1.β-adrenoceptor activation induces the apoptosis of human mesangial cells

Objective To investigate the effects of β-adrenoceptor (β-AR) activation on the apoptosis in human mesangial cells and it's mechanism.Methods Cultured HMC were used in experiments and were divied into four groups:the control group; β-AR activation (β-AR agonist NE/Pra) group; β-AR inhibitor (Prop) group; antioxidants group.The experiments technology including PCR,confocal scanning microscope,immunofluorescence and Tunel.Results The results of RTPCR and confocal scanning microscope showed that β1-AR and β2-AR were expressed in human HMC.β-AR activation induced reactive oxygen species (ROS) increase in human MCs,the relative levels of ROS were elevated as early as 0.5 h after β-AR activation,and gradually increased and peaked at 4 h on a concentration and time dependent manner.Tunel results demonstrated that β-AR activation induced apoptosis with ROS on a concentration and time dependent manner,β-AR blocking agent-propranolol significantly inhibited β-AR activation induced apoptosis.Antioxidants including vitamin C and NAC could inhibited β-AR activation induced apoptosis (all P < 0.01).Conclusions β-AR is functionally expressed in human mesangial cell,furthermore β-AR activation-induced ROS increase mediate apoptosis.Antioxidants can inhibit β-AR activation induced apoptosis.
2.Cost Effectiveness Analysis on Integrated Traditional Chinese and Western Medicine in ;Treatment of Leukemia

Objective To analyze the features of health economics in the treatment of leukemia;To evaluate the improvement trend of levels of clinical diagnosis, treatment and management. Methods TCM medical information dtatabase and electronic medical system in Zhejing Province TCM Hospital from January 2010 to December 2015 were retrived. According to inclusion and exclusion criteria, leukemia patients’ materials that met the critiria were screened. The total hospitalization fees, cost of Western medicine, the cost of Chinese patent medicine, cost of TCM decoction pieces, and cost of blood transfusion were set as costs, while white blood cell count, hemoglobin concentration, and platelet count were set as effectiveness indicators for cost effectiveness analysis. Results In patients with low white blood cell count increased unit white blood cell count, patients with high white blood cell count declined unit white blood cell count, patients with low hemoglobin increased unit hemoglobin count and patients with low platelet count increased unit platelet count, the total hospitalization fees, cost of Western medicine, the cost of Chinese patent medicine, cost of TCM decoction pieces, and blood transfusionfor the above increases showed a decreasing trend. Conclusion Health economics advantages in three blood routine indicators after treatment of integrated traditional Chinese and western medicine for leukemia is increasing year by year.
3.Microwave-assisted Extraction of Polysaccharides from Glechoma longituba

Objective To research the microwave-assisted extraction of polysaccharides from Glechoma longituba. Methods In present study, the processes of extraction of polysaccharides from Glechoma longituba by water bath and microwave-assisted extraction were optimized using orthogonal test, and the two methods were compared. Results The optimal process of water bath extraction was as follows:liquid-solid ratio 50∶1, time 180 min, temperature 90 ℃ and pH 8. 0. The optimal parameters of microwave-assisted extraction were liquid-solid ratio 30∶1, top microwave power, time 4 min, and pH 8.0. The yield of polysaccharides from Glechoma longituba by microwave-assisted and water bath methods was (4.58±0.09)% and (3.09±0.12)%, respectively, and the yield of microwave-assisted method was elevated by 48.2% compared with that of water bath extraction. Conclusion The microwave-assisted extraction of polysaccharides from Glechoma longituba not only improves the yield, but also reduces the extract volume, saves energy and time.
4.Ultrasounographic assessment of facet joint in the lumbar spine: a feasibility study

