1.The expression and significance of PAI-1,t-PA,u-PA and u-PAR in induced sputum in patients with asthma

Objective To detect the levels of plasminogen activator inhibitor(PAI)-1,tissue plasminogen activator(t-PA),urokinase-type plasminogen activator(u-PA)and urokinase-type plasminogen activator receptor(u-PAR)in induced sputum in patients with asthma and analyze their clinical significances in pathogenesis of asthlna.Methods The induced sputum was isolated from 15 normal subjects(control group),29 patients with acute attack of asthma(attack group)and 26 stable asthma patients(stable group).The levels of PAl-1,t-PA,u-PA and u-PAR were measured by ELISA.Meanwhile FEV1%pred of all groups were evaluated.Results The levels of PAI-1 and u-PAR of induced sputum were significandy higher in at-tack group[(23.32±21.64),(0.766±0.272)μg/L]and stable group[(17.23±9.40),(0.700±0.271)μg/L]than those in control group[(5.99±5.04),(0.516±0.197)μg/L](P<0.05).As for the induced sputum concentrations of u-PA and t-PA,there were no significant differences among the attack group[(0.287±0.235),(7.68±3.46)μg/L],stable group[(0.251±0.276),(9.88±4.68)μg/L]and control group[(0.239±0.322),(10.35±7.47)μg/L].In stable group,the level of PAI-1 had a negative correlation with the FEV1%pred(r=-0.756,P<0.01).In stable group,the level of PAI-1 had a positive correlation with the length of disease course(r=0.454,P<0.05).Conclusion PAI-1 and u-PAR maybe involved in pathophysiologic change of asthma.
2.Molecular evolution and binding free energy analysis of substrates of cephalosporinase ADC-57

Objective To analyze molecular evolution and binding free energies of cephalosporinase ADC-57.Methods Minimum Evolution method in MEGA 5.0 was used to analyze molecular evolution of cephalosporinase ADC-57 and other 19 kinds of beta-lactamases.Tertiary structure of ADC-57 was predicted by homology modeling referring to tertiary structure of CMY-2.The molecular docking of ADC-57 to 11kinds of beta-lactams substrates was performed using DOCK module in ArgusLab 4.1and the binding free energies (△G) was calculated.Results ADC-57,CMY-2,DHA-1,ADC-7,ADC-56 were all belong to class C beta-lactamase,and molecular evolution between ADC-57 and ADC-56 was closest.The top three antibiotics with declining binding free energy of beta-lactams were ertapenem,cefoxitin and ceftazidine,while the last two were clavulanic acid and aztreonam.Conclusions Catalytic activities of cephalosporinase ADC-57 to ertapenem,cefoxitin and ceftazidine are high,while to clavulanic acid and aztreonam are low. Hydrolytic activities of enzyme to beta-lactams (substrates) can be analyzed by molecular docking.
3.The risk assessment of antiplatelet medications in angiogram -negative subarachnoid hemorrhage

Objective The influence of antiplatelet medications on prognosis after non -aneurysmal subarachnoid hemorrhage(SAH)is unknown.This study aimed to evaluate the risk of antiplatelet mdications in devel-oping SAH.Methods 420 patients who underwent catheter cerebral angiography after presenting with nontraumatic SAH were included.Outcomes were assessed by using the modified Rankin scale.Results A total of 420 patients underwent catheter angiography for evaluation of SAH.Of these,63 cases (15%)were angiogram -negative.The fraction of patients presenting with angiogram -negative SAH as well as the frequency of antiplatelet use among these patients significantly increased during the study period.Antiplatelet use was more commonly associated with angiogram-negative SAH(18 /63,28.6%)than with angiogram -positive SAH(39 /357,11%;P =0.001).At 14 days after presentation,poor outcome was significantly more frequent among patients who took antiplatelet agents (20 /63, 31.7%)than among those who did not(12 /63,20%;P =0.017).Conclusion Antiplatelet medication use is asso-ciated with poor early,but not late,outcomes after angiogram -negative SAH.More studies are needed to confirm this association.
4.Investigation on the current situation and analysis of related influencing factors of xerosis in elderly inpatients

Objective:To investigate the prevalence and influencing factors of xerosis in elderly inpatients, to provide basis for effective prevention and management of xerosis.Methods:Using the cross-sectional survey method and the self-designed survey table of the current situation of xerosis and prevention of the elderly, the incidence of xerosis in 1 028 patients hospitalized in Suqian Hospital Affiliated to Xuzhou Medical University, Suqian City Hospital of Traditional Chinese Medicine and Siyang County Hospital of Traditional Chinese Medicine was investigated, and the related factors were analyzed.Results:The incidence of xerofosis was 20.23%(208/1028); the incidence of xerosis was 20.23% (208/1 028), which was mainly found in the lower limbs, with the incidence of 53.37% (111/208), among which the incidence of mild xerosis was the most, with the incidence of 51.44% (107/208). The incidence rate of the elderly patients with xerosis between different age, qualifications, nutritional status, comorbidity, Braden score, drug use, main caregivers, departments, and daily skin care were statistically significant ( χ2 values were 6.91-35.71, all P<0.05). Binary logistic regression analysis showed that age, nutritional status, comorbidity status, Braden score and daily skin care were the independent influencing factors of skin dryness in elderly inpatients. Conclusions:There is a common skin symptom of xerosis in the elderly. It is necessary to establish a standardized management process and take targeted preventive and nursing measures.
