1.Impact of the preservation of the intercostobrachial nerve in axillary lymphadenectomy due to breast cancer

Objective This study aimed to evaluate the relationships between preservation of the intercostobrachial(ICB) nerve and abnormal sensation of the arm,the total time of the surgery,and the number of dissected nodes in patients submitted to axillary lymphadenectomy in surgical treatment of breast cancer.Methods An prospective,randomized,and double blind intervention was performed on 85 patients at the Qinhuangdao Military Hospital in Hebei Province from July 2007 to August 2010.The patients according to whether the ICB nerve was preserved or not were randomly divided into two groups.The surgeries were performed by the same two surgeons.The postoperative evaluations were performed at 2 days,30 days,and 90 days.The abnormal sensation of the arm was subjectively evaluated with an questionnaire and objectively assessed with a neurologic examination.Results In the never preserved group,questionnaire found asymptomatic patients' proportion is up to 16/42,19/41,25/41 after 2 d,30 d and 90 d respectively; while in the resection group the result is 12/43,7/43,12/42.In the never preserved group,neurologic examination reported asymptomatic patients' proportion is up to 22/42,16/41,22/41 after 2 d,30 d and 90 d respectively; while in the resection group the result is 5/43,3/43,7/42.The results suggests that the abnormal sensation propotion in the intercostobrachial nerve preserved group is significantly less than the control group at the three above time points over (P < 0.05).No significant difference was observed on the total time of the surgery and the number of dissected nodes between the two groups.During the 36 months follow up,no local relapse was found in the two groups.Conclusions The research support that the preservation of the ICB nerve is feasible and can lead to a significant decrease in the alteration of abnormal sensation of the arm,without interfering with the total time of the surgery,the number of dissected nodes,and local relapse rate.
2.Effect of Reduning injection combined with blood purification on vital signs, renal fuction and immune function in elderly patients with multiple organ failure

Objective To investigate the effect of Reduning injection combined with blood purification on vital signs, renal fuction and immune function in elderly patients with multiple organ failure .Methods 76 patients with multiple organ failure were selected in the hospital and randomly divided into two groups.38 patients in control group were treated with actively conventional hematodialysis treatment, and 38 patients in experiment group were treated on the basis of control group with Reduning injection.The changes of vital signs, renal fuction and immune function were compared before and after treatment, and then the efficacy between two groups was compared.ResuIts Compared with control group post-treatment, in experiment group, the blood pressure and 24 h urinary volume were higher, the respiratory rate and heart rate were lower (P<0.05);serum creatinine (Scr), blood urea nitrogen (BUN), and uric acid (UA) were lower (P<0.05);the T lymphocyte cell subsets of CD3 +, CD4 +and CD4 +/CD8 +was higher, the CD8 +T was lower ( P<0.05 ) .ConcIusion Reduning injection combined with blood purification has a good clinical curative effect in treatment of multiple organ failure in elderly patients, and could effectively improve the patient′s vital signs, renal function, regulate immune function, and improve vital signs, which has the vital significance to the clinical therapy.

Objective To investigate the change of caspase-3 and F-actin in the apoptosis of prostate cancer androgen-independent cell line DU145 induced by all-trans retinoic acid (ATRA). Methods Using AO dying and fluorescent microscope to observe the shape of the cell,and the apoptosis peak can be observed by FACScan too.The effects of ATRA on the expression of caspase-3 was analyzed by Western blot and the F-actin’s structure was observed by immunofluorescent microscope. Results Flurescent microscope can observe more apoptotic cells about 85.0% in 48 h and FACScan can observe obvious apoptotic peak come into being after treated by ATRA. Meanwhile, the expression levels of cleaved caspase-3 was up-regulated.The structure of F-actin was destroyed. Conclusions ATRA could induce the apoptosis of DU145 cell mediated by caspase-3 and lead to the destroy of F-actin’s structure.

OBJECTIVE:To determine the retative bioavailability of Diclofenac sodium eye-ointment in rabbit eyes and to evaluate its pharmacokinetics following topical application METHODS:48 rabbits were divided into two groups 0 1% Diclofenac sodium eye-ointment and Diclofenac sodium eyedrops were applied topically to rabbit eyes and aqueous humor and iridic tissue were collected at given times The concentration of Diclofenac sodium in tissue were measured by HPLC RESULTS:Diclofenac sodium eye-ointment group had higher concentration and longer half-time than Diclofenac sodium eyedrops group,however,the peak time were the same in two groups The relative bioavailabilities of Diclofenac sodium ointment in aqueous humor and iridic tissue were 183 33% and 205 68% respectively,compared with those of Diclofenac sodium eyedrops CONCLUSION:Diclofenac sodium eye-ointment can be absorbed well and released slowly,which can reduce the frequency of application of drug It is especially suitable for operation patient or application at night
6.Progress of digital subtraction angiography in treatment of vascular complications after liver transplantation

Vascular complications after liver transplantation can seriously threaten the survival of patients.In the preoperative assessment and postoperative monitoring of patients' vascular conditions,although the digital subtraction angiography (DSA) is the golden standard for vascular lesion diagnosis,but not the first choice due to traumatic lesions.In some clinical circumstances,DSA can only be used when Doppler ultrasound,spiral CT,and MRI are not applicable,and in most cases DSA is used for treatment rather than diagnosis.This article reviewed the current interventional treatment of vascular complications to stress the important role of DSA in diagnosis and treatment of related complications after liver transplantation.
7.The impact on the endothelial cell function and renal blood flow through effective treatment for elderly patients with hypertension

