4.The changes of Endothelin-1 and tumor necrosis factor-? in the congestive heart failure and the intervention role of vitamin E and vitamin C

Objective To study the level changes of tumor necrosis factor ? (TNF ?)and endothelin 1 (ET 1) and the intervention role of vitamin E and vitamin C in the patients with congestive heart failure (CHF).Methods 65 cases of heart failure were randomized into A group of 32 cases and B group of 33 cases.A group were given cardiotonic,diuretic and extending blood vessel drugs;B group were given 200 mg vitamin E once a day and 200 mg vitamin C three times a day,in the duration of two weeks,besides the medications administered to A group.Specific radioimmunoassay and double antibody sandwich ELISA were used to measure the concentrations of TNF ? and ET 1 of 65 cases with heart failure and 32 healthy subjects.Results The TNF ? and ET 1 levels of congestive heart failure patients were significantly higher than those in the control group (P
5.Endovascular embolization treatment of intracranial aneurysms with detachable coils in the cerebral vasospasm time

Objective To evaluate methods and effect of the endovascular embolization of intracranial aneurysms. Methods From 2004,3 to 2004,8,14 patients with intracranial aneurysms, underwent intravascular embolization was reviewed. Clinic presentation: 6 cases males, 8 cases females, age range 45-78 years. 1 case ICA aneurysms, 8 cases PCA aneurysms, 5 cases ACA aneurysms . All patients were treated with DCS/MATRIX embolization. Results 11 cases aneurysms were 100% occluded, 2 cases aneurysms were 70-90% occluded,1 case aneurysm was 50% occluded; 6 patients found cerebral vasospasm.Conclusions In the Cerebral vasospasm time, endovascular embolization of intracranial aneurysms is a safe, minimal traumatic and effective method.
6.Clinical study of low dose rituximab for the therapy in elderly patients with chronic refractory immune thrombocytopenia

Objective To study the efficacy and safty of low dose rituximab therapy in elderly patients with chronic refractory immune thrombocytopenia(ITP) .Methods 36 elderly patients with chronic refractory ITP were collected from Xingtai People′s Hospital of Hebei ,the patients were given intravenous injection of intravenous rituximab at the dose of 100 mg once weekly for 4 consecutive weeks ,observing the efficacy and adverse reaction and following up for 12 months .Difference of platelet (PLT ) ,white blood cell (WBC) ,cluster differentiation 8 + (CD8 + ) ,CD19 + eukomonocyte number and immunoglobulin (IgG ,IgM ,IgA )were compared before and after treatment .Results The responses of complete remission (CR)were 17 casese(47 .2% ) ,remission (R) were 11 casese (30 .6% ) and no remission (NR) 8 casese (22 .2% ) respectively ,2 patients experienced mild dizziness and chest tightness ;PLT were significantly increased ( P = 0 .001) after treatment ,CD19 + lymphocyte count decreased significantly (P =0 .001) ,WBC ,CD8 + lymphocyte and serum immunoglobulin IgG ,IgM ,IgA showed no significant difference before and after treat-ment(P> 0 .05) .Conclusion Low dose rituximab had better clinical efficacy for the treatment of elderly patients with chronic re -fractory ITP ,low dose rituximab may be a effective treatment in elderly chronic refractory ITP patients with minor adverse reac -tions .
7.Effects of diazoxide pretreatment on expression of phosphatidylinositol 3-kinase mRNA and protein serine/ threonine kinase mRNA in rat myocardial microvascular endothelial cells exposed to hypoxia-reoxygenation

