1.The effect of on pharmaceutical care the decoction use compliance on pediatric community acquired pneumonia

Objective To investigate the decoction use compliance of the children with the pediatric community acquired pneumonia(CAP) by providing the pharmaceutical care from the clinical pharmacist.Methods 252 patients with CAP were divided into the observation group and control group.The patients of the observation group were provided with pharmaceutical care,including drug use education,drug use guiding,etc,while the patients in the control group were not given any pharmaceutical care from the clinical pharmacist.Results The rate decoction use compliance in the observation group was 78.1%,which was 39.1% in the control group,the difference between the two groups was significant (x2 =12.56,P < 0.05).Conclusion Pharmaceutical care can improve the rate decoction use compliance decrease the drug waste.
2.Application values of electromyography in diagnosis and differential diagnosis of diabetic polyneuropathy and alcoholic peripheral neuropathy

Objective To study the features and differences of electromyography of diabetic polyneuropathy (DPN) and alcoholic peripheral neuropathy (APN ), and to provide reference basis for the clinical application of electromyography.Methods 58 patients with DPN and 30 patients with APN were used as subjects. Nerve conduction studies (NCS)and sympathetic skin response (SSR)were performed in the patients, all data were analyzed.Results In the patients with DPN, the abnormalities of NCS and SSR were increased with the prolongation of the time diabetes,and the abnormality of SSR was higher than that of NCS(P<0.05).In the patients with APN, both demyelination and axonal loss in motor and sensory nerves were significantly involved, and the abnomalities of NCS and SSR were higher than those of the DPN patients (P<0.05 or P<0.01);but the proximal nerves were just involoved mildly. Conclusion Both DPN and APN have characteristic electrophysiological features.Early electromyography is useful for the early diagnosis of DPN and APN.According to the electrophysiological features of DPN and APN,the reason of peripheral neuropathy in the patients who have diabetes mellitus and alcoholism could be differentiated.
3.Survival time of heart allografts in rats can be prolonged by administration of donor apoptotic bone marrow cells via portal vein before transplantation

Objective To study the effects of administration of donor apoptotic bone marrow cells via portal vein on the survival time of heart transplantation in rats.Methods Wistar rats were selected as donors and SD rats as recipients, and divided into 4 groups at random. In groups A, B and C, the animals were injected via portal vein with RPMI 1640 0.5 ml 6 as control group, 5?107 donor bone marrow cells and 5?107 apoptotic donor bone marrow cells 6 days before transplantation, respectively , and not subjected to cyclosporin A (CsA). In group D, the animals were injected intra- peritoneally with CsA 5 mg/kg from 3 days before transplantation till 10 days post operation. Bone marrow cells apoptosis was induced by 60 Co? radiation. Heart transplantation models were established in each group. The survival time of donor heart and histological changes were observed. Serum concentrations of IL-10 and TGF-?_1 of recipients and mixed lymphocyte reaction (MLR) were measured. Objective To analyze the outcome of intra-artery chemotherapy for T1G3 bladder cancer , and its effectiveness and safety. Methods From June 2003 to May 2014, 39 patients with T1G3 bladder cancer chose intra-artery chemotherapy (Gemcitabine plus cis-platin), and close follow-up was required after 2 cycles of chemotherapy. During the follow-up, transurethral resection of bladder tumor was performed for non-muscle invasive bladder cancer, and cystectomy was performed for muscle invasive tumor. Results Of all patients, 32 were male and 7 were female. The median age was 56 years old (range: 32-82 years), and median follow-up time was 56 months (range: 12-136 months). Nineteen patients were primary bladder cancer, and 20 were recurrent tumor. During the follow-up, 17 patients developed recurrent tumors, including 8 progressed tumors and 3 died from tumor. Two-year and 5-year progressed-free survival were 88% and 74%, and 2-year and 5-year cancer-specific survival were 97% and 89%, respectively. During 5 years′ follow-up, 81% survivor preserved intact bladder, and only 1 patient cancelled chemotherapy for adverse effect. Conclusions Intra-artery chemotherapy (GC regimen) is a choice for T1G3bladder cancer, preventing from disease progression with good tolerance.
5.Survival analysis of 256 patients with oral cancer

