1.The Diagnostic Value of Colonoscopy Compared with Colon Double Contrast Radiography to Colon Carcinoma

Objective To evaluate the value of diagnose of colon carcinoma by colonoscopy and double contrast radiography.Methods The results and data of radiography diagnosis,colonscopy diagnosis,biopsy and post-operation pathology were analyzed retrospectively.Results The rate of accordance by X-ray diagnosis was 85.4%,colonscopy 80.5% and biopsy 84.6%.Conclusion The method of examination of colon double contrast radiography preceeds to colonoscopy examination when the patients were suspected suffering from the right colon or left colon carcinoma accompanying by masses;colonoscopy is the first method of diagnosis to choose when the patients were suspected suffering from carcinoma of sigmoid colon or colon polyps undergoing malignant changes.
2.Practice of and experience in reforming the property right system of enterprise-owned hospitals

The Workers' Hospital of Huangmian Group in our city, which succeeded in separating from the enterprise and becoming commercially oriented through reforming the property right system, has made some worthwhile attempts at transforming the property right system of enterprise-owned hospitals. The experience accumulated includes: commercialization of enterprise-owned hospitals through reforming the property right system is a must in adapting to the system of the socialist market economy and setting up the system of modern enterprises; it is also a must in the current reform of the health system; reform of the property right system of enterprise-owned hospitals gave strong impetus to the reform of operational mechanisms of personnel matters, distribution and management within the hospitals; appropriate policies are the key to ensuring the success of the reform; and hospitals with transformed ownership ought to maintain their original functions and continue to undertake such social tasks as medical treatment, prevention of diseases, healthcare and supervision of public hygiene.
3.Advantages and application of telemetry compared with the traditional methods in safety pharmacology

Objective To prove the advantages of telemetry by comparing with the traditional methods in safety pharmacology.Methods To monitor continuously the heart rate, respiratory rate, blood pressure and ECG of Beagle dogs by traditional and telemetry methods respectively, analyze and compare the changes between anesthetized and conscious dogs before and after feeding.Results Maintenance of anesthesia changed significantly the heart rate, respiratory rate, blood pressure and QT interval in the ECG of animals.The changes of physiological indicators in 24 h is not obvious in conscious animals, and showed a certain biorhythm.Compared with the conscious animals, the anesthetized dogs’ heart rate was significantly higher, blood pressure increased significantly, QRS and QTcf interval prolonged significantly, respiratory frequency decreased, heart rate increased significantly after feeding, and QTcf interval extended very significantly.Conclusions Traditional methods in safety pharmacology affect animal physiological indicators such as heart rate, blood pressure, respiratory rate and QT interval, which affect the objectivity of drug evaluation.Using conscious animals by telemetry can reduce these errors, however, the interference from outside should be eliminated.
4.Influence of different preservation conditions and times on the measurement of blood biochemical indicators in Beagle dogs

Objective Beagle dogs are commonly used animal for drug safety evaluation .As the necessary parameters , blood biochemical indicators are detected in acute or chronic toxicity tests .This study aims at assessing the influence of different preservation conditions and different preservation time on blood biochemical indicators to ensure the reliability of test results of long-term toxicity assessment .Methods Six Beagle dogs (3 males and 3 females) were used in this study .After collection and preparation of serum samples , biochemical indicators were detected after preservation in refrigerator at 2-8℃for 1, 2, 5, 8, and 12 hours;after preservation in ice transportation boxes at 2-10℃for 2, 5, and 8 hours;and after preservation in refrigerator at -20℃ for 1, 3, and 5 days.The biochemical indicators included alanine aminotransferase ( ALT ) , aspartate aminotransferase ( AST ) and alkaline phosphatase ( ALP ) , total protein ( TP ) , albumin (propagated), urea, creatinine (CREA), glucose (GLU), total cholesterol (TCHO), total bilirubin (TBIL), creatine kinase ( CK ) , gamma pancreatic acyl transferase ( GGT ) , calcium ( CA ) , lactate dehydrogenase ( LDH ) , phosphorus ( P) , high-density lipoprotein cholesterol ( HDL-C) , low density lipoprotein cholesterol ( LDL-C) , triglyceride ( TG) , sodium ( Na+) , potassium ( K+) and chloride ( Cl -) .Results Compared with the results of samples preserved for 1 hour, the LDL-C result of that preserved in refrigerator at -20℃for 5 days was significantly increased (P<0.05), and the coefficient of variation was 39%.The CK and LDH results of specimens preserved in refrigerator at -20℃ for 5 days was increased but not significantly ( P>0.05 ) , and the coefficient of variation of LDH was 41%.Conclusions According to the test results of blood biochemical indicators in the Beagle dogs detected after different preservation conditions and different preservation time in this study , detection test should be done within 1 hour, if not, detection should be done within 12 hours for the samples preserved at 2~8℃, or within 3 days for the sample preserved at -20℃.For transportation of serum samples , the serum samples should be placed in the ice box at 2~10℃, and detection test should be done within 8 hours .
5.Inhibitory effect of triacetylshikimic acid on plasma contents of vasoactive substances and brain myeloperoxidase activity during focal cerebral ischemia-reperfusion in rats

