1.The Analysis on the Reliability of MRI Malignant Signs of Soft Tissue Masses in Extremities

Objective To analyze the reliability of MRI malignant signs of soft tissue masses in extremities.Methods MRI features of soft tissue masses were analyzed retrospectively with emphasis on 30 cases with malignant MRI signs based on pathologic results,and these 30 cases were divided into two groups(benign lesions and malignant tumor group).The reliability of malignant signs for malignant tumor was evaluated.Results(1)The qualitative diagnostic accurate rate for benign and malignant masses was 89.7% and the histopathologic qualitative diagnostic accurate rate was 66.2% by MRI;(2)The findings of invasion and edema around the masses in diagnosis of malignant tumors had a remarkable statistical significance among five malignant signs(P0.05);(3)The sign of indefinite sturcture of muscular fibrosis had statistical significance between benign and malignant tumor diagnosis(P
2.The laboratory research of dual-source and dual-energy CT in evaluating the effect of rabbit VX2 tumor interventional therapy

Objective To explore the diagnostic value of the dual-energy technique with dual-source in the evaluation of therapeutic effect of interventional therapy for hepatocarcinoma.Methods 8 rabbits with tumor growing well were treated with intervention operation,paralleled with double energy CT scan and focal area image,CT and color order change were measured by dual energy liver OVERLAY workstation.Meanwhile,The necrosis and survival of tumor were evaluated by dual energy CT after hepatocarcinoma intervention therapy.Results Taking pathologic diagnosis as gold standard,compared with imaging and pathological results in the order of black,red,orange and red,accuracies were 87.2%,72.3%,71.5% and 83.6% respectively.Sensitivities were 95.1%,37.4%,54.7% and 63.8% respectively.Specificities were 85.4%,86.3%,81.5% and 85.8% respectively.Positive predictives value were 61.4%,54.2%,62.4% and 55.6% respectively.Negative predictive values were 96.3%,76.2%,79.7% and 91.3 % respectively.According to the Spearman relevance analysis,there was strong positive correlation between survival of tumor cells and image color scale (r =12.35,P < 0.05).Conclusion The necrosis and survival of tumor cells in rabbit VX2 liver tumor model can be preliminarily reflected by dual source CT dual energy enhancement scanning.The results provide good reference to the application of dual-source and dual-energy in clinical diagnosis and treatment.
3.Diagnostic value of spectral CT imaging on complications after breast augmentations with siliconeim plants

Objective To evaluate the diagnostic value of the spectral CT imaging on the complications after breast augmentations with silicone implants.Methods A lot of 22 cases with breast implants were scanned by the Gemstone spectral imaging (GSI) CT.The original data were loaded in processed workstation, analyzed by GSI analysis software, and then stored the series we needed.The features of the complications after breast augmentations with silicone implants were reviewed retrospectively on the spectral CT images.Results A lot of 44 breast implants scanned in our study and there were 23 breast implants with complications, including 7 breast implants with capsular rupture (6 breast implants with endometrial rupture detected by spectral CT, including 4 breast implants with endometrial rupture, 3 breast implants with outer membrane rupture and leakage), 13 breast implants with capsular contracture (classification according to Baker's classification, 6 in grade Ⅰand 7 in grade Ⅱ), 8 breast implants were moved and 2 breasts had foci of calcification.Conclusions The spectral CT imaging play important role in detecting the complications after breast augmentations with silicone implants by GSI analysis software.
4.Efficacy observation of acupuncture bloodletting and penicillin on treatment of children acute tonsillitis.

OBJECTIVETo observe differences of therapeutic effects among acupuncture bloodletting, penicillin and acupuncture bloodletting combined with penicillin for children acute tonsillitis and providea better treatment method in cli nic.
METHODSSeventy-five mild cases were selected into section of mild symptoms while seventy-five severe cases were selected into section of severe symptoms. Cases in the two sections then were divided into, an acupuncture bloodletting group, a penicillin group and a comprehensive group by random digital table method separately, 25 cases in each one. Qu-chi (LI 11), Hegu (LI 4), Dazhui (GV 14), Shaoshang (LU 11) and Erjian (EX 11) were selected in the acupuncture bloodletting group, intravenous injection of penicillin sodium was applied in the penicillin group and acupuncture bloodletting combined with penicillin was applied in the comprehensive group. Efficacy assessment was conducted after 3 days in the section of mild symptoms and after 5 days in the section of severe symptoms.
RESULTSFor the section of mild symptoms, the total effective rate was 96.0% (24/25) in the comprehensive group and 92.0% (23/25) in the acupuncture bloodletting group, which were both superior to 68. 0% (17/25) in the penicillin group (P<0.05), but no statistical significance was seen between the comprehensive group and acupuncture bloodletting group (P>0.05). For the section of severe symptoms, the total effective rate was 96.0% (24/25) in the comprehensive group, which was obviously superior to 60.0% (15/25) in the acupuncture bloodletting group (P<0.01) and 68.0% (17/25) in the penicillin group (P<0. 05), and no statistical significance was seen between the acupuncture bloodletting group and penicillin group (P>0.05).
CONCLUSIONThe efficacy of acupuncture bloodletting combined with penicillin is little different from that of acupuncture bloodletting for treatment of children acute tonsillitis with mild accompanied symptoms, which were both superior to intravenous injection of penicillin sodium. For severe accompanied symptoms, the efficacy of acupuncture bloodletting combined with penicillin is obviously superior to acupuncture bloodletting and penicillin.

