Introduction: Adrenal myelolipomas and lipomas are extremely rare benign tumors. They are usually asymptomatic and in most cases are incidentally found at radiological examinations or autopsy. Symptoms such as abdominal pain and increasing girth occur only when the tumor grows large. Objectives: To evaluate and treat this situation. Subjects: We report the case of a giant adrenal myelolipoma with dimensions of 9 x 8 x 8cm and a giant adrenal lipoma with dimensions of 20 x16 x 12cm. Firstly, in a 48 year old man admitted to hospital on the 18 January 2005 without symtoms of adrenal lipoma or adrenal mylolipoma and another one is a 25 year old man who was hospitalized on the 8 December, 2004 with pain in the right hip since two months prior, normal blood pressure and undiscovered tumor by hand. Result: Other tests were normal. Looking at the CT scan images, a dark and consistent block located between the liver and right kidney can be seen. It pushed and turned the right kidney. Both of them were resected by laparoscopy. Conclusion: We discuss the prevalence, clinical features, diagnosis and treatment of these uncommon tumors.

2.Prevalence of epilepsy in Phu Linh \ufffd?Soc Son \ufffd?Ha Noi, 2003.

Background: Epilepsy is a chronic disease, the incidence of epilepsy was from 5/1000 to 10/1000 population. In developed countries the rate of epileptic patients are managed much higher than in developing countries due to the differences in socioeconomic conditions as well as understanding of epilepsy. In Vietnam, few epidemiological studies about epilepsy were published. Objectives: To reveal the prevalence of epilepsy in a rural community in Viet Nam and the relation between the prevalence and age, career, level of education, economic condition of the patient. Subjects and method: This was a cross-sectional described study. Door \ufffd?to \ufffd?door survey based on WHO\u2019s questionnaires was conducted by neurologists and student of Faculty of Public healthy of Hanoi Medical University on people at Phu Linh commune, Soc Son district, Hanoi city. Data was processed by SPSS 10.0 program. Results: Periodic prevalence was 7.5\ufffd?(5.5\ufffd?was active epilepsy). Generalized epilepsy: 79.66%, focal epilepsy: 16.95% indeterminate epilepsy: 3.39%. There was a significant relation between education and prevalence of epilepsy. Conclusion: Periodic prevalence of Epilepsy in Phu Linh (2003) was 7.5\ufffd? Toninco-colonique seizures were dominant. A significant relation between number of epileptic patients and level of education was found.

3.Remark on the various clinical characteristics and treatment results of mobile tongue cancer at K Hospital from 1994 to 2004

Background: Tongue cancer is the most common cancer in oral cavity and often occurs in men over 50 years of age. Although it can be detected early, most patients have late diagnosis and treatment. Objectives: Remark on various clinical characteristics and treatment result of mobile tongue cancer at K Hospital from 1994 to 2004. Subjects and method: A prospective study was carried out on 290 patients with mobile tongue cancer treated in K Hospital between 1994 and 2004. Results: Among the 290 patients, there were 102 women and 170 men, the male/female ratio = 1.4:1. Cancer of the tongue was most seen at the age of 50-69 years old (52.2%). Common initial symptoms were the tumor of the tongue (27.6%) and tongue ulceration (28.3%). 91% of lesions were on the free border of the tongue. 45.9% patients had metastases in local lymph nodes. Most patients were diagnosed at the advance stage (stage III: 43.8%, stage IV: 25.5%). Squamous cell carcinoma accounted for 99.3%. 38.2% of cervical non-palpable nodes were pathological metastases. Surgical treatments and radiotherapy were commonly used (24.1% and 43.1%, respectively). Conclusion: Tongue cancer often metastasized early into cervical lymph nodes. It is required to diagnose at the early stage to improve treatment outcomes.