Objective To assess the feasibility and accuracy of ultrasound evaluation of facet joint in the lumbar spine by comparison of ultrasonography and CT measurements in normal and degenerative lumbar vertebra small joint.Methods Fifteen patients with chronic low back pain in Shengjing hospital of China Medical University were taken as the study group,and 20 healthy volunteers as the healthy controls from physical examination center of our hospital between January 2014 and December 2014.The ultrasonic and CT values were measured in L 1-S 1 small joints between normal and abnormal lumbar joints (height and width).The length and width of the left and right side small joints of healthy controls,ultrasonic measurement of healthy controls and patients with chronic low back pain,and value of ultrasonography and CT measurements were compared by using t test.Results The abnormal facet joint can be clearly showed by sonography,characterized by bone hyperplasia of edge joints,bone destruction,wear or thinning/thickening of articular cartilage.For the length and width of the lumber joints,the difference of ultrasonic measurement values between normal and abnormal lumbar joints were statistically significant [(1.26±0.03) cm vs (1.43±0.05) cm,t=2.626,P < 0.05;(0.18±0.01) cm vs (0.15±0.02) cm,t=2.264,P < 0.05].In patient group,the difference of length and width values in a total of 150 small joints between ultrasonography and CT were not statistically significant [(1.43 ± 0.17) cm vs (1.42 ± 0.16) cm],t=1.655,P > 0.05;(0.15 ± 0.03) cm vs (0.14±0.03) cm,t=1.656,P > 0.05].Conclusions The facet joint of the lumbar can be clearly shown by sonography and it is valuable in the diagnosis of small joint degeneration for lumbar vertebra.
5.Discussion on Research Focus of Ethical Review of Traditional Chinese Medicine Based on Co-Word Clustering Analysis Method

Objective: To tease relevant research on the ethical review of Traditional Chinese medicine in Chi-na and reveal hot topics and trends in the field.Methods: Taking the academic papers included in the domestic database on ethical review of Traditional Chinese medicine as literature samples,the BICOMB and SPSS22.0 soft-ware were used for document information mining,and the high-frequency keywords were analyzed used co-word clustering and thematic strategic diagram.Results: A total of 78 documents were retrieved,and 17 high-frequen-cy keywords were used.The four major hotspots in the field of ethical review of Traditional Chinese medicine in-cluded: institution building and system certification,GCP ethics optimization,ethical principle research,trial scheme design and project quality management.Conclusion: In recent years,the ethical review of Traditional Chi-nese medicine has been explored by scholars and institutions in China from multiple perspectives,and further re-search can be strengthened combined with the current weak areas,in order to increase the overall level of ethical review of Traditional Chinese Medicine.
6.Evaluation of the efficacy and safety of phacoemulsification under the IOL protection: a randomized controlled clinical study

Objective:To observe the therapeutic effect of intraocular lens (IOL) protected phacoemulsification (PHACO) in patients with hard nucleus cataract.Methods:A randomized controlled clinical study was conducted.A total of consecutive 120 patients (120 eyes) with hard nucleus cataract of Emery grade Ⅳ or Ⅴ were enrolled from January 2019 to May 2022.The patients were randomly divided into PHACO group receiving routine PHACO, IOL protected PHACO group receiving PHACO under IOL protection, and extracapsular cataract extraction (ECCE) group receiving ECCE, with 40 cases (40 eyes) in each group.Finally, 99 patients completed the follow-up, including 30 cases (30 eyes) in PHACO group, 35 cases (35 eyes) in IOL protected PHACO group, and 34 cases (34 eyes) in ECCE group.The total operation time, intraoperative PHACO time and cumulative energy release of each patient were recorded.The corneal endothelial cell density (ECD), coefficient of variation in endothelial cell area (CV), hexagonal endothelial cell ratio (6A), corneal astigmatism and the number of eyes with different grades of uncorrected visual acuity were measured and compared after 3-month follow-up.The intraoperative and postoperative complications were recorded.This study adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Yanbian University Hospital (NO.2023002).Patients were informed of study content and purpose and signed a consent form before treatment.Results:There was no significant difference in ultrasonic energy and time between PHACO group and IOL protected PHACO group ( P=0.691, 0.982).The total operation time was (38.81±2.73) and (36.45±3.45) minutes in PHACO group and IOL protected PHACO group, significantly shorter than (69.60±4.35) minutes in ECCE group (both at P<0.001).There was no significant difference in age, sex, lens nucleus hardness and other baseline data among the three groups before operation (all at P>0.05).Three months after operation, the number of patients with higher uncorrected visual acuity in PHACO group and IOL protected PHACO group was larger than that in ECCE group ( P=0.006, 0.007).The ECD and 6A in IOL protected PHACO group were (2 155.57±177.88)/mm 2 and (41.31±5.18)%, respectively, which were significantly higher than (1 912.64±224.11)/mm 2 and (36.18±3.27)% in PHACO group, and the CV in IOL protected PHACO group was (50.34±5.90)%, which was lower than (55.67±3.30)% in PHACO group, showing statistically significant differences ( P=0.007, 0.003, 0.005).At 1 week and 3 months after the operation, the corneal astigmatism was significantly lower in IOL-protected PHACO group than in ECCE group, but higher than in PHACO group, and the difference were statistically significant (all at P<0.05). Conclusions:Compared with conventional PHACO, IOL-protected PHACO can effectively reduce the damage of corneal endothelium caused by ultrasonic energy, shorten the operation time and reduce postoperative inflammatory reaction compared with ECCE, and does not significantly increase postoperative corneal astigmatism.IOL-protected PHACO is an effective improved surgical method for patients with hard nucleus cataract.