5.Comparative analysis of paravertebral lymphatic trunk by MR lymphangiography between intradermal and subcutaneous injection of Gadodiamide in rabbits

Objective To investigate intradermal injection and subcutaneous injection of Gadodiamide MR lymphangiography (MRL) in evaluation on the central conducting lymphatics in rabbits.Methods T1 weighted three-dimendional fast low angle shot (fl3d) sequence was used before and after administration of Gadodiamide to undertake MRL.Gadodiamide was administed intrademally in the left and right footpad in 8 rabbits,and then undertake MRL in 1.5T MRI system (intradermal injection group).Three days later,Gadodiamide was administered subcutaneously in the left and right footpad in 8 rabbits,and MRL were underdone with the same sequence (subcutaneous injection group).The degree of contrast enhancement within the lumbar lymphatic trunk and thoracic duct were evaluated using a 3-point scoring system.Results Intradermal injection group showed the popliteal lymph nodes,sacral lymph node and inferior aortic lymph node enhanced obviously in all 8 rabbits,and the enhancement duration time was about 30 90 mini the lumbar lymphatic trunk and cisterna chyli were detected in 6 rabbits,and part of the thoracic duct were detected in 5 rabbits.Subcutaneous injection group showed the popliteal lymph nodes enhanced obviously in all 8 rabbits,but the lumbar lymphatic trunk and the thoracic duct were not detected.The score of contrast enhancement within the lymphatic system of intradermal injection group and subcutaneous injection group had statistical difference (t=100.00,P=0.0002).Conclusion MRL with intradermal injection Gadodiamide are better than subcutaneous injection in evaluation of the lumbar lymphatic trunk.
6.Ultrastructure Characteristics of Different Chinese Medicine Syndromes of Helicobacter pylori-Correlated Gastric Diseases.

OBJECTIVE:
To explore the ultrastructure characteristics of patients with dampness-heat of Pi (Spleen)-Wei (Stomach) syndrome (DHPW) and Pi-qi deficiency syndrome (PQD), both of which are Helicobacter pylori (Hp)-correlated gastric diseases (HPCG), and implicate a helpful hint for the clinical microcosmic syndrome differentiation.
METHODS:
Fourteen gastric mucosa samples from 6 chronic gastritis (CG) and 6 active peptic ulcer (including 8 DHPW, 4 PQD) as well as 2 healthy volunteers were collected and tested for Hp infection. The ultrastructure of gastric mucosa was observed under the transmission electron microscope (TEM).
RESULTS:
Among 14 gastric mucosa samples, 8 of them were Hp positive (6 DHPW and 2 PQD), which were all accordance with the results screened by supermicro-pathological method. Under TEM, the normal gastric mucosa, with tidy microvilli and abundant in mucus granules, mitochondria and rough endoplasmic reticulum distributed evenly, and with smooth nucleus membrane. But in those specimens of DHPW with Hp infection, microvilli were presented with burr shape. Especially, those samples from dampness-heat syndrome with predominant heat type (DHSH) patients were more obvious, with microvilli damaged, mitochondria concentrated and distributed in disorder, secretory tubule extended. In dampness-heat syndrome with predominant dampness type (DHSD) patients, mucus granules aggregated obviously, mitochondria swelled and blurred, and rough endoplasmic reticulum crowded. For 2 samples of DHPW without Hp infection, their microvilli were intact, with mitochondria increased and gathered but well-distributed, and secretory tubule extended mildly. In 2 PQD patients with Hp positive, the specimens of microvilli were sparse, and their mucus granules and mitochondria were decreased, with fractured crests and vacuole, secretory tubules extension to nucleus membrane, and rough endoplasmic reticulum extension in a pool-like way, and nucleus condensed. The 2 samples from PQD patients without Hp infection were characterized with intact microvilli, decreased mitochondria, fractured crest and extended rough endoplasmic reticulum in a pool-like way.
CONCLUSION
It's obviously different in ultrastructure of DHPW and PQD patients under TEM, which may give a helpful hint for the microcosmic syndrome differentiation of HPCG.
7.MR Lymphangiography for Focal Disruption of the Thoracic Duct in Chylothorax of an Infant: a Case Report and Literature Review.

Chylothorax is a rare cause of pleural effusion in children, and it is usually difficult to identify the location of chyle leakage due to the small size of the thoracic duct in children. Herein we report an infant case with chylothorax whose leakage of the thoracic duct was successfully located by magnetic resonance lymphangiography (MRL) using pre-contrast MR cholangiopancreatography (MRCP) and gadodiamide-enhanced spectral presaturation inversion recovery (SPIR) T-weighted imaging, which demonstrate the imaging method is easy and effective for detecting the focal disruption of the thoracic duct in children with chylothorax and younger than 8 months old.
