1.Design, synthesis, and PPARgamma agonistic activity of novel indenone derivatives.

Agonists of peroxisome proliferator-activated receptor gamma (PPARgamma) are of interest as a treatment of type II diabetes, and indenone derivatives are a new class of non-TZD PPARgamma agonists. Based on existing indenone derivatives, a series of novel ones have been designed and synthesized. Meanwhile the structures have been comfirmed with 1H NMR and MS. Among them, 17b and 19 showed higher agonistic activities than rosiglitazone.
2.Clinical outcomes of advanced gastric cancer patients treated with chemotherapy of paclitaxel liposome combined with S-1 or oxaliplatin

Objective:This study aimed to compare the efficacy and adverse reactions of two different chemotherapeutic regi-mens. In particular, chemotherapy with paclitaxel liposome was administered in combination with either S-1 or oxaliplatin as the first-line therapy of advanced gastric cancer. Methods:A total of 118 patients with advanced gastric cancer were randomly divided into groups A (61 cases) and B (57 cases). In group A, paclitaxel liposome combined with S-1 was administered;in group B, paclitaxel lipo-some combined with oxaliplatin was applied. The short-term efficacy, adverse reactions, Karnofsky performance status score, median time to progression (mTTP), and median overall survival (mOS) of the two groups were observed and compared. Results:No signifi-cant differences were observed in the objective response rate, disease control rate, and mTTP between groups A and B (31.1% vs. 29.8%, 75.4%vs. 71.9%, 4.2 months vs. 3.8 months;P>0.05). The mOS rates were 10.5 and 8.9 months in groups A and B, respectively, with statistically significant differences (P=0.006). The incidence of degreesⅢtoⅣdiarrhea and peripheral nerve toxicity was signifi-cantly higher in group B than in group A (P<0.05). No statistical differences were found between the two groups in terms of other side effects. Conclusion:The two paclitaxel liposome-based regimens showed similar therapeutic effect in patients with advanced gastric cancer. S-1/paclitaxel liposome treatment could be more effective in terms of mOS and had a tendency of lower toxicity.
3.Neuromuscular and cardiovascular effects of rocuronium bromide

The neuromuscular and cardiovascular effects and endotracheal intubating conditions of bolus intravenous rocuronium 0.6mg/kg or 0.75mg/kg in 20 patients under balanced anesthesia,were studied, Intubating conditions were evaluated as excellent or good in all patients,except for one with poor intubating conditions following 0.6mg/kg. Onset times of both groups were 73.1 s and 67.1 s;neuromuscular blockade durations 24.9min and 32.0min; 25% recovery times 37.3min and 45.4min; 75% recovery times 46.4min and 58.7min;recovery index 9.1min and 13.3min,respectively. The cardiovascular effects after rocuronium in both groups were minimal.

6.Clinical effect of ulta pulse CO2 laser in treatment of 32 patients with primary amyloidosis of the skin

ObjectiveTo evaluate the clinical effect of ultra pulse CO2 laser in treatment of 32 patients with primary amyloidosis of the skin. Methods64 patients with primary amyloidosis of the skin were selected and randomly divided into two groups,the observation group(32 cases)and the control group(32 cases).The observation group were treated with ultra pulse CO2 laser,and the control group were treated by routine medicine method.Clinical effect were compared between the two groups after twelve weeks therapy. ResultsThe total effective rates of the observation group and the control group were 90.6%,53.2% respectively.There were a significant difference between the two groups(x2 = 11.13,P<0.01).There were some different irritations in two groups after therapy,to give symptomatic treatment could help relieve.The relapse rate in the observation group was significant lower than that in control group after treatment of 3 ~ 6 months(x2 = 9.471,P<0.01). ConclusionUltra pulse CO2 laser therapy of primary amyloidosis of the skin was safe and effective.
7.Influence of sevoflurane anesthesia on expression of GAP-43 and NCAM in hippocampal neurons of neonatal rats