ObjectiveTo investigate the effects of diazoxide pretreatment on expression of phosphatidylinositol 3-kinase(PI3K) mRNA and protein serine/threonine kinase(Akt) mRNA in rat myocardial microvascular endothelial cells exposed to hypoxia-reoxygenation (H/R).MethodsThe SD rat myocardial microvascular endothelial cells were cultured.The cells were seeded in 96-well plates ( 100μd/hole) or in 6 cm diameter dishes (2 ml/dish) with the density of 1 × 106/ml and randomly divided into 4 groups ( n =25 each):normal control group (group C),H/R group,diazoxide pretreatment group (group DZ) and diazoxide pretreatment + 5-hydroxydecanoate (5-HD,a mitochondrial ATP-sensitive potassium channel blocker) group (group DZ + 5-HD).The cells were exposed to 2 h hypoxia followed by 2 h reoxygenation.Diazoxide 100 μmol/L and diazoxide 100 μmol/L + 5-HD 100 μmol/L were added to the culture medium 2 h before hypoxia in groups DZ and DZ + 5-HD respectively.The cell viability,apoptotic rate and expression of PI3K mRNA and Akt mRNA were detected at the end of reoxygenation.ResultsCompared with group C,the cell viability was significantly decreased,while the apoptotic rate increased and expression of PI3K mRNA and Akt mRNA up-regulated in group H/R (P <0.05 or 0.01).Compared with group H/R,the cell viability was significantly increased,while the apoptotic rate decreased and expression of PI3K mRNA and Akt mRNA up-regulated in group DZ (P < 0.05 or 0.01).5-HD could inhibit diazoxide pretreatment-induced changes mentioned above (P < 0.05 or 0.01 ).ConclusionDiazoxide pretreatment can reduce H/R injury by promoting PI3K gene and Akt gene transcription through activation of mitochondrial ATP-sensitive potassium channels in rat myocardial microvascular endothelial cells.
8.Relationship of NIX expression with pulmonary alveolus apoptosis after severe thoracic collision injury

ObjectiveTo explore the relationship between the mRNA and protein expressions of NIX and the pulmonary alveolus apoptosis following severe acute lung injury (ALI).Methods Rat models of severe collision injury on the chest were built.The mRNA and protein expressions of NIX in the alveolar cells at 6,12,24,48,72 and 96 hours after injury were detected using immunohistochemistry,immunoblotting and RT-PCR.Meanwhile,apoptosis of the alveolar cells was checked at different time points with Tunel assay.ResultsThe protein expression of NIX in the alveolar cells was observed both in experimental and control groups,which increased at 6 h post injury,peaked at 48 h and then declined till approaching the pre-injury level at 96 h.In the meantime,NIX showed a high expression both in the vascular endothelial cells (VECs) and the renal interstitial fibroblasts.The apoptosis of alveolar cells mainly presented in bronchi,blood vessel endothelium (BVE) and alveolar epithelium at 24 h post injury.The post-injury apoptosis rate of the alveolar cells was significantly higher than the pre-injury rate ( P < 0.01 ),which reached the peak at 72 h and then decreased gradually.The changes of NIX protein in lung tissue showed a positive correlation with the apoptosis rate of alveolar cells after injury (r =0.303,P < 0.01 ).ConclusionsThe up-regulated expression of NIX takes part in the pathophysiological process of apoptosis of the alveolar cells and shows consistency with the apoptosis rate change of the alveolar cells,as may be the molecular basis for apoptosis of the alveolar cells after ALI.
9.‘Response to injury’ teaching based on the concept of global minimum essential requirements

Taking global minimum essential requirements(GMER)as standard,school of basic medical science in Wuhan University paid more attention to the cultivation of students' professional skills, innovation ability and communication ability. ‘Response to injury’integrated course was carried out. Based on the immunology,this course involves pathology and pathophysiology and explores the mecha-nism of the diseases' diagnosis and treatment. It took small classes,bilingual education,literature read-ing ,experiments and discussions in various forms . This reform aimed to improve the immunology , pathology and pathophysiology teaching and make our medical education meet the process of globalization.
10.EFFECT OF CHRYSALIS OIL ON BLOOD GLUCOSE AND OXIDATIVE STRESS IN DIABETIC RATS

Objective To investigate the effect of chrysalis oil on blood glucose and oxidative stress in diabetic rats induced by Streptozotocin (STZ).Method Sixty male SD rats were divided into 6 groups according to blood glucose:normal group,diabetic control group,high,medium,low dose (7.5,6.0,4.5ml/kg bw.) chrysalis oil groups and positive control group.Food and fluid intake were monitored every day.blood glucose and body weight were measured at each weekend,and at the end of experiment,glycosylated serum protein (GSP) was tested,and the effect of chrysalis oil on the content of malondialdehyde (MDA),reduced glutathione (GSH),total antioxidant capacity (T-AOC) in liver and pancreas were observed.Results (1) Diabetic symptoms were ameliorated and blood glucose and GSP were all lowered in therapeutic groups compared with diabetic control group,whereas,high dose chrysalis oil group had the same effect as positive control group.(2) In diabetic groups treated with chrysalis oil,the activity of GSH and T-AOC were significantly increased and the content of MDA decreased compared with diabetic control group.Conclusion Chrysalis oil could decrease blood glucose in a dose-and-effect relationship and improve antioxidant capacity.
