1.Effects of Buyang Huanwu Decoction on Contents of PCⅢ and Ⅳ-C in Blood Serum of Pulmonary Fibrosis Rats

Objective To discuss intervention effects of Buyang Huanwu decoction on pulmonary fibrosis (PF) rats. Methods Totally 144 healthy male SD rats were randomly divided into six groups:sham-operation group, model group, prednisone group, Buyang Huanwu decoction high-, medium-, low-dose groups, 24 rats in each group. Except for sham-operation group, the rest groups were given injection of BLM via tracheal intubation for the establishment of PF model, while sham-operation group was instilled the same amount of NS. From the second day, all the rats received a gavage by related medicine. Eight rats in each group were killed respectively on day 7, 14 and 28 after intratracheal instillation. The contents of PCⅢ and Ⅳ-C in blood serum of experimental rats were tested by radioimmanoassay method. Results Compared with the sham-operation group, the contents of PCⅢ and Ⅳ-C in blood serum of other groups increased gradually, and that of the model group was the most obvious (P<0.01). Compared with the model group, each medicine group had different decreases in the contents of PCⅢ and Ⅳ-C in blood serum (P<0.01). Compared with the prednisone group, the contents of PCⅢ and Ⅳ-C in blood serum of Buyang Huanwu decoction medium-dose group obviously decreased on the 14th and 28th day (P<0.05), as well as that of Buyang Huanwu decoction low-dose group on the 28th day (P<0.05). Conclusion Buyang Huanwu decoction could effectively reduce proliferation of collagen fibers and excess deposition of ECM in lung tissue of PF, and played an anti-fibrosis effect on lungs.
2.Effect of Sling-exercise Therapy on Cervicogenic Headache

Objective To investigate the efficacy of sling-exercise therapy on cervicogenic headache. Methods 60 patients with cervicogenic headache were divided into 2 groups with 30 patients each. Group I received paravertebral block of C2 once a week for 4 weeks, and Group II received sling-exercise therapy 3 times a week in addition. The Visual Analogue Score (VAS), frequency of pain (per month) before and 1, 3, 6 months after treatment were recorded, and the incidence of improvement was observed. Results In group I, VAS significantly improved 1 and 3 months after treatment (P<0.001), while it lasted 6 months in group II (P<0.001). The VAS improved more in group II 3, 6 months after treatment (P<0.001). It was similar in pain frequency. The incidence of improvement was 33.3% (10/30) and 73.3% (22/30) in group I and group II, respectively (P<0.01). Conclusion Sling-exercise therapy may improve the efficacy of paravertebral block on
cervicogenic headache, especially for the long-term.
3.Treatment with 125I seeds interstitial brachytherapy for the malignant tumor in digestive tract

Objective To inquive into 125I seeds interstitial brachytherapy on the malignant tumor in digestive tract.Methods Analyse retrospectively 104 patients with different malignant tumor of stage Ⅱ and stage Ⅲ in digestive tract.All patients received radical resection.125I intraoperative implantation to tumor bed and regional lymph nodes.Complete bland count was determined during postoperative 5-7 days and immuoassay,and X-ray film were taken to observe the distribution of 125I seeds,understand tumor recurs or shifie the situation.Results All patients are followed up a case by regular,Don't have death.Among them one of interstitial Lou happens 8 patients WBC count decreased to 3×109/L within one week and rose to 4×109/L after suit the remedy to the case.Immunity ralve was below normal survive patient apper serious complication that lead to the fact because plant into 125I seeds follow up a case by regular visits to.while being other.Conclusion 125I seeds interstitial beraehytherapy for the malignant tumor of stage Ⅱ and stage Ⅲ in digestive tract is simple,safe and effective especially good application prospects in basic wint.
4.Meta-analysis of the association between GSTT1 and the susceptibility to colorectal cancer among ;Chinese population

Objective To assess the association between glutathione S-transferase T1 (GSTT1 )gene and the susceptibility of colorectal cancer among Chinese population.Methods Clinical controlled trials of the association between GSTT1 and the susceptibility of colorectal cancer among Chinese population were searched in PubMed,EMBase,Cochrane Library,China Biology Medicine disc,VIP database,China National Knowledge Infrastructure and Wanfang database.According to the inclusion and exclusion criteria,data extraction and quali-ty assessment were done by two researchers independently.Outcomes were pooled with RevMan 5.1 .Results 326 studies were found and 10 clinical controlled trails including 2 983 cases of colorectal cancer and 4 386 cases of healthy objects were included in this analysis.The results of Meta-analysis showed that the lack of GSTT1 gene is associated with colorectal cancer susceptibility (RR =1 .11 ,95%CI:1 .06-1 .17,Z =4.26,P <0.000 1 ). Conclusion The loss of GSTT1 increase the risk of colorectal cancer among Chinese population.