4.Noise pollution, occupational failing hearing and occupational hearing loss among auto-engineering workers in Hue city

Background: Noise pollution has seriously negative effects on worker\u2019s health, especially their hearing ability. Occupational hearing loss is one of 21 insured occupational diseases. Occupational hearing loss is common in workers in transpostation, construction, energy heavy and light industry areas. Objectives: 1. To determine the degree of noise pollution in Thua Thien Hue Unity Auto Mechanical Joint Stock Company. 2. To evaluate occupational failing hearing and occupational hearing loss among workers in the company. 3. To promote preventive measures which imnimize occupational failing hearing and occupational hearing loss. Subjects and method:A cross-sectional, descriptive study was carried out in 254 employees in Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company. Results and Conclusion: 72.7% the employees of Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company have exposed to environmental noise at the level higher than the permitted one. The rate of occupational failing hearing among these people is rather high (33.1%), and occupational hearing loss rate is 11.8%. There is a increased rate of occupational hearing loss according to the employee\u2019s age and seniority. Proposed preventive measures should be immediately conducted (P < 0.05).\r\n', u'

5.Clinical characteristics of poststroke dementia patients with age of 60 years and older

Background: Dementia is one of the major causes of dependency after stroke. The prevalence of poststroke dementia (PSD)defined as any dementia occurring after stroke is likely to increase in the future.Objectives: This study have two purposes: 1) Clinical study of MCI and dementia after the first stroke of patients with age of 60 years and older; 2) Overview on clinical characteristics of memory disorders. Subjects and method: 30 patients with were diagnosed with the first ischemic stroke in Huu nghi hospital together with the same number in the control group were involved in this study. The subjects in the two groups were all satisfied with included/excluded criteria diagnosis. Clinical diagnosis of new - onset dementia or other mental disorders was determined using neuropsychological tests. Results: Many functions of the brain were impaired including: logical memory, visiospatial skills, executive function were statistically reduced in the research group compared to the control. However, language function was also impacted but not as much as others. The frequency of the poststrocke dementia in this study was 12.3% while the poststrocke mild cognitive impairment rate was 47%. Conclusions: Global cognitive functioning together with memory state was significantly declined in the ischemic stroke compared to the control group.

6.Application of neuropsychological battery tests in post-stroke dementia diagnosis among patients over 60 years old

Background: In Vascular Dementia (VaD) patients, the causes of blood vessels were common, and preventable and treatable, so that it is very important to detect and diagnose in the early stages of the disease. Diagnosis of dementia is based on clinical symptoms, and neuropsychological tests are useful tools. Objectives: (1) To evaluate the severity of VaD and Vascular Cognitive Impairment (VCI) after the 1st ischemic stroke in patients over 60 years old. (2) To make observations on the clinical features of post stroke dementia in these patient groups using neuropsychological battery. Subjects: 94 patients with 1st acute ischemic stroke, who were over 60 years old, conscious and literate, and cooperated well with physicians. A standard evaluation protocol was conducted at one month after an ischemic stroke for all the patients. Method: Prospective study. Data was analyzed by using SPSS software version 13.0. Results and conclusions: The rates of VCI and VaD after the first ischemic stroke were 21.3% and 25.5%, respectively. Clinical determinants of dementia were: visuoconstruction (65% patients), visual motor speed (50%), memory disorders (more than 40%, in which visual memory 45.8% and verbal memory 41.6%), executive function (37.5%), and language skill (37.5%). The attention and language functions were less affected (only 25% of the patients). Mini mental state examination score can be used to evaluate and classify clearly 3 groups: VaD, VCI patients and normal people.

7.Lead level in blood and urine of fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province

Background: In Viet Nam, there were many researches on determining lead level in blood, food and environment. But until now, there were not researches on determining lead level in fishermen community. Objectives: Determine trace amount of lead by von-ampe dissolve anot using electrode BiFE. Analysis on trace amount of lead in blood and urine of fishermen in Canh Duong village. Propose solutions for preventing lead poisoning in studied community. Subjects and method: Fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province. The method differential pulse-anodic stripping voltametry (DP-ASV) using Bismuth Film Electrodic in situ was successfully applied to measure lead levels in blood and 24h urine of fishermen in Canh Duong village. Results: The mean level of lead in blood of the suspected group (fishermen) in Canh Duong village (34,7 \xb5g/dL) was higher than that of the control group (16,3 \xb5g/dL). For both groups, 37 out of 40 blood samples (92,5%) had lead level higher than WHO\u2019s recommended level (10 \xb5g/dL). Especially, 6 of 40 blood samples (15%) had lead level higher than 50 \xb5g/dL. The lead level in 24h urine of suspected group had slightly higher (about 1,2 times) than that in the control group. The lead level of the two groups was much higher than the normal level. Conclusion: The method DP-ASV/BiFE in situ was applied successfully to determine lead level in blood and 24h urine. The investigative results of related information showed that: the risk of chronic lead poisoning in studied community very high.

