1.Laparoscopic abdominal trauma and the diagnosis and treatment of gastrointestinal tract injury in application

Objective To explore of laparoscopic abdominal trauma used in the diagnosis and treatment of gastrointestinal tract injury.Methods 180 cases of abdominal trauma and gastrointestinal injury were randomly divided into three groups,A group of 60 cases was given conventional nonoperative treatment,B group of 90 cases was given laparoscopic exploration to surgical treatment,C group of 30 cases was open to probe the surgical treatment.The clinical effect of three groups was observed.Results After treatment,A group of recovered 32 cases,improvement in 16 cases,invalid in 12 cases,the total effective rate 80.0%,length of hospital stay (8 ±4)d,B group healed 58 cases,30 cases improved,invalid in 2 cases,the total effective rate 97.8%,length of hospital stay (8 ± 3) d,C group of 19 cases were healed,better 10 cases,1 case ineffective,the total effective rate 96.7%,length of hospital stay (15 ± 5) d,A,B and C group total effective rate differences were statistically significant (x2 =13.45,4.50,all P <0.05).C group hospitalization time was longer than that in A,B group (t =5.24,3.34,all P < 0.05).Conclusion Laparoscopic exploration surgery for abdominal trauma and gastrointestinal injury has better diagnosis and treatment effect,which can reduce the time in hospital to improve the prognosis of patients.
2.Geriatric respiratory medicine: a field needs be furthered strengthened.

Respiratory diseases are common in the elderly and often the main causes of death among this population. In addition, it is expected that chronic obstructive pulmonary disease, lower respiratory tract infections, lung cancer, and pulmonary tuberculosis will be listed in the top ten diseases in 2020. Therefore, screening, diagnosis and management of respiratory diseases should be strengthened among the elderly; meanwhile, studies on geriatric respiratory medicine should be further enhanced.

3.Ultrasonographic assessment of fetal cardiovascular function in selective intrauterine growth restriction

Objective To evaluate the fetal cardiovascular function in selective intrauterine growth restriction (sIUGR)in monochorionic diamniotic(MCDA)twins by using 2D and color Doppler ultrasonography. Methods Twenty - four sIUGR pregnancies and 20 normal MCDA twins pregnancies were enrolled at the First Affiliated Hospital of Sun Yat - Sen University from December 2013 to September 2014,and cardiac structure of fetus and Doppler patterns of the umbilical artery(UA),umbilical vein(UV),pulsatility index of ductus venosus(DV - PI),atrioventricular valve, semilunar valve,tricuspid annulus systolic displacement(TAPSE),fractional shortening of left ventricle short axis(FS) and myocardial performance index(Tei index)of both ventricles were assessed. Results Cardiothoracic ratio,heart rate,DV - PI,FS,TAPSE,Tei index of the left ventricle(0. 34 ± 0. 03,0. 34 ± 0. 04),Tei index of the right ventricle (0. 35 ± 0. 03,0. 35 ± 0. 03)were not significantly different compared with those of the normal MCDA twins(all P ﹥0. 05). Cardiothoracic ratio,ventricular wall thickness,isovolumetric relaxation time( IRT),isovolumetric contraction time(ICT),TAPSE and tricuspid regurgitation(TR)in the larger twins of sIUGR were significantly bigger than those in the smaller twins of sIUGR(all P ﹤ 0. 05),Tei index of both ventricles in the larger twin of sIUGR was significantly higher than that in the smaller twins of sIUGR(Tei index of left ventricle:0. 43 ± 0. 06 vs 0. 35 ± 0. 04,Tei index of right ventricle:0. 43 ± 0. 06 vs 0. 34 ± 0. 04,P ﹤ 0. 05),the peak E in early diastolic / the peak A in atrial contraction (E/ A),the percentage of inflow during of cardiac cycle length(inflow/ CCL)in larger twin of sIUGR were significantly lower than those in smaller twin of sIUGR(all P ﹤ 0. 05). Conclusions There was no difference of cardiac function in normal MCDA twins,at the beginning of the diagnosis of sIUGR,cardiac dysfunction were found in larger twin,follow -up examination are helpful to clinical decision.
4.THE ANALYSIS OF FATTY ACIDS IN SEED OIL OF MICROULA SIKKIMENSIS

Microula Sikkimensis is a kind of plant only in China. Fatty acid composition of its seed oil was analyzed with a polar PEG-20M fused-silica capillary column GC combined with MS after the seed oil was saponified and methylated. 13 kinds of fatty acid methyl ester (FAMEs) were identified. The major constituents were found to be oleic acid 18.12%, linoleic acid 35.11%, ?-linolenic acid 8.33%, ?-linolenic acid 13.57% and stearidonic acid 1.21%.
5.Selective pregnancy reduction in second trimester