Objective To investigate the impact on the endothelial cell function and renal blood flow through effective treatment for elderly patients with hypertension. Methods 64 cases of elderly hypertensive patients over 70years old with effective treatment, according to pre-treatment blood pressure, were divided into two groups: hyptension level-2 group(30cases) and hypertension level-3 group(34cases), and 30 cases of elderly people with health physical examination during the same period were setlected as the control group. Endothelial cell function was detected, including the endothelin-1 (ET-1)、 nitrous oxide (NO), thromboxane B2 (TXB2)、 6-keto-prostaglandinF1α (6-K-PDGF1α) in plasma. Renal blood flow was explored by color doppler ultrasonic instrument ,involving peak velocity in systole(PSV) and lowest velocity in end-diastole(EDV) of renal arteries, segmental arteries and interlobar arteries.After indicators had reached the standard 1 month in the hypertension level-2 and hypertension leve1-3 patients with effective antihypertensive therapy, the difference of ET-1, NO, TXB2,6-k-PGF1o and PSV, EDV among the three groups were compared. Results The differences were statistically significant in ET-1, NO, TXB2,6-k-PGF1α, PSV and EDV before treatment among three groups (P < 0.05). After blood pressure treated had reached the standard 1 month,indicators were no statistically meaningful difference among the three groups (P > 0.05). Conclusion After effective clinical treatments, the endothelial cell function and renal blood flow of the elderly hypertensive patients can be improved. At the same time, to delay renal damage in elderly patients with hypertension provide a theoretical basis.
8.Effect of Electromyographic Biofeedback on Pointed Foot in Children with Spastic Cerebral Palsy

Objective To explore the clinical curative effect of electromyographic biofeedback on pointed foot in children with spastic cerebral palsy. Methods From June, 2014 to December, 2015, 80 children with spastic cerebral palsy and pointed foot were divided into con-ventional group (n=40) and electromyographic biofeedback group (n=40). The conventional group received exercise and massage, while the electromyographic biofeedback group received electromyographic biofeedback in addition. The passive range of motion (PROM) of ankle, the surface electromyographic results and the selective control results were compared before and eight weeks after treatment. Results After treatment, the PROM significantly decreased in both groups (t>9.142, P<0.001);the integrated electromyography and root mean square, and the selective control increased in both groups (t>3.456, Z>3.178, P<0.01), which were better in the electromyographic biofeedback group than in the conventional group (t>3.737, Z=-2.748, P<0.01). Conclusion Electromyographic biofeedback can further improve the foot dor-sal flexure and pointed foot gait of spastic cerebral palsy children.
9.Effect of Acupuncture Combined with Low Frequency Electrical Stimulation on Cerebral Palsy Salivation

Objective To explore the effect of oral sensorimotor therapy (OSMT), acupuncture, and low frequency electrical stimulation on salivation in patients with cerebral palsy. Methods From September, 2014 to October, 2015, 80 cerebral palsy patients with salivation were recruited. They were randomly divided into four treatment groups:group A was treated with OSMT, group B was treated with acupunc-ture, group C was treated with frequency electrical stimulation, and group D was treated with acupuncture and low frequency electrical stim-ulation, for eight weeks. Meanwhile, 20 cerebral palsy patients with salivation from outpatients were chosen as control group. Results There was no improvement in the root mean square (RMS) value of surface electromyography (sEMG) after treatment in both the control group and group A (t<1.668, P>0.05), and the RMS value significantly improved in the groups B, C and D (t>8.983, P<0.001). There was no sig-nificant difference in the salivation score and RMS value between the control group and group A (W=389.5, t=2.041, P>0.05), and they was better in the groups B, C and D than in the control group and group A (W>226.0, t>8.534, P<0.01), as well as in group D than in groups B and C (W>306.0, t>3.663, P<0.05). Conclusion Acupuncture and low frequency electrical stimulation could effectively improve salivation in children with cerebral palsy, and their combination was superior to either ones, while OSMT did not show obvious benefit.
10.Preparation and Characterization of Celecoxib-loaded PLGA Nanoparticles

OBJECTIVE:To prepare and characterize celecoxib-loaded PLGA nanoparticles. METHODS:Emulsification-solvent evaporation method was adopted to prepare celecoxib-loaded PLGA nanoparticles. With encapsulation efficiency and particle size as the indexes,Plackett-Burman design was preferred to screen the formulation and variables which had a significant effect on the property of nanoparticles. And then Box-Behnken response surface method was used to further optimize selected variables including mass concentration of PLGA,ultrasonic power and ultrasonic time,followed by verification. Malvern particle size analyzer was used to determine the particle size distribution of nanoparticles and Zeta potential of nanoparticle by the optimal formulation technol-ogy,and transmission electron microscope was used to observe the morphology of the nanoparticles,and their drug release in vitro behavior and stability(25,5 ℃)were also observed. RESULTS:The optimal formulation and technology was as follows as PLGA mass concentration of 30.0%,ultrasonic power of 180 W and ultrasonic time of 8 min. For the prepared nanoparticles,encapsula-tion efficiency and particle size were (85.7 ± 4.1)% and (226.1 ± 36.1) nm (n=3) respectively;particle size distribution was (176.2±41.2)nm,polydispersity index was 0.211±0.021,and Zeta potential was(-37.3±1.6)mV. Under the electron micro-scope,the nanoparticles were homogeneous in particle size and distributed spheroidally,with 24 h accumulative release of 52.4%. They were stable within 3 months at 5℃. CONCLUSIONS:Celecoxib-loaded PLGA nanoparticles have been prepared successfully.