AIM To explore whether plasma vasoactive substances and neutrophil infiltration make different contribution in cerebral ischemia-reperfusion injury and protective effect of triacetylshikimic acid(TSA). METHODSThe rat models of ischemia 90 min and reperfusion 3-48 h were prepared with middle cerebral artery occlusion. TSA 50-200 mg·kg-1 were given (ig) immediately and 60 min again after the onset of ische- mia. Serotonin and thromboxane B2 (TXB2) concentrations in plasma were detected by fluorescence spectrophotometry and radioimmunoassay respectively. Myeloperoxidase(MPO) activity in brain tissue was quantified by chemical analysis. RESULTS At 3-24 h after reperfusion, the concentrations of plasmic serotonin, TXB2, and brain MPO activity increased obviously in a time-dependent manner. At 48 h after reperfusion, the concentrations of serotonin and TXB2 decreased to the same level of sham. Nevertheless, brain MPO activity remained more elevated than the contralateral cortex. At 24 h after reperfusion, TSA (100 and 200 mg·kg-1) was shown to possess the ability to inhibit the increased plasmic serotonin, TXB2 concentrations, and brain MPO activity induced by focal cerebral ischemia-reperfusion. CONCLUSION Vasoactive substances in plasma and MPO activity in brain tissue show different time courses during focal cerebral ischemia-reperfusion and make different contribution to brain damage. TSA is effective to protect the ischemic brain tissue from ischemia-reperfusion injury.
6.Effect of tacrolimus on neural behaviors and memory of rats with traumatic brain injury

BACKGROUND:At present, many researches have shown that tacrolimus has protecting effect on peripheral nerves,however,whether tacrolimus has a similar effect on central neural system has not been known yet.OBJECTIVE:To investigate the effect of tacrolimus on neural behaviors and memory by observing the neural behaviors and detecting the expression of synaptophysin in cortex,hippocampus and basipodite.DESIGN:Parallel and before and post contrast study.SETTING:Department of neurosurgery, affiliated hospital of the two medical universities.MATERIALS:The experiment was carried out in the Laboratory of Department Neurosurgery,Shanghai Changzheng Hospital in November 2002.Twenty-four qualified Sprague-Dawley rats supplied by Shanghai Experimental Animal Center of medical institution of Fudan University,with body mass of 180-220 g,were randomly divided into three groups:control, injury and treatment,with 8 rats in each group.INTERVENTIONS:Anesthesia was induced by administration of sodium pentobarbital(with the concentration of 20 g/L,40 mg/kg) twenty-four hours before the operation.The coup energy was 151.95-172.21 kPa that was equal to medium injury. The control rats did not receive the coup. Tacrolimus was given to the treatment group in the dosage of 1 mg/kg body weight by intraperitoneal injection at 5 minutes post trauma. The drug was given once physiological saline. Before and post trauma (on the third and seventh day)all rats Accepted Beam Walking Test,Beam Balance Test and Memory Test,respectively. One week after trauma,all rats' heads was removed and the brain was taken out within seconds.The hippocampus,cortex and basipodite of the traumatic hemisphere were kept in methanal with the concentration of 4 percent. Immunohistochemistry and image analysis system were used to study the expression of synaptophysin in cortex,hippocampus and basipodite of the traumatic hemisphere.MAIN OUTCOME MEASURES: ①The performance records of neural behaviors and memory of the three groups. ②Results of the expression of synaptophysin in cortex, hippocampus and basipodite of the three groups.③Apoptotic cellscalculation of synaptophysin in cortex,hippocampus and basipodite.RESULTS:Totally 24 rats entered the final analysis. ①On the third day and the seventh day,the scores recorded in Walking Test and Balance Test of the treatment group were less than those in the injury group[Walking Test: (7.5±2.5) s,(5.5±2.1) s,(10.5±2.5) s,(8.2±2.5) s. Balance Test:(3.4±0.5)score,(2.5±0.2) score,(5.7±0.2) score,(5.0±0.5) score,P < 0.05-0.01].But the scores recorded in memory test of the treatment group were still more than those in the control group,memory test[(4.9±1.7) s,(6.2±2.3) s,(4.0±1.5) s,(4.4±2.6) s,P < 0.05-0.01]. ②The expression of synaptophysin in cortex,hippocampus and basipodite in the treatment group was still more than that in the injury group(140.36±3.87, 45.52±2.16,31.67±2.35,96.25±2.85,24.35±2.47,20.49±2.08,P < 0.01);The expression of synaptophysin control group was especially more than that in treatment group (162.24±3.52,50.58±2.31,42.69±2.53, P < 0.01).Apoptotic cells calculation of cortex,hippoeampus and basipodite in treatment group was far less than those in injury group,(10.37±2.12)%,(18.39±2.87)%, (12.78±2.45)%,(21.14±4.85)%,(38.57±3.78)%, (21.18±4.59)%,P < 0.01].And apoptotic cells calculation in injury group was more than those in control group [(3.85±2.56)%,(4.96±2.15)%, (3.52±2.17)%,P < 0.01].CONCLUSION:Tacrolimus can facilitate the expression of synaptophysin,refrain from calcineurin involving in the process of transcription-dependent and nontranscription-dependent apoptosis,and can also contribute to the recovery of memory and neural behaviors post trauma.
7.Effect of 3,4-oxo-isopropylidene-shikimic acidon free radical metabolism in the brain issue of rats subjected to middle cerebral artery thrombosis

