1.Community-based Rehabilitation in Heilongjiang Agricultural Reclamation: A Structural Design

Based on the investigation of the rehabilitation demand of disabled persons and the functional advantage of existing health care and services network in Heilongjiang provincial reclamation area, a structural design of community-based rehabilitation development has been drawn out, which including integrating existing three-level health care network, applying appropriate rehabilitation techniques, to establish a convenient, effective, and economical community-based rehabilitation system.
2.Advances in genetic modification technologies.

Genetic modification technology is a new molecular tool for targeted genome modification. It includes zinc finger nucleases (ZFN) technology, transcription activator-like effector nucleases (TALEN) technology and clustered regularly interspaced short palindromic repeat (CRISPR)-associated (Cas) (CRISPR-Cas) nucleases technology. All of these nucleases create DNA double-strand breaks (DSB) at chromosomal targeted sites and induce cell endogenous mechanisms that are primarily repaired by the non-homologous end joining (NHEJ) or homologous recombination (HR) pathway, resulting in targeted endogenous gene knock-out or exogenous gene insertion. In recent years, genetic modification technologies have been successfully applied to bacteria, yeast, human cells, fruit fly, zebra fish, mouse, rat, livestock, cynomolgus monkey, Arabidopsis, rice, tobacco, maize, sorghum, wheat, barley and other organisms, showing its enormous advantage in gene editing field. Especially, the newly developed CRISPR-Cas9 system arose more attention because of its low cost, high effectiveness, simplicity and easiness. We reviewed the principles and the latest research progress of these three technologies, as well as prospect of future research and applications.

3.The value of upright standing myelography in the diagnosis of lumbar disc herniation

Objective To evaluate the value of upright standing myelography in the diagnosis of lumbar disc herniation (LDH). Methods One hundred and ninety six patients with LDH diagnosed by CT or MRI were examined by upright standing myelography before surgery. Results The most diagnosis of myelography were accorded with CT or MRI, but 7 patients of myelography with LDH at L4-5 showed compression of nerve roots, while the images of these on CT or MRI only showed disc bulge or protrusion without nerve root compression. Conclusions Myelography in the upright standing position is perhaps superior to CT or MRI for the diagnosis of LDH, especially LDH at L4-5 with nerve root compression.
4.Angular dose dependency of MatriXX and its calibration

Objective To characterize angular dependency of MatfiXX and develop a method for its calibration in order to verify treatment plan with original gantry angles.Methods Absolute dose calibration was carried with thimble ionization chamber on the linear accelerator.so as to make sure 1 MU=1 cGy at the depth of maximum dose(dmax).A MatriXX was put into a Mutlicube phantom,and the ionization chamber matrix was calibrated with absolute dose.In order to determine a correction factor CF as a function of gantry angle θ.open beam fields of 10 cm×10 cm size were irradiated for gantry angles θ=0°-180°(every 5°)and every 1°for lateral angles θ in the range of 85°-95°.CF was defined as the ratio of the dose measured with ionization chamber and the dose from MatriXX.Results Relatively large discrepancies in response to posterior VS.anterior fields for MatriXX detectors(up to 10%)were found during the experiment and relatively large variability of response as a function of gantry angle.The pass rate of treatment plan in lateral beams was lower than that of other beams.The isodose distribution of corrected MatriXX matched well with the outcome from the treatment planning system. Conclusions The angular dose dependency of MatriXX must be considered when it is used to verify the treatment plan with original gantry angles.
5.The quality of ultrasonic root-end preparation: A quantitative study

Objective:To compare the quality of root-end preparation quantitatively between ultrasonic retrotips and traditional slow-speed handpiece. Methods: 27 maxillary second bicuspid teeth with two canals and an isthmus were randomly assigned to 3 groups.Root-end preparations were made respectively using ultrasonic diamond tip Berutti and niti tip RE2, ultrasonic 40~#K file and slow-speed handpiece with No.2 round bur. Image processing and analysis system were used to measure the preoperation and postoperation parameters for further evaluating the difference between the two techniques. Results: The cutting volume of ultrasonic instruments was less than slow-speed handpiece(P<0.000 1). Conclusion: Ultrasonic instrument is better in the root-end preparation than traditional slow-speed handpiece.
6.Evaluation of screening programs for cervical cancer