Objective:To analyze survival in patients with advanced oral cancer from prospective clinical trials. Methods:From 2008 to 2010, 256 patients with oral cancer at clinical stage III/IVA were randomly categorized into two groups. Patients in the experi-mental group received neo-adjuvant chemotherapy, surgery, and post-operative radiation, and patients in the control group underwent surgery and post-operative radiation. All patients were routinely followed-up after treatments. Survival was analyzed using Kaplan–Meier method and log-rank test, and differences were considered statistically significant at P value lower than 0.05. Results: Each group was composed of 128 patients. With the median follow-up period of 60 months, the 5-year overall survival rate was 61.7%and the disease-free survival rate was 53.9%. The overall survival rate (P=0.350) and the disease-free survival rate (P=0.160) were not sig-nificantly different between the experimental and control groups. Patients with positive pathological response to neo-adjuvant chemo-therapy exhibited significantly improved overall survival (P<0.05). Conclusion:Radical surgery should be emphasized to improve the prognosis of oral cancer. Functional reconstruction could also improve the quality of life and survival of patients. Despite that neo-adju-vant chemotherapy could not improve the survival of patients with advanced oral cancer in entirety, it could benefit patients exhibiting positive treatment responses.
6.Establishment of a rat femoral nonunion model by intramedullary fixation

BACKGROUND:Establishing an objective and standard animal model of bone nonunion is essential for experimental studies and treatment of nonunion. OBJECTIVE:To establish an objective animal model for experimental studies of nonunion. METHODS:Specific pathogen-free male Wistar rats were selected and prepared by cutting off a 5 mm bone defect in the middle femur,peeling off a large periosteum and removing bone marrow.Animal models were fixed with a 1.2 mm Kirschner wire.At 1,4 and 8 weeks,bone nonunion was observed by gross specimen observation,X-ray examination and histopathological examination. RESULTS AND CONCLUSION:The gross specimen,X-ray film and histopathological examination showed that there was no callus formation in the bone defect area,the broken end was filled with fiber tissue,and the bone callus was rare or even invisible.To conclude,the rat model of nonunion can be successfully established by osteotomy of the middle femur,large periosteum peeling and bone marrow removal.This modeling method is simple,reliable and effective.
7.Application of Bishop-Koop stoma in refractory congenital intestinal atresia.

OBJECTIVETo explore the feasibility and safety of Bishop-Koop stoma procedure in the treatment of neonates with refractory congenital intestinal atresia.
METHODSClinical and follow-up data of 25 neonates with refractory congenital intestinal atresia undergoing Bishop-Koop stoma procedure in our center from January 2011 to December 2014 were retrospectively analyzed. Of 25 neonates, 13 (52%) were male, 12(48%) were female, the birth weight was 1600-3800 g (mean 2920 g), the age of admission was 10 hours to 20 days, and the age of operation was 1-58 d (mean 7 d). Diameter ratio of proximal atresia intestine to distal atresia intestine was all greater than 4. Eleven cases(44%) were high jejunal atresia, 3 cases(12%) type III( b, 7 cases(28%) type IIII(, 14 cases(56%) were identified as complex meconium peritonitis, and 3 cases (12%) received reoperation.
RESULTSAll the cases completed their Bishop-Koop stoma operations successfully with median operative time of 3 (1.2-4.5) hours and median intra-operative blood loss of 3.5(1-18) ml. The postoperative complication rate was 20%(5/25), including 3 cases of cholestasis, 1 case of ileus, and 1 case of neonatal necrotizing enterocolitis with septicemia who died 6 days after operation resulting in the mortality of 4%. Besides, 1 case gave up treatment because of economic reason. For the rest 23 neonates, the median first feeding time was 11 days and mean time was 11(5 to 20) days; the median time of postoperative total parenteral nutrition (TPN) was 15 days and mean time was 21 (5 to 68) days; the median hospital stay was 33 days and mean hospital stay was 25(12 to 81) days, respectively. Two-stage stoma closure operations were performed in all the 23 cases afterwards and no postoperative associated complications were found. When discharge after Bishop-Koop stoma operations, Z score of body weight was normal in 3 cases(13.0%) and lower than normal in 20 cases(87.0%), while in hospitalization for stoma closure, Z score of body weight was normal in 19 cases(82.6%) and lower than normal in 4 cases (17.4%). Of 23 cases, serum albumin level was normal in 9 cases(39.1%) before operation, in 3 cases (13.0%) when discharge and in 22 cases(95.7%) in hospitalization for stoma closure.
CONCLUSIONBishop-Koop stoma procedure is safe and feasible in the treatment of neonates with refractory congenital intestinal atresia, and can obviously improve the nutritional status.