6.Comparison of serum markers between pure hepatitis B infection and superinfection of hepatitis B together with hepatitis D

Objective To understand the characteristics of the superinfection of hepatise B virus (HBV)together with hepatitis D virus(HDV)by comparing the differences of serum markers ALT,HBV DNA,HBVM,etc.In the pure HBV infection and the superinfection of HBV and HDV and to preliminarily invesitigate the pathogenesis of HDV.Methods The results of major bio-chemical indicators and hepatitis virus markers in 95 cases of HBV together with HDV infection and 100 cases of pure HBV infec-tion as the control group were statistically analyzed.Results Among 95 cases in the superinfection group,the incidence rate of chronic HBV was the highest,accounting for 66.32% and followed by liver cirrhosis.There was no statistical difference between the two groups according to the HBV DNA loads(P >0.05).The good positive correlation existed between the ALT abnormal rate of the liver function and HBV DNA in the superinfection group(r=0.90,P <0.05).The HBeAg negative mode was predominant in the superinfection group (P <0.05 ).There was no difference beween the two groups according to HBsAg > 250 IU/mL (P >0.05).According to HBsAg>250 IU/mL and HBeAg > 1 S/CO,the pure HBV infection group was higher than the super infec-tion group (P <0.05).The positive rate of HBcAb-IgM in the super infection group was significantly higher than that in the pure HBV group (P <0.05).Conclusion The superinfection of HBV together with HDV infection has the high occurrence rate in chro-nic hepatitis B.With the HBV DNA level increase,the abnormal rate of the liver function is also increased.HDV infection can inhib-it the HBeAg expression.The high positive rate of HBcAb-IgM in the superinfection patients might be related with chronic hepatitis B aggravation and recurrence.
7.Bevacizumab plus chemotherapy as first-line therapy for metastatic colorectal cancer: a systematic review of the efficacy and safety

Objective:To systematically assess the efficacy and safety of bevacizumab (BEV) plus chemotherapeutic agents as first-line therapy for metastatic colorectal cancer (mCRC). Methods:We retrieved randomized controlled clinical trials (RCTs) of BEV plus chemotherapeutic agents as first-line therapy for mCRC from the databases of PubMed (1966 to August 2009), Embase (1974 to August 2009), Cochrane Library (Issue 3, 2009), China Journal Full Text Database (CJFD, 1994 to August 2009), Chinese Bio-medical Literature Database(CBM, 1978 to August 2009) and Chinese Scientific Journals Full Text Database (CSJD, 1989 to August 2009). Then we evaluated the methodological quality of included studies. Meta-analysis was performed using RevMan 5.0 software developed by the Cochrane Collaboration. Results:Only 6 clinical studies were selected and 2 646 eligible patients were included in the systematic review. Meta-analysis showed that BEV plus chemotherapy increased the overall response rate (complete response+partial response) compared with chemotherapy alone. The relative risk was 1.27 (95%CI: 1.00-1.61, P=0.05), and the median survival time and progression-free survival (PFS) were longer. In terms of safety, there was a significant difference in the frequency of grade 3/4 adverse events, grade 3/4 hypertension, adverse events-induced study discontinuation and gastrointestinal perforation between the two groups. The relative risk was 1.12 (95%CI: 1.07-1.61), 4.51 (95%CI: 2.81-7.23), 1.37 (95%CI: 1.16-1.63)and 4.32(95% CI:1.24-15.05), respectively. There was no statistical difference between the two groups in the incidence of grade 3/4 bleeding, 60-day all-cause mortality, grade 3/4 proteinuria, grade 3/4 diarrhea, grade 3/4 leukopenia and pulmonary embolism. The relative risk was 1.50(95%CI: 0.87-2.57), 0.71(95%CI: 0.45-1.11), 2.26(95%CI: 0.69-7.33), 1.18(95%CI: 0.99-1.41), 1.17 (95%CI: 0.97-1.42)and 0.84(95%CI: 0.46-1.53), respectively. Conclusion:BEV plus chemotherapeutic agents as first-line the-rapy increases the response rate and prolongs PFS and median survival time of mCRC patients, but results in a higher incidence of any grade 3/4 adverse event, grade 3/4 hypertension and gastrointestinal perforation.