8.Laparoscopic surgery in Quang Tri General Hospital

Background: Laparoscopic cholecystectomy is the first laparoscopic surgery that was performed at Cho Ray hospital in September 1992. In 2005, Quang Tri general hospital has performed successfully the first case of laparoscopic appendectomy. Objectives: To access the preliminary results of application on endoscopic surgery in Quang Tri general hospital. Subjects and method: A prospective study was conducted on 140 patients with laparoscopic surgery or retroperitoneal surgery, was performed in Quang Tri general hospital from September, 2005 to April, 2006. Results:Among 140 patients was operated, there was only one case of postoperative intestinal obstructive complication (accounted for 0.7%), no case of death. Operative aged was between 11 and 70 years old. The average surgical time was 65 minutes for cholecystectomy, 35 minutes for appendectomy, 85 minutes for ureterolithotomy, 80 minutes for gynecological diseases. 2 cases of cholecystectomy and appendectomy changed open surgery. Conclusion: Laparoscopic surgery is a safe and effective method. It can be developed in provincial hospitals. Quang Tri general hospital has successful preliminary developed the new technique.

9.Results for cardiac intervention in National Hospital of Pediatrics

Background:The progress of the cardiac intervention technique has been changed the doctors' viewpoint in the treatment for congenital cardiac. In the past years, treatment for congenital cardiac needed to surgery but nowadays, the cardiac intervention technique are being applied in many Cardiac Centers with very good results. This technique also helped to prevent the complication in cardiac operation. Objectives:This study aims to report the results for cardiac intervention in National Hospital of Pediatrics. Subjects and method:A prospective study was conducted on 298 children diagnosed with patent ductus arteriosus, atrial septal defect (ASD), ventricular septal defect, coarctation, pulmonary stenosis (PS), aortic stenosis (AS), small patent ductus arteriosus (PDA) infant with pulmonary atresia - ventricular septal defect (VSD) and Dextro- transposition of the great arteries (d- TGA) with intact ventricular septum at National hospital of Pediatric between June 2004and October 2006. Results: Transcatheter closure of PDA, ASD by Amplatzer or Coil was safe and effective. Critically ill children diagnosed with PS, AS and Coarctation may be saved by valvuloplasty and angioplasty procedures. Catheter interventions avoided the needs for surgery without scars. Catheter interventions had low complications. Conclusion: The hospitalized duration was short so the expenditures reduced.

10.Epidemiological characteristics of HIV/AIDS patients and community care in Thua Thien Hue

Background: HIV/AIDS pandemic really become danger to mankind on the earth. Objectives: Description epidemiological characteristics of HIV/AIDS patients. Studying clinical expression, learning aboutdemands of infected patients and community care for infected patients. Subjects and method: 71 HIV/AIDS people in Thua Thien Hue were studied in 2006. Using descriptive method to give epidemiological characteristics and behaviour of HIV/AIDS people. People were determined HIV/AIDS infection based on the criteria of Ministry of Health. The information were collected by target study. Results: HIV/AIDS people mainly from the age of 20 to 39 (83,10%), males were 52,10% and in various levels of education and all kinds of occupation. The rate of sexually transmitted infection was 84,5% and infection transmission was 15,5%. The rate of sexually transmitted infection within the last 12 months was 57,7%. 63,4% of infected people used condom while having sex with their spouses or partners. 1,4% of infected people using drug infection within the last month. 38% of the infected people had manifestations of AIDS, 77,8% of HIV/AIDS were treated with antiviral drugs. Families and communities had positive attitudes and behaviors to the HIV/AIDS people. Infected people being remoted from the society was 12,7%, 88,7% of them receiving supports from their communities such as disease treatment, spirit and material supports and job opportunities. Conclusion: Strengthen communication activities to change behavior of HIV/AIDS people and in communitiy to reduce alienation, discrimination for infected people. Execute socialization in care and support for HIV/AIDS people.