Objective To investigate selective pregnancy reduction in second-trimester and its notices. Methods Selective pregnancy reduction in 19 cases of multiple pregnancy during 12~25 gestational age was performed,including 6 cases of twins,1 cases of triplets,1 case of quadruplet and 1 case of sextuplet. Ultrasonographically guided transabdominal intracardiac injection of high concentrate potassium chloride (KCl) solution was used. Results Performances of selective feticide in all twins were successful,except one monochorionic twins. In 11 cases of triplets,one case was aborted,one case was monozygotic triplet. Selective feticide was performed successfully in 7 of the rest 9 cases and the quadruplet Two cases were aborted after the procedure. The sextuplet was aborted after feticide. Conclusion Multifetal pregnancy reduction can be successfully performed in the second trimester without severe maternal complications. But abortion might occur afterwards. It is possible to reduce the abortion rate if monozygotic twin or triplet can be excluded before feticide,choosing the gestational sac which near the fundus,reducing the amniotic fluid volume of the target fetus or postponing feticide.
6.Study on Percolation Extract Process of Quyou Tincture

Objective To optimize the extract technology of Quyou Tincture with orthogonal design.Methods With the determination of peoniflorin and paeonol and the extraction rates as indexes,the extract conditions of Quyou Tincture was optimized by orthogonal design.Results The optimal preparation process of alcohol was as follows:adding 6 times alcohol(75%),immerse 24 hours and percolate with 1 mL/min.Conclusion The optimum extracting condition was simple,with a high extraction rate and low cost.
7.Effects of vitamin E on secretion of inflammatory mediators of human monocytes by advanced glycation end products

Objective To study the effects of vitamin E(VitE) on TNF-?,IL-1? and nitric oxide(NO) secretion from human monocytes stimulated by advanced glycation end products.Methods Human peripheral blood monocytes(PBMCs) from 20 hemodialysis(HD) patients and 15 normal volunteers were isolated.These PBMCs were cultured in vitro with advanced glycation end products(AGE) and vitamin E.All of these PBMCs were divided into control group,AGE-inducing group,different doses VitE group and different culture VitE group.The levels of TNF-? and IL-1? in supernatants were detected by ELISA.The levels of NO were detected by biochemical method.Results The basal secretion of TNF-?,IL-1? in monocytes from HD patients was more than those from normal volunteers.AGE induced significantly more secretion of TNF-? and IL-1?(P
8.Diagnosis, management and pregnancy outcomes of twin-twin transfusion syndrome

Objective To investigate the diagnosis, treatment and pregnancy outcomes of twin-twin transfusion syndrome(TTTS). Methods Eighteen cases of TTTS(TTTS group) were studied retrospectively from Jan 1991 to Oct 2005 in our hospital, and 620 twin pregnancies unaffected by TTTS (control group) were compared. Results (1)The overall incidence of TTTS was 2.8% in all twin pregnancies, and 8.1% in monochorionic twin pregnancies. (2) Fourteen cases of TTTS were staged, and 10 were terminated. Seven cases opted to be managed (1 case at stage Ⅱ, 5 at stage Ⅲ and 1 at stage Ⅳ). Transabdominal amnioreduction was performed in 3 cases, 2 of them progressed to stage Ⅴ and was terminated. 9.New simple method to detect uronic acids in polysaccharides

Object To develop a simple and reliable method to detect the presence of uronic acids in polysaccharides Methods Uronic acid containing polysaccharides and standard glucons were subjected to acid hydrosis for 2 h and 4 h respectively. Samples of the products, before and after hydrolysis were analyzed by TLC using standard uronic acid as the control and aminobenzen alizaric acid as the chromogenic agent, to compare the sensitivities in the detection of uronic acid used in the present conventional analysis. Results The fluorescent spots used as criterion in traditional TLC method were not shown by uronic acids, but by CF 3COOCH 3 and other by products in reaction 0 5 ?g of uronic acid could be detected with our method, without false positive reaction Conclusion The traditional TLC method seemed to lack theorectical background and may mislead structural study The present method in more reliable to detect uronic acids in polysaccharides by analyzing the complete hydrolytic products of polysaccharides using TLC with standard uronic acids as control and aminobenzene alizaric acid as chromogenic agent
10.Epidemiology and Analysis of Sensitivity for Antibiotics of Bacteremia in Intensive Care Unit

OBJECTIVE To study the epidemiology and the sensitivity to antibiotics of bacteremia in intensive care unit,and provide the reference for the clinical antimicrobial agents usage. METHODS 465 cases of bacteremia from Jan 2005 to Dec 2007 in the First Affiliated Hospital,Medical College of Zhejiang University were retrospectively analyzed. RESULTS Totally 1465 strains of pathogens were isolated in three years,and the positive rates were 22.6%.From them 155 strains were fungi(10.6%),611 strains were Gram-positive bacteria(57.9%) and 392 strains were Gram-negative bacteria(37.1%).G + strains mainly included coagulase-negative Staphylococcus(CNS,43.0%) and Staphylococcus aureus(2.1%);the sensitive antibiotics were vancomycin and teicoplanin.G-strains mainly included Acinetobacter baumannii(8.6%);Stenotrophomonas maltophilia(6.6%);Pseudomonas(5.1%) and Klebsiella pneumoniae(5.3%),the sensitive antibiotics were cefoperazone/sulbactam. CONCLUSIONS The positive rates of blood culture are higher in intensive care unit than in others.We should pay more attention to CNS for its high infection rate.The sensitive antibiotics of G + are vancomycin and teicoplanin.The sensitive antibiotics of G-are cefoperazone/sulbactum.Antimicrobial therapy should be conducted according to susceptibility test.The candidemia caused by non-Candida albicans(NCA) is increaseing in recent 3 year.