Objective To evaluate the influence of sevoflurane anesthesia on the expression of growth-associated protein 43 (GAP-43) and neural cell adhesion molecule (NCAM) in hippocampal neurons of neonatal rats.Methods Thirty-six pathogen-free Sprague-Dawley rats,aged 7 weeks,weighing 15-20 g,were randomly divided into 4 groups (n =9 each) using a random number table:control group (C group),1.5% sevoflurane 6 h group (L group),3% sevoflurane 2 h group (H1 group) and 3% sevoflurane 6 h group (H2 group).Group L inhaled 1.5% sevoflurane in oxygen for 6 h.H1 and H2 groups inhaled 3% sevoflurane in oxygen for 2 and 6 h,respectively.Group C inhaled 30% oxygcn only.When the neonatal rats were 14 days old,the rats underwent Morris water maze test for 7 consecutive days.Place navigation and spatial probe tests were carried out.After the end of Morris water maze test,the rats were sacrificed,and the hippocampus was obtained for determination of the expression of GAP-43 and NCAM in hippocampal neurons.Results Compared with group C,the escape latency was significantly prolonged,the time of staying at the original platform quadrant was shortened,and the expression of GAP-43 was down-regulated in L,H1 and H2 groups,and the frequency of crossing the original platform was decreased in L and H2 groups.There was no significant difference in NCAM expression among the four groups.Conclusion The mechanism by which sevoflurane anesthesia decreases the cognitive function may be related to down-regulated expression of GAP-43,but not related to NCAM expression in hippocampal neurons of neonatal rats.
8.Effects of sagital split ramus osteotomy surgery on hyoid position and pharyngeal airway with skeletal mandibular prognathism

Objective:To investigate the changes of hyoid position and pharyngeal airway before and after sagittal split ramus osteotomy (SSRO) of skeletal mandibular prognathism. Methods:Sixteen severe skeletal mandibular prognathism cases(Class Ⅲ, 5 males and 11 females) were chosen. Pre and post operation lateral cephalograms analysis reflecting pharyngeal airway and position of the hyoid bone was performed. Results: ① After SSRO, the width of airway decreased significantly. ② The distance between the mandible and the hyoid bone was also decreased significantly. No significant differences were found regarding measurements of the hyoid bone to the cranial base in male patients. Conclusion: The distance from the mandible to the hyoid bone and the width of airway were decreased significantly after SSRO. It suggests that the tongue and airway might be reformed functionally to new environments following SSRO surgical treatment.
9.Function and Application of Surgery Base Drug Management System in Our Hospital

OBJECTIVE:To construct surgery base drug management system,and to realize efficient and accurate drug man-agement in surgery room. METHODS:The functions of surgery base drug management system exploited by our hospital were intro-duced,and the effect of the system was evaluated in department of anesthesiology. RESULTS&CONCLUSIONS:The system pos-sesses the functions of surgery drug automated affiliated charge,standardized narcotics prescription autogeneration,discarded nar-cotics prescription auto-prescribing,and drug information summary statisticing and checking,etc. After the application of the sys-tem,nonstandard rate of prescription decreased from 12.7% of handwritten prescription to 0.1% of electronic prescription;the time of drug requisition and checking decreased from(8.5±1.6)min to 0 min and(7.6±1.0)min to(2.9±0.9)min(P<0.05 or P<0.01). The system standardizes medication behavior of physicians and improve their work efficiency,avoid the loophole of drug charge management,realize the consistency between the accounts and the real numbers of narcotics,improve the rate of quality pre-scriptions and narcotics management, and realize integration,automation,intellectualization and whole-course supervision of drug management in pharmacy and clinical departments.
10.Effects of Yiweining recipe on expressions of metalloproteinase-2 and cyclooxygenase-2 mRNAs in ectopic endometrium of rats with endometriosis.

OBJECTIVE: To explore the effects of Yiweining Recipe (YWNR), a compound Chinese herbal medicine, on expressions of metalloproteinase-2 (MMP-2) and cyclooxygenase-2 (COX-2) mRNAs in rats with endometriosis (EM). METHODS: Operational self-transplantation was applied in establishing the rat models. Detection of MMP-2 and COX-2 mRNAs was conducted with hybridization in situ. RESULTS: There were significant differences in the expressions of MMP-2 and COX-2 mRNAs between the untreated group and the high-dose YWNR-treated group. YWNR could reduce the expressions of MMP-2 and COX-2 mRNAs. CONCLUSION: YWNR can treat EM through reducing the positive expressions of MMP-2 and COX-2 mRNAs.
