTIVETo develop a high-performance liquid chromatography (HPLC) method for the simultaneous determination of rutin, hyperoside, isoquercetin, astragulin, quercetin, and kaempferol in Apocynum venetum and its extracts.
METHODThe separation was carried out on a Shim pack ODS (4.6 mm x 250 mm, 5 microm) colum eluted with in mobile phases of water containing 0.2% phosphoric acid and acetonitrile containing 0.2% phosphoric acid in acetonitrile gradient mode. The column temperature was 40 degrees C, and the flow rate was 1.0 mL x min(-1). The detection wavelength was set at 360 nm.
RESULTThe good seperation of six flavonoids was achieved within 40 min, with the relative standard deviations (RSD) of intra- and inter-day precision < or = 2.0%. Calibration curves of rutin, hyperoside, isoquercetin, astragulin, quercetin, and kaempferol showed good linear relationship (R2 > 0.999 7, n = 6). The average recoveries of the six flavonoids were within 97.30% - 105.8% (RSD 2.6%). Three batches of A. venetum and 2 batches of its extracts were determined.
CONCLUSIONThe developed method is simple, accurate, and repeatable, and can be readily used as a powerful tool for the quality control of A. venetum and its extracts.

10.Short-term MRI texture features of medial femoral cartilage after anterior cruciate ligament reconstruction

Objective:To investigate MRI texture features reflecting short-term medial femoral cartilage variations after anterior cruciate ligament (ACL) reconstruction.Methods:A retrospective case series analysis was conducted referring to the data of 20 patients who received ACL reconstruction at First Affiliated Hospital of Fujian Medical University from January 2017 to January 2020 including 17 males and 3 females, aged 23-43 years [(32.1±5.4)years]. The patients were evaluated before operation and at 1, 4, 12, 24, 48 weeks after operation: (1) the region of interest (ROI) in medial femoral cartilage was divided into ROI1 (meniscus anterior horn cartilage), ROI2 (meniscus body cartilage) and ROI3 (meniscus posterior horn cartilage) according to MRI image acquisition of the affected knee. The texture feature parameters were extracted for screening and analyzing to obtain the parameters reflecting sensitivity to cartilage variations; (2) the level of urinary C-telopeptide of type II collagen (uCTX-II) was measured by ELISA. At different time points, differences in texture feature parameters were compared to screen the parameters most sensitive to cartilage variations, and uCTX-II level were compared as well. Then, Pearson correlation analysis was used to determine the correlation between texture feature parameters most sensitive to cartilage variations and uCTX-II level.Results:After screening and analyzing, the horizontal direction of gray level non-uniformity (Horzl_GlevNonU) and Skewness of histogram (Skewness) of ROI3 were selected as sensitive texture feature parameters reflecting sensitivity to cartilage variations. There were significant difference in the Horzl_GlevNonU and Skewness of ROI3 before operation and at 1, 4, 12, 24, 48 weeks after operation (all P<0.05). Meanwhile, the Horzl_GlevNonU and Skewness of ROI3 were gradually increased over time, with a more significant tendency in the Horzl_GlevNonU. The Horzl_GlevNonU of ROI3 was similar at 4 weeks and 12 weeks after operation ( P>0.05), with significant differences at other time points (all P<0.05). The Skewness of ROI3 was significantly different at 1 week and 48 weeks after operation ( P<0.05), with insignificant differences at other time points (all P>0.05). The Horzl_GlevNonU of ROI3 was selected as the most sensitive texture feature parameter reflecting cartilage variations. Significant difference was reported in uCTX-II level before operation and at 1, 4, 12, 24, 48 weeks after operation (all P<0.05). After Pearson correlation analysis, the Horzl_GlevNonU of ROI3 was positively correlated with uCTX-II level before operation and at 1, 4, 12, 24, 48 weeks after operation ( r=0.554, 0.596, 0.550, 0.632, 0.756, 0.514, P<0.05 or 0.01). Conclusion:The Horzl_GlevNonU of ROI3 is the most sensitive texture feature parameter that reflects the early variations of medial femoral cartilage after ACL reconstruction and is positively correlated with uCTX-II level.