Objective To evaluate the values of visual inspection with acetic acid( VIA) and human papillomavirus( HPV) and thinprep cytology test( TCT) in the screening of cervical cancer.Methods VIA,HPV test and TCT in 946 women was conducted.Cervical biopsy or endocenical curettage( ECC) was performed in VIA positive women or HPV positive women or TCT positive women. The pathologic outcome was as the gold standard,and the out come more and equal to cervical intraepithelial neoplasia( CIN) Ⅱ were identified as positive.Results There was no significant difference among three groups in sensitivity rate( 73.l%,96.2,88.5% ) and negative predictive value ( 99. 1% ,99.9% ,99.7% ) ( all P > 0.05 ).The specificity rate (97.5% ) and positive predictive value ( 50.O% ) of TCT was better than those of HPV ( 93.5%,29.4% ),and HPV was better than those of VIA( 87.0%,13.7% ),and the difference had statistifical significance among three groups ( x2 =71.34,26.00,22.17,8.28,all P < 0.05 ).Conclusion Although the rate of missed diagnosis and misdiagnosis rate was higher than that of HPV and TCT,but VIA seems to be appropriate methods in the screening of cervical cancer owing to its low price.
7.MRI Features of Bone Marrow Edema in Avascular Necrosis of Femoral Head

Objective To discuss MRI features of bone marrow edema in avascular necrosis of femoral head(ANFH),and to evaluatethe relationship between bone marrow edema and the stage of ANFH.Methods MRI findings of 99 hips in 73 patients with ANFH wereretrospectively analyzed.Using SE T_1WI,T_2WI and STIR scaning,both coronal and axid were performed in all patients.The bone marrow edema in ANFH was graded into 0~3,which correlated with the stage of ANFH was concerned.Results Bone marrow edema of the femur was best demonstrated on T_2WI and STIR coronal images.Grade 1~3 bone marrow edema were seen in 61% diseased femoral heads.Bone marrow edema of the femur was increasing with progressing of the disease.Bone marrow edema was not commonly seen and localized in stageⅠof ANFH,but it was more commonly seen and extensive in stage Ⅱ,Ⅲ of ANFH.Conclusion Bone marrow edema is one sign that isaccompanied with ANFH.The probability and extent of bone marrow edema correlated well with the stage of ANFH.MRI is the mostsensitive and non-invasive means for demonstrating bone marrow edema.
8.Posterior atlantoaxial transpedicular screw fixation of unstable atlas fractures combined with rupture of transverse ligament

Objective To determine the outcome of unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation.Methods A retrospective case series study was made on 17 patients with unstable atlas fractures combined with rupture of transverse ligament treated by posterior atlantoaxial transpedicular screw fixation and fusion from January 2008 to December 2015.There were 13 males and 4 females,with age range of 34-69 years (mean,47.8 years).All atlas fractures were Jefferson fractures (Levine-Edwards type Ⅲ).Classification of transverse ligament rupture was type Ⅰ in 12 patients and type Ⅱ in 5 patients.No patients had neurologic deficit [American spinal injury association (ASIA) classification grade E].Operation time,blood loss,implant failure,bone fusion and visual analogue scale (VAS) were recorded after operation.Results Operation time was 85-120 min (mean,102 min).Blood loss was 90-150 ml (mean,115 ml).All patients were followed up for 10-20 months (mean,17.8 months).At the final follow-up,all patients achieved bone union,with no implant loosening or breakage happened.VAS was improved from preoperative (5.5 ± 1.8) points to (2.4 ± 1.5) points at tbe final follow-up (P < 0.05).ASIA Grade E remained in all patients.Conclusion Posterior atlantoaxial transpedicular screw fixation of unstable atlas fractures combined with rupture of transverse ligament is a safe and effective surgical procedure that is able to restore the atlanto-axial vertebral stability and relieve pain.

AIM: To investigate the changes of apoptosis in isolated pancreatic islet cells, insulin secretion, expression of Bcl-xL and Bax induced by combination of IL-1?, TNF-? and IFN-?, and effects of taurine on them. METHODS: Isolated pancreatic islet cells from Wistar rat were incubated in monolayer in vitro. NO-2/ NO-3 production, NOS activity, insulin secretion, the protein expression of Bcl-xL and Bax, percentage of islet cell apoptosis and DNA fragmentation in pancreatic islet cells incubated with combination of IL-1?, TNF-? and IFN-? were measured, and the effects of taurine on the changes of them were further investigated.
