Diabetic peripheral neuropathy is a common complication of diabetes mellitus, can occur in some patients with painful diabetic peripheral neuropathy. The occurrence of PDPN is related to many factors, such as long term severe hyperglycemia and metabolic disorders, deficiency of neurotrophic factors, disorder of microcirculation, increase of oxygen free radicals in oxidative stress and disorder of immune func-tion.Mitogen activated protein kinase (MAPK)is a kind of serine /tyrosine protein kinase ,which can widely expressed in cell, and can be involved in the pathophysiological process of stress, inflammation, cell cycle and apoptosis through increased nuclear transcription factors.P38MAPK signaling pathway is an im-portant branch of the MAPK,this overview focuses on the research progress of p38MAPK signaling pathway in the pathogenesis of PDPN.

BACKGROUND: Metabolic disorder of blood lipid can be observed in the non-diabetic first-degree relatives.OBJECTIVE: To explore the relationship between the level of blood lipid and insulin resistance in members of type 2 diabetic pedigrees.DESIGN: A retrospective case control study of the non-diabetic first-degree relatives of type 2 diabetic families.SETTING: The department of endocrinology of a university hospital.PARTICIPANTS: This study was carried out in the Department of Endocrinology of the First Affiliated Hospital of Chongqing Medical College from June to November 1999. A total of 125 non-diabetic first-degree relatives of type 2 diabetic pedigrees in Chongqing were recruited and divided into hyperlipemia group and normal control group according to their blood lipid level.INTERVENTIONS: Body height, body mass and blood pressure of all the subjects were measured. Their cubital venous blood samples were also obtained for determination of the levels of fasting plasma glucose (FPG), fasting insulin(FINS), fasting C-peptide(FCP), triacylglycerol(TG), total cholesterol(TC), low-density lipoprotein-cholesterol (LDL-C), and high-density lipoprotein-cholesterol (HDL-C).homeostasis model assessment insulin resistance(HOMA-IR) between the two groups.RESULTS: The level of FINS was markedly higher in the hyperlipemia group [(12.92 ±5.26) μU/L] than in the control group[(9.15 ±4.66) μU/L]( t = 2. 17, P < 0.05) . HOMA-IR was also significantly higher in the hyperlipemia group [ (2.94 ± 1.25 ) ] than in the control group [ (2.01 ± 0.94) ]( t = 2.70, P < 0.01 ) . In the hyperlipemia group, HOMA-IR was positively correlated with TC, TG and LDL-C ( r = 0. 34, 0. 78, 0.42), with TG most closely correlated( r = 0. 78, P < 0. 01), while HOMA-IR had no significant relation with HDL-C ( P > 0.05).CONCLUSION: There is a close correlation between lipid metabolic disorder and insulin resistance in the high-risk diabetic group.

5 cm?5 cm) (n=6).All patiens with complications were successfully treated except 8 with side branch closure, who were failed to response but cured by drugs after procedure.Three elder patients died, from sudden death(n=1),stoke (n=1),and kidney failure (n=1) after PCI. Conclusion:The complication incidence in patients with CTO lesions is relatively low and most of the complications can be cured by proper management.Revascularization by PCI for patients with CTO is an effective and safe treatment method.

Objective To study the efficacy and safety of transverse compression suture in the lower uterine segment for the control of postpartum haemorrhage in cesarean section caused by placenta previa. Methods From Jan 2011 to Jan 2013, 21 patients with postpartum haemorrhage in cesarean section due to placenta previa were given transverse compression suture in the lower uterine segment after routine medical treatment. And the hemostatic efficacy and safety were observed. Results 20 cases of the vaginal bleeding were controlled efficient-ly, with an efficiency of 95. 2%. There was no complication occurred, and menstruation were back to normal during the follow-up, and there was nothing abnormal in the uterine double accessories through B ultrasound reexamination. Conclusion Transverse compression suture in the lower uterine segment is an easy, safe and highly effective surgical technique, it is especially suitable for the control of haemorrhage in the lower uterine segment caused by placenta previa.

Objective To observe clinical curative effect of modified total cystectomy and Mainz Ⅱ neobladder. Methods Seventeen patients with bladder neoplasms were treated with modified total cystectomy and Mainz Ⅱ neobladder for urinary diversion. The paries posterior allantois with intestinum rectum and colon sigmoideum were taken 10 cm respectively, split the mesenterium edges, conduplicated and bilayer sutured from the junction of intestinum rectum and colon sigmoideum, bilateral ureters antireflux anastomosed respectively with colon sigmoideum and rectal papilla, then bilayer sutured paries anterior became Mainz allantois. Results There was no surgical mortality. The operative time was 340 ~ 420 mins (mean, 350 mins).Blood transfusion was 400 ~ 800 ml ( mean 600 ml). The follow-up was 4 ~ 18 months, urine and dejecta were shunt, uresis continence was fine and the operation had fewer severe complications. Conclusion Modified total cystectomy and Mainz Ⅱ neobladder to be an effective method for urinary diversion because of its simple operation, fewer severe complications, good uresis continence and high quality of life.

Objective: To evaluate the long-term efifciency of percutaneous transluminal septal myocardial ablation (PTSMA) for treating the patients with hypertrophic obstructive cardiomyopathy (HOCM).
Methods: A total of 66/94 (70.2%) HOCM patients received PTSMA in Shenyang PLA general hospital from 2001-10 to 2012-10 were retrospectively studied. The left ventricular out lfow gradient (LVOFG) was measured at before and after the operation, ECG and echocardiography were examined at 1 month, 6 months and 1 year after operation, and then examined once per year for (63.8±28.5) months.
Results: There were 26 patients lost contact during follow-up period, 40 returned to routine clinical check-up and 2 patients died thereafter, 1 because of sudden death and 1 because of cerebral bleeding. The pre-operative average LVOTG was (102.7 ± 47.5) mmHg, compared with the values at 6 months post-operation and long term (>6 months) after operation (33.9 ± 30.2) mmHg and (29.7 ± 25.4) mmHg,P<0.001. The pre-operative average inter ventricular septal (IVS) was (20.1 ± 3.6) mm, compared with the values at 6 months post-operation and long term after operation (17.5 ± 2.9) mm and (16.4 ± 3.6) mm, P=0.028 andP<0.001. There were 7 patients with NYHA class at II-III and having occasional chest suppression and short of breath. There were no heart transplantation, frequent premature ventricular contraction, tachycardia and other malignant arrhythmia occurred in 38 survivors.
Conclusion: PTSMA may reduce LVOTG, IVS thickness and improve the clinical symptoms in HOCM patients, the long-term efifcacy is reliable.