Anastomotic leakage is one of the common and serious complications after colorectal cancer surgery, and it should be detected, prevented and treated as soon as possible. In recent years, the causes, diagnosis and treatment of postoperative anastomotic leakage of colorectal cancer have always been the focus of clinical attention, and relevant reports and prediction indicators continue to emerge. This article reviews the current situation and progress of biomarkers for predicting postoperative anastomotic leakage of colorectal cancer, in order to provide theoretical basis for early clinical detection and treatment of anastomotic leakage.
9.Influence factors of delayed recovery after right ventricular-extrapulmonary arterial conduit reconstruction

Objective:To explore the risk factors of early delayed recovery after right ventricular-extrapulmonary arterial(RV-PA)conduit reconstruction.Methods:From 2017 to 2021, the children with RV-PA conduit reconstruction, who were treated in our hospital were retrospectively analyzed.The demographic data and peri-operative clinical data of the patients were collected for statistical analysis.Results:Fifty-five patients were included in the study.The patients were sequenced by the length of ICU stay.The time at the 75th percentile was defined as the critical value for grouping.According to the ICU stay time of the children, they were divided into normal recovery group(ICU stay ≤7 days, n=40)and delayed recovery group(ICU stay>7 days, n=15).The mechanical ventilator time in the whole group was 24(0, 1 408)h, and the ICU stay time was 4(1, 67)d.Six cases required extracorporeal membrane oxygenation (ECMO) support, and two cases died.In the multivariate Logistic regression analysis of two groups, long cardiopulmonary bypass(CPB) time( OR=1.034, 95% CI 1.009-1.061, P=0.009)and poor right ventricular function( OR=9.536, 95% CI 1.010-90.037, P=0.049)were independent risk factors for early delayed recovery. Conclusion:The risk of RV-PA conduit reconstruction is high.The proportion of ECMO support is increased.The mortality rate is higher.Right heart dysfunction and prolonged CPB time are risk factors for delayed postoperative recovery.
10.The experience in reoperation of Behcet's disease patient with perivalvular leakage after aortic valve replacement

Objective:To introduce the application of sleeve reconstruction with bovine pericardium in the reoperation of Behcet's disease with advanced aortic annulus invasion, and to report our experience and the early outcomes.Methods:From August 2012 to May 2022, 19 patients with Behcet's disease developed perivalvular leakage after previous surgery underwent aortic root replacement. They were analyzed retrospectively. Fifteen out of 19 cases underwent conventional aortic root replacement, 4 cases with advanced aortic annulus invasion underwent sleeve reconstruction with bovine pericardium. The medical records were reviewed. Demographic and perioperative data were collected, which included the aortic crossclamp time, intraoperative RBC consumption, length of ICU stay, and major postoperative complications.Postoperative follow-up was accomplished through telephone visit.Results:The median age was 39(36, 42) years old. Five were female. The median interval of diagnosis of perivalvular leak from index surgery was 5(3, 14)months. There was no perioperative death. Fifteen patients underwent conventional aortic root replacement. One patient had intraoperative ECMO impant due to low cardiac output as well as another one had IABP support. CABG was performed in 5 patients. One patient underwent permanent pacemaker implantation after surgery. The median follow-up time for the 15 patients was 55.00(29.25, 71.25)months. Two patients were lost during follow-up. One patient died of infection 3 years after surgery. One patient developed perivalvular leak. Two patient developed distal anastomotic pseudoaneurysm which need reintervention. The median follow-up time for the 4 patients with sleeve reconstruction was 5.5(5, 11.25)months. One patient underwent tracheotomy. Perivalvular leakage and pseudoaneurysm were not appreciated. Four patients were in NYHA class Ⅰ-Ⅱ.Conclusion:The sleeve reconstruction with bovine pericardium is safe and effective in reoperation treating patients with Behcet's disease and advanced aortic annulus invasion.