8.Effect of occupational skills relearning on hemiplegic arm function after stroke:a randomized controlled trial

Objective To explore the effect of occupational skills relearning programme on hemiplegic arm motor function and ac-tivities of daily living(ADL)in stroke patients. Methods From February,2022 to August,2023,74 stroke patients in Guangdong Work Injury Rehabilitation Hospital were enrolled and randomly divided into control group(n=37)and experimental group(n=37).The control group received conventional rehabilitation training,and the experimental group received additional occupational skills relearning programme,for three weeks.They were assessed with Fugl-Meyer Assessment-Upper Extremi-ties(FMA-UE),Functional Test for the Hemiplegic Upper Extremity-Hong Kong(FTHUE-HK),Motor Activity Log(MAL)-amount of use(AOU)and MAL-quality of movement(QOM),modified Barthel Index(MBI),and Stroke Impact Scale(SIS)-Hand and SIS-ADL before and after treatment. Results The scores in all assessments improved significantly in both groups(|t|>3.597,P<0.05)after treatment,while the scores of FMA-UE,FTHUE-HK,MAL-AOU,MAL-QOM were higher in the experimental group than in the control group(|t|>2.352,P<0.05). Conclusion Occupational skills relearning programme could promote the recovery of hemiplegic arm motor and activity,and facilitate the use of the hemiplegic arm in daily life in stroke patients.
9.Best evidence for the application management of over-the-counter medications for constipation in the elderly

Objectives:To search, evaluate, and summarize the evidence related to the application management of over-the-counter (OTC) medications for constipation in the elderly, providing evidence-based support for the rational use of these medications.Methods:A systematic search was conducted through computerized decision support systems, guideline websites, professional association websites, and databases for clinical decisions, guidelines, evidence summaries, and expert consensus on the management of OTC medications for constipation in the elderly. The search period spanned from January 2018 to March 2023. The quality of the literature was independently assessed, and evidence was extracted. The 2014 version of the Joanna Briggs Institute (JBI) evidence grading system was used to determine the levels of evidence.Results:A total of 18 article were included, consisting of two clinical decisions, five guidelines, seven expert consensus statements, and four evidence summaries. These covered five areas: medication principles for elderly individuals with different characteristics, medication selection, medication monitoring, health education, and precautions, culminating in a total of 30 best evidence.Conclusions:This study summarizes the best evidence for the management of OTC medications for constipation in the elderly, providing evidence-based support for the rational application of these medications.
10.Research progress on the physiological mechanism and regulatory network of orchitis treatment

Orchitis is a common male genitourinary disorder that significantly impacts patients' life quality.Current treatment strategies have certain limitations and side effects.Ongoing therapeutic strategies focus on the interactions and regulatory networks among pathways and factors involved in the progression of orchitis.The targeted pharmacological agents include inflammatory pathways (p38MAPK, NF-κB, PI3K/Akt), cytokines (TNF-α, IL-6), and the nitric oxide synthase (NOS) system.However, these studies are currently at the animal research stage, and further clinical investigations are necessary to validate their efficacy and safety before clinical use.This article reviews the preclinical animal studies on new treatment methods of orchitis from the aspects of autoimmunity and exogenous microorganism induction, including ketotifen furmarate, aspirin, L-NAME, activin A, cortisol, melatonin, methane, long non-coding RNA MEG3, Abaloparatide, recombinant type Ⅰ interferon, and so on.
