Objective To evaluate the efficacy of bispectral index (BIS) in guiding sevoflurane anesthesia in children. Methods Forty-eight ASA Ⅰ or Ⅱ pediatric patients aged 1-12 yr undergoing elective urological surgery were randomized into 2 groups ( n = 24 each) : group Ⅰ control and group Ⅱ BITS. Each group was further divided into 3 subgroups according to the age (n = 8 each) : subgroup A ( 1 yr≤age < 3 yr) ,subgroup B (3 yr≤ age<6 yr ) and subgroup C (6 yr≤age ≤ 12 yr). The patients were premedicated with IM atropine 0.015-0.02 mg/kg. Anesthesia was induced with 5% sevoflurane and 60% N2O in O2. Tracheal intubation was facilitated with vecuronium 0.1 mg/kg. Anesthesia was maintained with inhalation of sevoflurane and,50% N2O in O2 and intermittent Ⅳ bolnses of vecuronium and fentanyl. In BIS group, BIS was maintained at 40-60 during operation and at 60-75 during the 15 min before the end of surgery. BIS was monitored and recorded but not available to the anesthesiologist in control group and the depth of anesthesia was maintained based on hemodynamic changes and clinical signs. MAP, HR, end-tidal sevoflurane concentration, BIS, emergence time, extubation time and PACU discharge time were recorded. The amount of sevoflurane consumed was calculated. Results There was no significant difference in the demographic data between the cotresponding age groups of BIS and control group. The BIS were maintained at 40-60 in control group. The BIS was significantly higher in BIS group than in control group except the subgroup A. The end-tidal sevoflurane concentration was significantly lower, and the emergence time, extubation time and PACU discharge time were significantly shorter in BIS group than in control group. There was no significant difference in MAP and HR between the 2 groups. Conclusion BIS monitoring can reduce sevoflurane consumption and allow faster emergence from sevoflurane anesthesia in children over 1 yr of age.

Objective To study the clinicopathologic classification,and methods of individualized treatment of Budd-Chiari syndrome (B-CS).Methods Analysed the clinical data of 42 cases of B-CS in our hospital from March 2006 to August 2010.Results The 42 patients were divided into three types,including 5 subtypes,which of them underwent operation or the interventional therapy,All kinds of bypass,a total of 20 cases.After operation,1 case appeared hepatic encephalopathy and die from it,the others all recovered well.Radical lesion resection and thrombosis was taken out in 7 cases; a balloon catheter in 5 cases and postoperative recovered well.The spleen-lung fixation in 6 cases were cured after operation.The three cavity two capsule tube was used oppression hemostasis for 4 patients with emergency bleeding,three cases was cured of joint spleen cut + cutoff,1 case with conservative treatment is invalid,died.The 41 cases received follow-up,during the follow up period of 3 months to 3 years,2 patients had recurrence (4.9％),and the other patients recovered satisfactorily.Conclutions According to different the clinicopathologic classification of B-CS,Taking individualized treatment may further improve the clinical curative effect.

Objective To construct and expr es s a recombinant plasmid of nonstructural protein NS3-NS4 of hepatitis C virus ( HCV), and to identify the antigenicity of the expressed protein. Methods A gene region encompassing the nonstructural protei n NS3-NS4 of HCV was amplified by polymerase chain reaction (PCR) from the pUC1 9/HCV template. The recombinant expression plasmid containing the pBV220/NS3-NS 4 sequence was constructed, and the nonfused NS3-NS4 recombinant protein was ex pressed in E.coli DH5? efficiently. The recombinant protein was det ected by SDS-PAGE and ELISA. Results We successfully constructed and expressed the recom binant plasmid in prokaryote. Its antigenicity was detected with 50 standard ser a. Compared with the second-generation diagnostic Kit, the total detection rate was 96%. Conclusion The whole NS3-NS4 protein, a region of dominant immunogenicity, should be the effective component of the HCV diagnostic Kit and provide the clue for developing HCV DNA vaccine.

Objective To construct the expression box of fusion gene NT4-p53(N15)-Ant.Methods The gene of p53(N15)-Ant was obtained by PCR of two primers templating with each other and T-vector cloning method.The positive clone was identified and analyzed by the restriction enzymes and sequencing respectively.After digested with restriction enzyme,the interest gene of p53(N15)-Ant was subcloned into the plasmid pBV220/NT4.Results The gene of p53(N15)-Ant was confirmed by the digestion of restriction enzyme and sequencing.The recombinant plasmid pBV220/NT4p53(N15)Ant was identified by the digestion of restriction enzyme and agarose gel electrophoresis and the results conformed theoretical values.Conclusion The plasmid pBV220 containing the expression box of NT4-p53(N15)-Ant was successfully constructed by molecular cloning and recombination techniques in vitro,which will guide further study on gene therapy of cancer.

A sensitive method based on high-performance liquid chromatography-tandem mass spectrometry (LC-MS/MS) has been developed for the simultaneous determination of folic acid (FA) and its active metabolite, 5-methyltetrahydrofolic acid (5-M-THF), in human plasma. The analytes were extracted from plasma with methanol solution containing 10 mg/mL of 2-mercaptoethanol and 0.025% (v/v) ammonium hydroxide. FA and 5-M-THF were more stable after the addition of 2-mercaptoethanol and ammonium hydroxide in the sample preparation procedures of this study than they were in the previously published methods. Chromatographic separation was performed on a Hedera ODS-2 column using a gradient elution system of acetonitrile and 1 mM ammonium acetate buffer solution containing 0.6% formic acid as mobile phase. LC-MS/MS was carried out with an ESI ion-source and operated in the multiple reaction monitoring (MRM) mode. The assay was linear over the concentration ranges of 0.249-19.9 ng/mL for FA, and 5.05-50.5 ng/mL for 5-M-THF. The developed LC-MS/MS method offers increased sensitivity for quantification of FA and 5-M-THF in human plasma and was applicable to a pharmacokinetic study of FA and 5-M-THF.

Objective:To discuss treatment of complete response cases after neoadjuvant radiotherapy in rectal cancer. Methods:This retrospective study analyzed clinical data of 84 rectal cancer cases with pre-operative neoadjuvant chemoradiotherapy in our hospital from January 2010 to Augnst 2014. Results:After neoadjuvant chemoradiotherapy, 33 patients presented clinically complete response at a rate of 39.3%. After post-operative pathologic examination, among clinically complete response cases, six cases exhibited patho-logically complete responses at a rate of 18.2%. No recurrence or disease progression occurred within 12-36 months of post-operative follow up. Conclusion:Neoadjuvant chemoradiotherapy can significantly lower tumor stage and promote clinically complete remission of some patients. However, for clinically complete remission cases, further radical surgery should be provided.