AIM To study effect of 3,4 oxo isopropylidene shikimic acid(ISA) on free radical metabolism in the brain issue of rats subjected to middle cerebral artery thrombosis.METHODS Adopt a middle cerebral artery thrombosis model to determine the activity of T SOD, Cu Zn SOD, MDA and GSH px of brain issue of rats by the way of colorimetry.RESULTS ISA 100 and 200 mg?kg -1 ig for 2 times in 24 hours raised the activity of T SOD;ISA 50, 100 and 200 mg?kg -1 ig,increased the activity of Cu Zn SOD and GSH px in rats brain issue ;ISA 50, 100 and 200 mg?kg -1 ig for 2 times in 24 hours,decreased the content of MDA in rats brain issue. CONCLUSIONS ISA protect brain issue from the ischemic damage by modifying the free radical metabolism.
8.The Clinical Evaluation of MR Cholangiopancreatography

Objective To evaluate the clinical diagnostic value of magnetic resonance cholangiopancreatography ( MRCP ).Methods MPCP data of 42 cases with pancreatic and bile duct lesion proved by operation and pathology were analyzed retrospectively.MRCP was performed using single-shot-radio SE sequence (SSH/RAD). Of all cases included bile duct lithiasis in 19 , inflammation in 3,congenital cyst in 6 , carcinoma in 12,jaundice caused by diverticulitis of duodenum in 1 and Mirizzi syndrome in 1 .Results MRCP was succeeded at one time in all cases . On MRCP , the visual rate of intrinsic and extrinsic hepatic bile duct was 100% and that of common pancreas duct was 80.58%.The localized and qualitative diagnosis of lesions were up to 100%,88.2% respectively.Conclusion SSH/MRCP/RAD plays an important role in diagnosis of the pancreatic and bile duct lesion in clinical practice.
9.CT Diagnosis of Hepatic Abscess

Objective To analyse the CT features of hepatic abscess in different stages of pathology so that to improve the accuracy of CT diagnosis.Methods The plain CT and triphase contrast-enhanced CT findings of hepatic abscesses in 50 cases proved by surgery or other clinical methods were retrospectively analysed and compared with CT findings of hepatic carcinoma,hepatic metastases and hepatic hemangiomas.Results CT features of hepatic abscesses were divided into three types according to different stages of pathology.Early pyogenic hepatic abscess(10 cases)had two types:small cavitation(9 cases)and mass(1 case).Typical pyogenic hepatic abscess(38 cases)and granulomatous hepatic abscess(2 cases).Conclusion CT findings of liver abscesses are different in different stage of pathologic changes,triphase contrast-enhanced CT is specific in diagnosing hepatic abscess.
10.Liver Cystic Lesions:CT and MRI Diagnosis

Objective To explore CT and MR manifestations and its diagnostic value of liver cystic lesions.Methods The clinical and imaging data of 78 cases with liver cystic lesions were collected,including 21 cases of inflammatory cystic lesions,30 cases of tumor cystic degeneration and 27 cass of congenital growth cysts.Results The detected rate of liver cystic lesions with CT and MRI was all 100%.For inflammatory cystic lesions,tumor cystic degeneration and growth cyst,CT diagnostic accurate rate was 76%,90% and 93%,respectively,and MR was 83%,93% and 100% respectively.CT and MR manifestations:Of all inflammatory cystic lesions,hepatic abscess and liver echinococcosis cyst appeared as cystic occupying lesion,the cysts were of tension,low-density band on CT or water-like signal intensity on MR around the lesions in hepatic abscesses.Liver echinococcosis cysts were of big cyst with small cyst sign and enhanced moderately in cystic wall post contrast-enhanced scan.The metastasis cystic tumors showed single or multiple cystic lesions and mild or moderate circular enhancement.The primary liver tumor's cystc degenerative lesions showed cyst-solid lesions on CT and MR,and were of enhanced manifestations of the primary tumors.Growth cysts were single or multiple low-density or water-like signal intensity,generally there was not enhanced.Conclusion CT and MR have great diagnostic value for the location and nature of cystic lesions.
